Drug-impaired driving is a growing public safety issue. Addressing impairment due to drugs other than ethanol can be challenging for forensic toxicologists as many factors need to be considered including the type of drug(s), drug-drug interaction, the dose(s) and the individual's physiological condition and drug use history. Interpretation of blood drug test results is additionally difficult as drug concentrations in impaired driving cases may overlap levels typically viewed as toxic. This study compares blood concentrations of drugs in impaired driving cases to those in postmortem cases in Ho

INTRODUCTION: In Canada, alcohol-impaired driving is a persistent public health concern in need of effective community-based prevention strategies. This study examined the association between the number of rides offered by a safe-ride program in the province of Quebec every December and alcohol-related injury crashes during the 2000-2019 period.
METHOD: Safe-ride programs in four cities were examined (Montreal, Quebec, Sherbrooke and Trois-Rivières) using an ecological approach. The data set was structured as a balanced cross-sectional time series. Random-effects negative binomial regression

OBJECTIVE: The purpose of this study was to investigate factors contributing to the decline in the number of passengers riding with alcohol-impaired drivers involved in fatal crashes since 1982 and to examine the impact of simulated interventions on this group through 2050.
METHOD: Historical data were obtained from the Fatality Analysis Reporting System. We applied linear regression to analyze changes in the average numbers of passengers per alcohol-impaired young driver involved in fatal crashes between 1982 and 2020 by age and sex. We also extended our existing system dynamics simulation m

OBJECTIVE: Alcohol-impaired driving results in thousands of deaths annually. Alcohol ignition interlocks require a negative breath test to start a vehicle's engine, and 44 states have mandated some form of interlock law for drivers convicted of driving while intoxicated (DWI). The objective of this study was to estimate the association between interlock laws and fatal impaired-driving crashes.
METHODS: Differences in three interlock laws were evaluated by comparing alcohol-impaired passenger vehicle drivers involved in fatal crashes between 2001 and 2019 in the United States across state and

INTRODUCTION: Alcohol impairment is a major contributor to road traffic crashes and has increased across the United States in recent years. In 2022, over 13,000 people were killed in drunk driving crashes. Enforcement of impaired driving laws is an essential strategy to reduce alcohol-impaired driving and subsequent crashes. However, little is known about conviction outcomes related to alcohol-involved crashes. The aim of this study is to examine the association between charge combinations and conviction rates among alcohol-influenced drivers involved in crashes.
METHODS: Data for this study

In the United States, nearly 28 people die in alcohol-related motor vehicle crashes every day (1 fatality every 52 min). Over decades, states have enacted multiple laws to reduce such fatalities. From 1982 to 2019, the proportion of drivers in fatal crashes with a blood alcohol concentration (BAC) above 0.01 g/dl declined from 41% to 22%. States vary in terms of their success in reducing alcohol-related crash fatalities. The purpose of this study was to examine factors associated with changes in fatalities related to alcohol-impaired driving at the state level. We created a panel dataset of 50

This study aimed to investigate the predictors of recidivism in first-time driving under the influence (DUI) offenders, analyzing variables derived from medico-legal and toxicological examinations. The research was structured as a comparative study for the period 2012-2019. DUI offenders with a blood alcohol concentration >0.5 were included in the study. The case group consisted of recidivist offenders, while the comparison group consisted of first-time offenders. Personal data, socioeconomics, and parameters linked to the DUI were compared between the two groups. Significance was determined b

Cocaine is an illicit drug frequently encountered by forensic practitioners in driving under the influence of drugs (DUID) casework. Whole blood collected from a suspected drugged driver was found to contain 3.000 mg/L cocaine, 1.600 mg/L benzoylecgonine, and 0.260 mg/L methamphetamine. The high concentration of cocaine, while common in overdose death investigations, is unusual for an impaired driving case. Information from the officer revealed that the motorist swallowed cocaine during the traffic stop. Although a cocaine DUID charge could not be pursued, the blood methamphetamine concentrati

