Emergencies in diabetic foot ulcers management. Treatment of diabetic foot ulcers must be organized in specialized care centers to improve healing rates and reduce amputation rates. Some frequent and especially urgent situations will be described: infection, ischemia, Charcot joint. Other less common pathologies will be mentioned: bacterial arthritis, tenosynovitis, necrotizing fasciitis, gas gangrene. French diabetic foot clinics are easily found online.

A 67-year old man presented with a painful left foot and a putrid odor. His past medical history was significant for poorly controlled diabetes mellitus, coronary artery disease, and peripheral vascular disease. His surgical history included a prior right below-knee amputation for a diabetic foot infection three years prior, and a left third toe amputation for osteomyelitis one month ago. He was an active smoker. His laboratory data revealed a white blood count of 22 k/uL and a blood glucose of 381 mg/dL. Physical exam demonstrated an erythematous and edematous left foot with subcutaneous crep

Burn injury in diabetic patients has been a recent topic of interest in published studies. Previous studies have shown increased complications in diabetic patients compared with nondiabetic controls who have sustained these injuries. A paucity of research has been devoted to foot-specific diabetic burn injury. We present a case series evaluating the mechanisms and complications of diabetic foot burns.

The series of ulcers of the lower extremities known as "diabetic foot" is a common complication of diabetes and the chief cause of admission to hospital. The causes may be numerous but the main ones are distal symmetric neuropathy and peripheral obliterative arteriopathy, often complicated by infection. In this review, the Authors, after having illustrated the main pathophysiological aspects of the diabetic foot, describe the clinical characteristics of the disease, focusing particularly on the risk of suprainfection and vascular problems. The clinical and therapeutic approach to diabetic foot

Biomechanical factors play an important role in diabetic foot disease. Reducing high foot pressures (i.e. offloading) is one of the main goals in healing and preventing foot ulceration. Evidence-based guidelines show the strong association between the efficacy to offload the foot and clinical outcome. However, several aspects related to offloading are underexposed. First, in the management of foot complications, offloading is mostly studied as a single entity, whereas it should be analysed in a broader perspective of contributing factors to better predict clinical outcome. This includes assess

As diabetes takes on pandemic proportions, it is crucial for the orthopedic surgeon to be aware of the issues involved in diabetic foot. Ulceration is related to neuropathy and to arterial disease, a vital prognostic factor for healing; infection plays an aggravating role, increasing the risk of amputation. At-risk feet need to be screened for. Ulcer classification is essential, to set treatment strategy and determine prognosis. Before any treatment is decided on, neuropathy, vascular insufficiency and infection should individually be assessed by clinical examination and appropriate additional

In diabetic foot syndrome, a series of complications of late-stage diabetes affect the foot. These complications, which culminate in foot amputation, include peripheral vascular disease and neuropathy, Charcot arthropathy, plantar ulceration, and osteomyelitis. In recent years, the medical community has paid greater attention to diabetic foot syndrome, and our understanding of its pathophysiology and management has advanced. Although the podiatrist is charged with caring for the diabetic foot, as dermatologists we occasionally act as consultants. This review therefore offers dermatologists an

Diabetic foot infections are a common and often serious problem, accounting for a greater number of hospital bed days than any other complication of diabetes. Despite advances in both antibiotic therapy and surgical management, these infections continue to be a major risk factor for amputations of the lower extremity. Although a number of wound size and depth classification systems have been adapted for use in codifying diabetic foot ulcerations, none are specific for infection. In 2003, the International Working Group on the Diabetic Foot developed guidelines for managing diabetic foot infect

BACKGROUND: The diabetic foot is one of the main complications of diabetes mellitus, with a high risk of minor or major amputation. The preclinical foot lesions of patients without foot complaints were compared with healthy controls and analyzed.
METHODS: This study was conducted with 89 diabetic patients from an endocrinology clinic and 35 nondiabetic control patients. The patients were asked about the presence, types, and durations of pedal complaints; acquired and congenital foot deformities; and atrophy. Patient gaits were inspected for any swelling; skin and nail changes were also record

A comprehensive review of the literature relating to the pathology and management of the diabetic foot is presented. This should provide a guide for the treatment of ulcers, Charcot neuro-arthropathy and fractures involving the foot and ankle in diabetic patients.

A chronic diabetic foot ulcer requires a search for the etiology. The three main causes to search for are poor off-loading compliance, osteomyelitis, and peripheral vascular disease. The level of severity is measured with the U.T. classification and the level of infection with the classification of the International Consensus on the Diabetic Foot. Peripheral vascular disease must be precisely evaluated by Doppler ultrasound, which describes all the arteries of the lower limb. Angiography is required only in case of revascularization. Treatment of the ulcer includes strict off-loading, topical

MR imaging can be quite useful in the evaluation of pathologic processes related to the diabetic foot. The diagnosis of soft tissue processes such as callus formation, ulceration, foreign body granuloma, cellulitis, abscess, and gangrene can be made with the assistance of MR imaging. Osseous deformities such as hammertoe, rocker-bottom foot, neuropathic osteoarthropathy, and osteomyelitis can be detected and defined with MR imaging. Finally, although there are some overlapping features of neuropathic osteoarthropathy and osteomyelitis, the differentiation between the two entities is possible i

As the incidence of diabetes mellitus is increasing globally, complications related to this endocrine disorder are also mounting. Because of the large number of patients, foot ulcers developing in the feet of diabetics have become a public health problem. The predisposing factors include abnormal plantar pressure points, foot deformities, and minor trauma. Vulnerable feet usually already have vascular insufficiency and peripheral neuropathy. The complex nature of these ulcers deserves special care. The most useful prognostic feature for healing remains the ulcer depth, ulcers heal poorly if th

When treating diabetic foot ulcers it is important to be aware of the natural history of the diabetic foot, which can be divided into five stages: stage 1, a normal foot; stage 2, a high risk foot; stage 3, an ulcerated foot; stage 4, an infected foot; and stage 5, a necrotic foot. This covers the entire spectrum of foot disease but emphasises the development of the foot ulcer as a pivotal event in stage 3, which demands urgent and aggressive management. Diabetic foot care in all stages needs multidisciplinary management to control mechanical, wound, microbiological, vascular, metabolic and ed

Diabetes is a common disease with potentially devastating complications affecting the foot and ankle. Ischemia and infarction, neuropathic osteoarthropathy, callus, ulceration, and infection result from the underlying neurologic and vascular disease. The MR imaging appearance of these complications is discussed. Recognition of these MR imaging patterns is important for formulation of an appropriate treatment plan.

Allthough there has been dramatic progress in limb salvage in recent decades, management of nonhealing wounds in diabetic patients continues to present a dilemma for the reconstructive surgeon. However, the acceptance of free-flap resurfacing of diabetic foot ulcers has increased in recent years. This study reviews 10 microvascular free muscle flaps in nine patients over a mean follow-up period of 44 months. Five patients had evidence of peripheral vascular disease. There was one flap loss, and nine flaps were transferred successfully. No perioperative mortality was encountered. The operations
