[Efficacy and tolerability of long term topiramate in drug resistant epilepsy in adults].

OBJECTIVE: With the objective of evaluating the efficacy and tolerability of topiramate (TPM) in resistant epilepsy, we did a retrospective, open, multicentric analysis of 56 patients aged over 15 years in whom TPM was given as the second, third or fourth drug. All patients had already been on treatment for at least 18 months when topiramate was started. The average follow up was 27.2 months. RESULTS: At the close of the study 16% of the patients were asymptomatic, 23% had a 75% reduction in seizures and 36% a 50% reduction. The drug was withdrawn in 25% of the cases. The adverse effects note

Mechanisms used by cancer cells to tolerate drug-induced replication stress.

Activation of oncogenes in cancer cells forces cell proliferation, leading to DNA replication stress (RS). As a consequence, cancer cells heavily rely on the intra S-phase checkpoint for survival. This fundamental principle formed the basis for the development of inhibitors against key players of the intra S-phase checkpoint, ATR and CHK1. These drugs are often combined with chemotherapeutic drugs that interfere with DNA replication to exacerbate RS and exhaust the intra S-phase checkpoint in cancer cells. However, drug resistance impedes efficient clinical use, suggesting that some cancer cel

A multiplexable assay for screening antibiotic lethality against drug-tolerant bacteria.

Antibiotic screens typically rely on growth inhibition to characterize compound bioactivity-an approach that cannot be used to assess the bactericidal activity of antibiotics against bacteria in drug-tolerant states. To address this limitation, we developed a multiplexed assay that uses metabolism-sensitive staining to report on the killing of antibiotic-tolerant bacteria. This method can be used with diverse bacterial species and applied to genome-scale investigations to identify therapeutic targets against tolerant pathogens.

A Well-Tolerated and Effective Antiepileptic Drug for Patients With Myasthenia Gravis at Last?

Antiepileptic drugs have been known to worsen myasthenia gravis (MG) symptoms, and therefore, in patients who suffer both conditions (myasthenia and epilepsy), treatment selection is difficult. We report 2 cases of patients with MG who were safely treated with lacosamide. Evidence about antiepileptic drug treatment and adverse effects in MG is reviewed.

Tolerance to the increase of striatal homovanillic acid elicited by several anorectic drugs.

Several anorectic drugs affect dopamine metabolism in the rat striatum, increasing the concentration of homovanillic acid (HVA). However 1- and d-amphetamine and mazindol develop a tolerance to the HVA increase. On the contrary, the effect of fenfluramine and S 992 is not reduced by sub-chronic treatments. Moreover, a cross-tolerance to this biochemical effect develops between amphetamine and mazindol, but pretreatment with fenfluramine or with S 992 does not induce cross-tolerance to amphetamine.

The Role of Body Composition on Drug Tolerance in Cancer Therapy: A Structured Narrative Review.

PURPOSE OF REVIEW: The aim of the present review is to analyse the current knowledge on how body composition parameters, principally low muscle mass (frequently reported as "sarcopenia") and myosteatosis (low muscle quality), relate to drug tolerance and the occurrence of adverse events during cancer therapy across different malignancies. RECENT FINDINGS: Because dosing based on weight or body surface area does not capture body composition, its potential limitations have increasingly been questioned. The available research spans a highly heterogeneous set of studies with inconsistent results.

Non-antifungal medications administered during fungal infections drive drug tolerance and resistance in Candida albicans.

Introduction. Fungal infections are increasingly concerning, particularly in immunocompromised patients. These patients often suffer from comorbidities and receive multiple, non-antifungal medications.Gap Statement. The effects of these co-administered medications on fungal cells - and their potential to influence antifungal drug efficacy - are poorly understood.Aim. This study investigates non-antifungal medications commonly administered in parallel to antifungals and evaluates their impact on fungal susceptibility.Methodology. We systematically reviewed clinical guidelines to identify non-an

Incorporation of macrophage immune stresses into an intracellular assay of drug tolerance in Mycobacterium tuberculosis.

Development of new and improved tuberculosis (TB) chemotherapies is hampered by antibiotic resistance and drug tolerance by Mycobacterium tuberculosis (Mtb). Phenotypic drug tolerance, a phenomenon where Mtb populations can temporarily survive therapeutic antibiotic concentrations, represents a significant hurdle to TB treatment and is indeed one of the factors responsible for prolonged TB therapy. Assays that can identify compounds with improved efficacy against drug-tolerant Mtb are urgently required to improve TB treatment regimens. Here, we report the development of a 96-well plate assay c

The conserved protein DopA is required for growth, drug tolerance and virulence in Aspergillus fumigatus.

The majority of Aspergillus fumigatus reproduction occurs asexually, with large numbers of conidiophores producing small hydrophobic conidia dispersed aerially. When healthy hosts inhale conidia, the mucosal cilia and phagocytosis by the innate immune system can remove them. However, in immunocompromised hosts, the conidia are not removed, which allows them to germinate, forming mycelium that invades host tissues and causes disease. Previously we isolated a white A. fumigatus A1j strain incapable of producing conidia and screened several genes (including dopA) with significant expression diffe

Antifungal Tolerance and Resistance Emerge at Distinct Drug Concentrations and Rely upon Different Aneuploid Chromosomes.

