Multilevel Customized 3D-Printed Titanium Alloy Interbody Cages used to Treat Congenital Scoliosis: A Case Report.

INTRODUCTION: Certain elbow pathologies can give rise to considerable loss of joint function, which would severely affect the daily living activities of the patients. Total elbow arthroplasty (TEA) can be a safe and reasonable motion-preserving and pain-relieving option in such debilitating elbow conditions. The aim of this case series is to evaluate the functional outcome of TEA in young patients with four different indications after an 8-year follow-up. CASE REPORT: We report a case series of four TEA cases done for different indications, namely rheumatoid arthritis, chronic elbow dislocati

Radial Head Arthroplasty: A Systematic Review.

BACKGROUND: Despite the expanding body of literature on radial head arthroplasty, the increasing understanding of elbow anatomy, biomechanics, and kinetics, and the evolution of surgical techniques and prosthesis designs, there is currently no evidence to support one type of radial head prosthesis over another. The purposes of the present report were to review the literature and to explore the association between prosthesis design variables and the timing of surgery and the outcome of modern radial head arthroplasty. METHODS: The literature search was limited to studies involving skeletally m

Silicone Locking-Liner Socket with a Lightweight Aesthetic Prosthesis for Short Congenital Forearm Stumps: A Report of Two Patients.

Conventional prosthetic sockets can be uncomfortable and restrictive to elbow flexion when fitted on short forearm stumps due to their high trimline. We report improved patient experience after switching to a locking silicone-liner socket in conjunction with a lightweight aesthetic prosthesis in two congenital transradial amputees with short stumps who were previously dissatisfied with their conventional prosthesis. To reduce weight, the forearm section of the prosthesis was constructed using a lightweight design. Seven of eight attributes, namely aesthetics, comfort, suspension, ease of donni

Updates on Radial Head Arthroplasty in Trauma.

The radial head plays a crucial role in maintaining normal elbow kinematics, and radial head arthroplasty (RHA) can help restore normal elbow kinematics after radial head fracture. Most modern implants are made of polished metal alloy with stems that may be smooth, press-fit, or cemented. There is no consensus as to the superiority of smooth versus press-fit stems, and both can result in good clinical outcomes. Cemented stems are no longer indicated in routine RHA. Correct implant positioning is critical, and numerous methods of assessment have been described.

Kinematic analysis of impairments and compensatory motor behavior during prosthetic grasping in below-elbow amputees.

After a major upper limb amputation, the use of myoelectric prosthesis as assistive devices is possible. However, these prostheses remain quite difficult to control for grasping and manipulation of daily life objects. The aim of the present observational case study is to document the kinematics of grasping in a group of 10 below-elbow amputated patients fitted with a myoelectric prosthesis in order to describe and better understand their compensatory strategies. They performed a grasping to lift task toward 3 objects (a mug, a cylinder and a cone) placed at two distances within the reaching ar

Humeral amputation following total elbow arthroplasty.

PURPOSE: Total elbow arthroplasty (TEA) is associated with a relatively high complication rate, and exceptionally catastrophic complications might lead to amputation. The purpose of this study was to determine the incidence and aetiology of amputation performed at our institution in upper extremity limbs with a prior TEA. METHODS: Between 1973 and 2018, 1906 consecutive TEAs were performed at our institution. Upper extremity amputation was performed in seven (0.36%) elbows with five transhumeral amputations and two shoulder disarticulations. The group consisted of five females and two males w

Mechanical failure of the Coonrad-Morrey linked total elbow arthroplasty: A case report.

Semiconstrained (linked design) total elbow arthroplasty is indicated in a wide variety of cases. Long-term survival is better than with non-linked prostheses. However, mechanical failure of the hinge mechanism is a complication that may occur during follow-up. We report a case of failure of the axle assembly of a Coonrad-Morrey elbow prosthesis 8 years after implantation for nonunion of a supracondylar distal humerus fracture. Initial revision surgery included changing the axle and the polyethylene bushings. Revision surgery was necessary 1 year later when the axle failed again. A custom-desi

The acute bipolar radial head replacement for isolated unreconstructable fractures of the radial head.

For the acute treatment of radial head fractures, radial head arthroplasty can be considered if open reduction and internal fixation is not technically possible and if simple radial head resection is contraindicated. We report our experience with a bipolar Judet radial head prosthesis. After a mean follow-up of 41.7 months, 16 patients were assessed following radial head replacement. The outcome was assessed clinically, functionally, and radiographically. Outcome was satisfactory in 87.5% of cases. The mean post-operative ranges of motion were greater than the functional arcs both in flexion-e

A retrospective comparative cohort study of radial head arthroplasty versus resection in complex elbow dislocations.

