Academy of breastfeeding medicine founder's lecture 2009: Maternity care re-evaluated.

Abstract In the 1990s a rising tide of medical, surgical, and instrumental interventions served to make childbirth almost treated like a disease. This report supports a different approach to childbirth. A case and discussions of induction are presented. A national survey of 1,573 pregnant women throughout the United States was collected. Although most U.S. childbearing women are low risk, childbirth is "procedure intensive." Women reported significant interventions such as regional analgesia (76%), ruptured membranes (65%), forceful pushing (75%), and cesarean sections (32%). The U.S. rate of

"Like I Wasn't There, Except They Knew I Was": A Qualitative Analysis of Latina Perinatal and Postpartum Experiences in Support of Self-care.

OBJECTIVES: The purpose of this qualitative study was to understand the perinatal experiences of Latinas from pregnancy through the first year postpartum to inform the creation of a web-based tool to support postpartum self-care. This study is the second of a two-phase effort to gain insights into women's experiences and challenges with self-care and their recommendations for improving the perinatal and postpartum experience for other women. METHODS: Twenty-one Latinas (N = 21) between six months pregnant and six months postpartum participated in one of 10 conversations conducted in English o

Digital cognitive behavior therapy to reduce perinatal anxiety symptoms: perceptions of implementation barriers and facilitators among providers serving historically marginalized communities.

BACKGROUND: Perinatal anxiety is prevalent, particularly among historically marginalized groups who face barriers to accessing interventions like cognitive behavioral therapy (CBT). While digital CBT adaptations for this population show promise, research on implementation remains limited. PURPOSE: This study engaged potential implementers-perinatal clinicians, doulas, and psychiatrists-to explore perceived barriers and facilitators of digital CBT (BigHealth's Daylight app) use among racially marginalized and low-income perinatal people. METHODS: As part of the HOPE THRIVE study (transforming

Applying human-centered design to identify needs and generate recommendations to inform an integrated maternal health demonstration model.

BACKGROUND: Maternal mortality continues to rise in concerning rates in the United States. Maternal mortality risks are increased by clinical factors like cardiovascular disease or infection and nonclinical factors like food insecurity, unequal access to healthcare, and systemic racism. Integrated care coordination programs aimed at addressing care needs are emerging as best practices to improve maternal health outcomes at and post birth. Our goal was to (i) evaluate the care needs during pregnancy and postpartum and (ii) formulate recommendations for integrating a clinic-level model and commu

Community-Led Solutions to Address Black Maternal and Infant Mortality Through the TNT-PISP Model: A Qualitative Study.

OBJECTIVES: In both the U.S. and Wisconsin, Black women and infants experience significantly higher rates of morbidity and mortality than their white counterparts. Our research team set out to explore how a community-based and culturally informed perinatal support model could address the needs of Black mothers and their families. METHODS: We developed and implemented the Today Not Tomorrow Pregnancy and Infant Support Program (TNT-PISP), a community-based, culturally informed perinatal support model integrated with traditional obstetrical care. From October 2019 to August 2022, we held monthl

Experiences of Maternal Autonomy in Childbirth and Perinatal Care in the United States: A Systematic Review and Meta-Synthesis of Qualitative Studies.

BACKGROUND: Over the past decade, major legal and policy shifts have reconfigured the reproductive health landscape in the United States (US), renewing interest in reproductive and bodily autonomy. While much of this attention has focused on abortion access, autonomy during labor, delivery, and the immediate postpartum period remains underexamined. This review aimed to synthesize qualitative evidence on experiences of maternal autonomy in childbirth and the factors that influence perinatal care decision-making. METHODS: We searched Ovid MEDLINE, EMBASE, Scopus, JSTOR, and ClinicalTrials.gov f

Spheres of influence on contraceptive decision-making: qualitative interviews with postpartum mothers and providers.

BACKGROUND: Biological and behavioral factors influence interpregnancy intervals. Individualized family planning goals and preferences regarding contraception must be supported through shared decision-making approaches. Contraceptive counseling throughout the perinatal period is multifaceted whereby intersecting health-related choices including breastfeeding are co-occurring. This study explores multi-level factors influencing postpartum contraceptive decision-making among women and perinatal care providers in a rural region of the Southeast United States. METHODS: The qualitative study trian

Labour companionship and women's experiences of mistreatment during childbirth: results from a multi-country community-based survey.

