Stereo-EEG recordings extend known distributions of canonical movement-related oscillations.

Objective.Previous electrophysiological research has characterized canonical oscillatory patterns associated with movement mostly from recordings of primary sensorimotor cortex. Less work has attempted to decode movement based on electrophysiological recordings from a broader array of brain areas such as those sampled by stereoelectroencephalography (sEEG), especially in humans. We aimed to identify and characterize different movement-related oscillations across a relatively broad sampling of brain areas in humans and if they extended beyond brain areas previously associated with movement.Appr

Slow-wave modulation analysis during states of unconsciousness using the novel tau-modulation method.

Objective. Slow-wave modulation occurs during states of unconsciousness and is a large-scale indicator of underlying brain states. Conventional methods typically characterize these large-scale dynamics by assuming that slow-wave activity is sinusoidal with a stationary frequency. However, slow-wave activity typically has an irregular waveform shape with a non-stationary frequency, causing these methods to be highly unpredictable and inaccurate. To address these limitations, we developed a novel method using tau-modulation, which is more robust than conventional methods in estimating the modula

Epileptic seizure prediction in intracranial EEG using critical nucleus based on phase transition.

BACKGROUND AND OBJECTIVE: Epilepsy is the second most prevalent neurological disorder of brain activity, affecting about seventy million people, or nearly 1% of the world population. Epileptic seizures prediction is extremely important for improving the epileptic patients' life. This paper proposed a novel method to predict seizures by detecting the critical transition of brain activities with intracranial EEG (iEEG) signals. METHODS: This article used three key measures of fluctuation, correlation and percolation to quantify pre-ictal states of epilepsy. Based on these measures, a ritical nu

Increased delta power as a scalp marker of epileptic activity: a simultaneous scalp and intracranial electroencephalography study.

BACKGROUND AND PURPOSE: The purpose was to evaluate whether intracranial interictal epileptiform discharges (IEDs) that are not visible on the scalp are associated with changes in the frequency spectrum on scalp electroencephalograms (EEGs). METHODS: Simultaneous scalp high-density EEG and intracranial EEG recordings were recorded in nine patients undergoing pre-surgical invasive recordings for pharmaco-resistant temporal lobe epilepsy. Epochs with hippocampal IED visible on intracranial EEG (ic-IED) but not on scalp EEG were selected, as well as control epochs without ic-IED. Welch's power s

Typical somatomotor physiology of the hand is preserved in a patient with an amputated arm: An ECoG case study.

Electrophysiological signals in the human motor system may change in different ways after deafferentation, with some studies emphasizing reorganization while others propose retained physiology. Understanding whether motor electrophysiology is retained over longer periods of time can be invaluable for patients with paralysis (e.g. ALS or brainstem stroke) when signals from sensorimotor areas may be used for communication or control over neural prosthetic devices. In addition, a maintained electrophysiology can potentially benefit the treatment of phantom limb pains through prolonged use of thes

Magnetoencephalographic features in neurocysticercosis.

BACKGROUND: Magnetoencephalography (MEG) is a method of determining the brain activity noninvasively be detecting the magnetic fields associated with neuronal electrical activities. METHODS: By using 37-channel DC-superconducting quantum interference devices, MEG activity was recorded in a patient with neurocysticerosis, who had a long-term history of epilepsy. RESULTS: MEG clearly demonstrated accumulation of current dipoles originating from high-frequency waves around the cysticercal cyst, while scalp electroencephalogram failed to reveal paroxysmal discharge. Intraoperative electrocortico

Long-Term Stable Subdural Recordings Enabled by Fibrosis-Resistant Hydrogel-Integrated µECoG Arrays.

The long-term performance of microscale subdural arrays is often compromised by adverse tissue responses, leading to increased electrochemical impedance and signal deterioration during chronic applications. Here, the study presents an adhesive hydrogel-integrated micro-electrocorticography (aGel-µECoG) array that improves mechanical compatibility and adhesion to brain tissue, effectively mitigating tissue responses and enabling stable, high-performance electrical communication over days to months. The hydrogel consists of a hydrophilic polyvinyl alcohol and hydrophobic poly3-(trimethoxysilyl)

Biological Impact on the Stability and Reliability of Acute and Chronic Platinum based Thin Film Neural Interfaces in Vivo.

Reliability, stability and biocompatibility of an im-plant are the keys to transferring a preclinical study into the re-ality of clinical applications for diagnostics and therapy of pa-tients. Amongst the smallest and most critical components of neuroprostheses are the neural interfaces to the tissue. These could be seen as the most functional and yet most sensitive parts to connect to and interact with the nervous system. Thin film systems in the submicro- to nanometers range with a high num-ber of channels record biological signals and excite nerves aspiring high spatial sensitivity at the s

Towards predicting ECoG-BCI performance: assessing the potential of scalp-EEG.

Objective. Implanted brain-computer interfaces (BCIs) employ neural signals to control a computer and may offer an alternative communication channel for people with locked-in syndrome (LIS). Promising results have been obtained using signals from the sensorimotor (SM) area. However, in earlier work on home-use of an electrocorticography (ECoG)-based BCI by people with LIS, we detected differences in ECoG-BCI performance, which were related to differences in the modulation of low frequency band (LFB) power in the SM area. For future clinical implementation of ECoG-BCIs, it will be crucial to de

Integrated analysis of anatomical and electrophysiological human intracranial data.

