Dementia of different etiologies: vasoactive intestinal polypeptide in CSF.

Patients with normal-pressure hydrocephalus, multi-infarct dementia, and recent cerebral infarction had significantly lower vasoactive intestinal polypeptide (VIP) levels than age-matched controls (11 +/- 3 pmol/l, 17 +/- 4 pmol/l, 21 +/- 4 pmol/l, and 33 +/- 4 pmol/l, respectively). Three months after a shunt operation, the VIP levels had increased significantly in patients with hydrocephalus (54 +/- 13 pmol/l). VIP concentration in patients with senile dementia did not differ from that of controls.

Modern concept of vascular cognitive impairment.

BACKGROUND: Vascular cognitive impairment (VCI) has superseded vascular dementia and multi-infarct dementia as the concept to be used in cognitive decline due to cerebrovascular disease. METHOD: The literature was reviewed with regard to the concept of VCI and its incidence, pathophysiological substrate, clinical features and management. RESULTS: A change in the diagnostic paradigm from the current Alzheimer-based definition of vascular dementia to VCI will allow the earlier identification of cases and will identify a different population from that recognized using the current criteria for v

Cerebral blood flow and metabolism in aging and dementia.

There are structural, perfusive, and metabolic changes that occur in normal aging and in dementia. In Alzheimer's disease (AD), changes in blood flow and metabolism occur primarily in the parietal and temporal cortices. These changes are distinct from those seen in normal aging. These alterations correspond to known neuronal changes that occur in AD and are consonant with changes that are seen in cognitive function. Characteristic patterns are also emerging in other forms of dementia such as Pick's disease, multi-infarct dementia, and primary progressive aphasia. Finally, there is some prelimi

Diagnosis, differential diagnosis and nosologic classification of the dementia syndrome.

Some clinical aspects of the diagnosis, differential diagnosis, and the nosologic classification of the dementia syndrome are presented. In discussing the definition of the dementia syndrome and its differentation from other organic brain syndromes, the criteria of DSM III are taken into account. Considering the nosological classification of the dementia syndrome particular emphasis is placed on the most frequent diseases in the senium, the Multi-Infarct Dementias (MID) and the primary degenerative dementias (PDD, according to DSM III). The clinical problems in differentiating MID from PDD and

Dementia secondary to metabolic and nutritional abnormalities.

It is clear that dementia may result from nutritional and metabolic causes. Prior to labeling the demented person as having Alzheimer's disease or multi-infarct dementia, a thorough physical, historic, and laboratory evaluation is essential. Not only must other causes for the dementia be ruled out, but co-existing abnormalities that may confuse therapeutic intervention must be considered. Elderly persons notoriously under-report symptoms, in this case further complicated by the dementia. In addition, normal age-related changes, age-prevalent illness, and the atypical way in which diseases ofte

Indirect bright light improves circadian rest-activity rhythm disturbances in demented patients.

Light is known to be an important modulator of circadian rhythms. We tested the hypothesis than an enduring increase in the daytime environmental illumination level improves rest-activity rhythm disturbances in demented patients. Actigraphy was performed before, during, and after 4 weeks of increased illumination in the living rooms of 22 patients with dementia clinically diagnosed as probable Alzheimer's disease, multi-infarct dementia, dementia associated with alcoholism, or normal pressure hydrocephalus. The results indicated that during increased illumination, the stability of the rest-act

[A case of CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and lekoencephalopathy) with Notch 3 (Arg169Cys) mutation and typical granular osmiophilic materials in peripheral small arteries].

We report a 64-year-old Japanese woman with recurrent ischemic strokes and progressive dementia without any cardiovascular risk factors. Her first stroke was at 45 years old, and she has a family history of ischemic strokes compatible with an autosomal dominant trait. Marked leukoaraiosis and multiple lacunar infarcts were shown on brain MR images, and no atherosclerotic changes were observed in her extra- and intra-cranial arteries by cervical arterial echography and intracranial MR angiography. Excluded other inherited or metabolic diseases causing leukodystrophy by examination of her blood

Functional rating scales and psychometric assessment in Alzheimer's disease: applications in pharmacological trials.

New findings on cognitive aging show that decline in brain functions is not part of a general decline of functioning in old age. We know now that with increasing age there is a differentiation of cognitive functions into two independent dimensions (fluid and crystallized functions). This two-dimensional model allows a more precise description of the cognitive aging process and of pathological changes. While crystallized cognitive abilities can be improved by training till late in life, fluid cognitive functions are subject to a progressive decrease starting as early as age 30, becoming most ev

Should computed tomography appearance of lacunar stroke influence patient management?

Patients with a lacunar stroke syndrome may have cortical infarcts on brain imaging rather than lacunar infarcts, and patients with the clinical features of a small cortical stroke (partial anterior circulation syndrome, PACS) may have lacunar infarcts on imaging. The aim was to compare risk factors and outcome in lacunar syndrome (LACS) with cortical infarct, LACS with lacunar infarct, PACS with cortical infarct, and PACS with lacunar infarct to determine whether the clinical syndrome should be modified according to brain imaging. As part of a hospital stroke registry, patients with first eve

[Lacunar brain infarction in patients with pseudoxanthoma elasticum].

Brain infarctions in patients with pseudoxanthoma elasticum (PXE) are thought to be associated with the damage of cerebral arteries injured by connective tissue change caused by PXE. We experienced 3 cases of PXE with lacunar brain infarction and evaluated the relationship between the damage of small penetrating arteries and PXE. All patients' diagnosis of PXE were confirmed by histological examination. They were a 47-year-old man and two women with 54 and 62 years of age old. All patients had pseudoxanthoma and angioid streaks and showed pyramidal tract signs. In addition, one patient present

[Problems in CT diagnosis of the aging brain].

