ENG in the emergency room: subtest results in acutely dizzy patients.

A retrospective analysis of electronystagmography (ENG) subtests performed on 93 acutely dizzy patients in the emergency room (ER) shows that "pendular tracking" has high sensitivity and specificity for determining central pathology. Small combinations of subtests altered sensitivity or specificity only slightly over pendular alone. Detection of spontaneous nystagmus was also helpful in ruling out "other" diagnoses such as hyperventilation syndrome, psychogenesis, or malingering. Popularity of the shortened ENG in the ER was evidenced by the high rate of charting of results and the frequency w

Ototoxicity of netilmicin.

Ototoxicity and efficacy of netilmicin was evaluated in 76 consecutive patients with moderate to severe infections with Gram-negative rods and Staphylococcus aureus. Netilmicin alone was efficacious in 47 patients and in another 23 patients cure or improvement was seen with netilmicin combined with another antibiotic, usually penicillin. Therapeutic failure was seen in 4 cases, 2 of which were regarded as netilmicin failures. Vestibular function and hearing acuity was thoroughly examined by electronystagmography and audiometry. In spite of the extensive examination of the audiovestibular funct

Vertical oculomotor apraxia and memory loss.

A case of vertical oculomotor apraxia with severe memory loss and hypokinetic inertia is described in a 34-year-old woman who was taking oral contraceptives. The eye movement disorder was limited to paralysis of voluntary vertical saccades; pursuit movements, reflex eye movement, and horizontal saccades were normal. Random involuntary vertical saccades documented by electronystagmography qualified this as a true apraxia. Similar cases from the literature have in common involvement of both medial thalami, a localization supported in this case by distinctive abnormalities on CAT scan. The clinic

Alternobaric effects on the endolymph.

Hearing loss and vertigo experienced as a result of environmental pressure change has been the subject of many superficial inquiries. The mechanism of these conditions remains obscure. In human volunteers and a rhesus monkey, the effects of intratympanic as well as ear canal pressure changes on the endolymphatic system were measured using electronystagmography (ENG). For the monkey histological evaluation of the pressure effects on auditory structures was performed. We were unable to cause any ENG effects on the monkey neither did we produce intracochlear pathology. The data obtained in humans

Noise exposure and its effect on the labyrinth, Part II.

The neurootological data of 12 patients attending our ear, nose, and throat clinic were analyzed. The patients submitted to a neurootological routine evaluation consisting of patient history recording and audiometric and equilibriometric investigations (i.e., electronystagmography). The conclusion was that many tinnitus patients--even those who had no vestibular symptoms--showed some disturbance in the vestibular tests, such as spontaneous and positional nystagmus and nystagmus alternans. Laurikainen came to the same conclusion in 1995, after examining 10 patients with sudden deafness but no v

[Directional preponderance of caloric nystagmus in vestibular neuronitis].

The aim of this work was an analysis of caloric directional preponderance prevalence in patients with vestibular neuronitis and coincidence of above mentioned symptoms with spontaneous and positional nystagmus. The examinations were carried out in 36 patients, aged 27-51, 19 women and 17 men. The vestibular reactions were recorded by electronystagmography and directional preponderance and mean angle velocity of the slow phase were calculated. It was pointed out that in vestibular neuronitis patients the directional preponderance may appear early and in great number of cases with canal paresis.

Vestibular function in patients with cochlear implantation.

The aim of this work was to determine the influence of cochlear implantation (CI) on vestibular canal and otolithic function. Between 1995 and 1999, 15 patients (6 females, 9 males; 9-77 years old) underwent a vestibular examination before and after CI. Electronystagmography was performed between 5 and 8 days after CI in 9 patients, and with a time delay of 2-24 months in 10 patients. Pre- and postoperative evaluation included electronystagmography with caloric (44 degrees C, 30 degrees C, ice-water) and pendular rotatory testing. Otolithic function was measured postoperatively using off-verti

The Successful Application of Plasmapheresis in the Treatment of a Patient with Opsoclonus and Autoantibodies to Glutamate Receptor δ2.

Glutamate receptor δ2 (GluRδ2) is expressed in the neuronal postsynaptic densities at the junctions between the Purkinje cells and the parallel fibers. Recent reports have described patients with opsoclonus who possess anti-GluRδ2 antibodies. We report the case of a 53-year-old man with opsoclonus whose cerebrospinal fluid was positive for anti-GluRδ2 antibodies. Electronystagmography revealed abnormal sinusoidal eye movements, which were definitively identified as opsoclonus. The frequency and amplitude of saccadic oscillations diminished after plasmapheresis (PE). The patient's opsoclonus wa

Visual suppression test and ocular dysmetria: some electronystagmographic findings relevant to the assessment of size of cerebellopontine angle tumors.

Patients with cerebellopontine angle tumors underwent vestibular examination, testing of voluntary fast and slow conjugate eye movements and of visual-vestibular interaction (visual suppression test--VST) by electronystagmography (ENG). Both spontaneous and evoked nystagmus (Ny) and dysmetric alterations of the voluntary conjugate eye movements are relevant to cerebellopontine pathology. This study shows that ENG may reveal damage to the vestibular brain-stem and archicerebellar structures, the ENG signs of which very often precede the clinical signs of compression. In cranial nerve VIII neuro

[Pathologies of the cervical spine anatomy as a possible cause of young patients vertigo].