Human intracranial electroencephalography (iEEG) recordings provide data with much greater spatiotemporal precision than is possible from data obtained using scalp EEG, magnetoencephalography (MEG), or functional MRI. Until recently, the fusion of anatomical data (MRI and computed tomography (CT) images) with electrophysiological data and their subsequent analysis have required the use of technologically and conceptually challenging combinations of software. Here, we describe a comprehensive protocol that enables complex raw human iEEG data to be converted into more readily comprehensible illu

Bilateral epilepsy surgery in a 4-year-old child.

OBJECTIVE: Reports on bilateral epilepsy surgical interventions are anecdotal because of the possible neurological deficits caused by them. METHODS: We report on a four-year-old amaurotic child with catastrophic epilepsy due to bilateral occipital cortical dysplasia. After video-EEG monitoring and intraoperative electrocorticography he underwent a two-step bilateral occipital lobectomy. RESULTS: The first resection resulted in only temporary seizure cessation; however, he became seizure-free after the second operation (follow-up: 20 months). CONCLUSION: Patients with catastrophic epilepsy d

Brain mapping techniques to maximize resection, safety, and seizure control in children with brain tumors.

Intraoperative brain mapping techniques were used to localize language cortex, sensorimotor pathways, and seizure foci in children with supratentorial brain tumors. The methods of direct cortical and subcortical stimulation, in addition to electrocorticography, enabled us to maximize tumor resection, minimize morbidity, and eradicate epileptogenic zones which were always adjacent to, but not involving, the tumor nidus. Language localization was found to be quite variable in the children tested and anatomically unpredictable based on the preoperative neurological or radiological examination. Ph

Disorders of Electrophysiology Following Severe Traumatic Brain Injury.

Traumatic brain injury (TBI) produces electrophysiologic abnormalities that are critical to monitor. EEG is required to diagnose most seizures (Sz) that occur after severe TBI, provides information about background activity, and informs prognostic assessment. Non-invasive scalp EEG reflects large areas of the cortex, whereas direct recording of cortex, electrocorticography (ECOG), is invasive but provides more detailed information. The clinical management of electrophysiologic disorders has limited evidence and there is considerable equipoise about the prevention and management of Sz. Motor neuroprosthesis implanted with neurointerventional surgery improves capacity for activities of daily living tasks in severe paralysis: first in-human experience.

BACKGROUND: Implantable brain-computer interfaces (BCIs), functioning as motor neuroprostheses, have the potential to restore voluntary motor impulses to control digital devices and improve functional independence in patients with severe paralysis due to brain, spinal cord, peripheral nerve or muscle dysfunction. However, reports to date have had limited clinical translation. METHODS: Two participants with amyotrophic lateral sclerosis (ALS) underwent implant in a single-arm, open-label, prospective, early feasibility study. Unraveling tactile categorization and decision-making in the subregions of supramarginal gyrus via direct cortical stimulation.

OBJECTIVE: This study aims to investigate the potential of direct cortical stimulation (DCS) to modulate tactile categorization and decision-making, as well as to identify the specific locations where these cognitive functions occur. METHODS: We analyzed behavioral changes in three epilepsy patients with implanted electrodes using electrocorticography (ECoG) and a vibrotactile discrimination task. DCS was applied to investigate its impact on tactile categorization and decision-making processes. We determined the precise location of the electrodes where each cognitive function was modulated.

Channel Increment Strategy-Based 1D Convolutional Neural Networks for Seizure Prediction Using Intracranial EEG.

The application of intracranial electroencephalogram (iEEG) to predict seizures remains challenging. Although channel selection has been utilized in seizure prediction and detection studies, most of them focus on the combination with conventional machine learning methods. Thus, channel selection combined with deep learning methods can be further analyzed in the field of seizure prediction. Given this, in this work, a novel iEEG-based deep learning method of One-Dimensional Convolutional Neural Networks (1D-CNN) combined with channel increment strategy was proposed for the effective seizure pre

Reliability of visual review of intracranial electroencephalogram in identifying the seizure onset zone: A systematic review and implications for the accuracy of automated methods.

Visual review of intracranial electroencephalography (iEEG) is often an essential component for defining the zone of resection for epilepsy surgery. Unsupervised approaches using machine and deep learning are being employed to identify seizure onset zones (SOZs). This prompts a more comprehensive understanding of the reliability of visual review as a reference standard. We sought to summarize existing evidence on the reliability of visual review of iEEG in defining the SOZ for patients undergoing surgical workup and understand its implications for algorithm accuracy for SOZ prediction. We perf

Effects of hippocampal interictal discharge timing, duration, and spatial extent on list learning.

Interictal epileptiform discharges (IEDs) can impair memory. The properties of IEDs most detrimental to memory, however, are undefined. We studied the impact of temporal and spatial characteristics of IEDs on list learning. Subjects completed a memory task during intracranial EEG recordings including hippocampal depth and temporal neocortical subdural electrodes. Subjects viewed a series of objects, and after a distracting task, recalled the objects from the list. The impacts of IED presence, duration, and propagation to neocortex during encoding of individual stimuli were assessed. The effect
