Incidence and economic burden of prosthetic joint infections in a university hospital: A report from a middle-income country.

In this study, we aimed to evaluate the incidence and economic burden of prosthetic joint infections (PJIs) in a university hospital in a middle-income country. Surveillance data between April 2011 and April 2013 in the Orthopedic Surgery Department was evaluated. Patients (>16 years old) who had primary arthroplasty in Erciyes University were included in the study, and patients with preoperative infection were excluded. Patients were followed up during their stay in the hospital and during readmission to the hospital for PJI by a trained Infection Control Nurse. During the study period, 670 p

Skin, soft tissue, bone, and joint infections in hospitalized patients: epidemiology and microbiological, clinical, and economic outcomes.

BACKGROUND: Infections involving skin, soft tissue, bone, or joint (SSTBJ) are common and often require hospitalization. There are currently few published studies on the epidemiology and clinical and economic outcomes of these infections, whether acquired in the community or healthcare setting, in a large population. OBJECTIVE: To characterize outcomes of culture-proven SSTBJ infection in hospitalized patients, using information from a large database. DESIGN: We identified patients hospitalized in 134 institutions during 2002-2003 for whom specific International Classification of Diseases, N

Budgetary impact of treating acute bipolar mania in hospitalized patients with quetiapine: an economic analysis of clinical trials.

OBJECTIVE: To present a tool that allows estimation of the budget impact of treatments for acute mania in bipolar I disorder from a US healthcare payer perspective. METHODS: Using discrete event simulation, the course of individuals is simulated beginning with hospitalization. Discharge depends on symptom level measured by the Young Mania Rating Scale (YMRS). The treatment effect is determined using time-dependent regression equations derived from trial data, and decision rules obtained from clinical experts. Outcomes include: time to response and symptom resolution; proportion of subjects re

Mental disorders and economic crisis--a study on the development of admission into the psychiatric hospitals of Prussia between 1876 and 1906.

Social causes of mental disorders are a long-standing issue of controversy in psychiatric epidemiology. Following Brenner's (1977) analysis, we analysed the correlation between admissions to psychiatric hospitals and the employment figures for almost 3 decades of the last century in Prussia. The employment figures were taken as an indicator of social stress, whereas the admission figures were taken as an indicator of the incidence of mental disorders. Two groups with different diagnoses were considered in the analysis; nowadays they would more or less correspond to "functional psychosis" (ICD

Hospital Burden and Care Pathways of ANCA-Associated Vasculitis in Italy: A Focus on Clinical, Economic, and Geographic Dimensions.

BACKGROUND: Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAVs) are rare autoimmune diseases characterized by heterogeneous clinical presentation and substantial morbidity. Evidence suggests high healthcare utilization and economic burden of ANCA-associated vasculitis (AAV), particularly owing to hospitalization as well as geographic variation in the organization of specialized care. However, national-level analyses examining outcomes, resource use, and geographic differences in care pathways remain limited. OBJECTIVE: The study aimed to identify hospitalized patients wi

Opioid Intake and Quality of Life After Hospital Discharge From Major Surgery: A Health Economic Evaluation.

BACKGROUND: Earlier studies have not examined the extent to which reducing postoperative opioid use after patient discharge would improve the quality of life of opioid naïve patients, knowledge needed for economic evaluations of drugs and devices (applications) that reduce morphine milligram equivalents. We quantified associations between postoperative opioid use and patient-centered health outcomes using the EuroQol group's EQ-5D-5L values, recorded longitudinally. METHODS: Prospective observational data were collected for adult patients, without regular preoperative opioid use, undergoing i

Using Fetal Fibronectin Test to Reduce Hospital Admissions with Diagnosis of Preterm Labor: An Economic Evaluation Study.

