Disseminated histoplasmosis presenting with biliary obstruction and duodenal ulcer.

Disseminated histoplasmosis is an AIDS-defining illness which usually involves the liver and gastrointestinal tract, most commonly the small bowel. Abdominal pain, diarrhea, GI bleeding, hepatosplenomegaly and small bowel obstruction are well described presentations. Still gastrointestinal histoplasmosis often results in either vague symptomatology or no symptoms. Pancreaticobiliary disease related to disseminated histoplasmosis is not well characterized. We report the case of a young female with advanced HIV infection and biliary obstruction and a periampullary duodenal ulcer due to dissemina

Excision duodenoplasty: a new technique for congenital duodenal obstruction.

Slow anastomotic function is a common problem in the management of congenital duodenal obstruction. We describe a simple technique of excision duodenoplasty (ED) that results in a fixed open anastomosis, which facilitates early commencement of feeds and discharge from hospital. A retrospective case-note review (1981-2000) was undertaken to compare the results of ED with side-to-side duodenoplasty (SSD) and diamond-shaped anastomosis (DD). The outcome measures were days to commencement of feeds, duration of total parenteral nutrition (TPN), and length of hospital stay. In ED a 1.0 to 1.5-cm ell

Incomplete large bowel obstruction caused by a duodenal stent.

Gastric outlet obstruction secondary to benign or malignant disease can be treated with self-expanding metallic stents. This obviates the need for surgical bypass procedures and returns most patients to a normal diet. Prosthesis displacement is described as a late complication. A case of stent migration to the rectum, presenting with the symptoms and signs of incomplete large bowel obstruction, is described.

Duodenal and ampullary obstruction by a Peutz-Jeghers polyp.

We report a case of Peutz-Jeghers syndrome presenting with obstruction of the second part of the duodenum and the ampulla of Vater by a large intra-luminal polyp leading to duodenal obstruction and obstructive jaundice. CT scan of the abdomen showed a large polypoidal lesion, a caecal polyp and jejuno-jejunal intussusception. At surgery, two intussusceptions were reduced and leading polyps were excised via two enterotomies; the caecal polyp was excised via caecotomy. The duodenal polyp was excised by limited duodenectomy after frozen section has shown no evidence of malignancy. Histopathologic

Duodenal web causing gastric outlet obstruction in an adult.

Congenital duodenal web is a rare cause of gastric outlet obstruction in the adult. In this communication, we report a case in whom a preoperative diagnosis of chronic duodenal ulcer had been made. A review of diagnosis was forced by the finding of a normal pylorus at surgery.

Duodenal erosions and ulcers in patients with pancreatobiliary obstruction.

In order to determine whether obstructive pancreatobiliary lesions increase the risk of duodenal erosions and ulcers, the duodenal mucosa of patients with these lesions were prospectively examined before endoscopic retrograde cholangiopancreatography (ERCP). During the study period, 133 patients underwent ERCP for various reasons in the Department of Medicine, The Aga Khan University Hospital. One hundred and twenty-three patients were eligible for final analysis. Sixty-five patients with bilirubin > or = 35 mumol/L and alkaline phosphatase > or = 2.5 times normal levels along with radiologica

Duodenal obstruction by gallstone: case report of Bouveret's syndrome.

Bouveret's syndrome involves gastric outlet obstruction by gallstone. Herein we describe an unusual case of duodenal bulb obstruction by gallstone. An 80-year-old woman was hospitalized with a fifteen-day history of vomiting. Computed tomography (CT) showed pneumobilia and a round calcified mass in the second portion of the duodenum. Upper gastrointestinal tract series demonstrated the same sized oval radiolucency between the bulbus and the second portion of the duodenum. Endoscopic examination revealed a round black mass in the second portion of the duodenum, totally occupying the lumen. Endo

[Experiences with congenital duodenal obstruction in the Sophia Kinderziekenhuis].

