Evaluation of operational chronic infection endpoints for HCV vaccine trials.

Hepatitis C virus (HCV) is a leading cause of chronic liver disease. The natural history of HCV infection is heterogeneous, and a person infected with HCV can clear the virus or progress to a chronic infection. The chronic infection can remain asymptomatic for decades before the development of liver cirrhosis and/or carcinoma. Currently, there are no assays that can differentiate a transient infection (an acute infection that would clear) from a chronic infection, and serial HCV RNA testing is used to operationally define chronic hepatitis C (e.g. detectable HCV over 6 months). Therefore, HCV

Evidence-based treatment of stuttering: II. Clinical significance of behavioral stuttering treatments.

UNLABELLED: An evidence-based framework can be described as an empirically-driven, measurement-based, client-sensitive approach for selecting treatments. It is believed that using such a framework is more likely to result in a clinically significant outcome. For this paper, a clinically significant outcome was defined as a meaningful treatment change. It was suggested that there are at least three groups for whom a treatment's outcome is meaningful. These groups include clinicians/clinical researchers, the clients, and relevant others who have some interest in the outcome (e.g., parents of a c

Standing orthostatic blood pressure measurements cannot be replaced by sitting measurements.

As many elderly patients are not able to stand for several minutes, sitting orthostatic blood pressure (BP) measurements are sometimes used as an alternative. We aimed to investigate the difference in BP response and orthostatic hypotension (OH) prevalence between the standard postural change to the sitting and the standing position in a cross-sectional observational study. BP was measured with a continuous BP measurement device during two postural changes, from supine to the sitting and from supine to the standing position. Linear mixed models were used to investigate the differences in chang

Rank-based methods for assessing equivalence/non-inferiority with assay sensitivity in a three-arm trial with ordinal endpoints.

Various approaches have been developed to assess equivalence/non-inferiority with assay sensitivity in a three-arm trial with continuous or discrete endpoints. However, there is little work done on ordinal endpoints. Ordinal data do not have metric information, the method for analyzing metric endpoints can systematically lead to errors for ordinal observations. The win probability that a subject receiving one treatment achieves a better outcome (or "wins" against) compared to a subject receiving the other treatment, is developed to quantify the treatment effect. In this article, the equivalenc

Modeling Temporal Relationships Between Multivariate Repeated Markers Along With Clinical Endpoints: Application to Alzheimer's Disease and Related Dementias.

Diseases often involve multiple dimensions of interrelated impairments. Although significant advances have been made in joint models to simultaneously assess these processes in relation to clinical endpoints, they often fail to evaluate how these dimensions influence each other. We propose an original joint modeling framework to describe the temporal relationships between the processes involved in Alzheimer's disease and related dementias (ADRD) progression, and assess their association with ADRD diagnosis and death. The longitudinal submodel is a dynamic model that combines a structural multi

Association Between Progression-Free Survival and Overall Survival in Mantle Cell Lymphoma: A Trial-level Surrogate Endpoint Analysis.

INTRODUCTION: Survival outcomes of patients with mantle cell lymphoma (MCL) have substantially improved. In the setting of extended overall survival (OS), progression-free survival (PFS) is typically used as a primary endpoint in MCL clinical trials rather than OS to limit trial duration and cost. However, no prior analysis has assessed whether PFS can reliably predict OS. The aim of this study is to perform a systematic evaluation of PFS as a surrogate endpoint for OS in phase III MCL clinical trials. MATERIALS AND METHODS: A systematic review was conducted to identify phase III MCL trials.

Clinical trial endpoints for metabolic dysfunction and alcohol associated steatotic liver disease.

BACKGROUND: Metabolic dysfunction and alcohol-associated steatotic liver disease (MetALD) has emerged as a new subtype of steatotic liver disease, and presents a substantial challenge in clinical trial design due to a lack of data and consensus on endpoint selection. OBJECTIVE: This review proposes patient selection, stratification, and optimization in Phase II and III clinical trial designs for MetALD and provides reasonable stage-specific clinical trial endpoints. It provides a comprehensive review of data on non-invasive tests (NITs) and trial endpoints in metabolic dysfunction-associated

Decline in glomerular filtration rate as an endpoint in heart failure clinical trials: challenges and solutions.

