[Endocrine surgery for neck paraganglioma: operation, radiation therapy or wait and scan?].

BACKGROUND: Head and neck paraganglioma (HNP) represent rare endocrine tumors. Therapy is decided on genetic findings, tumor characteristics (e.g. tumor size, localization and dignity), age of patient and symptoms. In terms of local control radiation therapy is as equally effective as surgery but surgical morbidity rates secondary to cranial nerve injuries remain high. PATIENTS: Based on 6 patients with 11 solitary (4 patients) and multiple (2 patients) HNP (8 carotid body tumors, 1 vagal, 1 jugular and 1 jugulotympanic paraganglioma) the specific characteristics of the need for surgery as we

A Perspective on Integrative Oncology in India: Survival Conversations, Multidisciplinary Collaboration, and Healing Environments.

INTRODUCTION: Integrative oncology combines conventional cancer treatments with evidence-based complementary therapies to address the multidimensional needs of cancer patients. While existing literature largely focuses on therapeutic interventions and clinical outcomes, limited attention has been given to the lived experiences and professional realities of integrative oncology practitioners, particularly within the Indian healthcare context. OBJECTIVE: This perspective paper aims to explore practical, emotional, and interdisciplinary dimensions of practicing integrative oncology, while emphas

Urogynecologic Care for Women Who Are Incarcerated.

In the United States, approximately 180,700 women are incarcerated across jails and prisons, comprising a demographic with an aging population and a higher growth rate compared to men. Despite this demographic trend, research into urogynecologic care for women who are incarcerated is notably lacking, with few studies addressing pelvic floor disorders such as incontinence, and even fewer focusing on access to treatment options like vaginal pessaries or surgical interventions. Women who are incarcerated may face unique challenges in obtaining urogynecologic care, including limited access to medi

Dental health in persons with disability.

Systemic and structural barriers limit dental health for individuals with special healthcare needs (SHCN), who have poorer dental hygiene, higher rates of dental disorders, and less access to oral care. We aimed to understand these barriers directly from the patient and caregiver population as well as review the literature on oral health of individuals with SHCN. We reviewed the literature on individuals and caregivers of those with SHCN to identify barriers to dental healthcare faced by these individuals. We focused on clinical and educational interventions to support clinicians treating this

[Endocrine tumors: clinical overview].

The term endocrine tumor incorporates all neoplasms which originate from the various endocrine organs. Endocrine tumors can be characterized by different criteria: localization, endocrine function, dignity (i.e. tumorigenesis, sporadic or hereditary). These characteristics also determine the clinical outcome. The clinical history, symptoms and physical examination findings (e.g. amenorrhea, skin alterations, striae, virilization, increased blood pressure and flush) direct the diagnostic process of functioning endocrine tumors. Laboratory findings (endocrine parameters) are needed to establish

Mental Health Nurse experiences of delivering care to severely depressed adults receiving electroconvulsive therapy.

WHAT IS KNOWN ON THE SUBJECT?: ECT is a fast-working and potentially life-saving treatment, but it is still considered a controversial treatment choice. Due to lack of knowledge and public stigma, ECT can be an anxiety-inducing treatment for people experiencing severe depression. The nurse's role is to alleviate this anxiety, aid recovery and minimize the risk of relapse. They manage this onerous task ideally through the therapeutic relationship, and use the skills of assessment and risk identification in order to maximize patient outcomes. WHAT THIS PAPER ADDS TO EXISTING KNOWLEDGE?: The view

Existential distress in family caregivers: scoping review of meaning-making interventions.

OBJECTIVES: Family and friend caregivers often feel overwhelmed by and ill-prepared for their responsibilities. Many feel helpless living with uncertainty about the outcome of the patient's illness, which leads to existential distress. Supportive care interventions that address existential distress by promoting meaning and purpose buffer the negative effects of caregiver burden and promote resilience and growth. The purpose of this scoping review is to describe the depth and breadth of available interventions targeting caregiver existential distress. METHODS: We followed the Joanna Briggs Ins

Assessment of health system responsiveness in delivering HIV and AIDS care services at Urban sites of Pakistan - a protocol for cross sectional study.

