Dietary aspects of adverse reactions to foods in adults.

Dietary considerations play an important role in the diagnosis, treatment and management of immunologic and nonimmunologic reactions to foods. Food diaries and trial elimination diets may prove helpful in identifying the responsible foods. Elimination diets must be monitored carefully for nutritional adequacy and should be used no longer than absolutely necessary; in some instances appropriate vitamin and mineral supplementation may be necessary. Ideally the identification of foods that provoke symptoms should be confirmed by means of double-blind challenge testing. Avoidance of some problem f

Food hypersensitivity.

Food allergy is one of several causes of adverse reactions to foods. The underlying immunologic mechanism varies from one manifestation to another, hence no current single laboratory test can be expected to be positive in every case. The diagnosis could be suggested by information gathered from the medical history or screening procedures, such as trials of elimination diets, the food/symptom diary, skin testing, and in vitro-specific IgE antibody measurement. All foods suspected by any of the aforementioned methods should be subjected to verification by appropriate elimination-challenge testin

Eosinophilic esophagitis: is it all allergies?

Eosinophilic esophagitis (EE) is an increasingly recognized disorder in the adult population, most often manifested by symptoms of dysphagia and food impaction. Mechanisms involving eotaxin-3, interleukin 5, and signal transducer and activator of transcription 6 have been studied and may represent future therapeutic targets. Patients commonly have a personal and family history of atopy, and both food allergies and aeroallergens have also been investigated as triggers of EE. Traditional allergy-testing methods, including skin prick testing and specific IgE testing, have been used to identify fo

Health effects of salicylates in foods and drugs.

There is much (renewed) interest about the effects of salicylates on food intolerance, attention-deficit disorders, and cardiovascular disease. Current evidence for the efficacy of salicylate-elimination diets in the treatment of attention-deficit disorders and hyperactivity is weak, and further investigation is required on the relationship between salicylates and cardiovascular disease.

An Overview of Essential Nutritional Strategies and Products in the Treatment of Endometriosis.

BACKGROUND/OBJECTIVES: Recent reports on the co-occurrence of allergies and endometriosis have provided grounds for expanding research in this area, suggesting that diagnostics should be extended to women with endometriosis. However, numerous studies on nutrients and antioxidants do not specify the type of diet that supports the treatment process. In our review, we focus on the types of food elimination and dietary approaches that have been used. METHODS: This systematic review was conducted according to the PRISMA guidelines. We searched the EMBASE, PUBMED and SCOPUS databases, as well as th

[Nutritional factors associated with migraine].

Migraine is a chronic, highly prevalent, multidimensional, and complex disorder, influenced by genetic and environmental factors, among which is the diet. Medical treatments are partially effective, and that makes necessary to complement them with other therapeutic strategies. The nutrition plays a prevalent role. We will review dietary factors that have been linked to migraine and therapeutic nutritional guidelines about it: elimination, integral, ketogenic, epigenetic and hypocaloric diets, as well as diets that interest fatty acids, sodium, vitamins, and the gut-brain axis. To date, the evi

Food-driven inflammation in inflammatory bowel disease: Dietary instigators and emerging nutritional strategies.

Inflammatory bowel disease (IBD), including Crohn's disease and ulcerative colitis, is influenced not only by genetics and immunity but also profoundly by dietary factors. Emerging research identifies specific food components - such as ultra-processed foods, high-fat diets, and red meats - as potent exacerbators of intestinal inflammation through mechanisms involving dysbiosis, immune activation, and gut barrier disruption. Conversely, fiber-rich, fermented, and certain dairy products may confer protective effects by enhancing microbiota diversity and mucosal integrity. Patient-specific factor

Eosinophilic gastrointestinal disorders, a new epidemic.

PURPOSE OF REVIEW: Eosinophilic gastrointestinal disorders (EGID) are on the rise and impose a significant burden on patients. EGID other than eosinophilic esophagitis (EoE) remain diagnostically and therapeutically challenging. RECENT FINDINGS: We revised the current knowledge on primary EGID. Epithelial barrier dysfunction and type 2 inflammation are key elements in EoE pathogenesis, whereas the mechanisms underlying distal EGID are elusive. Clinical presentations of EGID vary by disease location and depth of tissue involvement. Diagnosis relies on biopsy, although eosinophilic histologic t

Dietary treatment of eosinophilic gastrointestinal disorders in children.

