Twenty-five years of shared life with vancomycin-resistant enterococci: is it time to divorce?

Twenty-five years ago, isolation of vancomycin-resistant Enterococcus faecium (VREm) was reported both in the UK and in France. Since then, VREm has spread worldwide in hospitals. Hospital outbreaks appeared to be related to the evolution since the end of 1980s of a subpopulation of E. faecium highly resistant to ampicillin and fluoroquinolones (the so-called clonal complex CC17) that later acquired resistance to vancomycin. CC17 isolates are presumably better adapted than other E. faecium isolates to the constraints of the hospital environment and most contain mobile genetic elements, phage g

Children of divorce in a Scandinavian welfare state: are they less affected than US children?

A fairly common view holds that children's risks of negative outcomes associated with family dissolution are generally small or even nonexistent in Scandinavia, and clearly smaller than what is usually found in the United States. This view was empirically examined in a recent large-scale study of 4,127 12-15-year-old children in Norway, of whom 623 had experienced parental divorce and lived in a single-mother family. The somewhat paradoxical pattern of findings was as follows: (a) The negative associations between parental divorce and various outcomes were found to be generally very similar in

The additive and interactive effects of parenting and temperament in predicting adjustment problems of children of divorce.

Investigated the interaction between parenting and temperament in predicting adjustment problems in children of divorce. The study utilized a sample of 231 mothers and children, 9 to 12 years old, who had experienced divorce within the previous 2 years. Both mothers' and children's reports on parenting, temperament, and adjustment variables were obtained and combined to create cross-reporter measures of the variables. Parenting and temperament were directly and independently related to outcomes consistent with an additive model of their effects. Significant interactions indicated that parental

Risk factors for bulimia nervosa: a controlled study of parental psychiatric illness and divorce.

Twenty five women with normal-weight bulimia nervosa were compared with 25 age- and weight-matched women without bulimia nervosa on measures of parental psychiatric illness. Case and control probands, as well as their parents, completed the Family History Research Diagnostic Criteria (FH-RDC) interview and a battery of self-report instruments. Case probands and controls were divided into two groups based on evidence for parental psychiatric illness. The assignment of parental psychiatric illness was made by (a) a positive parental history of alcoholism or depression from the FH-RDC; or (b) evi

After the honeymoon, the divorce: Unexpected outcomes of disease control measures against endemic infections.

The lack of effective vaccines for many endemic diseases often forces policymakers to rely on non-immunizing control measures, such as vector control, to reduce the massive burden of these diseases. Controls can have well-known counterintuitive effects on endemic infections, including the honeymoon effect, in which partially effective controls cause not only a greater initial reduction in infection than expected, but also large outbreaks during control resulting from accumulation of susceptibles. Unfortunately, many control measures cannot be maintained indefinitely, and the results of cessati

Faith, family, and stigma after parental divorce: psychological well-being among Indonesian Muslim adolescent girls.

BACKGROUND: Parental divorce can disrupt adolescents' identity, security, and belonging. In Muslim-majority Indonesia, divorce is also interpreted through cultural expectations of family unity, filial respect, family reputation, and religious meaning. These meanings may intensify stigma, but they may also provide adolescents with moral language for patience, hope, and self-restraint. OBJECTIVE: This study explored how Indonesian Muslim adolescent girls make sense of psychological well-being after parental divorce, with attention to Islamic cultural meanings, family relationships, stigma, and

Interparental conflict, not divorce, is linked to less positive marital attitudes among Chinese emerging adults.

BACKGROUND: Interparental relationship quality and attachment are theorized to be associated with young adults’ marital attitudes, yet evidence in Chinese cultural contexts is mixed. We tested whether interparental conflict versus divorce differentially relates to marital attitudes and whether meaning in life adds explanatory value. METHODS: Participants were 398 Chinese university students (mean age = 20.8 years; 40.6% male) who completed measures of parental marital status/conflict (harmonious; high conflict without divorce; divorced; bereaved), adult attachment, meaning in life, and marital

Real-world predictors of divorce among outpatients with bipolar disorder: sex differences and clinical implications.