The different methods of measuring the intracranial CSF spaces on CT images are described. The values obtained are demonstrated to separate the normal aging brain from the brain in senile dementia of Alzheimer's type. The CT criteria for the diagnosis of multi-infarct dementia are shown. The significance of CT studies in senile depression is discussed. The problem of vascular encephalopathy (leuko-araiosis) in normal aging of the brain and in dementia is considered in particular, and even the occurrence of intracranial space-occupying lesions and normal pressure hydrocephalus, as treatable cau

The ICD-10 criteria for vascular dementia.

The World Health Organization recently introduced the new ICD-10. Chapter V ('Mental and behavioural disorders including disorders of psychological development') contains operationalized diagnostic guidelines for the classification of psychiatric disorders, i.e. the term vascular dementia is clearly defined. Furthermore, the criteria of the ICD-10 allow a differentiation of vascular dementia into subtypes (vascular dementia of acute onset, multi-infarct dementia, subcortical vascular dementia, and mixed or unspecified types). The clinical feasibility of the ICD-10 criteria for subtypes of vasc

[Hypertrophic cardiomyopathy: a rare cause of vascular dementia. A case report].

Herein, we report a case of a 51 year old man who experienced three ischemic cerebral infarcts in a time of few months. The patient consulted after the third accident. Neurological presentation included pseudobulbar syndrome with a mild cognitive deficit, aphasia, left hemiparesia, hemiasomatognosia and homonymous lateral hemianopsia. Cerebral tomodensitometry and magnetic resonance imaging evidenced large infarcts images involving right middle cerebral artery territory and bilateral borderline zones in the junction of the territories of the middle and posterior cerebral arteries. Subcortical arteriosclerotic encephalopathy (Binswanger's disease).

Subcortical arteriosclerotic encephalopathy is a chronic vascular dementia with hydrocephalus characterized clinically by: (i) subacute focal neurological deficit; (ii) acute strokes; (iii) dementia; (iv) motor signs and pseudobulbar palsy; (v) hydrocephalus; (vi) persistent hypertension and systemic vascular disease; and (vii) a lengthy course. The pathogenesis is most probably ischaemic change related to subacute hypertensive encephalopathy. The pathological changes include severe central nervous system disease characterized by loss of white matter with gliosis, and arterial and arteriolar s

The pathology of ischemic-vascular dementia: an update.

In Western memory clinic-based series, ischemic-vascular dementia (IVD) is seen in 8-10% of cognitively impaired elderly subjects. Its prevalence in autopsy series ranges from 0.03% to 58% with reasonable values of 4-10%, while in Japan, IVD is seen in 22-35% and mixed-type dementia (MTD) (Alzheimer disease/AD+IVD) in 6-11%. In a large Viennese autopsy series, "pure" IVD was observed in 9.4% of demented elderly and in 2.9% of those clinically diagnosed as possible/probable AD MTD was observed in 3.1% and 1.3% respectively. The major morphological types of IVD are multi-infarct encephalopathy (

Imaging of vascular cognitive impairment.

Vascular cognitive impairment (VCI) is a major health challenge and represents the second most common cause of dementia. We review the updated imaging classification and imaging findings of different subtypes of VCI. We will focus on the magnetic resonance imaging (MRI) markers of each subtype and highlight the role of advanced MR imaging sequences in the evaluation of these patients. Small vessel dementia appears as white matter hyperintensity, lacunae, microinfarcts, and microbleeds. Large vessel dementia includes strategic infarction and multi-infarction dementias. Hypoperfusion dementia ca

Correlates of delayed referral for the diagnosis of dementia in an outpatient population.

BACKGROUND: Early diagnosis of dementia is critical, but there is usually a time lag between onset of symptoms and referral for neuropsychological testing and dementia diagnosis. We aimed to identify factors correlated with this delayed referral. METHODS: We studied 140 patients with cognitive deterioration referred to the Memory Clinic of the Catholic University (Rome) between 1995 and 1996. Alzheimer's disease or multi-infarct dementia was diagnosed according to National Institute of Neurological and Communicative Disorders and Stroke-Alzheimer's Disease and Related Disorders Association (N

The prevalence of dementia and depression in the elderly community in a southern European population. The Zaragoza study.

BACKGROUND: Provocative international disparities reported in the prevalence rates of dementia and depression require further investigation. This is the first psychiatric study, to the best of our knowledge, about the prevalence of DSM-III-R dementing and depressive disorders and their relationships in a representative, stratified community sample of the elderly in both a Spanish-speaking country and southern Europe. METHODS: A two-stage screening was completed in 1080 elderly. Sampling with replacement was done, and the cumulative response rate was 88%. In phase 1, lay interviewers administe

Glucose metabolism was measured by F-18 FDG PET in 33 patients with dementia. In patients with DAT, rCMRgl showed marked decrease in the associated areas in the temporo-parietal lobes, and it was relatively preserved in the primary cortices such as sensory, motor and visual areas. Frontal lobes were also involved in patients with advanced disease. In Pick disease, the frontal lobes were severely impaired. Multi-infarct dementia, Parkinson dementia and supranuclear palsy showed diffuse or multiple involvement throughout the entire brain. Correlation with rCMRgl and clinical severity was found i
