Pediatric diseases and operational deployments.

Many nations now export military health as a proactive arm of the nation's contribution to the maintenance of international peace in trouble regions of the world; and all nations are called upon from time to time in emergency and disaster situations to help out in their regions of interest. Children and young teenagers constitute some 50% of war-stricken populations. This paper explores this increasingly important role of military medicine from the point of view of a practicing pediatrician and career doctor-soldier. Many international operational deployments undertaken in the last 5 years hav

Scientific response to the 2023 Kahramanmaraş earthquake: A bibliometric study.

BACKGROUND: On February 6, 2023, Türkiye experienced two devastating earthquakes with epicenters in Pazarcık and Elbistan, measuring 7.7 and 7.6 on the Richter scale. These earthquakes resulted in over 50,000 deaths and widespread destruction of infrastructure. The disaster triggered a large-scale humanitarian crisis, presenting significant medical and psychological challenges. Understanding the scientific response to such events is crucial for enhancing future disaster preparedness and management. METHODS: A systematic search was conducted in the Web of Science database using the terms "Kahr

Dermatology aboard the USNS COMFORT: Disaster relief operations in Haiti after the 2010 earthquake.

On the 12th of January 2010, Haiti was struck by a 7.0 Richter magnitude earthquake that devastated its already fragile capital region. Approximately 230,000 people died immediately or during ensuing weeks, mostly due to acute trauma. Countless others suffered significant life- or limb-threatening injuries. As a part of the United States' response to this tragedy, eventually named Operation Unified Response, the United States Navy deployed hundreds of physicians and other medical response individuals on a hospital ship. Operation Unified Response was a military joint task force operation augme

[Disaster medical response concerns us all].

The flood disaster in the region of the Oder and Elbe Rivers or the disaster in Eschede, Kaprun, or Ramstein make us aware that disasters not only occur in distant regions of the world but also in our latitudes. They do not follow any rules; no one can predict the location, time, or type of a disaster. However, this lack of concrete predictability should not lead to our being unprepared to respond to catastrophic events. Detailed examination and analysis of medical and organizational activities involved in past disasters reveal that these types of incidents always entail similar medical and lo

[IT network establishment for neuropatients].

On March 11, 2011, big earthquake and subsequent gigantic tsunami killed more than 20,000 peoples in Tohoku area of Japan. Neurological patients were one such victim because they are usually very vulnerable to such a huge tragedy due to their physical disability including artificial ventilator-support. On occasion of the last tsunami, most cases showed "all or nothing" to lose life or to survive, and there were only a little cases who needed emergency surgical treatment. In the very early period, some neurological patients required electric power to keep their lifesupportive ventilator at evac

Surge capacity for healthcare systems: a conceptual framework.

This report reflects the proceedings of a breakout session, "Surge Capacity: Defining Concepts," at the 2006 Academic Emergency Medicine Consensus Conference, "Science of Surge Capacity." Although there are several general descriptions of surge capacity in the literature, there is no universally accepted standard definition specifying the various components. Thus, the objectives of this breakout session were to better delineate the components of surge capacity and to outline the key considerations when planning for surge capacity. Participants were from diverse backgrounds and included academi

Forgotten digital tourniquet: salvage of an ischaemic finger by application of medicinal leeches.

Individual finger tourniquets are appropriate to the management of a wide range of conditions presenting to an accident and emergency department. They are simpler and more comfortable to use than upper arm pneumatic tourniquets and commercially available digital tourniquets are not readily available in the accident and emergency unit. However, if a finger tourniquet is overlooked, ischaemia of the digit results, and gangrene may follow if the problem is not defused early enough, leading to potential disaster. We present one case where a digit was salvaged after 4 days of tourniquet application

Chest Ultrasonography in Modern Day Extreme Settings: From Military Setting and Natural Disasters to Space Flights and Extreme Sports.

Chest ultrasonography (CU) is a noninvasive imaging technique able to provide an immediate diagnosis of the underlying aetiology of acute respiratory failure and traumatic chest injuries. Given the great technologies, it is now possible to perform accurate CU in remote and adverse environments including the combat field, extreme sport settings, and environmental disasters, as well as during space missions. Today, the usage of CU in the extreme emergency setting is more likely to occur, as this technique proved to be a fast diagnostic tool to assist resuscitation manoeuvres and interventional p

Homeopathic treatment in emergency medicine: a case series.

Following a multiple-casualty construction disaster in Israel, members of The Center of Integrated Complementary Medicine joined in the emergency activity of the Shaare Zedek Medical Center. They administered homeopathic treatment to injured patients to supplement conventional orthopaedic treatment. This was to our knowledge the first time that complementary medicine had been used officially in conjunction with conventional medicine in an emergency situation. Our objective is to report and summarize the rationale, procedures and outcome of the complementary medicine intervention. Fifteen ortho

COVID-19 and Substance Use Disorders: Recommendations to a Comprehensive Healthcare Response. An International Society of Addiction Medicine Practice and Policy Interest Group Position Paper.

