[Course modifying therapy of Alzheimer's dementia].

The current therapy of Alzheimer's disease is primarily symptomatic. Drugs which aim to modify the course of the disease are currently being developed and tested in clinical trials. Given the complex and partly unknown pathogenesis of the disease, failure of such forms of therapy has to be taken into account. Clinical epidemiology suggests a possible neuroprotective effect of statins and non-steroidal anti-inflammatory drugs, however, the molecular basis of these effects has to be further unraveled. Therapies that modify the course of Alzheimer's disease are only likely to be effective years i

ABCG2 rs2231142 variant in hyperuricemia is modified by SLC2A9 and SLC22A12 polymorphisms and cardiovascular risk factors in an elderly community-dwelling population.

BACKGROUND: The ABCG2 rs2231142 single nucleotide polymorphism (SNP) is one of the most significant genetic variants associated with hyperuricemia (HUA) in Asian populations. However, the risk of ABCG2 rs2231142 variants for HUA could interact with other important HUA risk variants and cardiovascular factors. This study investigated the effects of the combined association among ABCG2 rs2231142 and multiple HUA genetic variants or cardiovascular risk factors on HUA risk and serum uric acid (sUA) levels in an elderly Chinese population. METHODS: A total of 1206 participants over 65 years old we

Prevalence, Incidence, and Risk Factors for Tinnitus.

How many people are affected by tinnitus? Is the risk of developing tinnitus on the rise or has it been declining over time? What modifiable lifestyle factors could help to prevent tinnitus? These population-based questions can be addressed through epidemiological research. Epidemiology refers to the underlying and basic science of public health. It describes the study of the distribution and determinants of health-related states or events in specified populations and the application of this study to the control of health problems. There are two key concepts in epidemiology: (1) measures of fr

Minimizing Risks of Liver Transplantation With Steatotic Donor Livers by Preferred Recipient Matching.

BACKGROUND: Donor livers with ≥30% macrosteatosis (steatotic livers) represent a possible expansion to the donor pool, but are frequently discarded as they are associated with an increased risk of mortality and graft loss. We hypothesized that there are certain recipient phenotypes that would tolerate donor steatosis well, and are therefore best suited to receive these grafts. METHODS: Using national registry data from the Scientific Registry of Transplant Recipients between 2006 and 2017, we compared 2048 liver transplant recipients of steatotic livers with 69 394 recipients of nonsteatotic

Glycemic control is an important modifiable risk factor for uveitis in patients with diabetes: A retrospective cohort study establishing clinical risk and ophthalmic disease burden.

AIM: To characterize the risk uveitis, scleritis or episcleritis in relation to diabetes, glycaemic control, and co-existence of retinopathy. METHODS: Using the Royal College of General Practitioners Research and Surveillance Centre database, we established the prevalence of acute uveitis and scleritis or episcleritis over a six-year period among populations without(n = 889,856) and with diabetes(n = 48,584). We evaluated the impact of glycaemic control on disease risk. Regression modeling was used to identify associations, adjusting for clinical and demographic confounders. RESULTS: Inciden

Mercury exposure and risk of cardiovascular disease in two U.S. cohorts.

BACKGROUND: Exposure to methylmercury from fish consumption has been linked to a potentially increased risk of cardiovascular disease, but evidence from prior studies is equivocal. Beneficial effects of the ingestion of fish and selenium may also modify such effects. METHODS: Among subjects from two U.S. cohorts (a total of 51,529 men and 121,700 women) whose toenail clippings had been stored, we prospectively identified incident cases of cardiovascular disease (coronary heart disease and stroke) in 3427 participants and matched them to risk-set-sampled controls according to age, sex, race, a

Potentially modifiable risk factors for acute kidney injury after surgery on the thoracic aorta: a propensity score matched case-control study.

Perioperative risk factors were identified for acute kidney injury (AKI) defined by the RIFLE criteria (RIFLE = risk, injury, failure, loss, end stage) after surgery on the thoracic aorta with cardiopulmonary bypass (CPB) in this case-control study. A retrospective review was completed for 702 patients who underwent surgery on the thoracic aorta with CPB. A total of 183 patients with AKI were matched 1:1 with patients without AKI by a propensity score. Matched variables included age, gender, body-mass index, preoperative creatinine levels, estimated glomerular filtration rate, a history of hyp

Differential response to early nutrition supplementation: long-term effects on height at adolescence.

BACKGROUND: The classical risk approach to predicting who benefits from an intervention is unsound because it relies on the theoretical assumption that those at risk will necessarily benefit. A better approach to systematically test who benefits from nutrition supplementation is proposed using interactive models. METHODS: Differential effects of nutrition supplementation during early childhood on stature at adolescence were studied in 245 males and 215 females to identify determinants of long-term benefit from food supplementation. Factors studied included family socioeconomic status (SES) an

Oral contraceptives and the risk of multiple sclerosis: a review of the epidemiologic evidence.

Multiple sclerosis (MS) is more frequent in women than in men, suggesting that sex hormones could play a role in the development of MS. For this reason, several studies have assessed whether use of oral contraceptives modifies the risk of MS. In this article, we review the methodology and results of published epidemiologic studies addressing this issue. On the whole, the existing epidemiologic evidence does not support an important effect of oral contraceptive use on the risk of MS, though it does suggest that oral contraceptives might delay the onset of the disease.

