Anterior Cervical Diskectomy and Fusion versus Minimally Invasive Posterior Cervical Foraminotomy for the Treatment of Cervical Radiculopathy: A Meta-Analysis.

BACKGROUND: Cervical radiculopathy can be managed surgically with anterior cervical diskectomy and fusion (ACDF) or minimally invasive posterior cervical foraminotomy (MIS-PCF). ACDF is widely used and provides indirect decompression and stabilization, while MIS-PCF preserves motion, avoids fusion-related complications, and may offer advantages in perioperative recovery. The aim of this meta-analysis was to compare surgical, patient-reported, and safety outcomes of MIS-PCF versus ACDF to guide clinical decision making. METHODS: A systematic literature search of PubMed, Scopus, Cochrane Librar

Radiologic and Clinical Evaluation 12 Months after Anterior Cervical Diskectomy and Dynamic Stabilization of the Segment with Dynamic Cervical Implant.

Background Anterior cervical diskectomy (ACD) is an established therapy for degenerative cervical radiculopathy and myelopathy. Controversy remains regarding the dynamic or rigid stabilization of the segment. Methods We studied our first 31 patients treated with ACD plus the Dynamic Cervical Implant (DCI) as an alternative to fusion or to arthroplasty, treated between February 2009 and 2011. Radiologic data were collected before and 12 months after surgery. Clinical results were documented 12 months after surgery. The radiologic analysis focused on the mobility of the segment, position of the

Comparison of Fusion versus Nonunion After 4-Level and 5-Level Anterior Cervical Diskectomy and Fusion With Anterior Plate Fixation.

BACKGROUND: Nonunion remains a concern in multilevel anterior cervical discectomy and fusion (ACDF), yet there are limited data on nonunion after 4 to 5-level ACDF. In fact, the largest series on 4-level or 5-level ACDF focused specifically on the swallowing outcomes. OBJECTIVE: To assess nonunion after 4 to 5-level ACDF. METHODS: Forty-one patients treated with 4 to 5-level ACDF with minimum of 12-month radiographic follow-up were retrospectively reviewed. Nonunion was found in 25 patients (61%) and 42 levels (25%) and complete fusion in 16 (39%) patients and 126 levels (75%). The 2 groups

Delayed Esophageal Perforation Diagnosed 12 Years After Anterior Cervical Diskectomy and Fusion: A Case Report and Review of Current Literature.

Esophageal perforation associated with anterior cervical diskectomy and fusion (ACDF) is a rare but serious complication. ACDF-related esophageal perforations can be acute or delayed. Delayed perforations more than 10 years after ACDF are exceedingly rare. Here, a delayed esophageal perforation discovered 12 years after a three-level ACDF is presented. This case highlights two main points. First, all diverticula after an ACDF warrant close clinical monitoring. Second, routine follow-up should be performed for patients with screw pullout to assist in early diagnosis of delayed esophageal perfor

Comparison of Using Intraoperative Computed Tomography-Based 3-Dimensional Navigation and Fluoroscopy in Anterior Cervical Diskectomy and Fusion for Cervical Spondylosis.

OBJECTIVE: Anterior cervical diskectomy and fusion (ACDF) is a highly successful procedure to treat spinal cord or nerve root compression; however, complications can still occur. With advancements in imaging, 3-dimensional (3D) reconstruction allows real-time instrument tracking in a surgical field relative to the patient's anatomy. Here, we compare plate positioning and short-term outcomes when using 3D navigation to fluoroscopy in ACDF for degenerative spine disease. METHODS: All ACDFs for cervical spondylosis performed by 6 surgeons at a single center between 2010 and 2018 were included. A

Long-term Radiologic and Clinical Outcomes after Three-level Contiguous Anterior Cervical Diskectomy and Fusion without Plating: A Multicentric Retrospective Study.

BACKGROUND: Anterior cervical diskectomy and fusion (ACDF) has been providing good surgical, clinical, and radiologic outcomes in patients suffering from cervical degenerative disk disease (DDD). However, the role of anterior plating is still debated, especially in three-level procedures. This study aimed to investigate long-term clinical and radiologic outcomes and complications after three-level contiguous ACDF without plating for cervical DDD. METHODS: Two institutional databases were retrieved (January 2009-December 2014) for patients treated with three-level contiguous ACDF without plati