PURPOSE OF REVIEW: To provide an overview of recent developments on dietary treatment of eosinophilic gastrointestinal disorders (EGID) in children. RECENT FINDINGS: Food antigens are the main triggers of eosinophilic esophagitis (EoE); however, currently available allergy tests cannot reliably identify eliciting antigens. Studies evaluating the six-food empiric elimination diet (6FED-milk, wheat/gluten, egg, soy/legumes, nuts and fish/seafood) have shown histological remission rates of 72%. Milk, egg, wheat/gluten, and, to a lesser extent, soy/legumes were the most frequent food triggers wit

The diet factor in attention-deficit/hyperactivity disorder.

This article is intended to provide a comprehensive overview of the role of dietary methods for treatment of children with attention-deficit/hyperactivity disorder (ADHD) when pharmacotherapy has proven unsatisfactory or unacceptable. Results of recent research and controlled studies, based on a PubMed search, are emphasized and compared with earlier reports. The recent increase of interest in this form of therapy for ADHD, and especially in the use of omega supplements, significance of iron deficiency, and the avoidance of the "Western pattern" diet, make the discussion timely. Eosinophilic oesophagitis.

Eosinophilic oesophagitis (EoE) is an antigen-driven pan-oesophagitis that is defined by the presence of at least 15 eosinophils per high power field on oesophageal histology in conjunction with upper gastrointestinal symptoms. EoE is closely associated with atopic disorders, in particular with food allergy, and as for other atopic diseases in childhood, there is a strong preponderance of male patients who have this disorder. The mechanisms leading to EoE have been characterised at the molecular level. Eotaxin-3, interleukin-5 and interleukin-13 are the key effector molecules in EoE pathogenes

Food allergy: from diagnosis to treatment.

Adverse food reactions, an adverse health effect arising from an immune or nonimmune response that occurs reproducibly on the exposure to a given food, can be divided into toxic and hypersensitivity reactions. When an immunologic mechanism has been shown, hypersensitivity food reactions should be referred to as food allergy that may be IgE- or non-IgE-mediated. Food allergy diagnosis is mainly guided by a correct and accurate history and physical examination, thus leading to prick test and elimination diets. The treatment gold standard is still represented by an elimination diet together with

Reactivity to potential cross-reactive foods in fruit-allergic patients: implications for prescribing food avoidance.

BACKGROUND: Prescribing therapeutic elimination diets in patients with fruit allergy should include recommendations on which other foods of the same family or group may be safely consumed. Evidence-based data on the management of fruit allergy are lacking; therefore, advice may vary from just avoiding the offending fruit, to overly restrictive diets of the entire botanical family. The aims of this investigation were to assess clinical reactivity to potential cross-reactive foods in fruit-allergic patients, and the implications for prescribing specific therapeutic elimination diets. Natural History of Food-Triggered Atopic Dermatitis and Development of Immediate Reactions in Children.

BACKGROUND: Case reports suggest that children with food-triggered atopic dermatitis (AD) on elimination diets may develop immediate reactions on accidental ingestion or reintroduction of an avoided food. OBJECTIVE: The objective of this study was to systematically study the incidence and risk factors associated with these immediate reactions. METHODS: A retrospective chart review of 298 patients presenting to a tertiary-care allergy-immunology clinic based on concern for food-triggered AD was performed. Data regarding triggering foods, laboratory testing, and clinical reactions were collect

[Rectal bleeding in infancy; could it be an allergy?].

Rectal bleeding in infants is frequently regarded as an alarming symptom for healthcare providers and often causes significant concern and anxiety among parents. In this article, we discuss two infants who were urgently referred to pediatric services for further evaluation. Both infants showed no signs of illness. The diagnosis in one case was ingested maternal blood and in the other Food Protein-Induced Allergic Proctocolitis (FPIAP), triggered by cow's milk. FPIAP is a non-IgE-mediated food allergy. FPIAP can also occur in exclusively breastfed infants when the lactating mother consumes cow'
