Atypical Cardiac Anatomy Leading to Subvalvar Aortic Stenosis in a 19-Month-Old Boy With a Murmur.

A 19-month-old boy presented with a murmur and was found to have an unusual etiology of subvalvar aortic stenosis with discrete subaortic membrane and anomalous attachment of the anterior mitral valve papillary muscle to the interventricular septum. Preoperative suspicion for mitral valve involvement impacted surgical planning.

"Acquired" subvalvular aortic stenosis after repair of several congenital cardiac defects.

Discrete subvalvular aortic stenosis is a progressive lesion. In this report we presented nine patients who had no significant left ventricular-aortic obstruction at initial cardiac catheterization or echocardiographic examination, but later developed significant subvalvular aortic stenosis. Associated lesions included ventricular septal defect in three, patent ductus arteriosus in two, aorticopulmonary window in one, tetralogy of Fallot in one, supramitral membrane in one, and ventricular septal defect and patent ductus arteriosus in one case. Nine patients were diagnosed with subvalvular aor

Left ventricular outflow tract obstruction in atrioventricular septal defects: a pathologic and morphometric evaluation.

Subaortic stenosis has been described with increasing frequency as an ominous feature of atrioventricular septal defect (AVSD), especially following surgical correction of the anomaly in non-Down's syndrome patients. In order to study the surgical anatomy of the left ventricular outflow tract in this malformation, 48 hearts featuring AVSD were examined. Obstructive lesions were classified into unequivocal forms (class A, 13.5%) and potential ones (class B, 10.8%). In the remaining hearts (class C, 75.7%) no obstruction was noted. In class A, subaortic stenosis was due to exaggeration of the an

Dual-Level Left Ventricular Outflow Tract Obstruction in an Adult.

BACKGROUND: Discrete subaortic membranes are an uncommon cause of left ventricular outflow tract (LVOT) obstruction in adults and may coexist with aortic valve disease. CASE SUMMARY: A 50-year-old man presented with acute decompensated heart failure. Transthoracic echocardiography showed reduced ejection fraction (30%-35%) and demonstrated severe calcific aortic stenosis with regurgitation. Transesophageal echocardiography revealed a discrete subaortic membrane causing additional subvalvular obstruction, consistent with dual-level LVOT obstruction. Invasive hemodynamics confirmed advanced aft

Recurrent subaortic membrane causing subvalvular aortic stenosis 13 years after primary surgical resection.

This case demonstrates the use of cardiac CT angiography as part of the preoperative evaluation of a recurrent subaortic membrane in a 23-year-old women. A subaortic membrane is a rare finding on coronary CT angiography and typically consists of a thin fibrous membrane in the left ventricular outflow tract (LVOT). The resulting turbulent flow may lead to LVOT obstruction and aortic insufficiency.

[Long-term postoperative outcomes after surgical correction of congenital subaortic stenosis].

OBJECTIVE: To analyze the results of surgical treatment of discrete subaortic stenosis and identify the main factors of left ventricular outflow tract (LVOT) restenosis in long-term postoperative period. MATERIAL AND METHODS: There were 87 surgical interventions in 63 patients with congenital subaortic stenosis between 2008 and 2023. Mean preoperative peak systolic LVOT pressure gradient was 72 mmHg (50-110 mmHg). Mean age of patients before surgery was 64.3±12.6 months. Fifteen (23.8%) patients had cardiovascular comorbidities; 27 (42.8%) ones underwent previous correction of congenital hear

Fixed subaortic stenosis with congenital aneurysm of sinus of Valsalva--cross-sectional and Doppler echocardiographic diagnosis of a rare association.

A 13-year-old girl was diagnosed by cross-sectional and Doppler echocardiography to have the rare combination of congenital aneurysm of the right sinus of Valsalva rupturing into the right ventricular outflow tract and a discrete subaortic fibrous shelf with severe subvalvar stenosis, moderate aortic regurgitation and left ventricular dysfunction. The findings were confirmed at cardiac catheterisation and surgery.

Midsystolic closure of the aortic valve in primary pulmonary hypertension.

Echocardiographic examination in a patient with primary pulmonary hypertension demonstrated midsystolic closure of the aortic valve, which has often been described in hypertrophic cardiomyopathy with or without obstruction, in discrete subaortic stenosis, and in ruptured aneurysm of the right coronary sinus of Valsalva. A possible mechanism for this finding in primary pulmonary hypertension and differential diagnosis from other diseases are discussed.

[Subaortic membrane in adults. A brief review of literature apropos of one case].

