The triple conjoined scapular-latissimus dorsi-groin flap for reconstruction of long lower extremity degloving injury: A case report.

Since the introduction of the angiosome concept, many attempts have been made to push the dimensional limits of free tissue transfers. While initially limited by the boundaries of random vascularization of adjacent angiosomes, wider flaps were transferred by combining contiguous source vessels. Combined flaps, either chimeric or conjoined, represent nowadays the largest reconstructive options in the human body. This is the first report of a triple conjoined free flap with two vascular anastomoses, able to cover the anterior surface of the entire lower limb, representing the longest flap ever r

A pedicled anterolateral thigh flap for abdominal reconstruction after previous degloving injury of the donor site: revascularisation of the donor site.

A pedicled anterolateral thigh flap was used to reconstruct an abdominal defect after traumatic degloving of the entire skin of the right upper leg two-and-a-half years earlier. There are few reports about revascularisation of skin flaps after previous interruption of the blood supply. As far as I know this is the first report of a revascularised (anterolateral thigh) perforator flap.

Treatment of distal fingertip degloving injuries using a cross-finger flap based on the dorsal branch of the proper digital artery at the middle phalanx.

This study reports our experience with reconstruction of distal fingertip degloving injuries using a single cross-finger flap based on the dorsal branch of the proper digital artery at the middle phalanx. From January 2009 to October 2011, 18 patients (18 fingers) presented with distal fingertip degloving injuries and were treated with this technique. The mean size of the soft tissue defects was 4.5 cm in length and 2.0 cm in width. The mean size of the cross-finger flaps was 4.7 × 2.1 cm. In the series, all flaps survived completely. No complication was reported, and no further flap debunking

A case of degloving injury of the whole hand reconstructed by a combination of distant flaps comprising an anterolateral thigh flap and a groin flap.

A case of degloving injury of the whole hand reconstructed by a combination of an anterolateral thigh (ALT) flap and a groin flap applied as a pedicled distant flap was reported. Despite complications of congestion of the ALT flap and superficial infection, both flaps were severed at 4 weeks after transplantation, and a useful hand was finally reconstructed. The combination of a pedicled groin flap and an ALT flap is not optimal for reconstruction of a whole-hand degloving injury but is considered to be an available procedure for covering a large skin defect without microsurgical procedures.

Utilizing a crossover reverse sural artery flap for soft tissue reconstruction of the plantar forefoot after a severe degloving injury.

Extensive soft tissue defects involving the weight-bearing areas of the plantar aspect of the foot often require coverage with flaps. The options often include free flaps, free muscle flaps with split-thickness skin grafting, or local flaps. When presented with high-energy-induced soft tissue injuries of the foot, choices become narrow, secondary to the associated zone of injury. Free flaps require a viable recipient vessel suitable for microvascular anastomosis. Split-thickness skin grafts applied to the plantar aspect of the foot are prone to persistent breakdown. Local flaps if available ar

Tendon Turndown to Bridge a Tibialis Anterior Gap and Restore Active Dorsiflexion After Degloving Foot Injury in a Child: A Case Report.

CASE: A child with a severe degloving injury to the dorsum of the foot and ankle had segmental loss of the tibialis anterior tendon and toe extensors. A tendon turndown procedure was used, and the repaired tendon was covered with a latissimus muscle free flap to restore active ankle dorsiflexion. Details of the procedure are described, and active dorsiflexion and functional ambulation were restored. CONCLUSIONS: The tendon turndown method is a simple and effective way to overcome segmental tendon loss of tibialis anterior and allowed restoration of active dorsiflexion in this case without rel

Successful One-Step Skin Replantation After Degloving Peno-Scrotal Injury in an 8-Year-Old Boy: A Case Report.

BACKGROUND Perineal injuries affecting the scrotum and penis are rare in pediatric patients, owing to the protective anatomy of the male genitalia. However, when such injuries do occur, timely surgical intervention is crucial. This kind of damage might not be life-threatening but could cause functional disorders and have a huge impact on the patients' psychological condition if not treated appropriately, especially as they enter puberty. This case illustrates the successful management of a severe genital injury in a young child. CASE REPORT We present the case of an 8-year-old boy who sustaine

Management of a Severe Degloving Injury With a Type 2 Open Tibia Fracture Using Negative Pressure Wound Therapy With Instillation and Dwell Time.

INTRODUCTION: Severe lower extremity trauma cases are challenging for most orthopedic surgeons if a degloving injury with open fracture is associated, especially in the elderly population. The management of the soft tissue is essential for bone union and reduction of infection. CASE REPORT: The authors present the case of an 87-year-old female admitted to their department after a road accident in which she sustained an open fracture of the distal tibia classified as Gustilo-Anderson Type II, a closed fracture of the lateral malleoli, and a degloving of the posterior and lateral aspect of the

A young female patient with severe traumatic hemifacial degloving avulsion injury successfully managed with reconstruction and full thickness skin graft: case report.

INTRODUCTION AND IMPORTANCE: Road traffic accidents can result in severe ocular injuries, jeopardizing vision. High-speed collisions may lead to facial avulsion and degloving injuries, which pose risks such as airway compromise, bleeding, soft tissue loss, disfigurement, and post-traumatic impairments that necessitate prompt reconstruction. CASE PRESENTATION: A 24-year-old woman sustained injuries in a motorcycle accident, which resulted in a left hemifacial avulsion, a degloving injury, and the loss of her left brow. Additionally, she lost the lower eyelid on the left side and a portion of t

A Chimeric SCIP Flap With a Sartorius Muscle Component for Single-Stage Reconstruction of Late-Onset Navicular Osteomyelitis Following Degloving Injury: A Case Report.

In degloving injuries without associated fracture, osteomyelitis may be difficult to recognize, potentially delaying definitive treatment. We report a case and describe the role of a chimeric superficial circumflex iliac artery perforator (SCIP) flap with a sartorius muscle component for simultaneous dead-space obliteration and soft-tissue resurfacing. A 44-year-old man underwent free anterolateral thigh flap coverage 1 month after a medial ankle degloving injury. Two years later, a painless noninflammatory cystic lesion developed on the flap and recurred over 4 years despite three local debri

Degloving Traumatic Lesser Toe Injury Reconstruction with Full-Thickness Skin Graft from Partially Amputated Autologous Toe Donor Site: A Case Report.

Complex soft-tissue injuries consist of difficult traumatic injuries caused by high-energy mechanisms such as motor vehicle accidents, lawnmower injuries, and crush injuries from heavy objects. Many times, because of the high-energy trauma, there is significant damage to the soft tissue and underlying bone, leading to a complex situation for healing. In this case report, a 43-year-old woman presented with extensive degloving injury and open fractures of the forefoot resulting from a lawnmower accident. After extensive irrigation and debridement, wound closure was achieved using a full-thicknes

The use of Integra with a sensate fasciocutaneous pedicled flap for the salvage reconstruction of a below knee amputation after pedestrian vs train multi-planar degloving injury.

Degloving injuries occur when there is sufficient tangential force to a body surface to disrupt the structures connecting skin and subcutaneous tissues to the superficial fascia. There may also be associated injuries to the underlying soft tissues, bone, nerves and vessels. The injury may be so severe that the limb is non-viable and requires amputation. Our patient sustained a severe lower limb degloving injury after an accident where the leg was caught and dragged along the platform by a train. There was insufficient skin to cover a below knee amputation stump. So, the dermal regeneration tem
