Emergency departments (EDs) absorb most acute unscheduled care (AUC) in the United States. Routing all of this care through one high-intensity system serves no one well: crowding delays care for time-critical patients; visits that turn on evaluation and coordination, and routine and chronic care defined by longitudinal needs, are delivered episodically and at high cost in a setting built for resuscitation and stabilization; and clinicians trained for emergencies spend much of their time on work the category does not fit. I argue that this is not only a capacity or workflow problem but a concep

Although a rare diagnosis in the Emergency Department, hypothermia affects patients in all environments, from urban to mountainous settings. Classic signs of death cannot be interpreted in the hypothermic patient, thus resulting in the mantra, "No one is dead until they're warm and dead." This comprehensive review of environmental hypothermia covers the clinical significance and pathophysiology of hypothermia, pearls and pitfalls in the prehospital management of hypothermia (including temperature measurement techniques and advanced cardiac life support deviations), necessary Emergency Departme

We report our 4-year experience at the Lariboisiere Hospital Emergency Headache Center. During this period, we provided care for 33,000 patients, two-thirds women, one-third men, 77 percent aged 20-50 years, 90 percent residing in Paris or the suburban areas. About two-thirds of our patients complained of primary headache, i.e migraine, tension type headache or cluster headache, but the number of patients with secondary headache has progressively increased to account for about 17.5 percent of our current recruitment. Vascular headache, particularly after meningeal hemorrhage has been prevalent

PURPOSE: Emergency cancer care constitutes a significant health care and patient burden. The purpose of this study was to identify characteristics of patients most fitting for treatment in an oncology-dedicated emergency department (OED).
METHODS: Electronic files of patients with cancer seeking emergency services between April and June 2017 were retrospectively obtained from the hospital registry. Efficacy parameters were compared between patients treated in the OED and those treated in the general emergency department (GED). Using descriptive statistics and logistic regressions, patient- an

The organization of emergency medical care varies from one country to another. However, the basic requirements applied to specialists in case of emergency curative diagnostic measures are common: fast feedback, professionalism, emotional stability. Therefore, the key characteristic of emergency medicine is to instantly react twenty-four hours per day. The medical workers of considered category are in condition of increased physical and mental loading. According the WHO concept, a negative impact of working environment is a key factor of development of professional burn-out syndrome. Despite th

PURPOSE: Utilization of telemedicine and telepharmacy services has become increasingly popular, as specifically noted during the coronavirus disease 2019 (COVID-19) pandemic. This article describes the implementation of and services provided by emergency medicine pharmacists (EMPs) as part of a telemedicine team in the emergency department (ED).
SUMMARY: This report describes the telemedicine and telepharmacy services provided to EDs in the Mayo Clinic Health System from the Mayo Clinic Rochester ED. Telepharmacy services provided by EMPs started in 2018. EMPs cover telepharmacy calls as part

We investigated social and logistic factors eg, distance from the medical center, language barriers, other children to care for, number of caregivers, etc.) for families to delay seeking immediate emergency care for neutropenic fever in a retrospective cohort study of all pediatric hematology-oncology patients who presented for fever in the setting of neutropenia to our emergency department or clinic from 2015 to 2020. Patients with a history of at least 2 prior admissions for neutropenic fever waited more often for a second fever before presenting versus those without such history (odds ratio

STUDY OBJECTIVE: We sought to describe diagnosis rates and compare common process outcomes between geriatric emergency departments (EDs) and nongeriatric EDs participating in the American College of Emergency Physicians Clinical Emergency Data Registry (CEDR).
METHODS: We conducted an observational study of ED visits in calendar year 2021 within the CEDR by older adults. The analytic sample included 6,444,110 visits at 38 geriatric EDs and 152 matched nongeriatric EDs, with the geriatric ED status determined based on linkage to the American College of Emergency Physicians' Geriatric ED Accred

BACKGROUND: Transient ischemic attack (TIA) is a brief episode of dysfunction in a confined area of the brain. The risk of stroke following TIA is approximately 4% within the first 2 days and 9% within the first month. Therefore, early diagnosis and treatment is critical to reduce mortality and risk of stroke in patients who have experienced a TIA.
OBJECTIVES: This systematic review aimed to investigate the impact of the urgent evaluation and initiation of treatment of patients with TIA on the risk of subsequent stroke and death.
DATA SOURCES: A literature search was performed for studies pu

Hand emergencies represent the most frequent traumatic injuries and outpatient surgery is still improving. It will achieve 70% of total surgeries by 2022. Our hand trauma center has been able to set up an emergency day surgery department in a university hospital center. With this article, we would like to report the ambula- tory care management and practice for hand emer- gencies in our university hospital center over 2 years. 892 patients suffering from hand traumas and managed in our day surgery department, were retro-spectively reviewed between January 2016 and December 2017. Standardized order sets are common in Emergency Departments (EDs), but the extent of use, effectiveness, and associated outcomes remain unclear. The objective of this scoping review was to describe the current evidence regarding standardized order set use in the ED by examining conditions for which order sets are used, implementation and evaluation strategies, and clinical outcomes. We searched MEDLINE, EMBASE, CINAHL, Scopus, Web of Science, and the Cochrane Library from inception to April 3, 2025 for studies comparing the use of standardized order sets to usual care for ED patients. Objectives MePACS is a triage and support-based personal alarm emergency response system designed to assist older and/or disabled people to live safely in their homes. The aim of this study was to estimate avoidable ambulance attendances and transports to emergency departments and quantify the cost savings attributed to MePACS compared with a comparison cohort without a personal alarm system. Methods Alarm activation and demographic data for clients registered in the program from June 2016 to May 2017 and funded through the Personal Alert Victoria program were extracted from routinely collecte

INTRODUCTION: Timely admission to a facility capable of providing highly specialised treatment is key in patients with spontaneous subarachnoid haemorrhage. We aimed to determine the time elapsed from the initial emergency telephone call to arrival at a neurosurgical department. Also, we aimed to determine the ambulance dispatch criteria used and the activated prehospital responses.
METHODS: This was a retrospective study. Patients admitted in the Capital Region of Denmark within a 3.5-year period were identified in the Danish National Patient Register. Data were extracted from medical record

Patients experiencing incarceration face a multitude of healthcare disparities. These patients are disproportionately affected by a variety of chronic medical conditions. Patients who are incarcerated often remain shackled throughout their hospital course, experience bias from members of the healthcare team, and have many barriers to privacy given the omnipresence of corrections officers. Despite this, many physicians report little formal training on caring for this unique patient population. In this narrative review, we examine the current literature on patients who are incarcerated, especial

Traditionally, the response to mass casualty incidents has focused on the front line. However, effective management of these incidents relies on the seamless coordination of emergency, municipal and community services activities. The coordinated, complex planning required for response and recovery requires a holistic planning perspective, extensive engagement and collaborative problem-solving approach. This case study looks at the challenges, opportunities and solutions encountered by the Calgary Emergency Management and Calgary Police Service in its collective planning process for mass casual

This paper explores the application of insights from the study of Complex Adaptive Systems (CAS) to emergency management in search of an adaptive approach to hazard management capable of functioning with increased effectiveness in dynamic, uncertain and unclear environments. This approach demands a shift in perception towards seeing emergency management at the local, tribal, state and national level as a CAS. To function successfully as a CAS, emergency management must move past its current linear hazard-based approach to a hazard-agnostic consequence-based systems approach. This must be predi