Antifungal drug tolerance is a response distinct from resistance, in which cells grow slowly above the MIC. Here, we found that the majority (69.2%) of 133 Candida albicans clinical isolates, including standard lab strain SC5314, exhibited temperature-enhanced tolerance at 37°C and 39°C, and were not tolerant at 30°C. Other isolates were either always tolerant (23.3%) or never tolerant (7.5%) at these three temperatures, suggesting that tolerance requires different physiological processes in different isolates. At supra-MIC fluconazole concentrations (8 to 128 μg/mL), tolerant colonies emerged

The chemotherapeutic drug carboplatin affects macrophage responses to LPS and LPS tolerance via epigenetic modifications.

Following re-exposure to lipopolysaccharide (LPS), macrophages exhibit an immunosuppressive state known as LPS tolerance, which is characterized by repressed proinflammatory cytokine production. LPS-induced tolerance in macrophages is mediated in part by epigenetic changes. Carboplatin, an anticancer chemotherapeutic drug, exerts its effect by inhibiting DNA replication and transcription, as well as through epigenetic modifications. Through an unbiased screen, we found that carboplatin rescued TNF-α and IL-6 production in LPS-tolerant macrophages. Transcriptomic analysis and gene set enrichmen

Inhibiting Drivers of Non-mutational Drug Tolerance Is a Salvage Strategy for Targeted Melanoma Therapy.

Once melanomas have progressed with acquired resistance to mitogen-activated protein kinase (MAPK)-targeted therapy, mutational heterogeneity presents a major challenge. We therefore examined the therapy phase before acquired resistance had developed and discovered the melanoma survival oncogene MITF as a driver of an early non-mutational and reversible drug-tolerance state, which is induced by PAX3-mediated upregulation of MITF. A drug-repositioning screen identified the HIV1-protease inhibitor nelfinavir as potent suppressor of PAX3 and MITF expression. Nelfinavir profoundly sensitizes BRAF

Apparent tolerance of turkey vultures (Cathartes aura) to the non-steroidal anti-inflammatory drug diclofenac.

The nonsteroidal anti-inflammatory drug diclofenac is extremely toxic to Old World Gyps vultures (median lethal dose -0.1-0.2 mg/kg), evoking visceral gout, renal necrosis, and mortality within a few days of exposure. Unintentional secondary poisoning of vultures that fed upon carcasses of diclofenac-treated livestock decimated populations in the Indian subcontinent. Because of the widespread use of diclofenac and other cyclooxygenase-2 inhibiting drugs, a toxicological study was undertaken in turkey vultures (Cathartes aura) as an initial step in examining sensitivity of New World scavenging

Tolerance Evaluation of Nonsteroidal Anti-Inflammatory Drug Hypersensitivity in Children: Is Age the Crucial Factor?

INTRODUCTION: Little is known about the natural history of pediatric nonsteroidal anti-inflammatory drug hypersensitivity (NSAID-H). The aim of this prospective study was to evaluate tolerance development in pediatric patients with confirmed, immediate NSAID-H and to determine the risk factors for NSAID-H persistence. METHODS: Children with a confirmed diagnosis of NSAID-H were assessed for tolerance by drug provocation test (DPT) at least 3 years after diagnosis. Factors associated with developing tolerance in NSAID-H were investigated. RESULTS: Of the 34 cases with confirmed NSAID-H diagno

Ketoconazole induces reversible antifungal drug tolerance mediated by trisomy of chromosome R in Candida albicans.

BACKGROUND: The emergence of tolerance to antifungal agents in Candida albicans complicates the treatment of fungal infections. Understanding the mechanisms underlying this tolerance is crucial for developing effective therapeutic strategies. OBJECTIVE: This study aims to elucidate the genetic and molecular basis of ketoconazole tolerance in C. albicans, focusing on the roles of chromosomal aneuploidy, Hsp90, and calcineurin. METHODS: The wild-type C. albicans strain SC5314 was exposed to increasing concentrations of ketoconazole (0.015-32 μg/mL) to select for tolerant adaptors. Disk diffusi

Acquired Tolerance to Ivermectin and Moxidectin after Drug Selection Pressure in the Nematode Caenorhabditis elegans.

Ivermectin and moxidectin are the most widely administered anthelmintic macrocyclic lactones (MLs) to treat human and animal nematode infections. Their widespread and frequent use has led to a high level of resistance to these drugs. Although they have the same mode of action, differences in terms of selection for drug resistance have been reported. Our objective was to study and compare changes occurring upon ivermectin or moxidectin selection in the model nematode Caenorhabditis elegans C. elegans worms were submitted to stepwise exposure to increasing doses of moxidectin. The sensitivity of

Preexposure to drug administration context blocks the development of tolerance to sedative effects of diazepam.

The development of tolerance to benzodiazepine (BDZ) and other psychoactive agents such as morphine, alcohol, and barbiturates is thought to be a contingent or learning phenomenon. In a previous report, we demonstrated a positive correlation between the development of tolerance to the sedative effects of diazepam (DZ) and hippocampal synaptic plasticity. The results of the present work show that the development of tolerance to the hypolocomotor action of DZ (5 mg/kg) for 4 days and the associated increase in synaptic plasticity are context specific. Because animal preexposure to the drug admin
