Paediatric duodenal injury complicated by common bile duct rupture due to blunt trauma: a multispecialist approach.

We report a case of late presenting duodenal perforation with common bile duct rupture secondary to blunt handlebar trauma in an 11-year-old boy. The patient presented with upper abdominal wall ecchymosis, pain and vomiting. He was discharged after 24 hours with resolving symptoms. However, the boy presented 2 days later febrile with signs of peritonitis. CT indicated duodenal perforation, which was confirmed during laparotomy where common bile duct rupture was also demonstrated. Primary repair of the duodenum was undertaken. Here, decompression was achieved with a nasogastric tube proximal to

[Biliary atresia and polysplenia syndrome].

Polysplenia syndrome is a rare malformation characterized by the association of multiple rates and other congenital anomalies dominated by cardiac, vascular, intestinal and bile malformations. We report the observation of a patient operated in the neonatal period (3 days) for an upper intestinal obstruction with situs inversus. Surgical exploration noted the presence of multiple rates, a preduodenal vein, a biliary atresia and a duodenal atresia. The surgical procedures performed were a latero-lateral duodeno-duodenostomy and hepatoportoenterostomy of KASAI with simple immediate and delayed ou

Trilogy of foregut, midgut and hindgut atresias presenting in reverse order.

We present a case of triple gut atresias (foregut, midgut and hindgut) with multiple congenital anomalies presenting as imperforate anus. Abdominal radiography showed the double bubble sign. Upper gastrointestinal study through a nasogastric tube confirmed duodenal obstruction. Exploratory laparotomy, duodeno-duodenostomy for duodenal atresia and a left descending colostomy for anorectal malformation were performed. During extubation, the nasogastric tube came out and could not be reinserted by the anaesthetic team under laryngoscopic guidance. A chest radiograph showed the tube curled in the

Urinary levels of activated trypsin in whole-organ pancreas transplant patients with duodenocystostomies.

Urinary levels of trypsinogen and active trypsin were measured in 34 urine samples from 14 patients an average of 23 months following whole organ pancreas transplantation and duodenocystostomy. Timed urine specimens obtained 30 min and 90 min following a time zero void were collected from 5 patients in an effort to define the kinetics of trypsinogen secretion and activation. Total urinary protein and urinary pH were correlated with urinary levels of trypsin and trypsinogen. Twenty-one specimens from 8 normal volunteers and a single specimen from a pancreas transplant patient with a duodenoente

Delayed Thoracoscopic Repair Following Abdominal Esophageal Banding in a 539-g Infant With Triple Atresia: A Case Report.

Triple atresia, comprising esophageal atresia, duodenal atresia, and anorectal malformation, has no established management strategy in extremely low-birth-weight (ELBW) infants. We report successful staged treatment in a 539-g infant with severe respiratory failure. Transabdominal esophageal banding and gastrostomy on Day 0 controlled airflow through the tracheoesophageal fistula without thoracic intervention. Duodeno-duodenostomy and transverse colostomy were performed on Day 18 (680 g), permitting enteral feeding. After growth to 1924 g, thoracoscopic repair of Gross type C esophageal atresi

Wilkie's Syndrome: Laparoscopic Duodenojejunostomy.

INTRODUCTION: Superior mesenteric artery syndrome is a rare condition caused by compression of the third portion of the duodenum between the aorta and the superior mesenteric artery, usually due to an aortomesenteric angle of less than 25°. It presents with chronic abdominal pain, vomiting, early satiety, and weight loss and is more common in young women. Diagnosis is confirmed by imaging studies, primarily computed tomography. CASE PRESENTATION: A 20-year-old patient with a history of an eating disorder presented with chronic abdominal pain, vomiting, and weight loss. Computed tomography rev

Comparison of Hepaticoduodenostomy with Hepaticojejunostomy in the Laparoscopic Management of Choledochal Cyst in Children: A Systematic Review and Meta-Analysis.

BACKGROUND: There has been systematic review and meta analysis (SRMA) on comparison of hepaticoduodenostomy (HD) and hepaticojejunostomy (HJ) in choledochal cysts (CDC), which included both open and laparoscopic procedures. There is no SRMA, comparing them in pediatric CDC which were managed exclusively by laparoscopy. METHODS: Studies comparing laparoscopic HD and HJ after CDC excision were identified from Medline, Ovid, Google Scholar, and Cochrane library. Suitable studies were reviewed and meta-analyzed. Outcomes evaluated included operative time, hospital stay, and incidence of postopera

Laparoscopic duodenoduodenostomy for duodenal atresia.

A 3,220-g newborn baby with trisomy 21 presented with duodenal atresia. No other congenital malformations were diagnosed. Informed consent for a laparoscopic approach was obtained. The child was placed in a supine, head-up position slightly rotated to the left at the end of a shortened operating table. The surgeon stood at the bottom end with the cameraperson to his left and the scrub nurse to his right. The screen was at the right upper end. Open insertion of a cannula for a 5-mm 30 degrees telescope through the inferior umbilical fold was performed. A carbon dioxide (CO2) pneumoperitoneum wi

[Congenital cystic disease of the biliary system in adults].

Congenital cystic disease of the biliary system is a complex syndrome of ectasies of the intra-, extra- or both situation of biliary tree. This disease has an unsure etiopathogeny. It is uncommon through the third age, with a greater incidence in child, teen-ager and young adult. The goal of our study is to evaluate the symptoms, diagnosis, treatment and histological aspects of the congenital biliary cysts. We performed a retrospective study from March 1988 to July 2003 about 11 patients with this disease treated in our surgical clinic. Clinical features, methods of diagnosis and surgical trea

[Oddi's sphincterotomy or sphincteroplasty. Considerations on the results obtained with a personal sphincteroplasty technic].

