An update on the diagnosis and management of non-IgE-mediated food allergies in children.

The spectrum of non-IgE mediated conditions includes well-defined conditions like Food Protein-Induced Enterocolitis Syndrome (FPIES), Eosinophilic Oesophagitis (EoE), Food Protein-Induced Enteropathy, and Food Protein-Induced Allergic Proctocolitis, but also the more controversial food protein-induced dysmotility disorders like food protein-induced gastroesophageal reflux disease (FPGORD) and food protein-induced constipation (FPC). Typically, non-IgE mediated reactions are delayed, with symptom onset from hours to days after exposure to a culprit food. The diagnosis is mostly clinical, and f

Autophagic elimination of Trypanosoma cruzi in the presence of metals.

Trypanosoma cruzi is an obligate intracellular parasite transmitted to vertebrate hosts by blood-sucking insects. Molecules present in parasites and mammalian cells allow the recognition and parasite internalization. Metallic ions play an essential role in the establishment and maintenance of host-parasite interaction. However, little is known about how parasites handle with essential and nonessential metal quotas. This study aimed to investigate the influence of metal ions on the biological processes of T. cruzi infected cells. Infected cells were incubated with ZnCl2, CdCl2, and HgCl2 for 12

Selective depletion of basophils ameliorates immunoglobulin E-mediated anaphylaxis.

Basophils, which are the rarest granulocytes, play crucial roles in protective immunity against parasites and development of allergic disorders. Although immunoglobulin (Ig)E-dependent responses via receptor for IgE (FcεRI) in basophils have been extensively studied, little is known about cell surface molecules that are selectively expressed on this cell subset to utilize the elimination in vivo through treatment with monoclonal antibody (mAb). Since CD200 receptor 3 (CD200R3) was exclusively expressed on basophils and mast cells (MCs) using a microarray screening, we have generated anti-CD200

An Unusual Psychiatric Presentation of the 3q29 Microduplication Syndrome.

The 3q29 microduplication syndrome is usually associated with an intellectual disability or global developmental delay and mild dysmorphisms. Other comorbid presentations reported in the literature include psychiatric disorders such as behavioral disorders, attention-deficit/hyperactivity disorders, elimination disorders, and autism spectrum disorders. The current case is of an adolescent girl with the 3q29 microduplication syndrome who had a diverse psychiatric presentation. The patient was a 14-year-old girl in institutional care, with a moderate intellectual developmental disorder, major be

Brain stars take the lead during critical periods of early postnatal brain development: relevance of astrocytes in health and mental disorders.

In the brain, astrocytes regulate shape and functions of the synaptic and vascular compartments through a variety of released factors and membrane-bound proteins. An imbalanced astrocyte activity can therefore have drastic negative impacts on brain development, leading to the onset of severe pathologies. Clinical and pre-clinical studies show alterations in astrocyte cell number, morphology, molecular makeup and astrocyte-dependent processes in different affected brain regions in neurodevelopmental (ND) and neuropsychiatric (NP) disorders. Astrocytes proliferate, differentiate and mature durin

Tobramycin Pharmacokinetics in Pediatric Patients With Primary Ciliary Dyskinesia.

BACKGROUND: Patients with primary ciliary dyskinesia (PCD) are commonly treated for pulmonary exacerbations with intravenous tobramycin, but data on tobramycin pharmacokinetics in PCD is lacking. The objective of this study was to compare tobramycin pharmacokinetics in pediatric patients with PCD to those with cystic fibrosis (CF). METHODS: This retrospective study included pediatric patients hospitalized for a pulmonary exacerbation between January 2018 and June 2023. Included patients were treated with systemic tobramycin and had two concentrations usable to calculate patient-specific pharm

Combating lead and cadmium exposure with an orally administered chitosan-based chelating polymer.

Heavy metals present a threat to human health, even at minimal concentrations within the body. One source of exposure is due to the consumption of low-level contaminated foodstuff and water. Lead and cadmium have been shown to be absorbed by and accumulate within organs like the kidneys and liver, and they have also been associated to many diseases including cardiovascular disease and kidney dysfunction as well as developmental disorders and neurodegenerative diseases. Since this contamination of lead and cadmium is found worldwide, limiting the exposure is complicated and novel strategies are

[Prevention in general practice: what is dangerous for liver cells?].

The liver is engaged in detoxication and elimination of toxic-nutritional foreign material. In addition it is of high affinity to viruses and parasites. Different influences may cause pathological reactions, which are of great consequence to the normal and even more to the previously damaged liver. In general, interactions and cumulative effects of different toxic agents occur. The increase of chronic hepatic disorders in the last years has to be noticed considering economic and social medical aspects. At the first place of agents with a toxic effect to the liver there is alcohol, followed by

Dietary Therapies for Gastrointestinal Disorders.

Alterations in gastrointestinal function (digestion, absorption, motility, secretion, and elimination) play important roles in the pathophysiology of many gastrointestinal disorders. Food also strongly influences gastrointestinal health and disease. Some foods act as antigens that trigger an enteric immune response, while others can serve as substrates with direct or indirect biological effects. Food can also be metabolized by gut microbes into bioactive molecules that alter physiology. This review discusses the current research evidence and the clinical use of "food as medicine" through dieta

Acute Kidney Injury and Renal Failure in Horses.

