Elastin-like polypeptide induced biomimetic mineralization for dentin hypersensitivity.

Dental hypersensitivity, a prevalent oral health issue, is closely associated with the exposure of dentinal tubules. Biomimetic mineralization has emerged as a promising approach to occlude these tubules and mitigate sensitivity. This study explores the potential of elastin-like polypeptide (ELP), a synthetic polypeptide, in dentinal tubule occlusion and its efficacy in reducing dental hypersensitivity. Building on previous findings that ELP can promote enamel regeneration, we extended its application to dentin repair. The structure of ELP was verified by time-of-flight mass spectrometry (TOF-

A silk fibroin-based hydrogel desensitizer achieving 660 μm dentin tubule occlusion for dentin hypersensitivity treatment.

Dentin hypersensitivity (DH) causes sharp pain due to external stimuli transmitted through dentinal tubules (DTs) in exposed dentin. Current desensitizers struggle to provide sufficient DT occlusion depth and density, limiting long-term effectiveness under daily erosion and friction. Inspired by fish schooling behavior, we introduce an adaptive hydrogel desensitizer (STU) composed of "sardines" (tannic acid and silk fibroin) and "predators" (urea). The "sardines" form a tightly packed hydrogel "shoal" through hydrogen bonds, while the "predators" disrupt these bonds, transforming STU into a lo

Pre-treatment of dentin with chondroitin sulfate or L-arginine modulates dentin tubule occlusion by toothpaste components.

PURPOSE: To evaluate the effect on dentin of chondroitin sulfate and L-arginine on dentin tubule occlusion. METHODS: The dentin samples were activated by submersion in an aqueous ( aq. ) solution of chondroitin sulfate ( ChS) or L-arginine prior to application of a commercial or custom-made toothpaste. After rinsing with water and ultrasonication, adhesion to dentin and occlusion of dentin tubules were evaluated by scanning electron microscopy and the elemental composition of the deposits was evaluated by energy-dispersive x-ray spectroscopy. RESULTS: Rinsing a dentin sample with a solution

Longevity of different in-office treatments for dentin hypersensitivity: A 6-month randomized and parallel clinical trial.

This longitudinal, randomized, parallel-design clinical trial aimed to evaluate the long-term effectiveness of different in-office treatments for dentin hypersensitivity (DH) over a 6-month period. A total of 192 teeth presenting DH associated with root surface exposure were treated with one of four desensitizing agents: fluoride varnish (Duraphat - FLU, active control), bioactive ceramic solution (Biosilicate - BIOS), universal self-etching adhesive (Single Bond Universal - SBU), and a bioactive, photoactivated varnish containing PRG fillers (SPRG). DH was assessed using a visual analogue sca

Biomimetic Dentin Repair: Amelogenin-Derived Peptide Guides Occlusion and Peritubular Mineralization of Human Teeth.

Exposure of dentin tubules due to loss of protective enamel (crown) and cementum (root) tissues as a result of erosion, mechanical wear, gingival recession, etc. has been the leading causes of dentin hypersensitivity. Despite being a widespread ailment, no permanent solution exists to address this oral condition. Current treatments are designed to alleviate the pain by either using desensitizers or blocking dentin tubules by deposition of minerals or solid precipitates, which often have short-lived effects. Reproducing an integrated mineral layer that occludes exposed dentin with concomitant p

Effect of bioactive glasses used as dentin desensitizers on the dentin-pulp complex in rats.

Bioactive glasses have been recommended for the occlusion of dentinal tubules in treating cervical dentin hypersensitivity. This study evaluates an in vivo model of dentin exposure, and tests the efficacy of bioglass treatments. Thirty male Wistar rats received gingival recession surgery on the upper left first molar. The treatments were applied over the surface of the exposed dentin every 4 days for 28 days. The groups were as follows: Naive; Gingival recession; Cavity varnish; Biosilicate®; Strontium bioglass; and Potassium bioglass. Changes in the dentin-pulp complex, and the presence of su

Molecular and neurovascular mechanisms of thermal sensitivity in teeth.

