Renal cortical pallor--a useful macroscopic marker for metabolic derangements at autopsy.

Renal cortical pallor was studied as a potential marker at autopsy of diabetic ketoacidosis in 23 cases, hyperglycemic nonketotic coma in eight cases, and alcoholic ketoacidosis in five cases (vitreous humor glucose level≥11.1 mM; β-hydroxybutyrate level≥5 mM). Renal cortical pallor was noted on macroscopic examination in 10 of 23 cases of lethal diabetic ketoacidosis (43.5%), three of eight cases of fatal hyperglycemic nonketotic coma (37.5%), and in two of five cases of alcoholic ketoacidosis (40%). Histologic examination revealed basal vacuolization of renal tubular epithelial cells in 12 c

Hashimoto's encephalopathy.

Hashimoto's encephalopathy (HE) is a completely treatable but relapsing neuroendocrine disorder associated with Hashimoto's thyroiditis (HT). Though numerous neurological manifestations are often associated with thyroid disorder, this entity is less documented. We are reporting a case of HE in a 65 year old female presenting with sudden onset deep coma following an attack of mild fever and vomiting for two days. Patient was a known diabetic. Hypoglycemic coma, diabetic ketacidosis and hyperosmolar coma were excluded by laboratory investigations. High blood sugar was corrected with insulin. She

Impaired cognitive processing speed in type 1 diabetic patients who had severe/recurrent hypoglycaemia.

AIMS: To detect whether adults with type 1 diabetes mellitus (T1DM) have lower cognitive performance than healthy individuals and to detect risk factors for low cognitive performance. METHODS: Twenty-six adults with T1DM and twenty-six healthy subjects matched for age, gender and educational level were compared for cognitive performance by a chronometric computerized test measuring visuo-spatial working memory (N-Back) and by two validated neuropsychological tests (Mini Mental State Examination, Animal Naming Test). Clinical data about diabetes duration, average daily insulin dosage, glycated

Rhabdomyolysis accompanying thyroid crisis: an autopsy case report.

Rhabdomyolysis is occasionally associated with metabolic disorders such as diabetic coma, severe electrolyte disturbances and myxedema coma. We describe rhabdomyolysis accompanying thyroid crisis. A 50-year-old man with Graves' disease developed rhabdomyolysis, congestive heart failure and hepatic failure during the course of thyroid crisis and then died of acute renal failure. Postmortem examination revealed rhabdomyolysis in the cardiac and psoas muscles, old myocardial infarction, hepatic centrilobular necrosis, renal cortical necrosis, and follicular hyperplasia in the thyroid. Circulatory

Hypernatremic hemorrhagic encephalopathy.

A 12-year-old juvenile diabetic was inadvertently given 500 ml of hypertonic saline intravenously. He developed hypernatremia, hyperosmolality, metabolic acidosis, and hyperglycemia. Seizures and stupor ensued, followed by coma and death. Computerized cranial tomography revealed numerous small subcortical hemorrhages that were verified postmortem.

Endocrine emergencies.

Endocrine emergencies are relatively infrequent events in clinical practice. Correct management requires prompt recognition and treatment, frequent without supporting laboratory studies. An understanding of the pathophysiology of these acute states of hormone excess or deficiency assists in establishing the correct diagnosis and initiating the proper therapy.

Endocrine emergencies.

The urgency for therapy in endocrine emergencies precludes the completion of a meticulous laboratory evaluation. However, a few critical diagnostic studies obtained concurrently with initiation of therapy may facilitate subsequent therapeutic decisions. Diabetic ketoacidosis, hypoglycemic coma, pheochromocytoma, hypercalcemia, and hypocalcemia are among the critical disorders discussed.

[Diabetic ketoacidosis in children. Effectivity of therapy with small doses of insulin applicated intravenously versus conventional therapy (author's transl)].

Since 1972, the treatment of diabetic coma, ketoacidotic or hyperosmolar, with regular infusions of low doses of insulin, is a well-established form of therapy. Pediatricians all over the world apply this regimen regularly since 1975. In contrast to this there is the regime of intravenous or intramuscular application of high doses of insulin. To control the effectivity of these two methods, we compared two approximately identical groups of patients and analysed 17 different parameters. The patients of the low dose insulin infusion regimen needed less insulin, potassium and bicarbonate and reac

Cerebral edema complicating nonketotic hyperosmolar coma.

