Pharmacological Interventions for Primary Psychodermatologic Disorders: An Evidence Mapping and Appraisal of Randomized Controlled Trials.

BACKGROUND: The lack of clinical guidelines for the treatment of primary psychodermatologic disorders (PPDs) hinders the delivery of optimal care to patients. The review aimed to identify, appraise, and summarize the currently available evidence about the safety and effectiveness of pharmacological management of PPDs through randomized controlled trials (RCTs). METHODS: The Preferred Reporting Items for Systematic Review and Meta-Analyses (PRIMSA) statement and the Global Evidence Mapping Initiative guidance were followed. Medline, Embase, PsycInfo, Cochrane and Scopus were searched, and two

[Induced "acarophobia"].

Although only ablut 240 cases of 'acarophobia' are on record in zoological and medical literature, it can be seen that this delusional syndrome without doubt leads to psychoses of association more frequently than any other mental disturbance. The literature contains many references, and the author can give two examples from personal esperience. At least every sixth patient suffering from delusions of parasitosis 'infects' relations. This really remarkable tendency to spread by psychological contagion on one or more dependent persons has been ignored by many writers. The supposition that such o

Managing the patient with psychiatric issues in dermatologic practice.

Patients often communicate emotions through their bodies and physical symptoms; the skin commonly serves as a means of expression in the patient-doctor relationship. It is important for the dermatologist to be able to indentify psychological issues that manifest in the skin and the interplay between psychiatric and dermatologic conditions. Delusional parasitosis, dermatitis artefacta, trichotillomania, and somatoform disorders all represent dermatologic conditions with underlying emotional causes. Many chronic dermatoses, such as psoriasis, atopic dermatitis, and acne, modulate and are influen

Delusional Infestation, Psychiatric Comorbidity, and Dementia: A Review of 146 Swedish Patients.

Delusional infestation is a rare but severe dermatopsychiatric disorder. The diagnosis is associated with great suffering for the patients and their relatives, as well as substantial use of healthcare resources. It has been observed that patients with delusional infestation often have psychiatric comorbid conditions. Neurodegenerative diseases like dementia have been reported to be associated, but only in a few case reports. An observational, retrospective search was conducted in the institutional database and all medical records of patients with a diagnostic ICD code in the range F40.0-F40.9

Drug-Induced Tactile Hallucinations Beyond Recreational Drugs.

In monosymptomatic hypochondriacal psychosis (MHP), such as delusional infestation (DI), the patient has a fixed, false, encapsulated belief associated with tactile hallucinations (TH), most commonly formication, which is defined as cutaneous sensations of crawling, stinging, biting, etc., without evidence of infestation. Drug-induced TH should be considered in patients with suspected MHP. Although recreational drugs such as cocaine, amphetamines, and narcotics are well known to induce TH, many busy practicing dermatologists may not be familiar with other types of medications that can induce T

Abnormal gray and white matter volume in delusional infestation.

Little is known about the neural basis of delusional infestation (DI), the delusional belief to be infested with pathogens. Case series and the response to anti-dopaminergic medication indicate disruptions in dopaminergic neurotransmission in the striatum (caudate, putamen), but did not allow for population-based inference. Here, we report the first whole-brain structural neuroimaging study to investigate gray and white matter abnormalities in DI compared to controls. In this study, we used structural magnetic resonance imaging and voxel-based morphometry to investigate gray and white matter v

[Psychotropics in dermatology].

In dermatology, psychotropics are sometimes useful. But it is not still popular in clinical dermatology. It is recommended to use them for the patients who have psychosomatic aspect, anxiety and depression due to skin disease. Psychotropics are also useful to alleviate the sensation of itch. Trichotillomania and excess scratch are treated with antidepressants, anticonvulsants or antipsychotics. Cutaneous dysesthesia and delusional parasitosis are treated with antipsychotics. In prescribing benzodiazepines and hypnotics, we have to check abuse and dependency. Decrease of dosage should be made g

Successful treatment of patients previously labeled as having "delusions of parasitosis" with antidepressant therapy.

Delusions of parasitosis (DOP) is a somatic subtype of delusional disorder, also known as monosymptomatic hypochondriacal psychosis. The management of DOP has been discussed extensively in the medical literature. Patients with suspected DOP have a broad differential diagnosis, including skin-based or systemic medical conditions and several kinds of psychiatric disease. However, there are many patients who have been labeled with DOP but do not meet the diagnostic criteria or who present with additional somatic complaints. These cases are a unique therapeutic challenge for dermatologists and psy

Morgellons-like Disease of the Scalp.

Morgellons disease remains a poorly understood condition, while the general medical consensus is that it is a form of delusional parasitosis in which individuals have sores they believe contain fibers. In fact, Morgellons disease may represent a culture-bound syndrome, i.e., a combination of psychiatric and somatic symptoms that are considered to be a recognizable disease within a specific society or culture. We present a case of Morgellons-like disease of the scalp and discuss its relationship to psychopathology and specific features of the scalp microenvironment. The propositus suffered from

A patient with delusional infestation by proxy: Issues for vulnerable adults.

