[Language mapping with direct electrical stimulation].

Language mapping with electrical stimulation in patients with lesions close to the language area aids surgery by identifying the language area in each individual. This method is quite important in a clinical setting, and is the most reliable for preventing persistent language deficits after resection. Furthermore, language mapping with electrical stimulation has revealed that the functional organization of the language area is more complex than previously suggested by functional MRI and other methods. The language-related sites where electrical stimulation induced abnormality during language t

Electrical stimulation for the suppression of pathological tremor.

Pathological tremor is manifested as an involuntary oscillation of one or more body parts. Tremor greatly decreases the quality of life and often prevents the patient from performing daily activities. We hypothesized that sensors-driven multichannel electrical stimulation could stabilize affected joints by activating the antagonistic muscles during involuntary activation of agonist muscles and vice versa (out-of-phase stimulation). Here, we present the new system (hardware and software) and the testing of its operation. The hardware consists of a multichannel stimulator and inertial sensors fo

[Peripheral electrical stimulation in neurogenic bladder].

For many years peripheral electrical stimulation has been performed to treat urinary tract dysfunction in neurogenic patients. Numerous methods have been used, involving sacral roots as peripheral nerve and pelvic organs. All of them are not valuable today. Sacral neuromodulation, tibial posterior and pudendal nerve stimulation have been successful for treatment of neurogenic detrusor overactivity in the short/middle term. Methods and respective interest of the main procedures will be discussed.

Functional electrical stimulation for neuromuscular applications.

Paralyzed or paretic muscles can be made to contract by applying electrical currents to the intact peripheral motor nerves innervating them. When electrically elicited muscle contractions are coordinated in a manner that provides function, the technique is termed functional electrical stimulation (FES). In more than 40 years of FES research, principles for safe stimulation of neuromuscular tissue have been established, and methods for modulating the strength of electrically induced muscle contractions have been discovered. FES systems have been developed for restoring function in the upper ext

Electrical and ultrasound stimulation for scaphoid fractures.

The standard treatment for delayed union or nonunion of the scaphoid is operative management. Electrical stimulation has been employed in these clinical situations in patients unable or unwilling to undergo surgical intervention. Recent interest has also focused on the use of low-intensity ultrasound as an adjunct to healing in distal radius and tibial fractures. Results with the use of ultrasound for scaphoid fractures is encouraging. A review of the mechanisms of action, clinical results, and possible indications is presented for these two nonoperative modalities.

Electrical stimulation as a treatment for stress incontinence.

Much research has been conducted into the use of electrical stimulation to restore function in weak/atrophied muscle and it is used widely in the field of muscle rehabilitation. As stress incontinence is a condition which is the result of pelvic floor muscle weakness, it is thought that the symptoms of this condition may be alleviated once the strength and endurance characteristics of this muscle group have been improved. Many studies have been conducted to evaluate the efficacy of various types of electrical stimulation, although definitive conclusions have yet to be drawn. Current forms of e

Electrically stimulated smooth muscle neosphincter.

BACKGROUND: Most patients undergoing total anorectal reconstruction suffer some degree of incontinence despite the incorporation of an electrically stimulated gracilis neosphincter. As smooth muscle has the ability to maintain prolonged contraction without fatigue, the aim of this study was to assess the feasibility of developing an electrically stimulated smooth muscle neosphincter. METHODS: Electrical stimulation of the rabbit colon was performed via intramural wire electrodes using a constant voltage DC stimulator. Contractile activity was recorded by serosal strain gauges and an intralumi

Electrical stimulation of upper airway musculature.

Investigators have postulated that pharyngeal collapse during sleep in patients with obstructive sleep apnea (OSA) may be alleviated by stimulating the genioglossus. The effect of electrical stimulation (ES) of the genioglossus on pharyngeal patency was examined in an isolated feline upper airway preparation and in apneic humans during sleep. We found that stimulation of the genioglossus (n = 8) and of the hypoglossal nerve (n = 1) increased maximum airflow through the isolated feline upper airway in humans during sleep. Additional findings in the isolated feline upper airway suggest that such

Clinical applications of functional electrical stimulation.

Functional Electrical Stimulation (FES) allows the restoration of controlled muscle contractions, and hence limb function via computer control, in patients with irrecoverable upper motor neuron lesions. Wide experience has been recorded in spinal cord injured paraplegics and tetraplegics and to a lesser extent in cerebrovascular accident victims. The electrodes, either surface or implanted, stimulate muscles electrically through a stimulator activated by a control source which is in turn activated by the remaining functions of the user. Future advances in electrode technology and control and c

Effects of electrical brainstem stimulation on tinnitus.

The primary aim of the auditory brainstem implant (ABI), an investigational device, is to restore auditory sensations in neurofibromatosis-2 patients deafened by bilateral acoustic neuroma surgery. An electrode is placed on the dorsal cochlear nucleus after the tumor is removed through the translabyrinthine approach. This procedure was performed on 18 patients: 13 were completely deafferentated after bilateral tumor removal and had received the ABI during surgery on the second-sided tumor; 5 were implanted at first-sided tumor removal. Three of these 5 patients had usable hearing remaining on

Electrical stimulation of osteonecrosis of the femoral head.

Osteonecrosis of the femoral head in the adult is a progressive condition that, if untreated, usually results in femoral head collapse and secondary osteoarthritis. The experimental application of electrical and electromagnetic fields has been shown to favorably affect a number of biological processes pertinent to osteonecrosis of the femoral head and has led to several clinical trials. The condition has been treated by the application of electrical fields invasively by the surgical implantation of electrodes within the femoral head and noninvasively by capacitative or inductive coupling. This

Transmucosal electrical stimulation of laryngeal muscles.

A new technique is described that enables discrete activation of individual laryngeal muscles by electrical stimulation across overlying mucosa. In 15 dogs, we defined six distinct motor points by transmucosal stimulation at 3 mA while observing the resulting characteristic position of the arytenoid and true vocal cord. Five dogs were then paralyzed with succinylcholine in order to simulate bilateral vocal cord paralysis. Application of a 3-mA stimulus at each motor point yielded no motion of the cords, but when the current was increased to 20 mA, characteristic responses were elicited. In fiv

Electrical stimulation and substance abuse treatment.

The use of electrical currents for the treatment of disease has been considered since antiquity but it has only been in recent years that suitable devices have been available to scientific investigators to provide stimulation currents for clinical use. These devices have been used extensively for the relief of intractable pain and are an accepted treatment modality today. Other investigators have turned to the investigation of the effects of tiny currents, less than one milliampere, applied to the head. Recent investigations have been successful in the alleviation of such symptoms as insomnia,

[Clinical use of electrical atrial stimulation].

Electric auricle stimulation (EAS) was realized in a total of 77 patients. In 60 of the patients EAS was performed as a hypoxic test (step-wise increase of heart rate, with ECG registration and in many of the cases -- registration of certain indices of general and intracardial hemodynamics) with a view to precise diagnosis of ischemic heart disease and assessment of myocardial contraction function. In 91 per cent of the patients with stenosed coronary pathology on coronarogram, the symptoms of coronary insufficiency were positive (pain attack, ECG changes via ischemic type and/or deterioration
