Effect of internal jugular vein catheterization on intracranial pressure and postoperative cognitive function in patients undergoing robot-assisted laparoscopic surgery.

BACKGROUND: We aimed to evaluate the effects of internal jugular vein (IJV) catheterization on intracranial pressure (ICP) and postoperative delirium (POD) during robot-assisted laparoscopic surgery by measuring the optic nerve sheath diameter (ONSD). METHODS: Data from a prospective single-center cohort study, conducted from October 2021 to February 2022, were used. Forty out of 80 patients scheduled for laparoscopic radical hysterectomy or prostatectomy were assigned to the group receiving IJV catheterization (Group I), and the other 40 only received peripheral venous cannulation (Group C)

Use of cimetidine as an oral antacid in obstetric anesthesia.

The H2-receptor antagonist, cimetidine, was used instead of magnesium trisilicate BPC as routine antacid therapy before both elective and emergency obstetric anesthesia. Two trials of its efficacy in increasing intragastric pH and decreasing the volume of gastric contents in parturients are reported. In the first trial, 400 mg of cimetidine given orally to patients being delivered by elective cesarean section effectively decreased gastric acidity, providing induction of anesthesia occurred 90-150 min after its administration. Of 62 patients requiring emergency anesthesia during active labor an

Cerebral gas embolism in a patient without right-to-left shunt after robotic partial nephrectomy.

BACKGROUND: Cerebral gas embolism is a rare but serious complication of laparoscopic surgeries, the risk of which is increased by the presence of right-to-left shunt. A case of cerebral gas embolism after robotic partial nephrectomy is presented. CASE PRESENTATION: A 71-year-old man underwent robotic partial nephrectomy. During tumor resection, end-tidal CO₂ (ETCO₂) decreased from 42 to 34 mmHg, followed by a decrease in mean arterial pressure (MAP) to < 65 mmHg and oxygen saturation (SpO₂) to 95%. Postoperatively, he exhibited delayed emergence from anesthesia and left conjugate gaze deviati

Delayed Emergence Due to Severe Respiratory Acidosis Following Prolonged Spine Surgery: A Case Report.

Delayed emergence from anesthesia is a frequent challenge in surgical settings. Common etiologies include residual drug effects, metabolic derangements, or neurological insults. We report a case of a 55-year-old woman who underwent an elective six-hour lumbar laminectomy and cervical implantation surgery. Despite an uneventful intraoperative course and routine neuromuscular blockade reversal, the patient exhibited somnolence and ventilatory failure after reversal and extubation. Arterial blood gas (ABG) analysis revealed severe respiratory acidosis. The patient required re-intubation and contr

[Usefulness of endovascular treatment for delayed massive epistaxis following endoscopic endonasal transsphenoidal surgery: a case report].

We report here a case of massive nasal bleeding from the sphenopalatine artery three weeks after endonasal transsphenoidal surgery. This 66-year-old male suffered from massive nasal bleeding with the status of hypovolemic shock. Under general anesthesia, an emergent angiography revealed an extravasation from the sphenopalatine artery. Trans-arterial embolization using coil and n-butyl-cyanoacrylate (NBCA) was performed following the diagnostic angiography. Complete occlusion of the injured artery was achieved. The patient showed good recovery from general anesthesia. Delayed nasal bleeding aft

Effect of Timing of Intravenous Fentanyl Administration on the Incidence of Posttonsillectomy Nausea and Vomiting.

OBJECTIVE/HYPOTHESIS: Fentanyl is commonly administered toward the end of tonsillectomy to prevent emergence delirium and reduce postoperative pain. However, it can delay emergence from anesthesia and increase the risk of postoperative nausea and vomiting (PONV). The goal of our study was to compare the risk of PONV based on the timing of fentanyl administration at the end of tonsillectomy in children. STUDY DESIGN: Prospective, double-blind, randomized controlled trial. METHODS: One hundred forty patients aged 3 to 7 years undergoing tonsillectomy were divided into two groups. Fentanyl (1 μ

Prefrontal neurophysiological changes associated with subanesthetic esketamine accelerating mice emergence from propofol anesthesia.

BACKGROUND: Recent studies have demonstrated that subanesthetic dose of ketamine or its S-enantiomer, esketamine, can paradoxically accelerate the recovery of consciousness in rodents following general anesthesia. However, the neural mechanisms underlying this "awakening-promoting" effect remain poorly understood. METHODS: Adult C57BL/6 J mice were anesthetized with propofol, 0and a low dose of esketamine (2 mg/kg) was administered intravenously to assess its awakening effects through behavioral tests. In vivo multichannel electrophysiological recordings, calcium imaging, and two-photon imagi

Acute intracranial hematoma formation following excision of a cervical subdural tumor: a report of two cases and literature review.

An intracranial hematoma is a rare, yet significant, complication following spinal surgery. The authors describe two cases with acute intracranial hematoma formation after excision of a cervical subdural schwannoma. One was a 14-year-old girl who developed bilateral intracranial extradural hematomas immediately following excision of the C4 subdural schwannoma. The other was a 59-year-old woman who had an acute cerebellar hematoma after removal of the C2-C5 subdural schwannoma. During the surgeries of both cases, spinal dura was partially removed together with the tumor and the dural sac could

The role of ventral tegmental area dopamine neurons in emergence from dexmedetomidine, fentanyl, and ketamine anesthesia in rats.

