Dexmedetomidine for the treatment of paroxysmal autonomic instability with dystonia.

INTRODUCTION: A 38-year-old man with severe head trauma complicated by paroxysmal severe intracranial pressure elevation associated with tachypnea, tachycardia, diaphoresis, and extensor posturing was diagnosed as suffering from paroxysmal autonomic instability with dystonia (PAID). These events were unresponsive to standard medical therapy, which included morphine, fentanyl, labetalol, lorazepam, metoprolol, and clonidine. METHODS: A trial treatment with dexmedetomidine, a central acting alpha2-agonist, to control symptoms of PAID was initiated 12 days after injury. PAID-related events subsi

Hypothermia-induced bradycardia in a neonate receiving dexmedetomidine.

Dexmedetomidine, an alpha(2)-adrenergic agonist, decreases lipolysis and thus prevents nonshivering thermogenesis in infants. We report the case of a two-day-old neonate who received dexmedetomidine for sedation and analgesia after undergoing primary repair of bladder exstrophy. After 9 hours, the patient developed bradycardia and profound hypothermia. The bradycardia was unresponsive to anticholinergics but resolved approximately two hours after radiant heat was applied. This report highlights the potential profound impact of dexmedetomidine on thermoregulation in neonates. Dexmedetomidine im

The in vitro effects of clonidine and dexmedetomidine on human myometrium.

BACKGROUND: alpha(2)-adrenergic agonists have been used extensively in the field of anaesthesia. Their direct effect on the human myometrium was investigated in this in vitro study, as this may have clinical repercussions in obstetric anaesthesia. METHOD: Strips of pregnant human myometrium obtained from six individuals at elective caesarean section were mounted on the Mulvany myograph in Krebs solution to which increasing concentrations of clonidine and dexmedetomidine (1x10(-11) to 1x10(-6) g/mL) were added. RESULTS: Dexmedetomidine increased uterine contractility at simulated clinical pla

Cardiopulmonary effects and pharmacokinetics of i.v. dexmedetomidine in ponies.

REASONS FOR PERFORMING STUDY: Currently available sedatives depress cardiopulmonary function considerably; therefore, it is important to search for new, less depressive sedatives. The study was performed to assess duration and intensity of cardiopulmonary side effects of a new sedative, dexmedetomidine (DEX), in horses. OBJECTIVES: To study pharmacokinetics and cardiopulmonary effects of i.v. DEX. METHODS: Pharmacokinetics of 3.5 microg/kg bwt i.v. DEX were studied in a group of 8 mature (mean age 4.4 years) and 6 old ponies (mean age 20 years). Cardiopulmonary data were recorded in mature p

Dexmedetomidine increases the cocaine seizure threshold in rats.

BACKGROUND: Central alpha adrenoceptors have been demonstrated to play an important role in the control of seizure activity; moreover, alpha adrenoceptors have been linked to electroencephalogram changes associated with cocaine. The purpose of this study was to determine if dexmedetomidine, a highly selective alpha -adrenoceptor agonist, alters the threshold for cocaine-induced seizure activity in rats. METHODS: Sprague-Dawley rats received a cocaine infusion (1.25 mg x kg(-1) x min(-1)) followed 15 min later by the coinfusion of either dexmedetomidine (20-microg/kg intravenous bolus followed

Dexmedetomidine alleviates ethanol withdrawal symptoms in the rat.

The effect of dexmedetomidine, a selective alpha 2-adrenoceptor agonist, on ethanol withdrawal symptoms was studied in chronically ethanol-fed rats. After a 4-day ethanol intoxication period the rats were given s.c. injections of dexmedetomidine (3, 10, or 30 micrograms/kg) or saline (control group) at 10, 16, 22, and 39 h after the last dose of ethanol. The severity of ethanol withdrawal symptoms (rigidity, tremor, irritability, hypoactivity) was rated up to 58 h, blind to the treatments. The results showed that dexmedetomidine at doses 10 and 30 micrograms/kg significantly diminished the sev

The effect of dexmedetomidine on nutrient organ blood flow.

The alpha 2-adrenergic agonist dexmedetomidine decreases not only heart rate, myocardial contractility, and oxygen demand, but also cardiac output (Q). To investigate whether this reduction in Q could critically impair perfusion of individual organs, we studied the effect of dexmedetomidine on nutrient blood flow to the heart, brain, kidney, spleen, skin, intestine, liver, and arteriovenous anastomoses using the radioactive microsphere technique. Studies were conducted in 14 dogs with an open chest and anesthetized with either chloralose/urethane (CU) or fentanyl/halothane (FH), to create diff

Anesthetic and hemodynamic interactions of dexmedetomidine and fentanyl in dogs.