A 57-year-old Caucasian female presented to our clinic with her 23-year-old son, who was profoundly autistic. Our patient was convinced that her son had an infestation with fibers and believed that he had developed the condition as a young child. She described the symptoms of the infestation in her son's skin on his behalf, as he was not able to communicate. She identified dietary factors as a causative factor and wanted them removed from his diet. Her son had seborrhoeic dermatitis on examination, with no evidence of an infestation. Our patient later identified that fibers were coming out of

Brexpiprazole for a Patient with Ekbom Syndrome with Intolerable Side Effect of Aripiprazole.

Ekbom's syndrome (ES), also referred to as delusional parasitosis, is a psychiatric disorder that is relatively uncommon. It is characterized by a robust and unwavering belief in infestation despite the absence of predominant hallucinations or formal thought disorders. Ekbom syndrome presents in 2 main forms: primary, marked by delusions and abnormal tactile sensations; and secondary, where symptoms arise due to another underlying organic condition. In primary ES, the therapeutic approach primarily relies on antipsychotic (AP) medications. This article presents a case with primary ES showing t

Collaboration between Psychiatrists and Other Allied Medical Specialists for the Treatment of Delusional Disorders.

BACKGROUND: There is increasing evidence that individuals with psychosis are at increased risk for cardiovascular disease, diabetes, metabolic syndrome, and several other medical comorbidities. In delusional disorder (DD), this is particularly so because of the relatively late onset age. AIMS: The aim of this narrative review is to synthesize the literature on the necessity for medical collaboration between psychiatrists and other specialists. METHODS: A non-systematic narrative review was carried out of papers addressing referrals and cooperation among specialists in the care of DD patients

A retrospective review of 12 patients with trichotillomania treated in a psychodermatology service.

Trichotillomania is a condition characterized by the pulling of hair from anywhere on the body and is classified as an obsessive-compulsive and related disorder. Patients with hair disorders are commonly referred to psychodermatology services, and can represent a management challenge. Few publications exist that report outcomes for patients with trichotillomania in real clinical practice. We report 12 such patients seen within our own psychodermatology service, who were managed using a variety of treatment strategies. The rate of defaulting of appointments was high, but improvements were seen

Formication with destruction of the nasal septum: A rare case report.

Delusions of parasitosis are known by several names, one of which is formication. It is an uncommon psychiatric disorder listed in the DSM V under the classification of delusional disorders. Patients experience an established, incorrect belief that they have an infection of living organisms such as parasites. There are two types of delusions of parasitosis, primary and secondary. In the primary form, the symptom is the delusion of a parasitic infection without secondary cause. The secondary form includes this symptom as well as additional psychiatric disorder, drug abuse, or medical illness. P

[A peek inside the field of psychodermatology].

Skin disorders are one of the most prevalent conditions observed in general practice, and they are often accompanied by other physical conditions. At dermatological clinics, increasing attention is being paid to psychosocial problems in patients with chronic skin disorders such as psoriasis and eczema or skin-related oncology problems. Psychological adjustment problems, severe itch-scratch problems, shame, social fear, or low therapy adherence are frequent reasons for a referral to psychologists trained in dermatological problems and other psychosocial health professionals. In addition, the tr

Patients labeled with delusions of parasitosis compose a heterogenous group: a retrospective study from a referral center.

BACKGROUND: There are few diagnostic tools available to the dermatologist to help in the diagnosis of patients with delusions of parasitosis (DOP). OBJECTIVE: We sought to find differences in presentation and clinical course between patients who received a final diagnosis of DOP and those who received a final diagnosis of a primary medical condition or other psychiatric disorder. METHODS: We performed a retrospective chart review of patients referred with a diagnosis of DOP. Each patient received a final consensus diagnosis. RESULTS: In all, 47 patients were included in the study. Patients

Exploring the association between Morgellons disease and Lyme disease: identification of Borrelia burgdorferi in Morgellons disease patients.

BACKGROUND: Morgellons disease (MD) is a complex skin disorder characterized by ulcerating lesions that have protruding or embedded filaments. Many clinicians refer to this condition as delusional parasitosis or delusional infestation and consider the filaments to be introduced textile fibers. In contrast, recent studies indicate that MD is a true somatic illness associated with tickborne infection, that the filaments are keratin and collagen in composition and that they result from proliferation and activation of keratinocytes and fibroblasts in the skin. Previously, spirochetes have been det

Delusional infestation managed in a combined tropical medicine and psychiatry clinic.

BACKGROUND: Delusional infestation (DI) is a well-recognised delusional disorder presenting as the persisting belief in the presence of parasitic or other infestations. Combined clinics have been run by dermatology and psychiatry in a small number of centres. Here we report the first few years of a unique combined clinic run with experts in infectious diseases/tropical medicine and psychiatric management of DI. METHODS: We reviewed all patients seen at the combined assessment clinics run at the Liverpool School of Tropical Medicine between 19 December 2011 and 31 October 2016. Data were colle

History of Morgellons disease: the same name for different psychodermatologic diseases?

"Morgellons disease" has been a controversial topic in the history of psychodermatology. The most consensual scientific opinion is that it is a primary psychiatric disorder, particularly, a delusional disorder, although others were also pointed out. Some authors have suggested that it may correspond to a common dermatosis with secondary psychopathology. The Morgellons Research Foundation has advocated that it is "an emerging infectious" entity. This paper intends to critically review the main ideas and controversies, since its first description.
