Osteomas in the middle ear are extremely rare and are generally solitary and small. We present a case of a 28-year-old female patient with multiple middle ear osteomas which arose from the promontory, epitympanum, and lateral semicircular canal. The osteomas were successfully removed by tympanomastoidectomy. This is the first reported case of a patient with middle ear osteomas arising from multiple sites.

Osteomas of the middle ear are rare benign neoplasms. Most of the cases caused conductive hearing loss and others were asymptomatic and diagnosed incidentally. We report a case of a huge middle ear osteoma along with compatible radiological findings filling the whole tympanum and the Eustachian tube, which caused intermittent otorrhea and conductive hearing loss.

The aim of this article is twofold: (a) to present the principles and the indications of surgical treatment of middle ear pathologies; and (b) to review the imaging findings after middle ear surgery, including the normal postoperative aspects and imaging findings in patients presenting with unsatisfactory surgical results or with suspicion of postoperative complications. This review is intentionally restricted to the most common diseases involving the middle ear: chronic otitis media and otosclerosis. In these specific fields of interest, CT and MR imaging play a very important role in the pos

Neuroepithelial cysts are lesions of the central nervous system that have previously been reported in cerebral parenchyma, in the spinal cord, and within the ventricles in association with the choroid plexus. We describe 2 cases of neuroepithelial cysts of the middle ear. One was diagnosed after surgery for a retraction pocket and chronic otitis media complicated by an intraoperative cerebrospinal fluid (CSF) leak. The other produced bilateral spontaneous CSF otorrhea and mimicked the presentation of arachnoid granulations. Both cases were verified with immunohistochemical stains. Neuroepithel

Virtual endoscopy is a computer-generated simulation of fiberoptic endoscopy, and its application to the study of the middle ear has been recently proposed. The need to represent the middle ear anatomy by means of virtual endoscopy arose from the increased interest of otolarygologists in transtympanic endoscopy. In fact, this imaging method allows the visualization of middle ear anatomy with high detail, but it is evasive and is essentially used for surgical guidance. Virtual endoscopy provides similar perspectives of the tympanic cavity but does not require the tympanic perforation. In the st

OBJECTIVE: The authors report a case of mature teratoma of the middle ear in a 3-year-old girl with a 1-year history of otitis media.
METHODS AND RESULTS: Radiologic investigation revealed a partially cystic lesion of the petrous portion of the right temporal bone. It produced opacification of the middle ear as well as destruction of septal air cells. The patient underwent a subtotal petrosectomy. Histologically, the tumor was composed of an intimate admixture of mature tissues representing all three germ layers, including brain, myelinated nerve trunks, skeletal muscle, bone, immature cartil

To rehabilitate most cases of conductive hearing loss closure of ear drum perforations and rebuilding of the ossicular chain can be performed. Due to the great number of biocompatible bone substitute materials available it is occasionally difficult for the surgeon to choose the most favorable substitute. Autogenous structures (ossicles, cortical bone, cartilage) and allogenous tissues (ossicles, cortical bone, cartilage, dentin) are possible bone replacement materials. Xenogenic tissue is currently not used in middle ear surgery. Ionomer cement is a hybrid material for replacement of bone but

The development of the murine middle ear was monitored both qualitatively and morphometrically by scanning electron microscopy from the 19th gestational day to the adult stage. At birth, the middle ear was less well developed than the inner ear. The tympanic membrane (TM) was obscured by occlusion of the external auditory canal. Ciliated cells and secretory granules were present in the middle ear epithelium already 5 days after birth (DAB). Keratin debris was discerned on the external layer of the TM 9 DAB. By 12 DAB, mesenchymal tissue had resorbed from the middle ear cavity, except around th

Adenomatous neoplasm occurring in the middle ear is rare. The clinical findings are non-specific and include otological symptoms as well as sometimes functional deficits of the lower cranial nerves. The final diagnosis will be made only after elaborate histological examinations because of the inconsistent radiological and clinical findings. DNA analysis yields some information on the prognosis of this tumour. We report on a case where a primary adenomatous neoplasm developed in the middle ear, and discuss our concept of therapy.

Carcinoid tumor of the middle ear is a very rare benign neoplasm, which may be mistaken for a malignant tumor. We present two new cases together with a review of 28 previously reported cases. Nearly all patients had progressive hearing loss, most often of the conductive type. About half of the patients complained of tinnitus and fullness of the ear. The typical otoscopic picture was erythema or lateral bulging of the tympanic membrane, which was intact in most patients. The tumor was most often localized to the middle ear with varying degree of extensions into neighboring areas. It often encap

Benign adenoma of the middle ear is a tumor from glandular mucosa. In most of the cases, it is revealed between 40 and 60 year old, by a conductive hearing loss with a greyish mass behind intact tympanic membrane. Scanography demonstrated a no specific tumor, without bone destruction. The ossicular chain is usually respected. The histologics findings show two distinctive type of cells: glandular and neuroendocrine differentiation. The treatment is surgical. A regularly follow up is necessary to diagnostic recurrence.

Osteomas of the middle ear are extremely rare lesions. Of the nine cases reported, eight presented with conductive hearing loss. We recently treated two patients whose osteomas, of the promontory and of the posteroinferior tympanic wall, were asymptomatic. We suggest that middle ear osteomas need not be removed when they do not impinge on the sound conduction system.

A case of a moderate essentially low tone unilateral conductive hearing loss without a previous history of secretory otitis media in a six-year-old female patient is described. The condition was found to be due to a dermoid cyst in the tympanic cavity, which was confirmed by pathological examination of the surgically removed specimen. This is the sixth documented case of dermoid cyst in the tympanic cavity.

The AA. inform about 8 patients suffering from congenital cholesteatoma of the tympanomastoidal cleft. They underline the difficulties for achieving a correct diagnosis when the eardrum remains intact and remark the need to keep in mind this process for differential diagnosis of the conductive hypoacusis in childhood. As a matter of fact the AA. emphasize the few cases of recurrences after surgery, in contrast with that of relapses in child's acquired cholesteatoma.

Odontoma of the middle ear cleft is an unusual cause of conductive hearing loss. We report two cases and discuss the developmental mechanism by which this anomaly may occur. The differential diagnosis of calcified masses in the middle ear is also discussed.

An unusual case of a patient with a gouty tophus in the middle ear is presented. The patient had no clinical or laboratory signs of hyperuricaemia. The findings in the middle ear are discussed with reference to current knowledge of the pathogenesis of gout.

One case of primary adenocarcinoma of the middle ear is reported, which developed from a chronic otitis media. Histological examination established the diagnosis of adenocarcinoma. Its clinical appearance was benign, being restricted to the middle ear and tympanic membrane and with no bone destruction or metastases. Two years after local surgery there was no evidence of recurrence.

A case of papillary adenocarcinoma of the middle ear is presented. The patient had an unusually short history of otalgia, aural discharge and facial palsy, and, at presentation the tumour was too large for surgical resection to be a feasible option. The management of this rare tumour is discussed and the relevant literature reviewed.