BACKGROUND: Anesthetic doses of dexmedetomidine (DMED), a highly selective alpha 2 agonist, are not well tolerated hemodynamically. The combination of an opioid with DMED might reduce the dosage requirements for each drug and thereby allow the same anesthetic depth to be achieved with lesser degrees of their individual side effects. METHODS: Dogs were anesthetized with enflurane. One group (n = 5) received intravenous doses of DMED from 0.1 to 10 micrograms/kg. Two other groups of five dogs each received fentanyl 15 micrograms/kg plus 0.05 microgram.kg-1.min-1 or fentanyl 45 micrograms/kg plu

Hemodynamic Safety of Dexmedetomidine-Fentanyl Sedation During TAVI.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) is an established alternative for patients with severe aortic stenosis who are unsuitable for surgical valve replace-ment. Conscious sedation is preferred to preserve spontaneous respiration and patient cooperation. Dexmedetomidine, a selective α2-adrenergic agonist, provides sedation, analgesia, and sympatholysis with minimal respiratory depression, making it suitable for high-risk TAVI patients. METHODS: We retrospectively analyzed 53 patients who underwent TAVI under dexme-detomidine-based sedation at a single center between Januar

Asystole in 2 Pediatric Patients During Dexmedetomidine Infusion.

INTRODUCTION: Bradycardia is a known side effect of dexmedetomidine. Reports of sinus pauses or asystole, however, are rare. We present 2 cases of pediatric patients who developed asystole on a dexmedetomidine infusion. SUMMARY OF CASES: An 8-week-old male with RSV bronchiolitis and acute hypoxemic respiratory failure was started on dexmedetomidine for sedation at 0.2 mcg/kg/h with a maximum dose of 0.7mcg/kg/h. On Hospital day (HD) 4, on dexmedetomidine at 0.7 mcg/kg/h, he developed intermittent episodes of bradycardia with heart rates in the 60 s. Echocardiogram on HD 6 showed normal functi

Midazolam and dexmedetomidine sedation impair systolic heart function.

BACKGROUND: Sedation is an essential part of clinical practice. Despite this fact, we still lack data describing the exact impact of sedation on heart function. PURPOSE: To compare the changes in heart function, induced after sedation with either midazolam or dexmedetomidine, using cardiac magnetic resonance imaging (MRI). METHODS: A total number of 30 volunteers were randomized into two groups: 15 participants in the midazolam group (MID) and 15 participants in the dexmedetomidine group (DEX). Every participant underwent a one-session cardiac MRI before and after sedation onset. The followi

Dexmedetomidine infusion overdose during anesthesia: A case report.

A 12-kg infant was given intravenous dexmedetomidine 0.2 µg kg-1  min-1 as an adjunct for general anesthesia. The 60-fold increase in dexmedetomidine infusion rate caused a biphasic response with initial hypertension followed by bradycardia and hypotension requiring inotropic support. No postoperative or long-term sequelae were noted. Dexmedetomidine infusion is usually delivered as µg kg-1  h-1 .

Dexmedetomidine pharmacodynamics in healthy volunteers: 2. Haemodynamic profile.

BACKGROUND: Dexmedetomidine, a selective α 2 -adrenoreceptor agonist, has unique characteristics, with little respiratory depression and rousability during sedations. We characterized the haemodynamic properties of dexmedetomidine by developing a pharmacokinetic-pharmacodynamic (PKPD) model with a focus on changes in mean arterial blood pressure (MAP) and heart rate. METHODS: Dexmedetomidine was delivered i.v. to 18 healthy volunteers in a step-up fashion by target-controlled infusion using the Dyck model. Exploratory PKPD modelling and covariate analysis were conducted in NONMEM. RESULTS: O

Association of dexmedetomidine with improved survival in ICU delirium.

BACKGROUND: Intensive care unit (ICU) delirium is common in critically ill patients and portends poor outcomes. Although dexmedetomidine (DEX) provides arousable sedation and possible neuroprotection, its survival benefit in established ICU delirium remains unclear. This study assessed the association between DEX initiation within 24 h of ICU admission and mortality in patients with early ICU delirium. METHODS: This retrospective cohort study used data from the Medical Information Market for Intensive Care IV database. ICU patients with a positive Confusion Assessment Method for the ICU scree

Intrathecal dexmedetomidine for cesarean delivery: a scoping review.

BACKGROUND: Off-label use of intrathecal dexmedetomidine (ITDex) is suggested in recent guidelines for pain management during cesarean delivery. The primary objective of this scoping review was to identify all publications reporting on ITDex for cesarean delivery, and secondarily to evaluate the evidence and characterize reported outcomes associated with its use. METHODS: Following PRISMA-Scr guidelines, we searched PubMed and Embase in October 2024 for articles reporting on ITDex for cesarean delivery and conducted a scoping review on randomized controlled trials (RCTs) and observational tri