BACKGROUND: Bipolar disorder is a mental illness characterized by recurring episodes of mania and depression and is known to cause social impairment. Additionally, it has been revealed that bipolar disorder increases the risk of divorce and loss of family member support, which can worsen the prognosis. However, there is limited evidence regarding the predictive factors of divorce among patients with bipolar disorder in real-world settings. METHODS: This study utilized an observational approach and involved psychiatrists from 176 member clinics of the Japanese Association of Neuro-Psychiatric

Parental divorce and smoking dependence in Lebanese adolescents: the mediating effect of mental health problems.

BACKGROUND: Lebanon has the highest prevalence estimates among Middle Eastern countries and Arab women regarding cigarette smoking, with 43% of men and 28% of women involved in such trends. Marital disruption is a tremendous source of irritability and discomfort that may hinder a child's healthy development, creating perturbing distress and increasing disobedience that may exacerbate smoking addiction. Additionally, Lebanese adolescents are inflicted by high emotional and economic instability levels, rendering increased susceptibility to distress and propensity to engage in addictive behavior.

Injurious REM sleep behaviour disorder in narcolepsy with cataplexy contributing to criminal proceedings and divorce.

We present a case of violent sleep-related behaviour in a patient with narcolepsy with cataplexy (NC) and REM sleep behaviour disorder (RBD), causing injuries to his wife that resulted in charge of assault, followed by a divorce case. This observation pinpoints that RBD in NC can arise early in a patient's life and that it can be violent, that pharmacological treatment for RBD has to be considered in this case, that lack (or delay) of RBD diagnoses may further exacerbate the psychosocial burden of NC, and that a general awareness of the nature of sleep related violence may avoid unfortunate le

Postdivorce living arrangements, parent conflict, and long-term physical health correlates for children of divorce.

The authors tested a biopsychosocial model in which young adults' long-term relationships with fathers and ongoing distress surrounding their parents' divorces mediated the relationship between disrupted parenting (i.e., exposure to parent conflict before the divorce and up to 5 years after, and amount of time with father postdivorce) and indicators of their physical health. University students whose parents divorced before they were 16 (n = 266) participated. Findings supported the model. The more time children lived with their fathers after divorce, the better their current relationships wer

[Multi-parent families as "normal" families--segregation and parent-child-alienation after separation and divorce].

Decisive for the question as to how children cope with their parents' divorce is whether or not the parents continue to perform their parental role together even after separation, or have at least made arrangements for the child to maintain a good relationship with each parent. These are the findings of a longitudinal study of 150 postdivorce families. The case of a multi-parent family after remarriage, which sees itself as a "normal" family and segregates the visiting parent, shows what consequences the breakdown of parent-child relationships has for the psychological health and the developme

Childhood family disruptions and adult well-being: the differential effects of divorce and parental death.

This study draws on attachment theory and social learning theory and uses data from the National Survey of Families and Households to examine the differential effects of childhood family disruptions on adult well-being. Comparisons are made between adults who experienced parental divorce, adults who experienced parental death, and adults who were raised in intact families (N = 4,341). The present study differs from previous research by making direct comparisons between different family disruption groups, assessing the effects of family disruptions that occur before age 19, and including multip

Sex differences in children's response to parental divorce: 1. Research methodology and postdivorce family forms.

This two-part review documents inconsistencies in the empirical basis for the hypothesis that boys are more negatively affected than are girls by parental divorce. An attempt is made to move beyond a global hypothesis to one specifying circumstances for the pattern. Part 1 asks whether methodological adequacy or postdivorce family forms in the studies help explain the discrepancy across studies that examine sex differences in children's response to parental divorce. Part 2, reviewing other possible sources for the discrepancy, will appear in a forthcoming issue of this Journal.

Echocardiography vs. CMR in the Quantification of Chronic Mitral Regurgitation: A Happy Marriage or Stormy Divorce?

Quantification of chronic mitral regurgitation (MR) is essential to guide patients' clinical management and define the need and appropriate timing for mitral valve surgery. Echocardiography represents the first-line imaging modality to assess MR and requires an integrative approach based on qualitative, semiquantitative, and quantitative parameters. Of note, quantitative parameters, such as the echocardiographic effective regurgitant orifice area, regurgitant volume (RegV), and regurgitant fraction (RegF), are considered the most reliable indicators of MR severity. In contrast, cardiac magneti
