Recent evidence suggests that immune mechanisms may underlie some cases of Meniere's disease. This study was conducted to determine whether an autoimmune mechanism is involved. Sera from 30 patients with Meniere's disease were reacted with human endolymphatic sacs and examined by indirect immunohistochemistry and fluorescence microscopy. Three of the samples (10%) showed positive staining, indicating immunoglobulin G (IgG) binding against the sac. No positive staining occurred when sera from healthy individuals or phosphate-buffered saline was used as a control. Clinical data showed an associa

The role of the endolymphatic sac in the pathoetiology of Menière's disease has been extensively studied but remains controversial. Likewise, the mechanism behind the efficacy of the endolymphatic-mastoid shunt procedure in Menière's disease remains obscure. The central hypothesis of this study is that the endolymphatic-mastoid shunt procedure is a destructive rather than a drainage procedure. Therefore removal of the extraosseous endolymphatic sac with scarring of the intraosseous portion may be more destructive and thereby more effective in the treatment of Menière's disease than conventiona

A detailed analysis of different aspects relative to endolymphatic sac surgery employed as conservative surgical treatment in Menière's disease is presented. The presumed basic principles for this type of surgery, which have led several otologists to adopt this procedure in the therapeutical protocol followed in Menière's disease, are discussed along with the good symptomatological results obtained. Particular emphasis is put on the multiple functions which have been attributed to the endolymphatic sac in the functional dynamics of the entire inner ear environment. Among these functions, relev

A growing body of evidence suggests that some cases of Menière's disease may be mediated by immune mechanisms. Because endolymphatic sac dysfunction is believed to be an underlying cause of Menière's disease, this study used immunohistochemical techniques to demonstrate the presence of immune complex deposition in the sacs of patients with Menière's disease. Positive immunoglobulin G (IgG) staining was noted in 10 of 23 sac biopsies from Menière's patients, with 2 specimens showing perivascular deposition. Only 1 of 5 control specimens was only slightly positive for IgG. Clinical correlation s

Immunofluorescent and light microscopic studies were performed on 21 small endolymphatic sac specimens biopsied during endolymphatic sac drainage surgery. The biopsies were taken from the extraosseous portions of the lateral side of the sac. Of the 21 specimens, only 5 displayed prominent histologic signs, including 1 with brown pigmentation and 4 with positive immunofluorescent reactions to either IgG, IgA, or IgM. No such histologic signs were seen in any of the other 16 specimens, although severe fibrous tissue change was found. One of the 4 specimens showing immunofluorescent reactions dis

Healthy human temporal bones and samples of the endolymphatic sac taken during surgery or from fresh cadavers were studied with regard to the presence of immunoglobulins. For demonstration of IgG, IgM, IgA and SIgA immunofluorescent methods and the immunoperoxidase-antiperoxidase method were used. Immunoglobulins are exclusively present within the endolymphatic sac whereas other structures of the healthy human inner ear do not show any positive reaction to the methods used for the demonstration of immunoglobulins.SIgA is clearly shown in epithelial cells of the rugosal part, whereas IgG predom

A specialized type of blood vessel is demonstrated within the dense soft tissue areas of the vestibular aqueduct and vascular channels of the surrounding bone, including the paravestibular canaliculus (PVC). The vessel wall is formed by the collagen-smooth muscle bundles of these areas. The lumen of these vessels is irregular, and segments appear to be closed by apposition of the bundles. The vessels are continuous with the capillaries of the endolymphatic sac. It is not clear whether they are on the venous side of the capillary system, or on the arterial side and might possibly be part of an

BACKGROUND: Decompression of the endolymphatic sac for Ménière's disease gives unpredictable results. This may be because the sac is difficult to identify and decompress accurately without causing surgical trauma.
METHODS: In order to test this idea, transmastoid decompression was simulated in 5 cadaver half heads and the anatomy of the endolymphatic sac was reviewed in a further 14 specimens.
RESULTS: The endolymphatic sac was found and confirmed by histology in all five simulated decompressions. A newly described feature, a trapezoid thickening of dura, was a useful guide. The review showe

BACKGROUND: Meniere's disease (MD) is an inner ear disorder, characterized by recurrent attacks of vertigo, low-frequency sensorineural hearing loss, tinnitus, and aural fullness. Endolymphatic sac surgery is an effective treatment to control vertigo attacks but without causing a hearing loss for intractable MD. However, the methods and effects of endolymphatic sac surgery have been controversial for many years, and the relationship between the vertigo control rates of different endolymphatic sac surgery methods is not well-documented.
OBJECTIVES: This study compared the vertigo control rate,

To verify whether injection of substances into the endolymphatic sac (ES) diffuses into the endolymphatic compartments of the human inner ear and in particular to the endolymphatic space of the scala media (ESp-SM), as demonstrated in animals, an exploratory investigation with magnetic resonance imaging (MRI) and intraoperative electrocochleographic recordings (ECoG) was conducted in patients with Ménière's disease (MD) treated with ES decompression. A mixture of dexamethasone and gadolinium (GD) in solution was injected into the ES of 4 patients. The results of the ES injection procedure were

From 1968 to 1983, 162 cases of Meniere's disease were treated surgically with our intramastoid drainage technique. One hundred forty of these cases could be followed up for two years or more. Treatment results were calculated according to the 1972 AAOO criteria and the 1985 AAO-HNS criteria. Using our technique, definitive vertigo spells could be controlled completely in 78% of cases and substantially controlled in 94%. Hearing worsened in only 15% of all cases. Therefore, prevention of worsening of hearing, especially in bilateral cases, and control of vertigo spells in patients with frequen

This study describes the technique and instrumentation necessary to perform a double-wall biopsy of the endolymphatic sac (ELS). Since this technique protects the lumen of the ELS, which is within the center of the biopsy sample surrounded by the lateral and medial wall, the ultrastructure of the ELS epithelium, luminal contents, and perisaccular connective tissue tends to represent the in vivo status. The biopsy specimen can be analyzed by electron microscopy for clinical-pathologic correlation. Ultrastructural analysis (1) provides definite identification of the ELS and epithelium, (2) may p

This study investigated the effect of a secondary immune response in the endolymphatic sac on the development of endolymphatic hydrops in guinea pigs. Following secondary keyhole limpet hemocyanin (KLH) challenge directly to the sac in systemically presensitized animals, hydrops rapidly developed within 1 week, then gradually reduced by 4 weeks. After 5 weeks, hydrops gradually recurred and developed again. Mean perilymph anti-KLH antibody levels rose from day 1 to 4 weeks and then decreased. The elevation of perilymph anti-KLH antibody levels on day 2 was due to an increased vascular permeabi

A 27-year-old female patient suffering endolymphatic sac tumor with intralabyrinthine hemorrhage was reported. The patient had hearing loss in the left ear with continuous tinnitus, and MRI showed the soft tissue shadow of endolymphatic sac. Considering that the tumor involved semicircular canal and vestibule, endolymphatic cyst tumor resection was performed by labyrinth route. After surgery, there was no cerebrospinal fluid leakage and facial nerve function was normal. More importantly, enhanced MRI of temporal bone showed no tumor recurrence 1 year after surgery.
