Postendodontic pain: a practical approach to diagnosis and management.

OBJECTIVES: Endodontic treatment is a routine procedure performed by general dental practitioners and endodontists on a daily basis. Fortunately, most patients undergoing endodontic therapy show a favorable outcome with uneventful healing. However, some patients develop pain following endodontic therapy. A majority of these patients develop acute, nociceptive pain ("flare-up") that resolves with appropriate treatment and subsequent healing. The dental profession is very adept at successfully managing the acute pain that occurs early following endodontic treatment. A minority of patients, howev

External cervical resorption: a three-dimensional classification.

This article describes a novel three-dimensional classification for external cervical resorption (ECR). The European Society of Endodontology and American Association of Endodontists & American Academy of Oral & Maxillofacial Radiology position statements advise that Cone beam computed tomography should be considered for the assessment and/or management of root resorption if it appears to be clinically amenable to treatment following clinical and conventional radiographic examination. The new classification takes into account the ECR lesion height (1: at CEJ level or coronal to the bone crest

Bilateral buccal radicular groove in maxillary incisors: case report.

AIM: To present the rare localization of a radicular groove on the buccal aspect of a tooth and to discuss the pathology and management of the concomitant endo-periodontal defect. SUMMARY: Bilateral buccal radicular grooves were found on the maxillary central incisors of a 60-year-old female Caucasian. One groove was associated with deep local pocketing resulting in pulp necrosis and the formation of a periodontal-endodontic lesion. After endodontic treatment of the affected tooth, periodontal surgery was performed during which an apicoectomy was carried out on the root-filled tooth. Both the

Ingrowth of Mineralized Tissue into the Root Canal of Immature Permanent Teeth after a Traumatic Injury: A Report of 3 Cases.

The aim of this case series was to describe the endodontic management of 3 immature permanent teeth that sustained traumatic injuries and subsequently presented with complete ingrowth of mineralized tissue into the canal space. Ingrowth of bone/mineralized tissue into the canal has been considered a poor long-term outcome with an inherent risk of ankylosis. In cases 1 and 2, no endodontic treatment was undertaken, except for emergency management requiring splinting. The cases were followed for 36 and 23 months, respectively. No ankylosis was evident over the review period, and normal teeth eru

A multidisciplinary approach to the treatment of a horizontally impacted mandibular second premolar: 10-year follow-up.

This report presents a multidisciplinary approach to the treatment of a rare case of a distally impacted mandibular second premolar indicating a 90-degree tilt of the long axis in a healthy, 15-year-old girl. It also describes the clinical and radiologic findings after a 10-year longitudinal follow-up period. The horizontally impacted mandibular second premolar was positioned behind the roots of the first molar and in close proximity to the root of the second molar, toward the lingual cortex of the mandibular body. Treatment methods required 2 years of cooperation among an orthodontist, an ora

Accuracy of Digital Radiography and Cone Beam Computed Tomography in Assessing Filling Material Extrusion Using Micro-Computed Tomography as Gold Standard: A Study in Human Cadavers.

OBJECTIVE: This study compared the accuracy of digital periapical radiography (DPR) and cone beam computed tomography (CBCT) in detecting extruded filling material, using a human cadaver model. Micro-computed tomography (Micro-CT) served as the gold standard. METHODS: A total of 27 single-rooted teeth embedded in cadaveric mandibular segments, obtained from a prior retreatment study, were included: 25 with confirmed apical extrusion of filling material on micro-CT and 2 without extrusion serving as negative controls. The segments were imaged using both DPR and CBCT. Two calibrated endodontist

Microendodontics at Different Levels From Routine to Complex Cases: A Case Series.

Over the past 30 years, the precision with which endodontics is performed has improved as a result of the advancements in technologies. Endodontists deal with intricate cases regularly, which appears to necessitate greater visual acuity. The invention of the dental operating microscope has been the most significant revolution. Nonetheless, due to a variety of behavioral variables, the use of magnification has yet to be adopted into general practice. The present case series aims to provide insight into the use of dental operating microscopes at different levels (routine to complex) in the field

The Preventive Effect of A Magnetic Nanoparticle-Modified Root Canal Sealer on Persistent Apical Periodontitis.