INTRODUCTION: The capacity of a health system to fulfill the justifiable demands of patients and their caregivers while delivering prompt, courteous and patient-centered treatment is known as health system responsiveness (HSR). HSR can be considered as a measure for the quality of the health system. Keeping aside the clinical aspects of healthcare, it focusses on the health system's capacity to meet the needs of patients in accordance with ethical values and service standards. HSR is comprised of eight major dimensions including autonomy, dignity, confidentiality, timely attention, communicati

Endocrinology of the aging male.

Despite enormous medical progress during the past few decades, the last years of life are still accompanied by increasing ill health and disability. The ability to maintain active and independent living for as long as possible is a crucial factor for ageing healthily and with dignity. The most important and drastic gender differences in aging are related to the reproductive organs. In distinction to the course of reproductive ageing in women, with the rapid decline in sex hormones expressed by the cessation of menses, men experience a slow and continuous decline. This decline in endocrine func

A Novel Flow Model for Demonstrating and Simulating Chemosaturation with Percutaneous Hepatic Perfusion in Unresectable Liver Metastasis.

RATIONALE AND OBJECTIVES: Diffuse unresectable liver metastasis of varying dignity are complex to manage and are associated with poor overall survival. Given the wide spectrum of liver-directed therapies including surgery, ablation, radiotherapy, brachytherapy, chemoembolization and chemosaturation an interdisciplinary approach is essential. Chemosaturation with percutaneous hepatic perfusion (CS-PHP) has shown promising developments in the treatment of metastasized uveal melanoma and colorectal carcinoma. However, the procedure is technically complex, involving isolation of hepatic circulatio

Multimodal Management of Pain in Oral Squamous Cell Carcinoma: A Mechanism-Based Approach.

PURPOSE OF REVIEW: Oral squamous cell carcinoma (OSCC) is frequently associated with severe nociceptive and neuropathic pain that adversely affects patient quality of life, functional capacity, psychological well-being, and prognosis. Pain in OSCC is increasingly recognized not only as a clinical symptom but also as a dynamic indicator of tumor progression and neuroinflammatory activity. This review summarizes the epidemiological, molecular, neurobiological, and clinical mechanisms underlying OSCC-associated pain and discusses current and emerging approaches for mechanism-based multimodal pain

"End-of-Life Care" in Cardiac Surgery.

Modern cardiovascular medicine and surgery enable the treatment of complex cardiovascular disease, even in elderly and multimorbid patients, through increasingly sophisticated procedures. Nevertheless, there are situations in which surgical intervention appears medically inappropriate or is explicitly declined by the patient. Similarly, it may become necessary in the postoperative course to revise the original therapeutic goals-particularly when continued survival depends upon prolonged intensive care unit treatment, or when the anticipated quality of life, from the patient's perspective, appe

Creating a Culture of Mobility: Using Real-Time Assessment to Drive Outcomes.

BACKGROUND: Recent studies have demonstrated the safety and feasibility of early mobilization of patients in the hospital setting. Promoting early mobility improves patients' ability to perform daily activities and attend to basic needs. It also preserves patients' dignity and independence. We implemented a culture of mobility program to promote awareness of the importance of early mobility among health care providers, patients, and family caregivers on an inpatient orthopedic unit. PURPOSE: The goal of the program was to empower clinical RNs, physical therapists (PTs), and occupational thera

Atypical melanocytic proliferations and new primary melanomas in patients with advanced melanoma undergoing selective BRAF inhibition.

PURPOSE: Selective inhibition of mutant BRAF by using class I RAF inhibitors in patients with metastatic melanoma has resulted in impressive clinical activity. However, there is also evidence that RAF inhibitors might induce carcinogenesis or promote tumor progression via stimulation of MAPK signaling in RAF wild-type cells. We analyzed melanocytic lesions arising under class I RAF inhibitor treatment for dignity, specific genetic mutations, or expression of signal transduction molecules. PATIENTS AND METHODS: In all, 22 cutaneous melanocytic lesions that had either developed or considerably