Coronavirus Disease 2019 (COVID-19) is escalating all over the world and has higher morbidities and mortalities in certain vulnerable populations. People Who Use Drugs (PWUD) are a marginalized and stigmatized group with weaker immunity responses, vulnerability to stress, poor health conditions, high-risk behaviors, and lower access to health care services. These conditions put them at a higher risk of COVID-19 infection and its complications. In this paper, an international group of experts on addiction medicine, infectious diseases, and disaster psychiatry explore the possible raised concern

[Expert consensus on the clinical application of anti-rabies virus monoclonal antibody (2025 edition)].

The anti-rabies virus monoclonal antibody (RmAb) is a novel passive immunization for rabies developed and produced through modern genetic engineering technologies. It specifically binds to the antigenic sites on the G protein of the rabies virus surface, thereby blocking the virus from binding to cellular receptors and exerting a preventive effect. Compared to traditional rabies immunoglobulin, RmAb demonstrates superior advantages in terms of controllable production yield, stable quality, high safety profile, and favorable specific activity. Currently, four RmAb products have been approved fo

Crisis Standards of Care Implementation at the State Level in the United States.

INTRODUCTION: In 2009, the Institute of Medicine published guidelines for implementation of Crisis Standards of Care (CSC) at the state level in the United States (US). Based in part on the then concern for H1N1 pandemic, there was a recognized need for additional planning at the state level to maintain health system preparedness and conventional care standards when available resources become scarce. Despite the availability of this framework, in the years since and despite repeated large-scale domestic events, implementation remains mixed. PROBLEM: Coronavirus disease 2019 (COVID-19) rejuven

Katrina, the tsunami, and point-of-care testing: optimizing rapid response diagnosis in disasters.

We assessed how point-of-care testing (POCT), diagnostic testing at or near the site of patient care, can optimize diagnosis, triage, and patient monitoring during disasters. We surveyed 4 primary care units (PCUs) and 10 hospitals in provinces hit hardest by the tsunami in Thailand and 22 hospitals in Katrina-affected areas. We assessed POCT, critical care testing, critical values notification, demographics, and disaster responses. Limited availability and poor organization severely limited POCT use. The tsunami impacted 48 PCUs plus island and province hospitals, which lacked adequate diagno

Sleep, circadian rhythm, and maritime safety: The Titanic disaster revisited.

OBJECTIVE: In 1912, the Titanic struck an iceberg and sank. The question of why the iceberg was detected too late to avoid collision has been debated ever since. The potential role of sleep deprivation and circadian rhythm disturbance has not previously been explored scientifically. Given that the diagnosis of sleep disturbances rests primarily on patient history and circumstantial evidence, this study systematically examines these factors. METHODS: An online search and literature review were conducted, drawing on extensive documentation provided by witnesses and experts who testified before

Emergency preparedness in high school-based athletics: a review of the literature and recommendations for sport health professionals.

Approximately 7.6 million high school students in the United States participate in sports. Although most sport-related injuries in adolescents are considered minor emergencies, life-threatening illnesses or injuries may occur, such as sudden cardiac arrest, heat stroke, status asthmaticus and exercise-induced asthma, catastrophic brain injuries, cervical spine injuries, heat- and cold-related illness, blunt chest/abdominal injuries, and extremity fractures resulting in compartment syndrome. Emergency preparedness in athletics involves the identification of and planning for medical services to

Desensitizing a pilot with a phobic response to required helicopter underwater escape training.

INTRODUCTION: A systematic desensitization program designed to help aircrew who have a phobic anxiety of the Helicopter Underwater Escape Training (HUET) has been developed. CASE PRESENTATION: A student pilot presented with a severe phobic anxiety of the HUET course--a result of a being trapped underwater on a marine survival training course. A treatment team was assembled and concluded his phobia could be treated by a systematic desensitization method. An exposure matrix of graded difficulty of in/underwater escapes was performed over 2 d. The student reported that his confidence increased a

Modernizing stockpiles of medical countermeasures against smallpox: Benefits, risks, and knowledge gaps.

OBJECTIVE: New smallpox medical countermeasures are entering the marketplace, offering the opportunity to modernize existing stockpiles. However, new smallpox countermeasures are developed under the animal rule, meaning that human efficacy data are lacking, and human safety data may be limited. Also, stockpile modernization would require prioritization of increasingly limited public funds. Approaches to address these issues are needed. METHODS: Smallpox vaccine data were gathered by literature search. The financial value of vaccination in the face of an outbreak was evaluated using a threatba

Implementing business continuity effectively within the UK National Health Service.

Whereas major incident planning is very well established within National Health Service (NHS) organisations in the UK, business continuity management (BCM) planning, in many cases, is a relatively new activity; however, a combination of factors including the emergence of H1N1 influenza, has led to growing interest in the subject. This paper draws on both the personal experience of the authors and published research in relevant fields to make a number of specific recommendations about the effective implementation of BCM within NHS organisations. These include the need to define the BCM project