INTRODUCTION: The management of radial head fractures for the treatment of complex elbow dislocations remains controversial. Until the publication of recent therapeutic algorithms, different actions have been carried out, including resection or arthroplasty of the radial head. This study aims to compare the clinical and radiological results of these two techniques. MATERIALS AND METHODS: A retrospective study was conducted between the years 2001 and 2016 in complex elbow dislocations associating comminuted fractures of the radial head (Mason type IV) for two cohorts: 20 patients who received

[Clinical results after replacement of comminuted radial head fractures (Mason III and IV) by the bipolar radial head prosthesis of Judet].

AIM: Long-term results after resection of comminuted fractures of the radial head (Mason III and IV) may lead to valgus elbow instability, radius proximalisation and rotatory instability. Radial head replacement has been used to prevent and treat these complications. The aim of this study was to define the value of the bipolar radial head prosthesis of Judet for treating comminuted fractures of the radial head. METHOD: Over a 5-year period, 14 radial head prosthesis were implanted in 12 patients. Retrospectively, we studied the clinical and radiological results. RESULTS: Eleven patients with

Radiocapitellar stability: the effect of soft tissue integrity on bipolar versus monopolar radial head prostheses.

INTRODUCTION: Radiocapitellar stability depends, in part, on concavity-compression mechanics. This study was conducted to examine the effects of the soft tissues on radiocapitellar stability with radial head prostheses. HYPOTHESIS: Monopolar radial head implants are more effective in stabilizing the radiocapitellar joint than bipolar radial head prostheses, with the soft tissues intact or repaired. MATERIALS AND METHODS: Twelve fresh frozen elbow specimens were used to evaluate radiocapitellar stability with monopolar and bipolar radial heads. The study variables focused on varying soft tiss

Souter-Strathclyde total elbow arthroplasty: medium-term results.

The Souter-Strathclyde unconstrained elbow prosthesis was prospectively studied in 36 patients (45 prostheses) with rheumatoid arthritis (Larsen grade 4 and grade 5). The mean age of the patients at the time of operation was 63 years (range: 39 to 75 years). Eight patients (9 prostheses) died within five years of implantation, from causes unrelated to the elbow arthroplasty. One patient was lost to follow-up, leaving 27 patients (35 prostheses) for review. The mean length of follow-up was 98 months (range: 60 to 174 months). At 8.2 years follow-up, the prosthesis showed a probability of surviv

Revision Total Elbow Arthroplasty.

Despite improvements in our understanding of elbow biomechanics and implant design, early revision are higher following total elbow arthroplasty (TEA) compared with hip and knee arthroplasty. Revision TEA remains a standard but technically challenging procedure with the potential for substantial morbidity. This review discusses the diagnosis and management of the failed primary TEA. Current concepts related to surgical treatment with revision TEA indicated for prosthetic joint infection, periprosthetic fracture, and aseptic osteolysis will be reviewed. In addition, strategies and adjunctive pr

Humeral fracture between a total elbow and total shoulder arthroplasty.

This article presents a case of a 71-year-old woman with a humeral fracture between a cementless reverse total shoulder arthroplasty and a cemented total elbow arthroplasty and discusses our treatment plan. Surgical treatment was performed after the patient was informed of possible complications and the benefits of surgery including: early, complete restoration of arm anatomy, greater functional improvement of the adjacent joints, and increased risk of nonunion with nonoperative treatment. The fracture was comminuted and extended proximally around the shoulder prosthesis. Through the posterior

Total Elbow Arthroplasty for Distal Humerus Fractures.

Total elbow arthroplasty is a good treatment alternative for selected patients with distal humerus fractures. Its attractiveness is related to several factors, including the possibility of performing the procedure; leaving the extensor mechanism intact; faster, easier rehabilitation compared with internal fixation; and overall good outcomes reported in terms of both pain relief and function. Implant failure leading to revision surgery does happen, and patients must comply with certain limitations to extend the longevity of their implant. Development of high-performance implants may allow expan

Lengthening of short stumps for functional use of prostheses.

The length of a stump plays an important role when using a prosthesis. It is essential to have an adequate length in the stump in order to apply the prosthesis. Therefore, it is necessary to attempt to lengthen short stumps. The authors have reviewed the results in 2 patients who had such lengthening of stumps one due to traumatic amputation of the forearm at 6.25 cm distally from the elbow and the other due to congenital deficiency of the radius and ulna treated by the Ilizarov techinque to improve the fitting of prostheses. Sixty percent (60%) and 40% lengthening was achieved respectively. A