BACKGROUND: Evidence has shown the benefits of labour companions during childbirth. Few studies have documented the relationship between the absence of labour companions and mistreatment of women during childbirth in low-income and middle-income countries using a standardised tool. METHODS: We conducted a secondary analysis of the WHO multi-country study on how women are treated during childbirth, where a cross-sectional community survey was conducted with women up to 8 weeks after childbirth in Ghana, Guinea, Nigeria and Myanmar. Descriptive analysis and multivariable logistic regression wer

Contribution of Health Care Practitioner and Maternity Services Factors to Racial Disparities in Alabama: A Qualitative Study.

OBJECTIVE: Black patients are three times more likely to die of pregnancy-related causes than White patients in the United States, and Alabama has the third worst maternal mortality rate in the nation. We sought to identify health care practitioner and maternity service factors contributing to disparities in Alabama, as well as potential strategies to address these contributors. METHODS: We conducted key informant interviews with obstetricians, nurses, doulas, lactation counselors, health system administrators, and representatives of professional organizations who deliver maternity care to ra

Listening to the Perspectives of Black Women on Perinatal Health Disparities: Reversing the Tide and Improving Outcomes.

INTRODUCTION: Pregnancy-related health in the USA fares worse than similarly resourced countries and the gap continues to widen. This trend however is disproportionately experienced by women of color. We have come to understand that this is due to the systems and structures that perpetuate racism. Despite our awareness of this, the voices of the community impacted by these systems have been largely left out of research. The authors aimed to utilize participatory research methods within a qualitative design to ask Black pregnant and parenting women about their lived experiences of pregnancy and

Reimagining maternal and infant wellbeing through AI-embedded integrative arts-based care: a public health perspective.

The peripartum period and early parenting are critical phases for maternal health, infant development, family functioning, and long-term wellbeing. Although maternal and child health services have traditionally prioritized pregnancy safety, childbirth security, and neonatal survival, contemporary public health agendas increasingly emphasize maternal mental health, positive birth and postnatal experiences, parent-infant bonding, and family-centered support. Evidence suggests that music therapy, mindful music listening, artistic expression, doula support, aromatherapy, parent-infant singing, and

Family practice maternity care in America: ruminations on reproducing an endangered species--family physicians who deliver babies.

BACKGROUND: The majority of family physicians do not deliver babies. One reason might be the family physician's intrinsic comfort with person- or patient-centered care compared with the common obstetric approach of disease or physician-centered maternity care. Another reason might be the uncritical intrusion of technology into maternity care. In addition, family physicians often are made to feel unwelcome in many maternity care systems. METHODS: The medical literature from 1984 to 1994 was searched for the topics of obstetrics, maternity care, family-centered birthing, and family practice edu

Startup activities and challenges implementing an integrated care model for pregnant and postpartum individuals with substance use disorders: a qualitative study.

BACKGROUND: Substance use has increased among pregnant and postpartum people in the last decade, yet few pregnant individuals receive prenatal care and treatment for substance use disorder (SUD). The Nurture Oregon model aims to integrate medical care with SUD treatment and provide destigmatized care through regular visits in an integrated care setting. We studied one family medicine, one behavioral health, and two SUD treatment organizations in rural counties with high SUD rates and limited resources that were funded to implement the model. METHODS: To examine the startup and early implement

Utilisation of complementary and alternative medicine (CAM) practitioners within maternity care provision: results from a nationally representative cohort study of 1,835 pregnant women.

BACKGROUND: There is little known about women's concurrent use of conventional and complementary health care during pregnancy, particularly consultation patterns with complementary and alternative medicine (CAM). This study examines health service utilisation among pregnant women including consultations with obstetricians, midwives, general practitioners (GPs) and CAM practitioners. METHODS: A sub-study of pregnant women (n=2445) was undertaken from the nationally-representative Australian Longitudinal Study on Women's Health (ALSWH).
