BACKGROUND: Transient, acute delirium after electroconvulsive therapy (ECT) occurs in approximately one in eight patients. This case report describes a man with no known risk factors who developed a rare form of delirium, which is of longer duration and occurs longer after electroconvulsive therapy.
CASE PRESENTATION: A man in his forties with major depression with psychosis was admitted to the acute psychiatric ward after his sixth ECT treatment. He was disoriented, walked around restlessly, and seemed suspicious. He could not understand instructions and he ate his meals while wandering arou

Acute delirium is a transient state of cerebral dysfunction reflecting an underlying medical decompensation. Toxicity from medications and other substances are a common cause of delirium. History and laboratory testing may be limited by alteration and lack of specific tests for certain compounds. Classes of compounds produce a constellation of symptoms and examination findings recognized as a toxidrome. Cessation of the offending agent, supportive care, and specific antidotal therapy are key to treatment. This article reviews the presentations of the anticholinergic toxidrome, sympathomimetic

OBJECTIVE: To report a case of delirium, without major autonomic symptoms, as the primary manifestation of concomitant use of alcohol while taking disulfiram.
CASE SUMMARY: A 50-year-old white woman with a history of bipolar disorder, type I, and alcohol dependence being treated with disulfiram was admitted to an inpatient psychiatric unit with a three- to four-day history of a change in mental status, including deficits in orientation, concentration, and visual hallucinations. Significant finding on review of systems included the spurious report of a 9.1-kg weight loss. Tachycardia and nonfo

Protracted alcohol withdrawal delirium is infrequent, but when it occurs significant morbidity can be anticipated. In this report, a case of protracted alcohol withdrawal delirium is presented. The patient's course was complicated by intracerebral and subdural hemorrhages, sedative-hypnotic drug synergism resulting in respiratory compromise, the failure of benzodiazepines to prevent delirium or shorten its duration, and the onset of Wernicke's encephalopathy.

BACKGROUND: The association between remimazolam, a novel ultra-short-acting benzodiazepine, and the risk of postoperative delirium (POD) and emergence delirium (ED) remains controversial, particularly following prolonged infusion.
METHODS: PUBMED, EMBASE, WEB OF SCIENCE and the Cochrane Library electronic databases were searched up to December 10, 2025. The primary outcome was the incidence of delirium. Secondary outcomes included postoperative nausea and vomiting (PONV), respiratory depression after extubation, extubation time and length of hospital stay. Subgroup and meta-regression analyse

BACKGROUND: To date delirium prevalence in the adult acute Italian hospital cardiac population is unknown. In a multicenter study, we assessed the prevalence of delirium over a single day among a population of patients admitted to acute cardiac hospital wards in Italy.
METHODS: This is a point prevalence study (called "Cardio Delirium Day") which involved 55 Italian cardiologic centers (23 North, 18 Central, 12 South, 2 Sardinia) that collected data on 152 patients older than 65 years hospitalized on March 15, 2023. Delirium was assessed on the same day in all patients using the Confusion Ass

The current study aimed to screen for delirium in hospitalized older adults and assess the validity of the Turkish version of the 4A's Test (4AT-TR) as a feasible tool to integrate in routine patient care. The point prevalence of delirium according to clinical evaluation in routine practice was detected among all patients aged ≥60 years in 12 pilot wards. Delirium screening was then conducted by two arms: (a) nurses using the 4AT-TR and (b) geriatricians according to Diagnostic and Statistical Manual of Mental Disorders, 5th edition (DSM-5) criteria. Prevalence of delirium according to clinica

Delirium, a neuropsychiatric syndrome characterized by an acute and usually reversible state of confusion, while dementia is a chronic, acquired cognitive impairment that significantly reduces a patient's ability to perform daily tasks, learn, work, and engage in social interactions. Previous studies indicates that individuals with dementia are more susceptible to delirium than the general population, and that delirium serves as an independent risk factor for the subsequent onset of dementia. However, a major controversy in this field concerns whether delirium is merely a marker of vulnerabili