The perception of dental pain and thermal stimuli is governed by the specialized structural and physiological organization of the dentin - pulp complex. Teeth are subjected to rapid thermal changes during normal oral functions, while the dental pulp - an intensely vascularized and innervated tissue - is confined within rigid mineralized structures. Under these anatomical conditions, even mild thermal stimuli - especially cold - can induce rapid and intense pain sensations. The mechanisms underlying thermal sensitivity of dentin have been explained by several theories. The direct neural theory

In vitro study on sealing dentinal tubules of children by combining EDTA gel with Nd: YAG laser.

OBJECTIVE: Dentin hypersensitivity is a common oral discomfort in children, mainly caused by exposed dentinal tubules responding to thermal, mechanical, and chemical stimuli. Effective sealing of dentinal tubules is crucial for alleviating sensitivity. This study aimed to evaluate, in vitro, the effect of EDTA gel combined with Nd: YAG laser on sealing dentinal tubules in children’s teeth. METHODS: Thirty extracted healthy premolars and molars from children were prepared into standardized dentin specimens. All samples were pretreated with 17% EDTA solution for 5 min to simulate tubule exposure

Consensus-based recommendations for the diagnosis and management of dentin hypersensitivity.

These consensus recommendations for the diagnosis and management of dentin hypersensitivity were developed by a broadly constituted board of dentists and dental hygienists drawn from general dental practice, specialist practice, academia and research from across Canada, joined by 2 international dentists with subject matter expertise. The need for consensus recommendations was made evident by the lack of clear and robust evidence in the dental literature, as well as confusion about diagnosis and management demonstrated by an educational needs assessment survey. High prevalence of the condition

Exploration of the covariation signal between cortical bone and dentine volumes across the upper limb bones and anterior teeth in modern humans and relevance to evolutionary anthropology.

Cortical bone and dentine are two mineralized tissues sharing a common embryological origin, developmental, and genetic background, distinct from those of enamel. Understanding their relationship is crucial to decipher the factors acting on their postnatal development, and shedding light on the evolutionary patterns of tissue proportions. Here, we investigate the coordinated variation between cortical bone and dentine volumes measured from arm and forearm bones (humeri, ulnae, radii) and upper anterior teeth (central incisors, lateral incisors, canines) of modern humans. Given the shared chara

Topical review. Dental pain and odontoblasts: facts and hypotheses.

Dental pain arises from exposed dentin following bacterial, chemical, or mechanical erosion of enamel and/or recession of gingiva. Thus, dentin tissue and more specifically patent dentinal tubules represent the first structure involved in dentin sensitivity. Interestingly, the architecture of dentin could allow for the transfer of information to the underlying dental pulp via odontoblasts (dentin-forming cells), via their apical extension bathed in the dentinal fluid running in the tubules, or via a dense network of trigeminal sensory axons intimately related to odontoblasts. Therefore, extern

Dentin hypersensitivity: pain mechanisms and aetiology of exposed cervical dentin.

OBJECTIVES: The paper's aim is to review dentin hypersensitivity (DHS), discussing pain mechanisms and aetiology. MATERIALS AND METHODS: Literature was reviewed using search engines with MESH terms, DH pain mechanisms and aetiology (including abrasion, erosion and periodontal disease). RESULTS: The many hypotheses proposed for DHS attest to our lack of knowledge in understanding neurophysiologic mechanisms, the most widely accepted being the hydrodynamic theory. Dentin tubules must be patent from the oral environment to the pulp. Dentin exposure, usually at the cervical margin, is due to a v

Comparison of quantitative light-induced fluorescence and near-infrared intraoral scanner imaging for proximal caries across predefined ICCMS radiographic thresholds: a diagnostic accuracy study.

BACKGROUND: Proximal caries detection remains challenging because early lesions are often inaccessible to visual inspection and bitewing radiography exposes patients to ionizing radiation. Quantitative light-induced fluorescence (QLF) and near-infrared imaging integrated into intraoral scanners are proposed as non-ionizing adjuncts. This study compared the diagnostic performance of a handheld QLF device (QrayPen C, Aiobio) and an intraoral scanner with near-infrared imaging (NIRI) (iTero Element 5D, Align Technology) for proximal caries across clinically relevant ICCMS radiographic thresholds.
