Immunoelectrophoresis tests showing Echinococcus granulosus arc 5 in human cases of Echinococcus vogeli and cysticercosis-multiple myeloma.

Human sera from one case of polycystic hydatidosis due to Echnincoccus vogeli and from a case of cysticercosis and multiple myeloma were positive to the immunoelectrophoresis (IEP) test for hydatidosis based on the E. granulosus arc 5 positivity criterion. Arc 5 can therefore be elicited in IEP tests of human sera not only by E. granulosus and E. multilocularis but also by E. vogeli antigens. Whether the cross reaction observed in the second serum was due to the multiple myeloma or to the cysticerci remains to be determined. Although arc 5 antigens are known to be present in Taenia hydatigena

Overview of Echinococcus multilocularis in Turkey and in the World.

Echinococcus multilocularis, a heteroxen and zoonotic parasite, is found in the intestine of carnivores, particularly foxes. Adult cestodes are regarded apathogenic in definitive hosts, while metacestode, the alveolar form, is high pathogenic for intermediate hosts. The alveolar cyst causes a maling tumor-like lesions with infiltrative, proliferative and destructive character which locates in the liver primarily, then metastasizes to other organs. If it is not treated in intermediate hosts it causes irreversible symtoms and death after located in vital organs such as liver, lungs, brain. Human

Pulmonary Echinococcus multilocularis metastasis in a dog.

A young adult Labrador retriever dog was presented for surgical debulking of hepatic alveolar echinococcosis. Computed tomography detected hepatomegaly with multiple large cavitary masses with extension of tissue from a lesion wall into the caudal vena cava and numerous nodules in all lung lobes. Following euthanasia, histology confirmed parasitic vesicles with granulomatous reaction in all lesions, and polymerase chain reaction (PCR) established the causative agent to be Echinococcus multilocularis. This report is the first to present imaging features of pulmonary E. multilocularis granulomat

The impact of globalisation on the distribution of Echinococcus multilocularis.

In the past three decades, Echinococcus multilocularis, the cause of human alveolar echinococcosis, has been reported in several new countries both in definitive hosts (canids) as well as in people. Unless treated, infection with this cestode in people is fatal. In previously endemic countries throughout the Northern Hemisphere, geographic ranges and human and animal prevalence levels seem to be increasing. Anthropogenic influences, including increased globalisation of animals and animal products, and altered human/animal interfaces are thought to play a vital role in the global emergence of t

Echinococcus alveolaris: presenting as a cerebral metastasis.

Echinococcus multilocularis (EM) is caused by the larval stage of the tapeworm. The main endemic regions for human alveolar echinococcosis are Central Europe, Russia, Turkey, Japan, China, Eastern France, North America. EM is an endemic disease in Turkey and especially common in the eastern Anatolia Region as seen in our case. The liver is the primary focus of the disease, cerebral localization is rare. Cerebral hydatid disease is approximately 5% of alveolar hydatid cases and generally considered to be fatal. Surgical treatment should be considered whenever possible. We present a case of cere

Echinococcus multilocularis in Europe--state of the art.

New information has become available on the epidemiological situation of Echinococcus multilocularis in Europe. Additional endemic areas have been identified, and, in some areas, prevalences in foxes have drastically increased. In addition, some neozootic mammals may by now be involved in the lifecycle. The parasite is now widespread in urban and suburban areas in central Europe. The number of human cases of alveolar echinococcosis is small, but still underreported, and is likely to increase in the near future. Trials to counteract the spread of this parasite have been performed by application

[Rare case of huge echinococcus of the liver].

A huge echinococcus, 26-16 cm in size, of the right lobe of the liver was diagnosed by means of echoscopy and radiology in the man 53 years of age. During operation 3000 ml of transparent liquid with daughter bubbles were taken out from the echinococcus cavity. Partial resection of the right lobe of the liver and drainage of the echinococcus cavity were made transhepatically with two drainages. During the postoperative period everyday sanitation of the cavity with antiseptic solutions was done. Periodically echoscopic monitoring of the echinococcus cavity was also done. The cavity gradually de

Ecology and epidemiology of Echinococcus multilocularis in Europe.