BACKGROUND: Dopamine neurons in the ventral tegmental area (VTA), a midbrain structure, have been widely implicated in promoting emergence from general anesthesia. However, most studies have focused on anesthetics that primarily target GABAA receptors. Whether these neurons also facilitate emergence from mechanistically diverse drug-induced unconscious states is not known. Here we examine the specific contribution of VTA dopamine neurons in restoring arousal following sevoflurane-, dexmedetomidine-, fentanyl-, and ketamine-induced unconsciousness. METHODS: Adult Sprague Dawley rats received v

Mild hypercapnia with hyperventilation attenuates recovery from anesthesia in elderly patients.

PURPOSE: Mild hypercapnia with hyperventilation has been reported to significantly decrease recovery time from inhaled anesthesia in young and middle-aged patients. However, its efficacy has not yet been clarified in elderly patients, although delayed emergence can deteriorate their quality of recovery. METHODS: We enrolled 30 elderly patients (≥65 years) and 30 middle-aged patients (45-64 years) who were scheduled for ophthalmic surgery and allocated them to the control or the device group. Anesthesia was maintained with 1.5% sevoflurane. Mild hypercapnic hyperventilation was induced by the

Clinical Ethics Framework for Last-Minute Pediatric Refusal of Anesthesia Induction.

Induction of anesthesia is one of the most distressing moments for pediatric patients during their perioperative experience. They may consequently refuse induction at the last minute. Anesthesia providers are then placed in a challenging ethical dilemma: they must seek to gain the child's cooperation, force the child to undergo induction using restraint, or delay the case. In the moment, it can be difficult to parse the relevant clinical and ethical factors to determine the most appropriate course of action. The purpose of this perspective paper is to provide a clinical ethics framework to sup

Pseudocholinesterase Deficiency Considerations: A Case Study.

Pseudocholinesterase deficiency, sometimes called butyrylcholinesterase deficiency, is a rare disorder in which the neuromuscular blocking drugs succinylcholine and mivacurium cannot be metabolized properly in the blood plasma. This disorder can either be acquired as a result of certain comorbidities or it can be inherited genetically. Anesthesia providers must understand the pathophysiology of pseudocholinesterase deficiency and be prepared to safely and effectively manage patients who show signs and symptoms consistent with the disorder after the use of the indicated neuromuscular blocking d

Fournier's gangrene: A retrospective analysis of 26 cases in a Canadian hospital and literature review.

We performed a systematic review of all cases of Fournier's Gangrene (FG) at our hospital over a 12-year period. A total of 26 cases were assessed. Our goal was to determine the mortality rate and to identify risk factors associated with FG. We also wanted to examine three potential prognostic factors in relation to patient survival, including the time delay from emergency room admission to surgery, the initial extent of the disease, and the impact of more than one debridement procedure under general anesthesia. The time between emergency room admission and the beginning of surgical debridemen

Altered Behavioral Responses Show GABA Sensitivity in Muscleblind-Like 2-Deficient Mice: Implications for CNS Symptoms in Myotonic Dystrophy.

Considerable evidence from mouse models and human postmortem brain suggests loss of Muscleblind-like protein 2 (MBNL2) function in brain is a major driver of CNS symptoms in Myotonic dystrophy type 1 (DM1). Increased hypersomnia, fatigue, and surgical complications associated with general anesthesia suggest possible sensitivity to GABAergic inhibition in DM1. To test the hypothesis that MBNL2 depletion leads to behavioral sensitivity to GABAA receptor (GABAA-R) modulation, Mbnl2 knock-out (KO) and wild-type (WT) littermates were treated with the anesthetic sevoflurane, the benzodiazepine diaze

Bilateral pallidal deep brain stimulation for X-linked dystonia-parkinsonism.

BACKGROUND: X-linked dystonia-parkinsonism (XDP) is a progressively debilitating movement disorder that begins with focal dystonia and eventually generalizes. It exclusively affects Filipino inhabitants of the island of Panay. We report a case of XDP successfully treated by deep brain stimulation (DBS) and review the literature. METHODS: A 36-year-old man with XDP failed medical management and underwent bilateral globus pallidus internus DBS. A search of the PubMed database was performed to identify all articles discussing DBS and XDP. "Stimulation," "DYT3," "Lubag," "torsion dystonia," and "

Myocardial Ischemia-Associated Arterial Embolism Management from Pulmonary Cryotherapy.

Watchman devices are increasingly implanted in patients with atrial fibrillation who cannot be anticoagulated. We report a case of a patient undergoing Watchman device insertion complicated by cerebral arterial gas embolism. The treatment was delayed because the diagnosis was not initially considered. The patient was eventually treated with hyperbaric oxygen but sustained permanent disabilities. Watchman device implantation has a near 1% risk of arterial gas embolism. The possibility of cerebral gas embolism should be considered in patients emerging from anesthesia with neurologic deficits.
