Dysmenorrhea: evaluation with cine-mode-display MR imaging--initial experience.

PURPOSE: To prospectively evaluate uterine contractility during menstruation and its relation to primary dysmenorrhea by using magnetic resonance (MR) imaging with cine mode display. MATERIALS AND METHODS: The university ethics committee approved the study protocol, and all subjects gave written informed consent. Nineteen healthy women were examined during menstrual cycle days 1-3 by using a 1.5-T-magnet unit. Sixty serial half-Fourier rapid acquisition with relaxation enhancement MR images of the uterus were obtained every 3 seconds for 3 minutes and displayed in cine mode. MR images were an

Clinical evidence on integrated Chinese-Western medicine for primary dysmenorrhea.

BACKGROUND: Primary dysmenorrhea (PD), a common gynecological disorder, adversely affects the quality of life, mental health, and overall well-being of many young women. Given concerns about adverse effects of some Western medications, Chinese medicine (CM) and integrated Chinese-Western medicine (ICWM) approaches have gained attention. This study aims to systematically identify PD-related clinical evidence on CM and ICWM treatments, appraise and categorize the evidence by study design, and provide evidence summaries based on pre-defined clinical questions to inform evidence-based practice. Accessory cavitated uterine mass: a rare cause of severe dysmenorrhea.

We present a rare Müllerian anomaly known as "accessory cavitated uterine mass (ACUM)," which causes severe, refractory dysmenorrhea, and review the literature. A 17-year-old patient experienced severe, cyclic left-sided pelvic pain, correlating with menstruation. Magnetic resonance imaging (MRI) revealed a cystic lesion surrounded by myometrial tissue. Medical treatment with oral contraceptives was unsuccessful. Laparoscopic excision of the mass was performed, and histopathology confirmed the diagnosis of ACUM. At six months postoperatively, she remained asymptomatic, and follow-up MRI showed

Impaired PIEZO1 function drives uterine hypercontractility in adenomyosis-associated dysmenorrhea.

STUDY QUESTION: Does PIEZO1 play any role in adenomyosis-associated dysmenorrhea? SUMMARY ANSWER: PIEZO1 downregulation in the myometrium reduces the expression of endothelial nitric oxide synthase (eNOS) and nitric oxide (NO) production, leading to increased and irregular contractility that contributes to dysmenorrhea in women with adenomyosis. WHAT IS KNOWN ALREADY: Aberrant uterine contractility has long been documented in women with adenomyosis, but our knowledge of the molecular mechanisms governing uterine contractility is quite limited. Oxytocin receptor (OTR) expression in myometrium

Protein expression profiling of rat uteruses with primary dysmenorrhea syndrome.

PURPOSE: The aim of this study was to investigate differentially expressed proteins (DEPs) and their functions in the uteruses of primary dysmenorrhea (PD) rats using label-free quantitative proteomics analysis. METHODS: The PD rat model was induced by injecting both estradiol benzoate and oxytocin. Twenty rats were equally divided into two groups: a control group (normal rats), a PD model group (PD rats). Writhing scores and serum levels of Prostaglandin E2 (PGE2) and Prostaglandin F2α (PGF2α) were used to evaluate the success of the rat PD model. The DEPs were identified and analyzed by lab

What women say about their dysmenorrhea: a qualitative thematic analysis.

BACKGROUND: Dysmenorrhea is highly prevalent and is the leading cause of absence from school and work among women of reproductive age. Evidence suggests that dysmenorrhea may also be a risk factor for other chronic pain conditions. Limited research has examined women's experience with dysmenorrhea using qualitative data. Research is warranted to address issues and needs that are important from women's own perspectives. Therefore, the purpose of this study was to describe women's salient thoughts about their experiences of dysmenorrhea. METHODS: We analyzed data collected from an open-ended qu

What we know about primary dysmenorrhea today: a critical review.

BACKGROUND: Primary dysmenorrhea, or painful menstruation in the absence of pelvic pathology, is a common, and often debilitating, gynecological condition that affects between 45 and 95% of menstruating women. Despite the high prevalence, dysmenorrhea is often poorly treated, and even disregarded, by health professionals, pain researchers, and the women themselves, who may accept it as a normal part of the menstrual cycle. This review reports on current knowledge, particularly with regards to the impact and consequences of recurrent menstrual pain on pain sensitivity, mood, quality of life and

Surgical treatment of primary dysmenorrhea with laparoscopic uterine nerve ablation.

