Involvement of the lesser sigmoid notch in elbow fracture dislocations.

BACKGROUND: This study addressed the primary null hypothesis that there is no difference in the articular surface area of the lesser sigmoid notch involved among Mayo classes. Secondarily, we analyzed the fracture line location and the pattern of lesser sigmoid notch articular surface involvement among Mayo classes. METHODS: Using quantitative 3-dimensional computed tomography, we reconstructed and analyzed fractures involving the lesser sigmoid notch articular surface in 52 patients. Further, we assessed the surface area involved in the fracture, the number of fracture fragments, and the loc

Biepicondylar fracture presenting with elbow dislocation: a case report.

INTRODUCTION: Biepicondylar fracture of the elbow is very rare, and to date there have only been three reports of this injury and its treatment in the English scientific literature. This case report evaluates the surgical internal fixation of a biepicondylar fracture of the elbow with an associated dislocation. CASE PRESENTATION: We report the case of a 15-year-old Turkish girl with a biepicondylar fracture dislocation of the left elbow. Open reduction and an internal fixation operation were applied. There were no complications. CONCLUSION: In these injuries, open reduction and internal fixa

Posteromedial dislocation of the elbow with lateral condyle fracture in children.

Posteromedial dislocation of the elbow in children is an extremely rare injury. The current study describes four boys with posteromedial dislocation of the elbow associated with a displaced fracture of the lateral humeral condyle. The patients ranged in age from 6 to 12 years (average age, 9 years 6 months). Closed reduction of the elbow dislocation and open reduction of the lateral humeral condyle fracture with fixation by Kirschner wires by a lateral approach was done in each patient. This was followed by 4 weeks immobilization wearing a long-arm cast. There were three excellent results and

Fractures of the elbow's lateral column radial head and capitellum.

Fractures of the lateral column of the elbow involve radial head fractures or capitellum fractures. As the biomechanics of the elbow have become better understood, the importance of the radiocapitellar joint in stabilizing the elbow has been emphasized. Preservation of the radial head has gained importance. For nondisplaced radial head fractures, good results are generally obtained with a short period of immobilization followed by early range of motion. In managing displaced radial head fractures, improved techniques and more versatile instrumentation have helped make preservation more feasibl

Transarticular fixation with the capacity for motion in fracture dislocations of the elbow.

Post-traumatic stiffness of the elbow joint is a frequent result of immobilisation leading to severe disability in the use of the upper extremity. Recognition of the tendency to stiffness leads to the assumption that the strong self-healing forces of the capsule and ligament apparatus converts the initial instability of the joint after ligament disrupture, into a high-grade undirected stability following immobilisation. Directed stability as it is produced by the natural ligament apparatus of the joint on the other hand produces a guided movement of the joint in one direction. These theoretica

[Fractures of the lateral condyle of the elbow in children. Radiologic aspects].

Fractures of the lateral condyle of the humerus classified as type IV in Salter schema are difficult to diagnose when the capitellum is not ossified. Therefore it is necessary to know exactly the radiological lines which are constructed from the humerus and the radius to the capitellum. Diagnosis is also difficult when the ossification center of the capitellum humeri is only slightly displaced. The epiphyseal fracture line which is always visible at surgery can be missed on X-Ray when ossification has not yet appeared. Laminograms are necessary in some cases to be sure that the fracture is dis

Fractures and dislocations about the elbow in the head-injured adult.

Of 548 head-injured adults, 16 sustained injuries about the elbow (3%). Sixteen of 18 patients (89%) developed heterotopic ossification (HO). All elbows which had been dislocated developed new bone in the substance or adjacent to the medial or lateral collateral ligaments, or both. Some dislocations also developed significant HO in all planes. Final motion correlated with the amount of HO in the joint. Supracondylar fractures generally developed a moderate amount of anterior or posterior HO, or both. Motion was moderately compromised. Posteriorly located HO developed in olecranon fractures. Ra

[Immediate surgical treatment of supracondylar fractures of the elbow in childhood].

The results of surgical treatment, in children, of 10 supracondylar factures of the humerus, complicated by vascular and neurological disturbances were so satisfactory that open reduction is proposed for all those cases with severe lesions and oedema requiring a prolonged period of continuous trans-skeletal traction even without neurological and vascular problems.

Radial head arthroplasty: A pillar of stability in complex elbow fractures.

Radial head (RH) arthroplasty (RHA) has emerged as a critical intervention in the management of complex elbow fractures, particularly Mason type III and IV injuries where the native RH is irreparable. Beyond its role in pain relief and joint congruity, RHA serves as a biomechanical cornerstone for restoring the lateral column and ensuring elbow stability, especially in the presence of associated ligamentous injuries or fracture-dislocations. This editorial synthesizes current evidence on RHA in Mason type III and IV RH fractures, with attention to biomechanical rationale, implant design, and c

Elbow hemiarthroplasty in distal humeral fractures: Indication, surgical technique and results.