Noise and cardiovascular disease: a review of the literature 2008-2011.

The association between noise and cardiovascular disease has been studied for several decades and the weight of evidence clearly supports a causal link. Nevertheless, many questions remain, such as the magnitude and threshold level for adverse effects of noise, how noise and other cardio-toxic pollutants (such as particulate matter) may interact in disease causation, identification of vulnerable populations, of exposure modifiers (i.e., location of bedrooms) and of other effect-modifiers (i.e., gender), and how epidemiologic methodology can be improved. This review describes contributions to l

Statin therapy for Alzheimer's disease: will it work?

Disease-modifying therapies are being developed for Alzheimer's disease (AD). These are expected to slow the clinical progression of the disease or delay its onset. Cerebral accumulation of amyloid beta (A beta) peptides is an early and perhaps necessary event for establishing AD pathology. Consequently therapies aimed at attenuating brain amyloidosis are expected to be disease modifying. Based on the epidemiological evidence pointing to a link between cholesterol metabolism and AD and the numerous laboratory studies implicating cholesterol in the process of A beta production and accumulation,

Interaction of prenatal bisphenols, maternal nutrients, and toxic metal exposures on neurodevelopment of 2-year-olds in the APrON cohort.

BACKGROUND: Epidemiological studies suggest that Bisphenol-A (BPA) is a developmental neurotoxicant, but the modifying effects of maternal nutrient status or neurotoxicant metal co-exposures have not been reported. Bisphenol-S (BPS) is being used as a BPA-alternative, but few epidemiological studies have evaluated its effects. OBJECTIVES: To examine if prenatal maternal BPA or BPS exposure are associated with children's neurodevelopment at two years of age while adjusting for effect-measure modification by sex, maternal nutrients, and co-exposure to neurotoxic metals. METHODS: Total BPA and

Pain, sleep complaints, and depressive symptom clusters and their association with cognitive decline in older Mexican Americans.

INTRODUCTION: Older Mexican Americans, a fast-growing segment of the U.S. population, have high rates of pain, depression, and sleep complaints. Data are lacking on how these symptoms overlap and affect cognition in this population. We examined patterns of pain-sleep-depressive symptom clusters and their association with cognitive decline in older Mexican Americans. METHODS: We used data from the Hispanic Established Populations for the Epidemiologic Study of the Elderly (2010-2016), a study of older Mexican Americans in Southwestern U.S. We used latent class analysis to identify clusters bas

Revisiting the interaction between birth weight and current body size in the foetal origins of adult disease.

The four models proposed for exploring the foetal origins of adult disease (FOAD) hypothesis use the product term between size at birth and current size to determine the relative importance of pre- and post-natal growth on disease in later life. This is a common approach for testing the interaction between an exposure (in this instance size at birth) and an effect modifier (in this instance current size)--incorporating the product term obtained by multiplying the exposure and effect modifier variables within a statistical regression model. This study examines the mathematical basis for this ap

Interaction of apolipoprotein E epsilon 4 with other genetic and non-genetic risk factors in late onset Alzheimer disease: problems facing the investigator.

The Apolipoprotein E4 allele (Apo-epsilon4) is the major susceptibility gene for late onset Alzheimer Disease (AD) but epidemiological data suggest that the effect of this allele is modified in different individuals by genetic or environmental factors. Age and head injury are major non-genetic factors modifying the Apo-epsilon4 risk. There is conflicting data as to whether alleles of other chaperon proteins (such as alpha-1-antichymotrypsin (ACT)) or Apo-epsilon4 receptors (such as the VDRL receptor) modify the Apo-epsilon4 risk for AD. We analyze problems posed by genetic association studies

Progression of degenerative mitral stenosis: insights from a cohort of 254 patients.

BACKGROUND AND AIM OF THE STUDY: Degenerative mitral stenosis (DMS) is an increasingly common echocardiographic finding, yet the clinical and biological behavior and rate of progression of the condition are unknown. METHODS: A total of 254 patients was identified from the authors' echocardiographic database with DMS, defined as severe mitral annular calcification with extension into the mitral leaflets resulting in transmitral flow acceleration with a mean diastolic gradient of >2 mmHg in the absence of commissural fusion. Each patient required paired echocardiograms to have been recorded at

The Urgent Need to Address Modifiable Risk Factors to Prevent Young-Onset Colorectal Cancer in Mexico.

Colorectal cancer (CRC) represents a major cause of morbidity and mortality worldwide and has traditionally been considered a disease of older adults. However, in recent decades, there has been a sustained increase in incidence and mortality among younger populations, particularly in low- and middle-income regions such as Latin America. This article aims to provide an updated overview of CRC epidemiology in Mexico, with emphasis on early-onset disease and modifiable lifestyle-related risk factors. A narrative review of epidemiological studies, population-based reports, and regional case-contro

Exercise and smoking: health rivals revealing shared protective mechanisms in Parkinson's?

Compelling epidemiological evidence suggests that exercise and smoking are modifiable risk factors that are linked to a reduced risk of Parkinson's disease. These two risk factors represent opposite ends of a spectrum: exercise is universally embraced, while smoking is rightly eschewed for its established adverse health effects. Yet, intriguingly, preclinical evidence suggests that at their biological cores, exercise and some of the many components of tobacco may share strikingly similar working mechanisms that may favorably modify PD risk or disease course, for which definitive evidence is st