Human alveolar echinococcosis (AE), caused by the larval stage of Echinococcus multilocularis has a high mortality rate in untreated patients. The life-cycle of E. multilocularis in Europe predominantly involves foxes as definitive hosts. However, experimental studies demonstrated a comparable biotic potential of E. multilocularis in dogs and raccoon dogs but an insignificant potential in cats. AE occurs in central and eastern Europe at low incidence rates. Recent studies in foxes have shown that E. multilocularis has a wider geographic range (including Italy) than previously thought. Wilderness in the city: the urbanization of Echinococcus multilocularis.

A distinct increase in fox populations, particularly in urban areas, has been observed in Europe. This is of particular concern in endemic regions of the small fox tapeworm Echinococcus multilocularis, the aetiological agent of human alveolar echinococcosis. Novel tools have facilitated the investigation of the ecology of urban foxes and have demonstrated the urban wildlife cycle of E. multilocularis. Such studies are essential for estimating the risk of transmission to humans and to determine the basics for the development of control strategies.

[Current spread and epidemiology of Echinococcus multilocularis].

Recently, a wealth of new data was collected on the distribution and ecology of E. multilocularis. The parasite is now known to occur at surpisingly high prevalence rates in e.g. Belgium and northwestern Germany, new records exist for the Netherlands, and the parasite was found to be widespread in Poland and the Czech Republic. In addition, foxes in continental Europe have adapted their behaviour and are now common in many towns and cities where they are also known to carry the parasite. New data exist on endemicity regions in western Asia. In addition to new informations on the parasite's ran

[Atypical Echinococcus cysticus (granulosus) of the liver].

An atypical case of cystic echinococcosis of the liver is reported. During a routine examination of a 75-year-old man an asymptomatic hepatic lesion was found. US, CT and MRI showed a solid, avascular, non-invasively growing lesion. Echinococcus serology was negative. Histological samples taken by biopsy and after cyst extirpation confirmed the diagnosis of an old, scarred Echinococcus cyst. Such findings are rare in echinococcosis and cause difficulties with the diagnosis. Be- cause of the increasing age of the population and the presence of tourists and immigrants from endemic areas, this co

Presentation of echinococcus infection as lung abscess.

A 35-year-old man presented with clinical and radiological features of lung abscess. Despite broad spectrum antibiotics for 5 weeks he remained systemically unwell. Underlying Echinococcus infection was suspected when repeat radiology of his chest showed a water lily sign, and this diagnosis was confirmed at thoracotomy. Hydatid disease may present as lung abscess and cause delay in diagnosis and appropriate therapy.

New perspectives in the diagnosis of Echinococcus disease.

Echinococcosis, an endemic disease on the Asian continent, is caused by the tapeworm Tenia Echinococcus, which produces cysts in the liver and other organs. I reviewed 157 patients with hydatid disease of the liver. The hydatid cysts were single in 125 patients and predominantly in the right lobe, but multiple in 31 patients, localized in both lobes or in the lung, spleen, pancreas, heart, or peritoneum. In 14 patients, early complications were manifested by rupture of the cyst into the biliary tract, the lung, or the peritoneum. Infection of the cysts was rare. Late postsurgical complications

Chemotherapy of Echinococcus infection in man with albendazole.

Since 1983 data have been collected on the outcome of treatment of apparently active Echinococcus granulosus hydatid cysts with albendazole. Most patients received 800 mg albendazole daily in cycles of 28 d with 14 d between cycles, with a mean duration of 2.5 cycles (range 1-12). From an initial set of over 500 cases, 253 patients were evaluated for efficacy, with 269 hepatic, 86 pulmonary, 50 peritoneal and 51 cysts at other sites being individually assessed. 72 patients (28.5%) were regarded as cured, 129 (51%) as improved, 46 (18.1%) as unchanged and 6 (2.4%) were worse. 47 patients underw

The role of sonography in the diagnosis of Echinococcus alveolaris.

Abdominal sonography and computed tomography were performed in eleven patients with operatively and histologically proven Echinococcus alveolaris of the liver and the detailed appearances were compared. Sonography is a useful screening method in the assessment of E. alveolaris, but its previous importance is being gradually superseded by computed tomography, which is capable of providing more detailed information in the diagnosis of Echinococcus infestation.
