Analysis of skeletal status by quantitative ultrasonometry in a cohort of postmenopausal women with high blood cholesterol without documented osteoporosis.

Osteoporosis and atherosclerosis are leading causes of mortality and morbidity in the Western world. A link between osteoporosis and atherosclerosis was proposed by epidemiologic and laboratory data. In the present study, we investigated skeletal status in postmenopausal women with hypercholesterolemia using quantitative ultrasonometry (QUS). Six hundred healthy postmenopausal subjects were enrolled within a 2-mo period by primary care physicians. Information on lifestyle and calcium intake was collected for each enrolled subject. Subjects (n = 256) were divided into two groups according to li

Regulatory Approaches to Cybersecurity Risk Management for AI-Enabled Medical Device Software in Korea, the United States, and the European Union: Comparative Document Analysis.

BACKGROUND: Software-based and AI-enabled medical devices are increasingly networked and updatable, expanding the attack surface and making cybersecurity governance intersect with quality management and postmarket oversight. Regulated device risk management nevertheless remains primarily oriented toward patient-safety harms under ISO 14971 frameworks, which may not fully capture cybersecurity risks affecting data integrity, system resilience, or service continuity. OBJECTIVE: This study aimed to compare how Korea's Ministry of Food and Drug Safety (MFDS), the US Food and Drug Administration (

Is capacity to consent to healthcare a uniformly defined concept? Insights from a document analysis of laws and national guidelines across six European countries.

BACKGROUND: Capacity to consent or decision-making capacity to healthcare is a key prerequisite for a valid informed consent. A clear understanding of this concept is important to protect patients' autonomy. Without it, clinical assessments may vary, jeopardising patients' participation in decisions about their care and increasing the risk of delayed, denied or harmful treatment. Despite substantial international debate and reforms concerning norms on capacity assessment, there remains a paucity of scholarly literature comparing national approaches, which could enable countries to learn from o

The Vaccine That Was Never Mentioned: A Comparative Medico-Legal Analysis of the Duty to Inform and Document Preventive Immunization, with Proposed Practice Guidelines.

Background/Objectives: Vaccine-preventable disease may generate medico-legal disputes not only after an adverse event following immunization, but also when a clinician is alleged to have failed to mention, recommend, revisit, refer for, or document a clinically relevant vaccine. We examine this "unmentioned vaccine" problem while distinguishing existing law (lex lata) from proposed good practice (lex ferenda). Methods: We conducted a documented purposive narrative and comparative medico-legal review. PubMed/MEDLINE, PubMed Central, structured scholarly web searches, publicly accessible legal r

A document analysis of the South African national guidelines for medical male circumcision and the implications for infant and child male circumcision decision-making.

South Africa's 2016 medical male circumcision (MMC) guidelines ("the guidelines") provide direction for the MMC programme's implementation in South Africa. The aim of our document analysis was to assess the guidelines, particularly in lieu of changing guidance from WHO and PEPFAR. We then assessed how the guidelines might be applied to infant and child male circumcision (ICMC). The analysis was performed by reviewing South Africa's guidelines, along with international documents used in developing those guidelines, to identify the historical development of the guidelines, as well as the implica

Regulating the drafting fiction: A qualitative content analysis of official guidance and regulator documents on AI scribe adoption and use in healthcare.

Artificial intelligence scribes use automatic speech recognition and generative language models to document clinical consultations, but official expectations for safe adoption are scattered across privacy, professional, medical device, safety and health-system guidance. We conducted a qualitative content analysis of 74 unique English-language official documents issued by regulators, government bodies, medical colleges and health-system organisations. Documents were identified through a staged grey literature search and coded using a hybrid deductive and inductive framework. We developed the an

International Medicinal Product Information Documents: A Quantitative Content Analysis of Instructions for Preventing, Mitigating, and Monitoring Adverse Drug Reactions.

INTRODUCTION: Medicinal product information documents (PIDs) detail clinical characteristics and instructions for monitoring, preventing, and mitigating adverse drug reactions (ADRs). They vary across countries, but there have been no recent international comparisons. We have therefore quantified and compared the completeness of the information given in drug labelling from different countries. METHODS: From the websites of 35 regulatory agencies, we retrieved the PIDs of medicinal products that were involved in signals communicated by regulators in 2014-2019. We developed a data extraction fr

Evaluation of Documented Infections by Provider Specialty in Patients with Multiple Sclerosis Receiving Disease-Modifying Therapies: A Claims Database Analysis.

INTRODUCTION: People with multiple sclerosis (pwMS) have an increased risk of infections. There are limited data characterizing who initially diagnoses infections in pwMS and how these are communicated among healthcare providers. METHODS: Retrospective US claims data from January 1, 2017 to August 31, 2024 were used to identify newly diagnosed pwMS aged 18-64 years who were taking any disease-modifying therapy (DMT) for ≥ 90 days. Common, Intermediate and Well-Documented HLA Alleles in the Brazilian Population: An Analysis of the Brazilian Bone Marrow Donor Registry (REDOME).

This study investigates the HLA allele diversity in Brazil, a reflection of the country's unique history of population admixture. The international comparison of findings emphasises the importance of incorporating underrepresented populations into global HLA databases. We present a comprehensive analysis of HLA alleles within the Brazilian population, utilising high-resolution sequencing data from 298,000 unrelated haematopoietic stem cell volunteer donors registered with the Brazilian Bone Marrow Donor Registry (REDOME). Our research encompasses donors from all regions of Brazil, identifying

Utilizing the integrated dual recording system to enhance documentation and analysis of airway management in children with difficult airway: A case report.

