Magnetic resonance imaging evidence for widespread orbital dysinnervation in dominant Duane's retraction syndrome linked to the DURS2 locus.

PURPOSE: High-resolution, multipositional magnetic resonance imaging (MRI) was used to demonstrate extraocular muscles (EOMs) and associated motor nerves in Duane retraction syndrome (DRS) linked to the DURS2 locus on chromosome 2. METHODS: Five male and three female affected members of two autosomal dominant DURS2 pedigrees were enrolled in the study. Coronal T(1)-weighted MRI of the orbits was obtained in multiple gaze positions, as well as with heavy T(2) weighting in the plane of the cranial nerves. MRI findings were correlated with motility. RESULTS: All subjects had unilateral or bilat

Atypical Duane's retraction syndrome: congenital adduction palsy with synergistic divergence in association with aberrant trigeminal innervation and facial hypoplasia.

The authors describe the atypical and unique features of trigeminal-oculomotor synkinesis in a 6-year-old boy with left congenital adduction palsy and synergistic divergence with facial hypoplasia. Adducting movements of the left eye were also seen on mastication. To the best of the authors' knowledge, a case of this nature has not previously been reported. This case illustrates the absence of changes in palpebral aperture and globe retraction on attempted adduction and also the complete absence of any abduction deficit of the involved eye. Additionally, presence of adducting movements on mast

An Analysis of 5 Duane's Retraction Syndrome Patients with Preoperative Abnormal Face Turn Reversal and (or) Worsening after Standard Horizontal Eye Muscle Surgery.

BACKGROUND AND PURPOSE: Although horizontal extraocular muscle rectus recession is an accepted treatment for abnormal face turn (AFT) in Duane's Retraction Syndrome (DRS), in some situations it can worsen or reverse the AFT. Patients referred to the author with this complication over a 4 year period (2002-2006) were reviewed to identify potential risk factors. METHODS: Retrospective medical record review of DRS patients referred because of reversal and (or) worsening of the preoperative AFT immediately after technically uncomplicated horizontal rectus eye muscle recession surgery. RESULTS: A

Transient pseudo-Duane syndrome after lateral orbital trauma.

Duane syndrome is a dysinnervation disorder that frequently involves secondary anomalous eye movement, in particular, ocular retraction and the narrowing of the palpebral fissure on the adduction. Pseudo-Duane syndrome is caused by a mechanical restriction that produces clinical findings similar to those of Duane syndrome. Most patients with pseudo-Duane syndrome have restrictive pathologies on the medial side of the globe, such as entrapment of the medial rectus muscle. We report the case of a 44-year-old man with pseudo-Duane syndrome in whom we identified a lateral orbital wall fracture and

Bilateral Duane syndrome and bilateral aniridia.

Duane retraction syndrome has been reported in association with structural abnormalities of the eye, including epibulbar dermoid, keratoconus, iris dysplasia, heterochromia iridis, persistent fetal vasculature, cataract, choroidal coloboma, microphthalmia, and optic nerve dysplasia. A novel association, that of bilateral Duane syndrome with bilateral aniridia, is the subject of this report.

Duane syndrome associated with oculocutaneous albinism.

A 5-year-old female presented with signs of Duane's retraction syndrome and oculocutaneous albinism. Duane's syndrome is associated with anomalous innervation of the lateral rectus by the oculomotor nerve. Oculocutaneous albinism is associated with an increased proportion of crossed retinal ganglion fibers at the chiasm. Because Duane's syndrome and oculocutaneous albinism are both examples of ocular miswiring, a common mechanism is suggested.

Synergistic divergence and jaw-winking phenomenon.

Synergistic divergence is a congenital syndrome of monocular adduction deficit, with simultaneous abduction of both eyes on attempted gaze into the field of action of the paretic medial rectus muscle. Based on electromyographic data, it has been speculated that it is a neural miswiring disorder related to Duane's retraction syndrome. The association between Duane's retraction syndrome and the Marcus Gunn jaw-winking phenomenon, another neural miswiring disorder, has been previously reported. The authors describe a patient with both bilateral synergistic divergence and Marcus Gunn jaw-winking p

A novel form of aberrant innervation in congenital cranial dysinnervation disorder.

Certain forms of congenital incomitant strabismus are associated with abnormal cranial nerve development and resultant abnormal orbital innervation (eg, Duane retraction syndrome, congenital fibrosis of the extraocular muscles [CFEOM]); such conditions can be considered congenital cranial dysinnervation disorders (CCDDs). In addition to duction limitation and/or ptosis, orbital CCDD phenotypes include inappropriate extraocular muscle and/or levator innervation by nerves intended for innervation of other structures (eg, some of the innervation intended for the medial rectus muscle inappropriate

Expansion of the CHN1 strabismus phenotype.

PURPOSE: Hyperactivating CHN1 mutations have been described in individuals with Duane retraction syndrome with or without vertical gaze abnormalities. This was a study of five family members with distinctive ocular dysmotility patterns that co-segregated with a novel hyperactivating CHN1 mutation. METHODS: Participating members of a family segregating pleomorphic incomitant strabismus underwent ophthalmic examinations, and several underwent high-resolution magnetic resonance imaging (MRI) of the orbits and brain stem. Participant DNA was extracted and amplified for haplotype analysis encompas

Clinical Profile and Surgical Outcomes of Bilateral Duane Syndrome with Exotropia.

OBJECTIVE: To study the clinical profile and surgical outcomes of patients with bilateral Duane syndrome with exotropia. METHODS: The records of all consecutive patients diagnosed with bilateral Duane syndrome with exotropia in primary gaze (2010-2024) were reviewed. Patient demography, age at presentation, and type of Duane syndrome were studied in the clinical profile. The pre-operative and post-operative details of angle of deviation, abnormal head posture, horizontal ocular ductions, grade of globe retraction, and overshoot were evaluated for the surgical outcomes. RESULTS: Forty-five pa

Orbital leiomyoma presenting as inverse globe retraction syndrome: a unique presentation of a rare disease.

Inverse globe retraction syndrome is a rare ocular motility disorder characterized by limited abduction, with globe retraction and up- or downshoots on attempted abduction, differentiating it from globe retraction due to Duane retraction syndrome, seen on attempted adduction. It can be congenital or acquired. We report the case of a 3-year-old girl who presented with classical features of inverse globe retraction syndrome secondary to an underlying orbital tumor involving the medial rectus muscle. Incisional biopsy confirmed the diagnosis of a leiomyoma. At 10 months' follow-up, vision, ocular

Gustatory lid retraction: an unusual congenital cranial dysinnervation disorder.

Congenital cranial dysinnervation disorders are developmental abnormalities of cranial nerves that often include abnormal synkinesis. Among the most common ophthalmic congenital cranial dysinnervation disorders are Duane retraction syndrome and the Marcus-Gunn jaw-winking phenomenon. This report documents gustatory lid retraction as an unusual congenital cranial dysinnervation.
