Hearing loss following ventriculoperitoneal shunt in communicating hydrocephalus patients: a pilot study.

OBJECTIVES/HYPOTHESIS: Hearing loss can be associated with a decrease in cerebrospinal fluid (CSF) pressure because changes in CSF pressure induce changes in perilymph pressure. Hearing loss after neurosurgical procedures have been reported, but clinical information on hearing loss after the placement of ventriculoperitoneal (VP) shunts, the most commonly used CSF shunt for hydrocephalus patients, is limited. This study is aimed to show the relationship between VP shunt and hearing loss. STUDY DESIGN: Prospective study. METHODS: Pure tone threshold and electrocochleography were preoperativel

Surgical management of Meniere's disease in the era of gentamicin.

OBJECTIVE: For many years, surgery was the mainstay of therapy for medically refractory patients, but recently, transtympanic gentamicin perfusion has attracted increasing interest and is a method frequently used for treating Meniere's disease. Many otologists question the relevance of surgical treatments, and traditional options are rarely discussed or offered to patients. The purpose of this study is to describe results of labyrinthectomy, vestibular nerve section, and endolymphatic mastoid shunt surgery for patients with Meniere's disease and to compare them with published results for genta

Meniere's disease: Medical management, rationale for vestibular preservation and suggested protocol in medical failure.

Meniere's disease is a peripheral audiovestibular disorder characterized by vertigo, hearing loss, tinnitus, and aural fullness. Management of these symptoms includes medical and surgical treatment. Many patients with Meniere's disease can be managed using nonablative therapy, such as intratympanic steroids and endolymphatic shunt surgery, prior to ablative techniques such as intratympanic gentamicin. Recognition of concurrent migraine symptoms may aid in medical therapy and also underscore the importance of preserving vestibular function where possible. The goal of this review is to explain t

Surgical treatment of Menière's disease.

Surgical treatment is used only for patients with Menière's disease in cases that do not respond to medical therapy. At the present time, the endolymphatic sac shunt operation is performed in cases where the auditory threshold is fluctuant around 30 dB. When the auditory threshold is between 40 and 70 dB we use the vestibular neurectomy by the retrosigmoid approach. In patients with Menière's disease with profound hearing loss the translabyrinthine neurectomy is used. The advantages of the vestibular neurectomy by the retrosigmoid approach compared to the retrolabyrinthine and middle fossa app

Low-frequency harmonic acceleration in the evaluation of surgical treatment of Meniere's disease.

23 patients with classical findings of Meniere's disease and so diagnosed by a neurotologist were tested with harmonic acceleration. 7 of these patients were selected to receive endolymphatic-mastoid shunts for intractable vertigo and/or highly progressive fluctuant hearing loss. All 7 patients showed an increase in the impairment in their phase relationships at the lowest frequency (0.01Hz) following surgery which was considered to be secondary to the procedure. Later this phase lag returned to a level approaching normal. They also showed a definite decrease in the asymmetry (labyrinthine pre

Pathology of Ménière's disease as it relates to the sac and tack procedures.

The rationale for endolymphatic sac and tack operations is to prevent the further accumulation of endolymph, the former by draining endolymph into extralabyrinthine tissues and the latter by shunting endolymph into the perilymphatic space. While the basic concepts are reasonable enough, the probability of achieving these objectives seems remote. There appears to be adequate clinical evidence, however, to show that these procedures are of therapeutic value in selected cases. It is tempting to speculate that these surgical insults to the labyrinth, with the associated inflammatory and biochemica

Endolymphatic sac surgery: analysis of 100 operations.

One hundred endolymphatic mastoid shunt operations in 89 patients with classical Menière's disease were analysed. The disease was bilateral in 18 patients (20%). The patients were carefully pre-selected by a comprehensive protocol of audiovestibular and metabolic investigations. All patients had definite electrophysiological evidence of endolymphatic hydrops with an enhanced negative summating potential on transtympanic electrocochleography. The surgical results were analysed both by the original American Academy of Ophthalmology and Otolaryngology Guidelines (AA00, 1972) and the more recent m

Reconstruction of mastoidectomy defect with titanium mesh.

OBJECTIVE: Intact-canal-wall mastoidectomy procedures leave an unsightly depression in the postauricular area. Until now, there have been few reports of successful reconstruction of mastoidectomy defects, and none using titanium mesh. When secondary mastoidectomy is not anticipated, as in endolymphatic sac shunt procedures, the postauricular defect resulting from mastoidectomy can be eliminated by reconstruction using titanium mesh. This is a retrospective study of 14 patients who underwent reconstruction of a mastoidectomy defect with titanium mesh. MATERIAL AND METHODS: All 14 patients unde

Management of Meniere's disease. Surgery may help if medication fails.

Meniere's disease is the classic triad of fluctuating hearing loss, episodic vertigo, and severe tinnitus. It usually occurs unilaterally, although about 15% to 20% of patients may have bilateral disease. The disease involves a series of acute exacerbations followed by a period of remission; it progresses until the ear has essentially "burned itself out," causing symptoms to subside. Treatment of Meniere's disease should be tailored to the severity of symptoms and the patient's life-style. Initial treatment involves conservative medical management and psychologic support. If this fails, endoly

Evidence of surgical treatments for intractable Meniere's disease.