PURPOSE: Delirium remains underdiagnosed in hospitalized older patients. The Single Question in Delirium (SQiD) has emerged as a promising screening tool with a high negative predictive value in previous studies, but only a few have investigated the SQiD in geriatric patients. This study aimed to investigate the added value of combining the 4AT and SQiD for detecting delirium in hospitalized older patients. Secondary objectives included determining the point prevalence of delirium and healthcare professionals' awareness of delirium.
METHODS: On World Delirium Awareness Day 2024, all patients

METHODS: A cross-sectional study using delirium screening and falls reports was used to measure the association between delirium and falls. All inpatient data from August, 2018, to January, 2020, at a large academic medical center were analyzed. A multivariable logistic regression of 29,655 hospital admissions was used to understand the association between in-hospital delirium and falls.
RESULTS: Analysis revealed a delirium rate of 12.5% (n = 3,707) of all admissions and 286 (0.9%) admissions with falls; of the falls studied, 37.6% of these patients screened positive for delirium during thei

BACKGROUND: Neuroinflammation and neuronal injury have been reported to be associated with the development of postoperative delirium in both preclinical and clinical settings. This study aimed to investigate the potential correlation between biomarkers of neurofilament light chain and glial fibrillary acidic protein and emergence and postoperative delirium in elderly patients undergoing surgery.
METHODS: Patients who developed emergence delirium (n = 30) and postoperative delirium (n = 32), along with their matched controls, were enrolled after obtaining ethics approval and written informed c

30 patients with delirium tremens were given in a double-blind trial--beside the basic treatment with chlormethiazol (Distraneurin)--aprotinin (Trasylol) or placebo. Duration of the delirium and the amount of chlormethiazol used were the criteria for successful treatment. It was shown that the additional application of aprotinin did neither shorten significantly the duration of the delirium not save the amount of chlormethiazol used. Methodologically, special attention was given to the question of duration of the delirium and of registering symptoms. A delirium-rating scale was devised and its

INTRODUCTION: There is a paucity of data on the effect of preinjury substance (alcohol, drugs) abuse on the risk of delirium in patients with traumatic brain injury (TBI). This study aimed to assess the incidence of delirium among patients with blunt TBI in association with different substances.
METHODS: We analyzed the 2020 American College of Surgeons-Trauma Quality Improvement Program. We included all adult (≥18 y) patients with blunt TBI who had a recorded substance (drugs and alcohol) screening. Our primary outcome was the incidence of delirium.
RESULTS: A total of 72,901 blunt TBI pati

BACKGROUND: Since 2015, the American Delirium Society (ADS) Research Committee has conducted an annual survey of the delirium literature for presentation in its year-in-review session. Our objectives were to describe the review process used for the 2021-2022 and to summarise the selected publications.
METHODS: Each member of the ADS Research Committee nominated up to 6 publications considered to be the most impactful primary delirium research published from September 1, 2021, to July 31, 2022. The 24 nominated studies were divided into three categories balanced by number of articles: medical

BACKGROUND: Fluctuating course of delirium and complexities of ICU care mean delirium symptoms are hard to identify or commonly confused with other disorders. Delirium is difficult to diagnose, and clinicians and researchers may combine assessments from multiple tools. We evaluated diagnostic accuracy of different combinations of delirium assessments performed in each enrolled patient.
METHODS: Data were obtained from a previously conducted cross-sectional study. Eligible adult patients who remained admitted to ICU for >24 hours with at least one family member present were consecutively enrol

BACKGROUND: A crucial part of delirium care is determining if the delirium episode has resolved. Yet, there is no clear evidence or consensus on which assessments clinicians should use to assess for delirium recovery.
OBJECTIVE: To evaluate current opinions from delirium specialists on assessment of delirium recovery.
DESIGN: Online questionnaire-based survey distributed internationally to healthcare professionals involved in delirium care.
METHODS: The survey covered methods for assessing recovery, the importance of different symptom domains for capturing recovery, and local guidance or pa