OBJECTIVE: The objective of this study was to look for dose- and concentration-effect relationships in experimental studies on single-dose administration of morphine on traffic-relevant behavioral tests by a systematic literature review and possibly to see whether a dose/concentration could be defined below which few or no tests would be affected.
METHODS: Searches for corresponding literature were conducted using MEDLINE, EMBASE, and PsycINFO, throughout March of 2016. The search strategy consisted of words colligated to cognitive and psychomotor functions of relevance to driving, in relatio

BACKGROUND: In recent years, electric scooters (e-scooter) have emerged as an alternative mode of urban transport due to their availability and effortless use. However, e-scooter-related trauma and injuries, especially to the head, have received wide media coverage and raised public concern about their safety. We aim to determine and compare clinically relevant variables, incidence, and severity between bicycle and e-scooter-related facial fractures and potential protective measures for injury prevention.

BACKGROUND AND AIM: Harmful alcohol use can be a sensitive topic, and discussions with patients about alcohol can be challenging. In some cases, alcohol use can result in patients being deemed unfit to drive. The main aim of this study was to explore general practitioners' experiences with their dual role as clinician and expert assessor in relation to patients' alcohol use.
MATERIAL AND METHOD: This is a qualitative study in which general practitioners attending a seminar on changing alcohol habits in general practice were invited to participate. Audio recordings from group discussions in fo

Driving while intoxicated (DWI) or driving under the influence of drugs (DUID) poses a continued public safety risk in Texas, which has one of the highest alcohol impaired traffic fatality rates. This study aimed to identify alcohol and drug use trends seen in DWI/DUID cases in the city of Houston from 2014 to 2018 to better understand the populations at risk. Blood samples submitted to the Houston Forensic Science Center (HFSC) were evaluated for blood alcohol concentration (BAC), drug concentrations and demographics. During the 5-year period, 12,682 Houston driver blood samples had a mean (m

Assessing fitness to drive in applicants with a historical or current substance use disorder presents a specific clinical challenge. The Australian guidelines require evidence of remission and absence of cognitive change when considering applications for re-licensing driver or individuals applying to reengage in safety-sensitive work. This paper reviews some of the clinical and biochemical indicators that determine whether a particular person is in 'remission' and meets the criteria for return to driving or other safety-sensitive occupation. It provides an overview of the challenges in establi

INTRODUCTION: Alcohol taxation is a widely used public policy tool to reduce alcohol-related harm. However, studies directly examining the impact of alcohol taxes on alcohol-related crashes remain limited, particularly in middle-income countries. This study investigates how changes in specific alcohol excise taxes in Türkiye affect the number of drunk drivers involved in traffic crashes.
METHOD: For this purpose, the study uses a monthly province-level panel dataset covering 81 provinces and 10,692 observations between 2013 and 2023. A product-weighted real tax index is constructed by weighti

INTRODUCTION: Drink driving is a dangerous behavior associated with significant road trauma. The ability to estimate one's alcohol plays an important role in the decision to drink and drive. This systematic review aimed to synthesize the evidence regarding what factors are associated with the accuracy of self-estimated blood and breath alcohol concentrations (BAC/BrAC) and discuss relevant implications for drink driving.
METHOD: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses, a literature search was conducted on four databases, followed by a manual search usi

On-site drug screening of oral fluid samples has gained attention because of its convenience and rapid results. The aim of this investigation was to compare the results of preliminary screening for drugs in oral fluid samples collected from suspected drug-impaired drivers using DrugWipe 6S and WipeAlyser reader with the results obtained from blood samples. Additionally, we compared the DrugWipe test results with findings of drug traces detected within the used DrugWipe devices. Police officers selected a sample of 355 suspected drug-impaired drivers in 2023. They used DrugWipe 6S for prelimina

BACKGROUND: Motor vehicle collisions (MVC) continue to be a leading cause of mortality for youth in the United States. Since 2010, seven states have revoked mandatory laws that suspended licenses for underage alcohol use, also known as use/lose laws. This study analyzed whether each state's policy change was associated with increased youth MVC mortality.
METHODS: State mortality data for youth ages 15 years to 20 years in MVCs involving a young driver (i.e., ages 15-20 years) were obtained from the Fatality Analysis Reporting System. Population data was retrieved from the Centers for Disease