A total of 21 cases are presented, of patients in whom Oddian sphincteroplasty was performed, according to an original technique. In 19 cases the indication was due to Oddian stenosis associated to lithiasis or hepatic hydatic cysts which had ruptured in the biliary pathways. In two other cases security sphincteroplasty of the Oddi sphincter was performed for lithiasis of the choledocus. The decision was based on macroscopic changes of the biliary pathways, on the control of the main biliary pathway. Sphincteroplasty was achieved with the aid of an original probe, and the incision of the ampul

Biliary Bezoar: the sump syndrome and choledochoenterostomy.

The sump syndrome has been reported to occur infrequently following a choledochoenterostomy (duodenostomy or jejunostomy) for stone disease. The enterostomy stoma usually decreases in diameter following surgery allowing for reflux into the distal, inactive segment of the bile duct without adequate egress of the material. This stagnant portion of the bile duct functions as a reservoir for collection of debris and/or lithogenic bile resulting in intermittent obstruction of the stoma producing symptoms of pain and clinical and biochemical evidence of cholestasis and pancreatitis. This symptom com

Endoscopic ultrasound guided biliary interventions.

Endoscopic Retrograde Cholangiopancreatography (ERCP), even in expert hands, may fail in 5-10% of cases, especially in cases of papillary infiltration, malignant gastric outlet obstruction, or surgically altered anatomy. Percutaneous transhepatic biliary drainage (PTBD) has represented the traditional rescue therapy, despite associated with high rate of adverse events, need for re-interventions and an inferior quality of life. The evolution of Endoscopic Ultrasound (EUS) from a diagnostic to a therapeutic tool offers an effective and safe alternative for internal biliary drainage (BD) into the

Emergent endoscopic ultrasound-guided cholecystoduodenostomy does not prevent R0 resection in a pancreaticoduodenectomy for pancreatic cancer.

Endoscopic ultrasound-guided biliary drainage, mainly choledochoduodenostomy, is commonly used as rescue therapy after Endoscopic Retrograde Cholangiopancreatography (ERCP) in malignant distal biliary obstruction due to un-resectable pancreatic cancer. An alternative when the cystic duct is patent and choledochoduodenostomy is not feasible is performing an Endoscopic Ultrasound-gallbladder drainage. The advent of the Lumen Apposing Metal Stents (LAMS) has shortened and simplified this procedure. However, many concerns exist about the performance of these procedures with metal stents preoperati

Congenital pyloric atresia and associated anomalies: a case series.

Congenital pyloric atresia (CPA) is a very rare surgical condition. Eleven patients with the diagnosis of CPA treated at our hospital were retrospectively studied for the age at diagnosis, sex, presenting symptoms, associated anomalies, operative findings, treatment and outcome. Male: Female is 8:3. The age at diagnosis ranged from one day to three years. Associated anomalies were seen in four (36.6%). These included epidermolysis bullosa (EB) in two, oesophageal atresia with distal trachea-oesophageal fistula in one, colonic atresia in one, sensorineural deafness and dysplastic kidney in one

[A study of concomitant chronic pancreatitis in forty-one cases of adult congenital biliary dilatation].

Forty-one cases of adult congenital biliary dilatation (CBD) were studied for concomitant chronic pancreatitis. Of the 41 cases, 8 had had such previous operations as choledochojejunostomy or choledocho-duodenostomy without biliary diversion (reoperated cases) and the other 33 had not received previous operations (primary cases). All of the 36 cases whose type of pancreatico-biliary junction could be examined revealed anomalous pancreatico-biliary junction. Four cases (10%) of CBD (1 of the primary and 3 of the reoperated cases) were complicated by chronic pancreatitis. On examining the elapse

Forme fruste choledochal cyst: long-term follow-up with special reference to surgical technique.

PURPOSE: Forme fruste choledochal cyst (FFCC) is a choledochal cyst that has minimal or no dilatation of the extrahepatic bile duct (EHBD) and is associated with pancreaticobiliary malunion (PBMU). The authors reviewed the long-term outcome of their patients with FFCC. METHODS: Inpatient and outpatient records of 281 patients with choledochal cyst were reviewed. In this study, minimal dilatation of the EHBD was defined as its maximum diameter being less than 10 mm. RESULTS: There were 17 cases of FFCC identified. In all 17 patients, PBMU was present. The mean age at EHBD excision was 2.9 yea

Pancreatic transplantation with duodenoduodenostomy drainage: technique, normal radiological appearance and complications.

Pancreas transplantation is considered the curative treatment for severe type 1 diabetes mellitus in selected cases. Since the first procedure in 1966, surgical techniques have been improved. The current trend among most medical centers, as well as at our Institution, is enteric drainage and systemic venous or portal anastomosis. The aim of this pictorial essay is to describe the main imaging features of pancreatic transplantation with duodenoduodenostomy drainage.

Disrupted pancreatic duct treated with a combination of endoscopic cystoduodenostomy and pancreatic duct fistualization through a pseudocyst wall using a lumen-apposing metal stent.

Management of disconnected pancreatic duct syndrome is complex and is increasingly approached using emerging endoscopic techniques. Herein, we present a case of recurrent pancreatitis in an elderly woman complicated by pseudocyst formation and complete pancreatic duct disruption. Her condition was treated with a novel combination of transpapillary stenting and cystduodenostomy in which a pancreatic duct stent was placed into an opening in the pseudocyst wall through a lumen-apposing metal stent. This allowed for effective drainage into the gastrointestinal tract and resolution of duct disrupti
