Laparoscopic implantation of electrodes for stimulation of the hypogastric nerve and the vas deferens in dogs.

The minimal invasiveness of laparoscopy prompted us to investigate its use for placement of electrodes for stimulation of the hypogastric nerve and the vas deferens. In an acute canine model, pressure changes in the vas deferens secondary to electrostimulation were recorded. The laparoscopic approach included four ports. Monopolar cuff electrodes were placed around the hypogastric nerve and the vas deferens, and the leads of both were pulled through the abdominal wall. After multiple percutaneous stimulations (20 mAmp., 20 Hz. and 200 microsecond pulse width), the abdomen was opened through a

Pacemaker electrode implantation in patients with persistent left superior vena cava.

Four out of 661 consecutive patients with permanent endocardial VVI pacing had the pacing lead introduced through a persistent left superior vena cava. It was difficult to introduce the lead from the right atrium into the right ventricle because the tip of the lead tended to be deflected away from the tricuspid orifice. This difficulty was overcome by shaping the lead into a pigtail with 3-4 cm wide loop. This avoided the risk of entering a branch of the coronary sinus in order to reach the right atrium. This technique made it easier to position the lead in the right ventricular apex. There we

Test Electrode-Based Estimation of Achievable Insertion Depth in Cochlear Implantation.

Processing the optimal length of electrode arrays for cochlear implants (CIs) is vital for achieving maximum effectiveness, and the results tend to differ between the preoperative radiological estimations and the depth reached at the time of surgery. This study evaluates the feasibility of using a flexible insertion test electrode with colored depth markers to determine the practically achievable electrode insertion depth before CI electrode array placement. The study was conducted at a tertiary center and included patients with inner ear anomalies, reimplantation cases, and profound deafness

Synchrotron-Based Trauma Assessment of Robotic Electrode Insertions in Cochlear Implantation.

OBJECTIVES: Minimizing intraoperative trauma during cochlear implant electrode insertion is crucial for preserving residual hearing. Manual insertions are subject to variability due to surgeon experience and anatomical differences, often leading to inconsistent outcomes. Robotic-assisted systems have been developed to improve insertion precision and reduce trauma. This study evaluates the safety and efficacy of the OTODRIVE system for cochlear implant electrode insertion. METHODS: Fifteen cadaveric human temporal bones were implanted with a variety of electrodes (FLEX26, FLEX28, FLEXSOFT, FLE

Multicargo Porous Cochlear Electrode Coating for Antifibrosis After Cochlear Implantation.

Severe to profound sensorineural hearing loss seriously affects the communication and cognitive ability of the patients. Cochlear implantation (CI) is currently the most effective treatment, while it may damage the remaining inner ear function due to its poor biocompatibility and the resultant fibrosis. Herein, a porous methacrylated poly(dimethylsiloxane) (MA-PDMS)-coated cochlear electrode is presented for CI and hearing protection. The porous MA-PDMS is filled with a hybrid hydrogel system made of dexamethasone sodium phosphate (Dex), Ti3C2Tx MXene (MXene), and methacrylate gelatin (GelMA).

Multifaceted understanding of human nerve implants to design optimized electrodes for bioelectronics.

Bioelectronic medicine is a promising venue for treatment of disabilities using implantable neural interfaces. Peripheral neurostimulation of residual nerves recently enabled multiple functional benefits in amputees. Despite the preliminary promising impact on patients' life, the over-time stability of implants and the related nerve reactions are unclear. To unveil the mechanisms and inform the design of better nerve-electrode interfaces, we engaged a multifaceted approach, merging functional responses from patients, their histological data, and corresponding computational modelling. Chronic nerve health following implantation of femoral nerve cuff electrodes.

BACKGROUND: Peripheral nerve stimulation with implanted nerve cuff electrodes can restore standing, stepping and other functions to individuals with spinal cord injury (SCI). We performed the first study to evaluate the clinical electrodiagnostic changes due to electrode implantation acutely, chronic presence on the nerve peri- and post-operatively, and long-term delivery of electrical stimulation. METHODS: A man with bilateral lower extremity paralysis secondary to cervical SCI sustained 5 years prior to enrollment received an implanted standing neuroprosthesis including composite flat inter

Otoprotection to Implanted Cochlea Exposed to Noise Trauma With Dexamethasone Eluting Electrode.

Cochlear implantation (CI) is now widely used to provide auditory rehabilitation to individuals having severe to profound sensorineural hearing loss (SNHL). However, CI can lead to electrode insertion trauma (EIT) that can cause damage to sensory cells in the inner ear resulting in loss of residual hearing. Even with soft surgical techniques where there is minimal macroscopic damage, we can still observe the generation of molecular events that may initiate programmed cell death via various mechanisms such as oxidative stress, the release of pro-inflammatory cytokines, and activation of the cas

The Impact of a Cochlear Implant Electrode Array on the Middle Ear Transfer Function.