With approximately 25% of dysmenorrheic patients reporting no improvement with nonsteroidal anti-inflammatory drugs, a study was devised to evaluate the effectiveness of a laparoscopic technique for the interruption of the uterosacral nerves. In a double-blind study of 21 patients with primary dysmenorrhea, 81% (9 of 11) reported significant relief from menstrual pain after the surgery. Performed as an outpatient procedure, laparoscopic uterine nerve ablation may alleviate dysmenorrheic complaints when other modalities have failed. Half the treated women reported continued relief of menstrual

Analyzing the Risk Factors for Recurrence of Endometrioma and Dysmenorrhea in Patients with Endometriosis.

BACKGROUND: Recurrence of endometrioma and dysmenorrhea after surgery remains a major clinical challenge. Identifying reliable risk factors is essential to optimize individualized treatment and follow-up strategies. However, large-scale data from Chinese populations are limited. MATERIALS AND METHODS: We retrospectively analyzed 1,598 reproductive-aged women with pathologically confirmed endometriosis who underwent surgery between 2009 and 2010 and were followed for up to 7 years. Clinical, surgical, and follow-up data were collected. Kaplan-Meier survival analysis and Cox proportional hazard

Mechanism of action of Taohong Siwu decoction in the alleviation of primary dysmenorrhea.

BACKGROUND: As one of the most common gynecological disorders, PD significantly impacts the quality of life for women. TSD, a well-known traditional Chinese medical prescription, has gained popularity for its use in treating gynecological cold coagulation and blood stasis syndromes such as PD. However, the lack of comprehensive data hinders our understanding of its molecular mechanism. PURPOSE: The objective of the present study is to investigate the therapeutic effects of TSD on PD and elucidate its plausible mechanism. METHODS: HPLC was employed to confirm the presence of the principal met

[Dysmenorrhea and pelvic pain secondary to an accessory and cavitated uterine mass].

An accessory and cavitated uterine mass (ACUM) is a rare anomaly with an embryological origin of dysfunctionning female gubernaculum. It is an accessory mass internally lined with normal endometrium, separated from the uterine cavity and located near the insertion of the round ligament. ACUM's clinical manifestations are severe dysmenorrhea and/or chronic pelvic pain. It is a relatively unknown condition, which makes its diagnosis complicated and suggests a large differential diagnosis. We report the case of a 31-year-old female presenting with pelvic chronic pain and crippling dysmenorrhea. T

Frequency-Specific Alterations in Brain Function in Patients with Primary Dysmenorrhea.

OBJECTIVE: Recent neuroimaging studies have found that brain function is abnormal in primary dysmenorrhea (PDM). The present study aimed to explore frequency-specific brain alterations and their occurrence in the PDM. METHODS: Forty-seven patients with PDM and 36 matched healthy controls were enrolled in the study and underwent resting-state functional magnetic resonance imaging. The alterations in brain function in patients with PDM were assessed with different frequency filter bands (Slow5, Slow4, Slow3, and full low frequency) and a functional connectivity density (FCD) approach. The clini

ACOG Committee Opinion No. 760 Summary: Dysmenorrhea and Endometriosis in the Adolescent.

Dysmenorrhea, or menstrual pain, is the most common menstrual symptom among adolescent girls and young women. Most adolescents experiencing dysmenorrhea have primary dysmenorrhea, defined as painful menstruation in the absence of pelvic pathology. When the patient's history suggests primary dysmenorrhea, empiric treatment should be initiated. When a patient does not experience clinical improvement for her dysmenorrhea within 3-6 months of therapy initiation, her obstetrician-gynecologist should investigate for possible secondary causes and for treatment adherence. Secondary dysmenorrhea refers

A randomized controlled trial of single point acupuncture in primary dysmenorrhea.

BACKGROUND: Acupuncture is often used for primary dysmenorrhea. But there is no convincing evidence due to low methodological quality. We aim to assess immediate effect of acupuncture at specific acupoint compared with unrelated acupoint and nonacupoint on primary dysmenorrhea. METHODS: The Acupuncture Analgesia Effect in Primary Dysmenorrhoea-II is a multicenter controlled trial conducted in six large hospitals of China. Patients who met inclusion criteria were randomly assigned to classic acupoint (N = 167), unrelated acupoint (N = 167), or non-acupoint (N = 167) group on a 1:1:1 basis. The

Involvement of oxytocin and vasopressin in the pathophysiology of preterm labor and primary dysmenorrhea.

Important sources of oxytocin and vasopressin in the human, apart from the supraoptic and paraventricular nuclei of the brain, may be the fetus during labor as well as the endometrium and decidua of the uterus itself. The release of oxytocin and vasopressin to plasma is under influence of ovarian steroids. The two hormones stimulate uterine contractions in pregnant and non-pregnant women via myometrial oxytocin and vasopressin V1a receptors. At the onset of human labor preterm or at term no clear rise in the maternal plasma concentration of oxytocin and/or vasopressin has been demonstrated, bu