The surgical management of distal humerus fractures in adults generally consists in open reduction internal fixation (ORIF) or total elbow arthroplasty (TEA). Hemi humeral hemiarthroplasty (EHA) is a treatment option for unreconstructable intra-articular distal humerus fractures. It is a reasonable option in patients over the fifth decade and its potential advantages are to eliminate the complications related to the ulnar component such as wear of the hinge (busching wear) or the aspetic loosening of the ulnar component. The potential disadvantages are the risk of instability with the possibil

Prospective longitudinal evaluation of elbow motion following pediatric supracondylar humeral fractures.

BACKGROUND: Temporary elbow stiffness after the treatment of a supracondylar humeral fracture in a child is often a concern of parents. However, little attention has been devoted to documenting, longitudinally, the time required for motion recovery. The purpose of the present study was to provide a prospective, longitudinal evaluation of elbow motion in a large population of pediatric patients undergoing treatment of a supracondylar humeral fracture. METHODS: We prospectively examined 373 patients (375 fractures) who presented to our urgent care center between March 1, 2007, and September 30,

[Primary total elbow replacement for complex intra-articular distal humerus fractures].

INTRODUCTION: Open reduction and internal fixation of multifragmentary intra-articular fractures of the distal humerus often do not provide satisfactory results in elderly patients with osteoporosis. METHOD: From December 2001 to January 2008 a total elbow arthroplasty (Coonrad-Morrey, Zimmer, USA) was performed on 12 patients (average age 81+/-9 ears) who presented with a type C distal humeral fracture. The mean time of follow-up with clinical and radiological assessment was 28+/-17 months. RESULTS: The Mayo score showed a good functional result with an average of 81+/-9 out of 100. DASH an

Medial elbow approaches for coronoid fractures: risk to the ulnar nerve.

BACKGROUND: Coronoid fractures often require open reduction internal fixation (ORIF) to restore elbow stability. The flexor pronator split, flexor carpi ulnaris (FCU) split, and Taylor and Scham (T&S) approaches are frequently used medial approaches to access the coronoid. The ulnar nerve can be released or transposed when performing these exposures. The optimal medial surgical approach and management of the ulnar nerve has not been clearly defined. The purpose of this study was to compare postoperative ulnar nerve complications in coronoid fractures undergoing ORIF following a medial surgical

Humeral trochlear morphology does not influence coronoid fractures in elbow dislocation.

PURPOSE: Traumatic elbow dislocation is the second most frequent joint dislocation, even though the elbow is a congruent and stable joint. Individual variability in anatomical congruence of the elbow and how it relates to simple or complex instability has rarely been studied in the literature; we hypothesized that a greater articular coverage by the humeral trochlea would be more likely to result in complex dislocation. The aim of this study is to analyze trochlear morphology in simple (SED) and complex elbow dislocation (CED), to assess whether the degree of humeroulnar joint congruence influ

Three dimensional printing technology and materials for treatment of elbow fractures.

PURPOSE: 3D printing is a rapid prototyping technology that uses a 3D digital model to physically build an object. The aim of this study was to evaluate the peri-operative effect of 3D printing in treating complex elbow fractures and its role in physician-patient communication and determine which material is best for surgical model printing. METHOD: Forty patients with elbow fractures were randomly divided into a 3D printing-assisted surgery group (n = 20) and a conventional surgery group (n = 20). Surgery duration, intra-operative blood loss, anatomic reduction rate, incidence of complicatio

Difficult supracondylar elbow fractures in children: analysis of percutaneous pinning technique.

After treatment with two lateral percutaneous pins, 80 children with grade II-III supracondylar elbow fractures were reviewed. The result was good in 55 patients (68%) and was the same for extension- and flexion-type injuries. These patients had less than 10 degrees change in carrying angle and less than 20 degrees total change in range of motion. This method eliminates risk of iatrogenic injury to the ulnar nerve but is technically very demanding. Redisplacement, joint penetration, and damage to the capitellar side of growth plate can be avoided only by positioning the pins divergently.

Surgical outcomes for post-traumatic stiffness after elbow fracture: comparison between open and arthroscopic procedures for intra- and extra-articular elbow fractures.

HYPOTHESIS: We hypothesized that arthroscopic osteocapsular arthroplasty has a comparable outcome to that of the corresponding open procedure. METHODS: Patients treated with osteocapsular arthroplasty for post-traumatic stiffness were assigned to open procedure (OPEN) and arthroscopic procedure (ARTHRO) groups. The clinical outcomes were measured based on range of motion (ROM), Mayo Elbow Performance Score (MEPS), and visual analog scale (VAS) score. Based on the initial trauma, the patients were grouped into either intra-articular fracture (I) or extra-articular fracture (E) groups, followed