A 54-year-old man, ex smoker with high blood pressure and a history of possible Wolff-Parkinson-White syndrome was admitted for presenting an episode suggestive of acute coronary syndrome with immediate syncope and left bundle branch block, while performing physical activity. Angioplasty and a drug-eluting stent were performed in the left circumflex artery. Subsequently, Doppler echocardiography disclosed an image suggestive of a subaortic membrane. Given these findings, the patient underwent a 3D transesophageal echocardiogram and a magnetic resonance study, which confirmed the diagnosis of a

Discrete subvalvular aortic stenosis.

Discrete Subaortic Stenosis is one of the many lesions responsible for left ventricular outflow tract (LVOT) obstruction. It may present as in an isolated from as membranous or fibromuscular ring below the aortic valve or in association with other congenital anamolies such as VSD, PDA, coarctation of aorta, hypoplastic aortic annulus, double chamber right ventricle among others. The condition is rarely diagnosed antenataly or in infancy but often manifests in the first decade of life with features of progressive LVOT obstruction, LV hypertrophy and dysfunction aortic regurgitation due to damag

[Valvulopathies (XIV). The role of percutaneous valvuloplasty in congenital valvular diseases].

The present article describes the cooperative experience Córdoba-Las Palmas since 1983 in 123 patients with congenital right or left ventricular outflow obstructions who were treated by percutaneous balloon dilatation. Seventy of them had pulmonary stenosis (63 valvular and seven supravalvular types); two had pulmonary atresia, 25 valvular aortic stenosis and 27 discrete subaortic stenosis. In patients with valvular pulmonic stenosis the gradient and the right ventricular pressure decreased significantly. No significant changes were observed in the degree of pulmonary regurgitation. One neonat

Rapid evolution from "normal" left ventricular outflow tract to fatal subaortic stenosis in infancy.

Serial clinical, echocardiographic, haemodynamic, and angiocardiographic data support the conclusion that over four months severe discrete subaortic stenosis evolved from a previously angiocardiographically normal left ventricular outflow tract in an infant who had a successful repair of thoracic aortic coarctation. This case supports those who suggest that a myocardial factor may play an important role in the change in form and function of some congenitally malformed hearts.

Echocardiographic features of combined membranous subaortic stenosis and acquired calcific aortic valvulopathy.

The M-mode echocardiographic features of aortic valve structure and motion in a 45-year-old male with combined congenital subaortic diaphragm and acquired deformity of the aortic valve are described. Clinical, hemodynamic, and angiographic studies suggested calcific aortic valve disease with stenosis and insufficiency, but the additional presence of a subaortic diaphragm was not appreciated. Cardiac ultrasonography demonstrated multiple, central diastolic aortic valve cusp echoes consistent with a thickened, calcified, tricuspid aortic valve. Despite calcification of the cusps, however, enough

Accelerated ventricular rhythm in children: a review and report of a case with congenital heart disease.

We report a child with accelerated ventricular rhythm (AVR) and congenital heart disease. Three children with congenital heart defect associated with AVR were previously reported, but in each AVR occurred only postoperatively. Because our patient's 24-hour electrocardiograph recording showed AVR rates, and differences between sinus and AVR rates, exceeding published childhood limits, we reviewed the topic. On the basis of our review, we suggest guidelines for diagnosing AVR and differentiating it from ventricular tachycardia.

[Echocardiographic diagnosis of subpulmonic obstruction in hypertrophic cardiomyopathy (a case report) (author's transl)].

The echocardiogram of a patient presenting with a systolic ejection murmur and ECG evidence of left ventricular hypertrophy revealed asymmetric septal hypertrophy but no abnormal systolic anterior motion of the mitral valve as in IHSS. Her pulmonary echo disclosed small A waves, partial valve closure in early systole and coarse systolic fluttering. This motion pattern--which is similar to that of patients with infundibular pulmonary stenosis and which resembles the aortic valve motion pattern in discrete subaortic stenosis--led us to the diagnosis of hypertrophic cardiomyopathy with subpulmoni

ADAMTS19-associated heart valve defects: Novel genetic variants consolidating a recognizable cardiac phenotype.

Recently, ADAMTS19 was identified as a novel causative gene for autosomal recessive heart valve disease (HVD), affecting mainly the aortic and pulmonary valves. Exome sequencing and data repository (CentoMD) analyses were performed to identify patients with ADAMTS19 variants (two families). A third family was recognized based on cardiac phenotypic similarities and SNP array homozygosity. Three novel loss of function (LoF) variants were identified in six patients from three families. Clinically, all patients presented anomalies of the aortic/pulmonary valves, which included thickening of valve