This case report highlights the use of an integrated Dual Recording System (iDRS) to document and analyze airway management. The iDRS combines video laryngoscopy and a wide-angle camera to capture dual-view recordings of the larynx and operating room during airway procedures. We utilized this system in a 5-month-old boy with a difficult airway who underwent pulmonary artery banding under general anesthesia. Retrospective video analysis showed discrepancies between written information and iDRS video findings, including differences in the desaturation cause, description of airway procedures, and

Mixed-Methods Analysis of Provider-Documented and Patient-Reported Urinary Tract Infection Symptoms Among Veterans With Neurogenic Bladder.

Inappropriate urinary tract infection diagnosis in patients with neurogenic bladder may result from ambiguous symptoms experienced by these patients and contributes to antibiotic overuse. Characterization of patient-reported signs and symptoms may help providers more appropriately diagnose urinary tract infections. A previous study collected signs and symptoms recorded in electronic medical records of patients with neurogenic bladder due to spinal cord injury/disorder, multiple sclerosis, and Parkinson's disease with at least one urinary tract infection diagnosis between 2017-2018 at four medi

Policy and Programming Towards Addressing Treatment Gaps in Adolescents Living with HIV: A Content Analysis of Policy and Programme Documents in Namibia.

Adolescents living with HIV (ALHIV) face unique challenges resulting in persistent treatment gaps, particularly viral non-suppression. Country programs adopt policies, guidelines, and innovations, based on WHO recommendations and best practices from elsewhere. However, it is unclear to what extent these tools address the management of adolescents with viral non-suppression. We report on a review of guidelines for the provision of HIV services to ALHIV in Namibia. We conducted a systematic document review using Content Analysis and Thematic Analysis methodology, and the READ approach. We identi

Prospective documentation and analysis of the pre- and early clinical management in severe head injury in southern Bavaria at a population based level.

Treatment of patients suffering from severe head injury is so far restricted to general procedures, whereas specific pharmacological agents of neuroprotection including hypothermia have not been found to improve the outcome in clinical trials. Albeit effective, symptomatic measures of the preclinical rescue of patients (i.e. stabilization or reestablishment of the circulatory and respiratory system) or of the early clinical care (e.g. prompt diagnosis and treatment of an intracranial space occupying mass, maintenance of a competent circulatory and respiratory system, and others) by and large c

[Endocrinologic after-care of brain tumor patients: initial retrospective analysis of HIT-study documented auxologic and endocrine parameters].

BACKGROUND: In 1987, a pilot trial (HIT 88/89) was initiated by GPOH to investigate the effectivity and toxicity of intense preradiation chemotherapy. The aim of this study was to improve the treatment of children with malignant brain tumors, but provided no standardized recording of auxological and/or endocrinological parameters. In 1993 a sub-project, "Endocrinological late effects", was started during the ongoing study to examine the late effects of treatment, and a retrospective evaluation was performed in order to standardize parameters. PATIENTS: We considered auxological and hormonal d

Contextualizing Changes in e-Cigarette Use During the Early COVID-19 Pandemic and Accompanying Infodemic ("So Much Contradictory Evidence"): Qualitative Document Analysis of Reddit Forums.

BACKGROUND: Understanding how social media platforms facilitate information exchange and influence behavior during health crises can enhance public health responses during times of uncertainty. While some risk factors for COVID-19 susceptibility and severity (eg, old age) were clear, whether e-cigarette use increased risk was not clear. People who used e-cigarettes had to navigate both the COVID-19 infodemic and a conflicting, politicized, and changing information environment about the interaction between COVID-19 and e-cigarette use. OBJECTIVE: This study aims to characterize and contextuali

Readability and complexity of written information presented to hospitalised patients for trial consent during the COVID-19 pandemic in the UK: a retrospective document analysis.

OBJECTIVES: Patient information sheets (PISs) and informed consent forms (ICFs) are essential tools to communicate and document informed consent for clinical trial participation. These documents need to be easily understandable, especially when used to take informed consent from acutely unwell patients. Health literacy guidance recommends written information should be at a level between reading ages 9-11. We aimed to assess the readability and complexity of PISs/ICFs used for clinical trials of acute therapies during the COVID-19 pandemic. DESIGN: Retrospective document analysis. SETTING: PI

A Retrospective Analysis Demonstrates That a Failure to Document Key Comorbid Diseases in the Anesthesia Preoperative Evaluation Associates With Increased Length of Stay and Mortality.

BACKGROUND: The introduction of electronic health records (EHRs) has helped physicians access relevant medical information on their patients. However, the design of EHRs can make it hard for clinicians to easily find, review, and document all of the relevant data, leading to documentation that is not fully reflective of the complete history. We hypothesized that the incidence of undocumented key comorbid diseases (atrial fibrillation [afib], congestive heart failure [CHF], chronic obstructive pulmonary disease [COPD], diabetes, and chronic kidney disease [CKD]) in the anesthesia preoperative e

Ischemic heart disease mortality with documented tobacco use in the United States, 1999-2023: a nationwide population-based analysis.

BACKGROUND: Tobacco use is a major risk factor for ischemic heart disease (IHD), substantially increasing the likelihood of myocardial infarction, stroke, and premature mortality. This study examined U.S. trends in IHD mortality with documented tobacco use disorder from 1999 to 2023. METHODS: CDC mortality data for adults aged ≥ 25 years with IHD (ICD-10 codes: I20-I25) and documented tobacco use (ICD-10 code: F17) listed as contributing causes of death were analyzed. Age-adjusted mortality rates (AAMRs) per 100,000 population were calculated. Joinpoint regression was used to estimate annual