In the department of Paediatric Surgery, Sophia Children's Hospital, University Hospital, Rotterdam, we investigated whether the presence of Down's syndrome influenced the chances of survival of patients with a congenital duodenal obstruction. The retrospective study comprised all 109 patients admitted to the Sophia Children's Hospital with a congenital duodenal obstruction in the period 1971-1989. The additional presence of Down's syndrome itself did not influence the prognosis. The presence of other additional anomalies (especially cardiac anomalies) did influence the mortality rate. In the

[Duodenal obstruction due to a biliary calculus (Bouveret's syndrome)].

We present a case of duodenal obstruction caused by biliary calculus (Bouveret's syndrome) an unusual cause of gallstone ileus. The clinical presentation simulated a pyloric stenosis, the gastrointestinal X-ray study being that which allowed us to establish diagnosis. Surgery was carried out only on the duodenal obstruction with successful results.

Gastric outlet and duodenal obstruction from inflammatory pancreatic disease.

During a ten-year period, 16 patients with gastric outlet and duodenal obstruction due to inflammatory pancreatic disease were seen. The cause of obstruction was chronic pancreatitis in ten patients, pseudocysts with associated pancreatitis in five patients, and pancreatic abscess in one patient. All patients had nausea and vomiting, 14 had abdominal pain, and five had weight loss greater than 4.5 kg. Diagnosis was made by plain abdominal film in one case, upper gastrointestinal tract roentgenographic series in 15 cases, and endoscopy in 11 cases. Mobilization of the duodenum relieved the obst

Prenatal demonstration of gastric and duodenal obstruction by ultrasound.

Prenatal sonographic findings in one case of gastric obstruction due to an imperforate antral diaphragm and in one case of duodenal atresia are presented. The combination of either one or two prominent sonolucent areas in the upper or mid fetal abdomen and polyhydramnios is virtuallydiagnostic of high obstruction.

Adult Distal Duodenal Obstruction: A Diagnostic and Therapeutic Challenge.

Distal duodenal obstruction (DDO) can be succinctly defined as features of gastric outlet obstruction with bilious vomiting and radiological or endoscopic evidence of post-bulbar obstruction. Obstructions of the third (D3) and fourth (D4) parts of the duodenum are rare and present significant diagnostic and surgical challenges, particularly when the cause is malignant. In the following three case reports, we discuss three distinct aetiologies of this rare syndrome and highlight important considerations surrounding the early investigation and management of these individuals. The first patient i

Use of trans-anastomotic tubes in congenital duodenal obstruction.

AIM: Despite data to suggest benefit of trans- anastomotic tube (TAT) feeding in infants following repair of congenital duodenal obstruction (CDO), TAT usage is limited. We aimed to report a large series of infants with CDO treated with or without TAT in order to improve the evidence underlying this simple intervention. METHOD: Single centre retrospective review of all infants CDO over a 20-year period (January 1999 - November 2020, inclusive). Important outcomes were compared between infants treated with or without TAT. Data are median [IQR]. RESULTS: Ninety-six infants were included. A TAT

Duodenal volvulus: a rare cause of small bowel obstruction.

Small bowel volvulus is a rare but life-threatening emergency. Volvulus of the duodenum is even rarer without the presence of predisposing factors. The clinical presentation is vague, including abdominal pain, nausea and vomiting, prompt diagnosis of volvulus therefore relies heavily on radiographs. The treatment options lie between conservative or surgical management, where the decision is influenced by the patient and their presentation. This case is of a 100-year-old female with an extensive surgical and medical background presenting with signs of small bowel obstruction. With the help of i

Duodenal Obstruction Due to Giant Gallstone: A Case Report.

BACKGROUND: Duodenal obstruction due to a gallstone, also known as Bouveret's syndrome, is one type of gallstone ileus. This is a rare complication of cholelithiasis. Among gallstone ileus cases, duodenal obstruction is alsorare. Apart from rareness, diagnosis is challenging due to unspecific clinical manifestation. Treatment options have benefits and drawbacks with each as well. Therefore, setting an appropriate option in a certain patient is an important issue. CASE REPORT: An 85-year-old woman presented clinically with a gastric outlet obstruction. Upper gastrointestinal (GI) endoscopy was
