Hypothalamic digoxin, hemispheric chemical dominance, and Alzheimer's disease.

This study assessed the changes in the isoprenoid pathway and the consequences of its dysfunction in Alzheimer's disease (AD). The isoprenoid pathway and digoxin status were also studied for comparison in individuals of differing hemispheric dominance to find the role of cerebral dominance in the genesis of Alzheimer's disease. There was elevation in plasma HMG CoA reductase activity, serum digoxin, and dolichol levels, and a reduction in serum magnesium, RBC membrane Na(+)-K+ ATPase activity, and serum ubiquinone levels. Serum tryptophan, serotonin, strychnine, nicotine, and quinolinic acid w

Digoxin-like immunoreactive substances in chronic liver disease.

Digoxin-like immunoreactive substances, which cross-react with digoxin antibody, have been found to have natriuretic effect and Na+,K+-ATPase inhibitory effect. The role of digoxin-like immunoreactive substances in chronic liver disease was studied by radioimmunoassay in 63 serum and 60 urine samples from 58 patients with chronic liver disease and compared with 16 controls. Although the mean serum digoxin-like immunoreactive substances level of compensated chronic liver disease patients (0.06 +/- 0.05 ng per ml, p less than 0.01) was higher than that of controls (0.02 +/- 0.03 ng per ml), only

Diuretic-induced CNS magnesium alteration and digoxin intoxication.

Male Wistar rats were injected intraperitoneally for 3 consecutive days with hydrochlorothiazide (HCTZ; 15 mg/kg), chlorothiazide (CTZ; 100 mg/kg), bendroflumethiazide (BFTZ; 1.5 mg/kg), chlorthalidone (CHLOR; 15 mg/kg), methyclothiazide (METH; 1.0 mg/kg) or metolazone (MET; 1.5 mg/kg). Magnesium content was measured in the hypothalamus, medulla oblongata, cerebral cortex, heart, skeletal muscle and serum. Although there was no consistent alteration of Mg in the serum, skeletal muscle and heart, there was a significant effect on hypothalamic and medullary Mg. Compared to a control value of 17.

Supratherapeutic Digoxin Levels in Children with Cardiac Disease.

We characterized our institution's experience with supratherapeutic digoxin levels (> 2.0 mcg/L) which is reported to occur in 0.8-4% of treated patients. Children < 18 years with a measured digoxin level between January 2014 and December 2024 were included. Data were obtained from the electronic health record including digoxin dose, indication, renal function, and clinical interventions. Descriptive patient-level analyses were performed in children with supratherapeutic levels. Laboratory sample-level analyses were performed using all measured digoxin levels comparing high (> 2.0mcg/L) verses

Digoxin is still useful, but is still causing toxicity.

Digoxin, the oldest known cardiovascular drug, is still used today to treat heart failure and atrial fibrillation. Because it has a narrow therapeutic index and multiple interactions, it frequently causes toxicity with a wide range of symptoms and cardiac arrhythmias. More importantly, elevated serum digoxin levels have been linked to a higher risk of death in patients with heart failure or atrial fibrillation, even without signs or symptoms of toxicity. This article reviews the current state of digoxin use, its pharmacologic principles, and the mechanisms, clinical presentation, and managemen

[Myocardial infarction in the course of digoxin and theophylline poisoning].

The aim of this paper is to present a case of the patient who was hospitalized in the Toxicology and Cardiology Department because of suicidal digoxin and theophylline intoxication. Both drugs have complicated mechanism of action and affect cardiovascular system differently, but both predispose to ventricular and supraventricular arrhythmias. In the presented patient conduction disorders, cardiac muscle repolarization disorders and ventricular arrhythmias typical to digoxin poisoning were observed. During hospitalization the patient experienced the ECG and biochemical abnormalities resulting f

Effect of lapatinib on oral digoxin absorption in patients.

The potential for an interaction between lapatinib and absorption of the P-glycoprotein (ABCB1) substrate digoxin at a therapeutic dose in breast cancer patients was characterized. Seventeen women with HER2-positive metastatic breast cancer received a single oral 0.5-mg dose of digoxin on days 1 and 9 and oral lapatinib 1500 mg once daily on days 2 through 9. Digoxin pharmacokinetic parameters were determined on day 1 (digoxin administration alone) and on day 9 (coadministration of lapatinib and digoxin), and parameters were compared to determine the effects of lapatinib on digoxin absorption.

Digoxin as a treatment for patients with idiopathic intracranial hypertension.

Idiopathic intracranial hypertension (IIH)-sometimes called pseudotumor cerebri-is a neurologic condition distinguished by any of the following symptoms: headache, increased cerebrospinal fluid pressure, papilledema, vision loss, diplopia, tinnitus, deafness, nausea and vomiting, or sixth nerve palsy. Medical and surgical management options are available for patients with IIH. The authors report a case of IIH that was successfully resolved with digoxin after standard avenues of therapy were exhausted.

Advances in the management of digoxin toxicity in the older patient.