Persistent apical periodontitis is a critical challenge for endodontists. Developing root canal filling materials with continuous antibacterial effects and tightly sealed root canals are essential strategies to avoid the failure of root canal therapy and prevent persistent apical periodontitis. We modified the EndoREZ root canal sealer with the antibacterial material dimethylaminododecyl methacrylate (DMADDM) and magnetic nanoparticles (MNPs). The mechanical properties of the modified root canal sealer were tested. The biocompatibility of this sealer was verified in vitro and in vivo. Multispe

C-shaped root canal systems in the bilateral mandibular first molars: a case report and literature review.

BACKGROUND: Mandibular first molars typically exhibit complex root and canal anatomy. A thorough understanding of their morphological variations is crucial for endodontists to achieve successful root canal treatment. A C-shaped root canal system is a unique anatomical variation characterized by a C-shaped or semi-circular cross-sectional morphology, observed predominantly in mandibular second molars. The prevalence rate of the C-shaped root canal system is approximately 2.7-48.7% in the mandibular second molar, while the condition is rarely seen in mandibular first molars, particularly in bila

Preventive Endodontics: Mitigating Crack Development and Tooth Loss.

Cracked teeth represent a growing concern in endodontic practice, driven by biomechanical, evolutionary, and lifestyle factors. While traditionally approached as a condition requiring reactive treatment, contemporary endodontics must now shift toward prevention. This article explores the multifactorial nature of cracked teeth, including parafunctional habits, malocclusion, modern dietary patterns, and the evolutionary mismatch between human dentition and present-day lifestyles. The paper outlines key risk factors such as bruxism, malocclusion, and enamel demineralization, while also discussing

Endodontic Reapproach in a Tooth With External Resorption: Case Report.

INTRODUCTION: Endodontists face various challenges, and some factors are associated with endodontic treatment failure, such as the presence of resistant microorganisms resulting from inefficient disinfection and nonadhesive coronary sealing. The quality of the execution of the operative protocols, cautious irrigation, and the presence of a hermetic seal from the crown to the apex lead to successful endodontic retreatment. METHODS: This descriptive cross-sectional study approached a female patient, aged 30 years. The data obtained from the literature were collected from PubMed, SciELO, and Goo

Short-Term Pain Evolution and Treatment Success of Pulpotomy as Irreversible Pulpitis Permanent Treatment: A Non-Randomized Clinical Study.

The objective of this work was to evaluate (1) the short-term evolution of pain and (2) the treatment success of full pulpotomy as permanent treatment of irreversible pulpitis in mature molars. The study consisted of a non-randomized comparison between a test group (n = 44)-full pulpotomy performed by non-specialist junior practitioners, and a control group (n = 40)-root canal treatments performed by specialized endodontists. Short-term pain score (Heft-Parker scale) was recorded pre-operatively, then at 24 h and 7 days post-operatively. Three outcomes were considered for treatment success: ra

Hidden in Plain Sight: Diagnostic Challenges of Extraoral Sinus Tracts of Dental Origin: A Literature Review with Case Reports.

UNLABELLED: Extraoral cutaneous lesions of odontogenic origin are frequently misdiagnosed due to their asymptomatic dental causes and innocuous presentation. This literature review included 42 articles that highlight the diagnostic challenges of odontogenic cutaneous sinus tracts (OCSTs), which are often mistaken for skin infections, cysts, or neoplasms. As a result, patients typically consult dermatologists, general practitioners, or surgeons before reaching a dental specialist, with a diagnostic delay (Median = 12 months). Misdiagnosis can lead to unnecessary investigations, increasing patie

Smart Root Canal Therapy: Intelligence Refinement of Clinical Procedures.

Root canal therapy (RCT) is the primary treatment modality for pulpal and periapical diseases. Although the fundamental objective of RCT has remained unchanged for over a century, its technical details have been continuously refined and transformed. Preoperative diagnostic approaches have evolved from two-dimensional imaging to three-dimensional volumetric reconstructions and, more recently, to artificial intelligence-assisted analytical frameworks. Treatment modalities have progressed from manual instrumentation to motor-driven systems and guided or robotic-assisted interventions. Disinfectio
