Successful Drug-eluting Stent Implantation in a Male Patient with Dextrocardia: A Case Report.

Situs inversus with dextrocardia is a rare condition, with complete transposition of all the body organs, including the heart. Percutaneous coronary intervention (PCI) in these patients is technically difficult because of the mirror image of organs. Here, we describe a 56-year-old man with coronary heart disease with known situs inversus with dextrocardia and coronary percutaneous intervention was performed for stenosis in the right coronary artery. A drug eluting stent was implanted at this site successfully. This case suggested that the interventional management of such patients follows the

Novel route of insulin delivery using an implant-mediated drug delivery system.

Patients with diabetes mellitus (DM) frequently require multiple painful injections of insulin to manage their blood glucose level. A newly introduced, modified titanium implant, the implant-mediated drug delivery system (IMDDS), has the advantage of sustained, needle-free drug delivery. We evaluated the feasibility of the IMDDS as an alternative route of insulin delivery in an alloxan-induced rabbit DM model. Among a total of 30 rabbits with IMDDSs, 25 survived the induction and had a blood glucose level (BGL) higher than 300 mg/dl. The animals were divided into two groups: 13 in the test gro

Everolimus-induced pneumonitis after drug-eluting stent implantation: a case report.

Despite the wide use of everolimus as an antineoplastic coating agent for coronary stents to reduce the rate of restenosis, little is known about the health hazards of everolimus-eluting stents (EES). We describe a case of pneumonitis that developed 2 months after EES implantation for angina. Lung pathology demonstrated an organizing pneumonia pattern that responded to corticosteroid therapy. Although the efficacy of EES for ischemic heart disease is well established, EES carries a risk of pneumonitis.

[Withdrawal in a patient with an implantable drug delivery system in spite of continuation of opiate therapy].

A male patient with severe chronic pain has been treated with a combined drug therapy scheme of morphine and clonidine. After cessation of clonidine the patient reported opiate-withdrawal symptoms with agitation, polyuria, diarrhea, and hyperalgesia. It is known that the combination of these two substances causes synergistic analgetic effects. The abrupt cessation of clonidine after a combined administration with morphine seems to lead to a relative decrease in opiate effects and furthermore excite opiate-withdrawal symptoms.

Impact of periprocedural myonecrosis on clinical events after implantation of drug-eluting stents.

BACKGROUND: High level of creatine kinase myocardial band isoenzyme (CK-MB) elevation has been associated with late mortality after coronary intervention. We sought to evaluate the impact of periprocedural myonecrosis on clinical events in patients undergoing drug-eluting stents (DES) implantation. METHODS: A total of 1807 patients (2550 lesions) with successful DES implantation were followed for mean duration of 13 + or - 7 months. Patients with acute myocardial infarction and those with elevated CK-MB at baseline were excluded. Based on the CK-MB levels after stenting, patients were classif

A totally implantable drug delivery system for local therapy of the middle and inner ear.

Local therapy of middle and inner ear diseases is being used, but is restricted to cases of ear drum perforation or to repeated invasive intratympanic drug application by the physician. In accordance with the Medical Device Directive (class III), a bone-anchored, totally implantable drug delivery system (TI-DDS) has been developed. It includes a micropump for subcutaneous, patient-controlled activation, a drug reservoir and a septum port. A thin guide-wired catheter leads from the pump outlet to the point of application in the mastoid or middle ear cavities. Local inner ear therapy with suitab

A new vitreal drug delivery system using an implantable biodegradable polymeric device.

PURPOSE: The authors evaluated the feasibility of using an implantable biodegradable polymeric device to deliver drugs into the vitreous humor. METHODS: Two types of devices were prepared by compression-molding polymers of poly(DL-lactic acid) of two different molecular weights. The molecular weights of the poly(DL-lactic acid) used were 5,600 (device-1) and 9,100 (device-2). Sodium fluorescein (NaF) served as a hydrophilic drug marker. The release of the dye from the devices was studied in vitro. The intravitreal kinetics of NaF was evaluated in rabbits in vivo by fluorophotometry. The eyes

Clinical Effects of the U.S. Food and Drug Administration's Newest Restrictions on the Etonogestrel Implant.

In 2026, the U.S. Food and Drug Administration introduced a new Risk Evaluation and Management Strategy (REMS) for the etonogestrel contraceptive implant. Although the intent was to maintain safe practices for device insertion and removal, the large body of long-standing safety data should raise questions about the need for these new requirements. In the absence of safety concerns regarding device insertion and removal, a REMS requirement on the etonogestrel contraceptive implant may lead to decreased reproductive health care access and provision.

From implantable to wearable: recent advances in microfluidic platforms for precision drug delivery.

Wearable and implantable microfluidic drug delivery systems have emerged as promising platforms that address key limitations of conventional systems, including poor dosing control, variable bioavailability, and systemic toxicities. By enabling precise manipulation of fluids at the microscale, microfluidic systems could provide precise control over drug dosing rate, timing, and localization, supporting continuous or controlled patient-centered therapy. Recent advances in materials science, microfabrication, and soft bioelectronics have further enabled devices that conform to body tissues, opera

Meta-analysis of outcomes from drug-eluting stent implantation in femoropopliteal arteries.

OBJECTIVE: In recent years, studies of drug-eluting stent (DES) for femoropopliteal artery diseases (FPADs) have been gradually published. To explore whether this type of stent is superior to the traditional bare metal stent (BMS), we performed this study. METHODS: A systematic search for randomized controlled trials (RCTs) in Excerpta Medica Database (Embase), PubMed, Web of Science (WOS), and Cochrane Library was performed on November 29, 2022. We innovatively adopted the hazard ratio (HR), the most appropriate indicator, as a measure of the outcomes that fall under the category of time-to-

Assessing platelet reactivity after drug eluting stent implantation: state of the art.

Introduction: Platelets play a pivotal role in the occurrence of recurrent ischemic events in coronary artery disease patients who are treated with drug-eluting stents and are on dual antiplatelet therapy (DAPT).Areas covered: High platelet reactivity (HPR) to adenosine diphosphate during clopidogrel therapy is a strong predictor of post-stenting ischemic event occurrences. However, uniform use of potent P2Y12 receptors blockers to overcome HPR is associated with elevated bleeding risk. Selective de-escalation of P2Y12 receptor blocker therapy based on PFT in patients with acute coronary syndr

Neoatherosclerosis after drug-eluting stent implantation: a novel clinical and therapeutic challenge.

The recognition that obstructive disease of the epicardial coronary arteries, causing ischaemic heart disease, can be treated with a percutaneous coronary intervention (PCI) has been a major discovery in cardiology in the last 40 years contributing, in particular, to the reduction of mortality associated to acute myocardial infarction (AMI). However, even in the era of drug-eluting stent (DES) implantation, a sizable proportion of patients who undergo PCI may develop late or very late post-implantation complications, that occur in the form of restenosis, neoatherosclerosis, and/or in-stent thr

Long-term drug delivery using implantable electrospun woven polymeric nanotextiles.

A woven nanotextile implant was developed and optimized for long-term continuous drug delivery for potential oncological applications. Electrospun polydioxanone (PDS) nanoyarns, which are twisted bundles of PDS nanofibres, were loaded with paclitaxel (PTX) and woven into nanotextiles of different packing densities. A mechanistic modeling of in vitro drug release proved that a combination of diffusion and matrix degradation controlled the slow PTX-release from a nanoyarn, emphasizing the role of nanostructure in modulating release kinetics. Woven nanotextiles, through variations in its packing