Digoxin intoxication is a common problem in the elderly. In its mildest forms it may go undiagnosed, but in severe cases it is often fatal. Altered digoxin pharmacokinetics, attributable to the physiological changes associated with aging, underlying illness, and drug-drug interactions all contribute to the occurrence of digoxin toxicity. Advanced age, male gender, initial hyperkalaemia, underlying heart disease, and advanced atrioventricular block at the time of admission are poor prognostic factors. Supportive care alone is often insufficient. Digoxin-specific Fab therapy may result in dramat

Redefining the role of digoxin in the treatment of atrial fibrillation.

Atrial fibrillation (AF) encompasses a variety of discrete clinical syndromes, including paroxysmal, chronic, acute, and postoperative. Digoxin, long considered the mainstay of therapy for rate control in all types of AF, appears to have only modest electrophysiologic effects, which are mediated primarily by the autonomic nervous system. Digoxin has less potency than the calcium antagonists or beta-blocking drugs with respect to atrioventricular nodal blockade. Although less potent than calcium antagonists or beta-blocking drugs on the atrioventricular node, digoxin provides positive inotropic

[Should chronic atrial fibrillation be treated with digoxin?].

For decades, atrial fibrillation was treated exclusively with digoxin. The introduction of Ca-antagonists and beta-blockers has, however, produced valuable alternatives as ventricular function can now be controlled both at rest and during exercise whereas, with digoxin, ventricular function could only be controlled at rest. This review deals with the etiology and electrophysiology of atrial fibrillation together with the effects of Ca-antagonists and beta-blockers on conduction and control of the heart rate during exercise. Documentation of the effects of the different drugs, separately and co

Reassessment of indications for digoxin. Are patients being withdrawn?

Several studies have shown that the majority of patients receiving digoxin can be successfully withdrawn. A medical record review was conducted to determine whether, in practice, patients were being withdrawn from digoxin. Original indications for digoxin therapy in 163 outpatients were as follows: congestive heart failure (CHF), 50%; supraventricular tachycardia (SVT), 23%; CHF and SVT, 10%; and unknown/unclear, 17%. One third of these patients were withdrawn during the 3.5-year study, and 79% remained stable, off digoxin. The most significant predictor of withdrawal was chart indication of r

[Use of digoxin in patients with paroxysmal supraventricular tachycardia].

The effectiveness and electrophysiologic mechanisms of antiarrhythmic effect of digoxin were examined in 27 patients with paroxysmal atrioventricular nodal reciprocal tachycardia (PAVNRT) and supraventricular tachycardia (SVT) due to latent complementary conductive pathways, i. e. latent Wolff-Parkinson-White (WPW) syndrome. To assess antiarrhythmic action of digoxin, transesophageal pacing and plasma digoxin radioimmonoassays were used. Preventive antiarrhythmic efficiency of digoxin was 53% in PAVNRT patients, and 25% in SVT patients with latent WPW syndrome. Antegrade atrioventricular condu

Hemodynamic effects of digoxin on congestive heart failure.

To assess the hemodynamic effects of digoxin (0.01 mg/kg) on congestive heart failure, we evaluated 19 patients with decreased contraction force of left ventricle (old myocardial infarction n = 9, and dilated cardiomyopathy n = 10, group 1) and 8 patients with mechanical impaired left ventricular filing (mitral stenosis n = 8, group 2). In groups 1 and 2, heart rate and pulmonary capillary pressure significantly decreased (p less than 0.05). In group 1, stroke volume increased, but not significantly. In group 2, stroke volume increased significantly (p less than 0.05). There were no significan

Sinus node dysfunction caused by methyldopa and digoxin.

Symptomatic sinus bradycardia developed in two patients while they were taking methyldopa and digoxin. In one patient, bradycardia did not occur with either digoxin or methyldopa alone. The other patient, who had never taken methyldopa alone, did not demonstrate bradycardia with digoxin alone. In each patient, normal sinus node function was demonstrated after methyldopa and digoxin therapy was discontinued. Both patients continued to receive digoxin without recurrence of bradycardia.

Effect of quinidine on positive inotropic action of digoxin.

To determine whether digoxin-quinidine interaction alters the inotropic effect of the glycoside, the response of peak isometric force and maximal rate of force development (dF/dt) in isolated feline right ventricular papillary muscles to digoxin and quinidine alone, and in various combinations, was examined. The administration of 1.3 or 2.6 x 10(-5) M of quinidine after 2 x 10(-7) M of digoxin resulted in an increase in contractile performance in each animal. Although 1.3 x 10(-5) M of quinidine alone produced a positive inotropic effect, 2.6 x 10(-5) M of quinidine produced no such effect. Be

Cardiac toxicity of digoxin in newborn and adult rats.

We investigated the age-specific arrhythmogenic effects of digoxin in the rat. Adult female rats (n = 26) and one-day-old newborns (n = 20) were anesthetized with pentobarbital and injected subcutaneously with varying doses of digoxin. Electrocardiograms (ECG) were monitored continuously for four and one-half hours following digoxin administration. The arrhythmogenic dose 50 (AD50) and lethal dose 50 under anesthesia (LD50) were determined using the method of Litchfield and Wilcoxon. AD50 in adults was 13.0 +/- 1.0 mg/kg (X +/- SD) compared with 2.9 +/- 0.3 mg/kg in the newborns (P less than 0