Cerebral edema as a complication of the therapy of diabetic coma has been described for over 50 years, although modern awareness dates to about 1967. Almost all cases have occurred in patients with diabetic ketoacidosis (DKA). Although a few cases of cerebral edema have been reported in patients with nonketotic hyperosmolar coma (NKHC), these are in general not well documented by either autopsy data of cat scans. Over a period of 9 years, I have encountered 5 patients who developed cerebral edema as a complication of the therapy of NKHC. The initial plasma glucose in these patients was 1,496 +

Acute rhino-orbito-cerebral mucormycosis.

Report of the typical clinical course and pathological findings in acute rhino-orbito-cerebral mucormycosis following diabetic coma. Invasion of orbital nerves by the fungus with neural and perineural changes may contribute to the neuroophthalmological symptoms in this disease besides the predilection for blood vessels with consequent thrombosis.

[Non-ketosic hyperosmolar hyperglycemic coma. Case reports and therapeutic consideration].

Non-ketonic hyperosmolar hyperglycaemic coma (N.K.H.H.C.) is by no means uncommon in diabetes. Its picture includes sensorial depression, hyperglycaemia, hyperazotemia, marked dehydration and plasma hyperosmolarity. It is mostly found in elderly subjects with non-serious diabetes. Reference is made to 6 personal cases observed during a period of 14 months. The incidence of N.K.H.H.C. noted during this period was 2.2%; this was higher than that of ketoacidotic coma. Two patients died from hypovolaemic shock and one from septic complications. Three survived the episode. Treatment was based on th

Para-aminosalicylic acid-induced hypoglycaemia in a patient with diabetic nephropathy.

A 62-year-old Indian with diabetic nephropathy controlled with metformin, developed miliary tuberculosis for which he was treated with rifampicin, isoniazid and ethambutol. Soon afterwards he developed cholestatic hepatitis and visual disturbance. Rifampicin and ethambutol were stopped. Streptomycin caused vertigo and had to be stopped. The introduction of para-aminosalicylic acid (PAS) led to hypoglycaemic coma. Metformin was stopped. Hypoglycaemic coma recurred. PAS was stopped and the patient's blood glucose concentrations became normal. Treatment with isoniazid and ethambutol led to total

[Bilateral posterior ischemic optic neuritis in an adolescent with diabetes mellitus with decompensated blood glucose homeostasis].

BACKGROUND: Diabetogenic loss of visual acuity is very rare in juvenile onset diabetics. PATIENT: A 14-year-old adolescent with type-I diabetes mellitus since 7 years is reported. She experienced an episode of ketoacidotic diabetic coma after one year of very bad glycemic control. Even two weeks before onset of the diabetic coma her visual acuity decreased to 8/200 in both eyes. The perimetry revealed central and paracentral socotomas, the examination of the fundi showed only some microaneurysms and a normal optic disc. Electrophysiological results (normal ERG and EOG, pathologic pattern-VECP

Effects of severe hypoglycemia on the human brain. Neuropathological case reports.

The neuropathological findings in two cases of irreversible hypoglycemic brain injury are described. A 26-year-old diabetic man injected insulin without adequate food intake and died after 2 months in coma. An 84-year-old nondiabetic man accidentally received 10 mg of glibenclamide and died after 3 months in relatively superficial coma. In the first case, an extensive necrotizing injury with gliosis was present in the cerebral cortex with temporal preponderance, as well as in the amygdalae and hippocampus. Lesions were also present in the putamen and caudate nucleus whereas the globus pallidus

Acute obstructive hydrocephalus due to brain-stem edema caused by hyperosmotic insult. Case report.

The case of a 63-year-old man with acute obstructive hydrocephalus is presented. To the authors' knowledge, this is the first reported case of acute obstructive hydrocephalus associated with nonketotic hyperosmolar diabetic coma. It is believed that the plasma hyperosmolality resulted in osmotic endothelial injury leading to brain-stem edema.

Cerebellar venous infarction: case report with clinicopathologic correlation.

A diabetic man developed a severe hyperosmolar state resulting in coma. CT of the head showed bilateral cerebellar hemorrhages. Despite medical treatment, he deteriorated and died. At autopsy, the straight sinus was thrombosed. There were bilateral, hemorrhagic, cerebellar venous infarctions. This condition is rare because of abundant collateral venous drainage.