Background: Preterm labor and delivery remain a major problem in obstetrics accounting for perinatal morbidity and mortality. The challenge is to identify those with true preterm labor to avoid unnecessary hospital admissions. The fetal fibronectin (FFN) test is a strong predictor of preterm birth and can help identify women with true preterm labor. However, its cost-effectiveness as a strategy for triaging women with threatened preterm labor is still debatable. Objective: To evaluate the effect of FFN test implementation on hospital resources by reducing the admission rate of threatened prete

The Economic Effects on Acute Care Surgery in a New York City Public Hospital During the COVID Pandemic.

BACKGROUND: Since the start of the COVID-19 pandemic, less acute care surgical procedures have been performed and consequently hospitals have experienced significant revenue loss. We aim to investigate these procedures performed before and after the start of the COVID-19 pandemic, as well as their effect on the economy. METHODS: This is a retrospective analysis of patients who underwent cholecystectomies and appendectomies during March-May 2019 compared to the same time period in 2020 using Chi-square and t-tests. RESULTS: There were 345 patients who presented with appendicitis or cholecysti

High discrepancies in the mortality of hospitalized patients with COVID-19 in the two most economically important states in Brazil.

OBJECTIVE: The aim of this study was to compare the proportion of deaths among hospitalized cases of COVID-19 in São Paulo and Rio de Janeiro, stratified by private and public services. METHODS: Hospitalization data for severe acute respiratory syndrome (SARS) were obtained using the SIVEP-Gripe Database. All hospitalized adults who were diagnosed as COVID-19 or unspecified SARS, between January and December 2020, were included in the analysis. Logistic regression models were fitted to evaluate the risk of death between Rio de Janeiro and São Paulo, adjusted for age, sex, and comorbidities.

A behavioral economics-based telehealth intervention to improve aspirin adherence following hospitalization for acute coronary syndrome.

PURPOSE: A significant number of patients with acute coronary syndrome (ACS) are nonadherent to aspirin after hospital discharge, with an associated increased risk of subsequent cardiovascular events. The purpose of this pilot study was to test the efficacy of a telehealth intervention based on behavioral economics to improve aspirin adherence following hospitalization for ACS. METHODS: We enrolled 130 participants (c¯X = 58 ± 10.7 years of age, 38% female, 45% black) from two hospitals. Patients were eligible if they owned a smartphone and were admitted to the hospital for ACS, prescribed as

Economic consequences of early inpatient discharge to community-based rehabilitation for stroke in an inner-London teaching hospital.

BACKGROUND AND PURPOSE: In an inner-London teaching hospital, a randomized trial of "conventional" care versus early discharge to community-based therapy found no significant differences in clinical outcomes between patient groups. This report examines the economic consequences of the alternative strategies. METHODS: One hundred sixty-seven patients received the early discharge package, and 164 received conventional care. Patient utilization of health and social services was recorded over a 12-month period, and cost was determined using data from provider departments and other published sourc

Clinical and economic outcomes of empiric parenteral antibiotic therapy for pneumonia: a retrospective study of 1,032 hospitalized patients.

The spiraling increase in pharmaceutical costs in hospitals means that the economic aspects of pharmacological therapy can no longer be ignored. The aim of the present study was to evaluate care and cost outcomes following initial therapy with parenteral antibiotics in a hospital setting for the treatment of lower respiratory tract infection (pneumonia and pleurisy with or without complications). From January to December 1996, a total of 1,032 patients were reviewed. The mean age of the patients was 71.9 years (range, 16-100 years). Piperacillin was most frequently prescribed (n=369), followed

Impact of a hand hygiene campaign in a tertiary hospital in South Korea on the rate of hospital-onset methicillin-resistant Staphylococcus aureus bacteremia and economic evaluation of the campaign.

BACKGROUND: Hand hygiene (HH) is the most important factor affecting health care-associated infections. METHODS: We introduced a World Health Organization HH campaign in October 2010. The monthly procurement of hand sanitizers per 1,000 patient days was calculated, and the monthly incidence of methicillin-resistant Staphylococcus aureus bacteremia (MRSAB), classified into community- and hospital-onset (HO), was measured from a microbiologic laboratory database. Trends of MRSAB incidence were assessed using Bayesian structural time series models. A cost-benefit analysis was also performed base