Nephrotoxic and hemodynamically mediated disorders are the most common causes of acute renal failure (ARF) in horses and foals. Leptospira spp. is the most common infectious cause of ARF. Initial treatments for ARF include elimination of nephrotoxic drugs, correction of predisposing disorders, and fluid therapy to promote diuresis. Horses and foals with polyuric ARF often have a good prognosis, while those with oliguric or anuric ARF have a guarded to poor prognosis. When fluid therapy is unsuccessful in improving urine production, various drugs treatments have been used in an attempt to incre

Nitric oxide inhalation therapy for newborn infants.

Binding of NO to heavy metal-containing proteins probably accounts for many of its physiologic actions. NO inhalation is a promising new treatment for various disorders of neonates. The therapy is most likely to benefit premature neonates who are hypoxemic despite breathing pure oxygen and those who suffer from impaired carbon dioxide elimination. Newborn infants who have congenital heart disease may benefit from inhaled NO therapy if their disease involves some form of pulmonary venous hypertension or if they have recently undergone surgery involving cardiopulmonary bypass grafting. The use o

Canine intrahepatic biliary disease.

The intrahepatic biliary tract is comprised of a complex network of conduits that are essential in the transportation and formation of bile with components needed for fat digestion and absorption as well as the elimination of catabolised compounds. Derangements in this system can have dangerous sequela that affects morbidity and mortality. This review aimed to summarise clinically relevant updates on intrahepatic biliary disorders in dogs including human corollaries.

Elastosis perforans serpiginosa.

Elastosis perforans serpiginosa (EPS) is a cutaneous connective tissue disorder featuring transepidermal elimination of altered elastic tissue. Morphologically and histologically EPS presents characteristic patterns; the case reported herein classically illustrates these patterns. In approximately one-fourth of reported cases, EPS has been associated with systemic connective tissue disorders and may thus be a useful clue in the early recognition of diseases such as Down's syndrome or pseudoxanthoma elasticum.

Localized cervical lymphadenopathy induced by diphenylhydantoin sodium.

Diphenylhydantoin sodium is a standard drug in the treatment of convulsive disorders. Numerous untoward reactions have been reported. Lymphadenopathy related to drug-induced hypersensitivity has occurred. A yound woman undergoing treatment for seizures developed a large, tender, localized neck mass, associated with trismus, spasmotic torticollis, fever, eosinophilia, and skin rash. She was thought to be suffering from a deep cervical fascial space abscess. Symptoms subsided rapidly after elimination of anticonvulsant medication.

"Imaging Findings of Eosinophilic Gastrointestinal Diseases in Adults".

Eosinophilic gastrointestinal (GI) disorders are a group of conditions marked by pathologic eosinophilic infiltration of one or multiple locations in the GI tract. Conditions include eosinophilic esophagitis, eosinophilic gastritis, eosinophilic enteritis, and eosinophilic colitis. The site and depth of eosinophilic infiltration of the GI tract usually determines clinical presentation. These conditions should be considered in the differential diagnosis for several GI symptoms, such as food impaction or dysphagia. Histopathology is the gold standard for diagnosis of eosinophilic disorders. Neve

Pruritus as a manifestation of systemic disorders.

A significant internal disorder is present in only a limited number of patients with essential pruritus. The mechanisms for the production of pruritus in these disorders are poorly understood and frequently are unrelated to liberation of histamine. Proteolytic enzymes in lymphoproliferative disorders, bile acids in obstructive hepatobiliary disease, parathyroid hormone in chronic renal failure, and prostaglandins and kinins are but a few of the chemicals that may ultimately be responsible for the development of pruritus in these and other purely cutaneous disorders. Patients with essential pru

Genetic epidemiology of Greenland.

The Inuit (Eskimo) gene pool is in many respects similar to that of East Asian populations. Some polymorphisms imply frequent occurrence of disorders comparatively rare in Western Europe (e.g. lactose and sucrose malabsorptions). Low frequencies of alleles for slow isoniazid acetylation and sparteine/debrisoquine oxidation indicate slow elimination of a multitude of drugs. Autoimmune disorders (e.g. rheumatoid arthritis, insulin dependent diabetes mellitus, Graves' disease and psoriasis) are rare, possibly explained by the associations of these disorders with HLA-alleles rare in Inuit (e.g. HL

Acquired Perforating Dermatosis With Giant Lesions on the Scalp: A Unique Presentation in a Hemodialysis Patient.

Acquired perforating dermatosis is a rare skin disease characterized by umbilicated hyperkeratotic papules or nodules and transepidermal elimination of dermal components such as collagen and elastic fibers. It has been reported in association with various systemic disorders, particularly chronic renal failure and diabetes mellitus. Common locations include the extensor extremities and trunk. We describe a patient who developed multiple lesions on the scalp only, an unusual and rarely described location.
