The middle ear, which lies between the external auditory canal and the inner ear (cochlea), comprises the tympanic membrane, the ossicular chain (i.e. malleus, incus, and stapes), as well as the associated muscles, ligaments, and the middle ear cavity. Its primary function is to transmit vibratory energy (sound pressure) from the air to the cochlear fluids via the ossicular chain. This part of the ear can be damaged by cholesteatoma, which can affect all three ossicles, necessitating ossiculoplasty to restore sound transmission. Ossiculoplasty is the preferred intervention for restoring the me

OBJECTIVES: Our aims were to measure and compare anatomical parameters of the ossicles in normal, congenital aural stenosis (CAS), and congenital aural atresia (CAA) ears.
METHODS: This retrospective study was performed using three-dimensional reconstructed images derived from computed tomography scans of 20 normal subjects, 20 CAS patients, and 20 CAA patients.
RESULTS: The lengths of the malleus handle and long process of the incus were greater in normal ears than in CAS and CAA ears (all P < 0.05). The angles of the incudostapedial joint and between the short and long processes of the inc

BACKGROUND: Development of the auditory ossicles is initiated by induction of the cartilage primordium of each ossicle between the fifth and seventh fetal week. It is well known that primordium of the upper part of the ossicular chain is derived from the first branchial arch and that of the lower part develops from the second branchial arch. Previous studies have suggested that auditory ossicular malformations are caused by deficiencies in the early period of induction of the cartilage primordium. However, auditory ossicular malformations can occur at any developing stage, and their clinical f

William Cheselden's famous anatomical treatise spanned the entire 18th century period with its 15 editions. The aim of this study is to analyze the otological knowledge described in all these editions, to identify key 18th century otological advancements, and to study their concordance. In the first edition (1713), Cheselden notably mentioned four middle ear ossicles: malleus, incus, fourth ossicle, and stapes; four auditory muscles: "external tympani," "external oblique," tensor tympani, and stapedial; and a small opening in the tympanic membrane. In subsequent editions, minimal changes appear

Temporal bone CT was used to examine a group of 87 patients with chronic purulent otitis media (103 temporal bones). The patients' age ranged from 2 to 74 years. A scheme was developed and proposed to evaluate the temporal bone by CT. The CT signs of chronic purulent otitis media uncomplicated by cholesteatoma and those of cholesteatomic purulent otitis were identified. The CT symptomatology of chronic purulent otitis includes: sclerotic changes in the bone tissue of the mastoid process, impaired pneumatization of the cavities of the middle ear, including the tympanic cavity, destructive chang

The characterization of the vibrations of the middle ear ossicles during sound transmission is a focal point in clinical research. However, the small size of the structures, their micrometer-scale movement, and the deep-seated position of the middle ear within the temporal bone make these types of measurements extremely challenging. In this work, dynamic synchrotron-based X-ray phase-contrast microtomography is used on acoustically stimulated intact human ears, allowing for the three-dimensional visualization of entire human eardrums and ossicular chains in motion. A post-gating algorithm is u

Otological surgery is performed under the operating microscope with special microinstruments. The introduction of new technical means such as the argon laser, equipped with a fiberoptic handpiece, and the sceeter microdrill have improved the results according to the statistic of Berne. An important aspect is the approach to the structures in the temporal bone. The classical retroauricular and endaural incisions are replaced whenever possible by the less traumatic transmeatal approach directly through the external ear canal without external skin lesion. Ear drum perforations and cholesteatomas

The aim of the study was to describe ossicle resorption in chronic otitis with cholesteatoma and correlate it with clinical parameters such as age, contralateral ear condition, tympanic membrane aspect, cholesteatoma pathogenesis and extension, associated lesions and hearing threshold. Preoperative clinical data were collected for 140 patients with chronic otitis with cholesteatoma, whose ossicles were evaluated during surgery. 82% of patients showed ossicle resorption, with incus damage in 78% of cases. Multiple involvement was found in 45% of cases and the incus-stapes association was the mo

The anterior anchoring of the malleus of 30 extant species of Lagomorpha (rabbits, hares, pikas) has been studied on the basis of histological serial sections and µCT-scans. It is shown that former studies of Oryctolagus, Lepus, and Ochotona are incomplete, because the rostral part of the processus anterior of the malleus is always lacking due to damage of this extremely delicate structure. Our study shows that in perinatal stages of Leporidae the praearticulare develops a prominent processus internus that fits into a groove at the ventral side of the tegmen tympani; this "tongue and groove"-a

High-resolution computed tomography (HRCT) provides excellent contrast between osseous structures, air and soft tissue in conjunction with high spatial resolution. Therefore, thin-section HRCT with bone window setting is the method of choice for the examination of the middle ear structures. The indications are acute and chronic inflammatory changes, cholesteatoma and tumor, the "postoperative middle ear", and malformations. In most cases, HRCT enables differentiation between inflammatory changes, cholesteatoma, and tumor. The excellent depiction of subtle osseous details enables the identifica

BACKGROUND: Congenital malformations of the auricle are often combined with atresia of the outer ear canal and malformations of the ossicles, representing aesthetic as well as functional deficits. Optimal treatment should therefore address both aspects equally. This report describes a new approach, combining the reconstruction of the auricle with implantation of an active middle ear hearing aid, stimulating the round window membrane.
METHOD: A 33-year-old male patient, with bilateral ear microtia, fibrous atresia of the external ear canals and malformation of the ossicles due to Treacher Coll

The white rhinoceros is the largest of the five extant rhinoceros species. The population is declining rapidly because of intense poaching. However, normal anatomical descriptions in this species are lacking. The purpose of this study is to describe the osseous anatomy of the middle and inner ear of the southern white rhinoceros using micro-focus X-ray computed tomography imaging. Four temporal bones obtained from two 1-day old southern white rhinoceros preserved in 10% formalin were scanned. Tri-dimensional reconstructions were obtained and volumes of the middle ear ossicles and inner ear str

OBJECTIVE: Otitis media is one of the most common illnesses which may cause ossicles destruction and conductive hearing loss. However, nowadays the damaged middle ear bones may be replaced by a partial or total ossicular replacement prosthesis. The main aim of this article was to confirm the proper functioning of the new middle ear prosthesis.
METHODS: This work describes first clinical trials conducted on a group of three patients with the case of interrupted ossicles continuity and chronic otitis media. The clinical trials were performed according to permission No. 157/KBL/OIL2016. The pati

In order to model the hearing capabilities of marine mammals (cetaceans), it is necessary to understand the mechanical properties, such as elastic modulus, of the middle ear bones in these species. Biologically realistic models can be used to investigate the biomechanics of hearing in cetaceans, much of which is currently unknown. In the present study, the elastic moduli of the auditory ossicles (malleus, incus, and stapes) of eight species of cetacean, two baleen whales (mysticete) and six toothed whales (odontocete), were measured using nanoindentation. The two groups of mysticete ossicles o