Meniere's disease is an inner ear disease, characterized by recurrent rotatory vertigo, sensorineural hearing loss and tinnitus. There are some with frequent vertigo attacks, progressive hearing loss and persistent annoying tinnitus even through the continuous standard medical treatments. These cases are thought to account for 10%-20% of all cases of Meniere's disease. In this review article, we would like to demonstrate the evidences for surgical treatments according to the previous papers, and consider the next therapeutic strategies including surgical options according to the international

[Endolymphatic sac drainage and steroid-instillation surgery (EDSS) for intractable Meniere's disease].

Endolymphatic sac surgery is one of the most widely accepted techniques used to treat intractable Meniere's disease. To improve this surgery, we developed the following techniques: A simple mastoidectomy was used to expose the endolymphatic sac between the sigmoid sinus and inferior margin of the posterior semicircular canal. The sac was opened and filled with a mass of prednisolone. A bundle of absorbable gelatin films was then inserted into the sac lumen to expand it, followed by gelatin sponges dipped in a high concentration of dexamethasone. Long-term results (17-32 months) in 20 patients

Class A results in Menière's disease.

A series of 101 patients with Menière's disease treated surgically with an intramastoid endolymphatic shunt operation and with a minimum follow-up of 2 years is reported. The results included success in 76 percent and failure in 22 percent. Thirteen of these patients maintained a hearing level of 15 dB or better, while 11 patients experienced hearing gains of 25 dB or better. These results are compared to other reports, and the difficulty of making such comparisons is illustrated. A derived "efficacy ratio" is defined, and its use is suggested to aid in making statistical inference. The conclu

Water permeability of the mammalian cochlea: functional features of an aquaporin-facilitated water shunt at the perilymph-endolymph barrier.

The cochlear duct epithelium (CDE) constitutes a tight barrier that effectively separates the inner ear fluids, endolymph and perilymph, thereby maintaining distinct ionic and osmotic gradients that are essential for auditory function. However, in vivo experiments have demonstrated that the CDE allows for rapid water exchange between fluid compartments. The molecular mechanism governing water permeation across the CDE remains elusive. We computationally determined the diffusional (PD) and osmotic (Pf) water permeability coefficients for the mammalian CDE based on in silico simulations of cochl

Surgery for vertigo (you don't have to live with it).

Surgery for vertigo is recommended where there is objective evidence of a unilateral inner ear disorder, when medical therapy fails, and when the patient demands definitive therapy. Common surgical procedures are the endolymphatic subarachnoid shunt, retrolabyrinthine vestibular neurectomy, cochleovestibular neurectomy, and the post ampullary nerve section (singular neurectomy). Vertiginous disorders which can be surgically cured are Ménière's disease, vestibular neuronitis, chronic labyrinthitis, and benign positional vertigo. Due to the recent advances in microsurgery and the development of

Austin endolymph dispersement shunt surgery for Menière's disease.

This paper analyses the results of 53 Austin endolymph dispersement shunt procedures as compared to previously reported data on 87 Arenberg inner ear valve implants and 339 cases employing various other surgical techniques. Overall results are similar regardless of the surgical method involved, however findings indicate that capillary endolymph dispersement both affords more class A results, with isolated spectacular hearing gains, and creates a greater chance of completely eliminating dizziness in both definite and adjunctive spells. Furthermore, Austin's endolymph dispersement drain evidence

Meniere's disease of syphilitic etiology.

Congenital or acquired syphilis is the cause of Meniere's disease in six percent of all cases. The pathogenesis is endolymphatic hydrops and osteitis of the otic capsule which produces the characteristic fluctuating hearing loss, tinnitus and ear pressure associated with episodic spells of vertigo. The ear symptoms are, at first, typical of Meniere's disease of other etiologies. Failure to establish the specific etiologic diagnosis can result in unnecessary surgical treatment or inappropriate medical therapy. In some cases, if a steroid, the specific treatment, is not given promptly, hearing c

Pathologic significance of Meniere's symptom complex. A histopathologic and electron microscopic study.

Histologic and ultrastructural findings in the temporal bones of a patient clinically diagnosed as having unilateral Meniere's disease are presented. The patient underwent two endolymphatic subarachnoid shunt operations that failed to relieve symptoms and a middle fossa vestibular nerve section that completely relieved episodic vertigo and stabilized hearing in the ear operated upon. Histopathologic examination of the temporal bones showed no endolymphatic hydrops in either ear. The maculae and cristae on the side operated upon showed severe degenerative changes attributed to vestibular neurec

Intraoperative mitomycin C in endolymphatic sac surgery for Ménière's disease: A pilot study.

The long-term results of endolymphatic sac (ES) shunt surgery in Ménière's disease have been reported to be unsatisfactory compared to the short-term results, probably because of fibrosis and/or reclosure of the incised ES. To solve this problem, we tried to apply mitomycin C (MMC) intraoperatively to the incised ES as used in trabeculectomy for glaucoma. MMC has an antiproliferative effect as well as an antineoplastic effect. Thus, it is expected that MMC would have a benefit to prevent rapid fibrosis and/or reclosure of the ES. MMC did not show any ototoxicity in our animal study and we star