OBJECTIVES: As a treatment for partial deafness with residual hearing in the lower frequency range, the combined acoustic and electric stimulation of the cochlea has become widespread. Acoustic stimulation is provided by a hearing aid's airborne sound and the electric stimulation by a cochlear implant electrode array, which may be inserted through the round window or a cochleostomy. To take advantage of that concept, it is essential to preserve residual hearing after surgery. Therefore, the intracochlear electrode array should not compromise the middle ear vibration transmission. This study in

Cochlear Histopathology as Observed in Two Patients With a Cochlear Implant Electrode With Positioner.

HYPOTHESIS: This study reports the cochlear histopathology of two patients who during life underwent cochlear implantation with a positioner. BACKGROUND: A silastic positioner introduced by the Advanced Bionics Corporation in 1999 was designed to position the electrode of the cochlear implant close to the modiolus. The positioner was recalled in the United States in July 2002 because of an apparent higher incidence of bacterial meningitis in patients in whom the positioner had been placed. METHODS: Four celloidin-embedded temporal bones from two patients with cochlear implants with a positio

Regenerative peripheral nerve interface viability and signal transduction with an implanted electrode.

BACKGROUND: The regenerative peripheral nerve interface is an internal interface for signal transduction with external electronics of prosthetic limbs; it consists of an electrode and a unit of free muscle that is neurotized by a transected residual peripheral nerve. Adding a conductive polymer coating on electrodes improves electrode conductivity. This study examines regenerative peripheral nerve interface tissue viability and signal fidelity in the presence of an implanted electrode coated or uncoated with a conductive polymer. METHODS: In a rat model, the extensor digitorum longus muscle w

Percutaneous implantation of gastric electrodes - a novel technique applied in animals and in patients.

Temporary electrodes implanted under general anaesthesia, or via an oral or percutaneous endoscopic gastrostomy route have been used for testing of gastric electrical stimulation (GES). We have developed a principle for percutaneous electrode implantation. Leads were constructed so that the tip could be anchored to the gastric submucosa under gastroscopic control. Acute experiments were performed in anaesthetized pigs. Three patients referred for nausea and/or vomiting and non-established indications for GES (chronic intestinal pseudo-obstruction, functional dyspepsia without gastroparesis) we

Changes in Electrode Impedance and Cochlear Microphonic in Cochlear Implant Recipients in the First Year After Cochlear Implantation.

OBJECTIVE: We examined changes in electrode impedance and cochlear microphonic (CM) during the first year after implantation and investigated the extent to which changes in impedance related to changes in CM. In addition, we explored whether residual hearing was related to impedance and/or CM. STUDY DESIGN: Prospective longitudinal design. SETTING: Tertiary referral center in Utrecht, the Netherlands. PATIENTS: Twenty-one adults underwent cochlear implantation surgery between January 2020 and January 2025. INTERVENTIONS: Impedance and CM responses to pure-tone stimuli (125, 250, and 500 Hz

Intraoperative Visual Identification of Extra-Cochlear Electrodes in Cochlear Implantation Using Stimulation Current-Induced Non-Stimulating Electrode Voltages.

OBJECTIVES: Extra-cochlear electrodes (EEs) are a frequent complication in cochlear implant (CI) surgery, potentially leading to reduced speech-perception outcomes. Standard intraoperative tools such as contact impedances and electrically evoked compound action potentials often fail to detect these misplaced electrodes. Stimulation Current-Induced Non-Stimulating Electrode Voltages (SCINSEVs), or electrical field imaging (EFI) in the advanced bionics device, are presented here as a faster and more accurate intraoperative measure for identifying EE. Adopting this method clinically could reduce

Modiolar Proximity of Slim Modiolar Electrodes and Cochlear Duct Length: Correlation for Potential Basis of Customized Cochlear Implantation With Perimodiolar Electrodes.

OBJECTIVES: Recent studies have shown that cochlear duct length (CDL) varies among individuals and could significantly influence the final position of the electrode and its trajectory in the cochlea. Given this, we hypothesized that the degree of modiolar proximity of novel slim modiolar electrodes, such as CI532 and CI632, can also be affected by CDL. To test this hypothesis, we retrospectively evaluated individual CDL to determine if there is any significant correlation of CDL with degree of modiolar proximity. METHODS: Fifty-one ears from 38 subjects implanted with slim modiolar electrodes