Vertigo occurs in many diseases, especially in older people. It is a specific diagnostic problem in young people. In the paper, 86 young patients with incidental vertigo were clinically assessed. Electronystagmography, eye-tracking pattern test, optokinetic test and cervical spine X rays were used in examination. The results were compared with the control group of young and healthy people and also with the older patients group with degenerative changes of the cervical spine. In the examined group the disturbances of physiologic cervical lordosis were found in 33.8% and positional nystagmus in

Nystagmus episodes related to sea sickness.

In order to test the possible presence of nystagmus during episodes of sea sickness, we performed electronystagmography (ENG) on nine subjects before and during sailing on a 35-ton vessel in a somewhat heavy sea. In addition, the accelerations of the x, y and z axes of the ship were also monitored and simultaneously recorded. Nystagmus was not detected in pre-sailing recordings, while all of the subjects demonstrated clear positional nystagmus during sailing. The nystagmus was episodic, direction-fixed and its intensity was greater in sea-sick than in non-sick subjects. Our findings suggest th

The authors analyze otoneurological symptoms in 120 patients with pineal tumors. A marked cochleovestibular syndrome was found characterized by bilateral neurosensory hypoacusis with vocal-tonal dissociation, head noises, vertigo, multiple spontaneous nystagmus including a converging one, impaired optokinetic nystagmus, enhanced vestibulovegetative reactions. 39 cases are reported with pineal tumors removed microsurgically. Otoneurological symptoms were followed up in 32 patients early after surgery (within 20 postoperative days) and in 6 patients 1 to 5 years after radiotherapy. As evident fr

[The diagnostic value of the Stepping Test of Unterberger (author's transl)].

Of the vestibulo-spinal reactions, the "stepping test" is considered to be the most important, especially for unilateral peripheral vestibular disturbances. Electronystagmography was performed on seven patients after transtemporal neurectomy of the vestibular nerve, 17 patients with a nearly compensated vesitbular defect after vestibular neuronitis, and seven patients after suboccipital removal of acoustic neuromas. These results were then compared to the spontaneous stepping test. The test could not be performed on six of the seven patients with the operated acoustic neuroma, and on three oth

Computerised dynamic posturography: its place in the evaluation of patients with dizziness and imbalance.

As well as vestibular information, computerised dynamic posturography (CDP) measures visual and proprioceptive contributions to the balance system. Three hundred and seventy-five patients were evaluated by computerised dynamic posturography and the results were compared to electronystagmography (ENG) results. CDP demonstrated abnormalities in the balance system in many of the patients who had normal ENGs. It can hone the physician's skill in recognising subtle balance disease; it does not provide localization to one side; it does not offer disease-specific information; however, taken in conjun

[The visual fixation test in the study of visual-vestibular interactions. General aspects and results in normal subjects and patients].

Whenever we need to study visual-vestibular interactions, our method of choice among the ones based on thermal stimulation is the visual suppression test. In the vestibular laboratory of the II E.N.T. Clinic of the University of Parma the V.S.T. has been employed since 6 years systematically in all patients undergoing Electronystagmography. Our study was based on 10 normal patients and on 30 patients suffering from different pathologies. These patients were divided into groups according to the site of the lesion. The results we obtained are interesting: this test gives us important clinic data

[Disseminated sclerosis: diagnostic value of neurophysiological investigations (author's transl)].

Demyelinisation lesions of disseminated sclerosis actually present within the white matter are much more numerous than clinical manifestations would suggest that the majority of these lesions are clinically latent. Ophtalmologists were the first to seek a functional deficit reflecting the demyelinisation which so often affects the optic pathways. In practice, the recording of evoked visual potentials has proved to be the most reliable method. The brain stem is another region where demyelinisation lesions are frequent and quite often latent. Electronystagmography offers a highly sensitive means

Audiologic and other clinical findings in a case of basilar artery aneurysm.

A patient with a large, right-sided basilar artery aneurysm was evaluated. Major symptoms included progressive hearing loss, facial numbness, occipital headaches, dizziness, and diplopia of less than a year's duration. Audiologic results indicated a low-frequency sensorineural hearing loss with marked discrepancies between ascending and descending pure-tone thresholds for the ear ipsilateral to the lesion. Auditory brain-stem response demonstrated bilateral abnormalities, but the early waves were normal. Contralateral acoustic reflexes were absent on the right and elevated on the left. Electro

Sudden deafness with vertigo as a sole manifestation of anterior inferior cerebellar artery infarction.

Sudden deafness without associated neurological symptoms and signs is typically attributed to a viral inflammation of the labyrinth. Although sudden deafness occurs with anterior inferior cerebellar artery (AICA) infarction, the deafness is usually associated with other brainstem or cerebellum signs such as crossed sensory loss, lateral gaze palsy, facial palsy, Horner syndrome or cerebellar dysmetria. An 84-year-old woman suddenly developed right-sided tinnitus, hearing loss, vertigo and vomiting. Audiometry and electronystagmography documented absent auditory and vestibular function on the r