Chronic kidney disease (CKD) and cardiovascular disease are tightly interconnected, with common mechanisms that underlie the development and progression of both diseases, recently articulated into the framework of the cardiovascular-kidney-metabolic syndrome. CKD and heart failure commonly coexist in the same individual, with increasing evidence for common therapies in both disease states. It is valuable for patients, clinicians, and regulatory agencies to understand how to best assess CKD progression in patients with heart failure for evaluation of individual patients and as part of an endpoi

VO 2 Peak: The Emerging End Point For Cardiovascular Outcome Trials in Nephrology.

Cardiovascular outcome trials are challenging to conduct in patients with CKD. Despite this, well-designed randomized controlled trials are critical to inform optimal management strategies and improve clinical care. Unfortunately, many cardiovascular outcome trials in nephrology have not demonstrated a treatment benefit. Contributing to this are the difficulties associated with end point selection and the limitations of many traditional end points such as resting left ventricular geometric measures and circulating biomarkers in patients with CKD, which are well known to be a major impediment t

End Points for the Next-Generation Bladder-Sparing Perioperative Trials for Patients With Muscle-Invasive Bladder Cancer.

PURPOSE: The evolving treatment landscape of muscle-invasive bladder cancer (MIBC) increasingly warrants novel trial design to evaluate perioperative strategies aimed at bladder preservation. To establish standardized outcome measures for evaluating organ preservation strategies in MIBC, the International Bladder Cancer Group (IBCG) and the Global Society of Rare Genitourinary Tumors (GSRGT) assembled an international, multidisciplinary consensus panel. METHODS: The IBCG and GSRGT gathered global bladder cancer experts and patient advocates to establish a framework for risk-adapted bladder-sp

Evaluating time-to-event surrogates for time-to-event true endpoints: an information-theoretic approach based on causal inference.

Putative surrogate endpoints must undergo a rigorous statistical evaluation before they can be used in clinical trials. Numerous frameworks have been introduced for this purpose. In this study, we extend the scope of the information-theoretic causal-inference approach to encompass scenarios where both outcomes are time-to-event endpoints, using the flexibility provided by D-vine copulas. We evaluate the quality of the putative surrogate using the individual causal association (ICA)-a measure based on the mutual information between the individual causal treatment effects. However, in spite of i

Investigation of differences in sensitivity between 3 strains of Daphnia magna (crustacean Cladocera) exposed to malathion (organophosphorous pesticide).

Acute and chronic ecotoxic effects of organophosphorous insecticide malathion (Fyfanon 50 EC 500 g L(-1)) were investigated on three strains of Daphnia magna. The nominal effective concentrations immobilizing 50% (EC50) of Daphnia after 24 and 48 h were 0.53 and 0.36 μg L(-1), 0.70 and 0.44 μg L(-1), and 0.75 and 0.46 μg L(-1) for the strains 1, 2, and 3 respectively. There was an increase in malathion ecotoxicity with time of exposure as confirmed by chronic studies. In fact, after 21 days of exposure, significant effects on survival (lowest observed effect concentration or LOECs) were record

The intubation scoop (i-scoop) - a new type of laryngoscope for difficult and normal airways.

The i-scoop is an intubation device with a curved guiding bar with laterally located lenses at its tip, rather than a blade. Twenty-five anaesthesiologists intubated a manikin that simulated first a normal and then a difficult airway. All participants were able to intubate the difficult airway with a good view of the glottis using the i-scoop. None was able to intubate using seven other laryngoscopes (Macintosh laryngoscope, GlideScope(®) GVL and AVL, McGrath(®) (Series 5/MAC), C-MAC(®) , A.P. Advance(™) ). Intubation was successful only with the Airtraq(®) (n = 10), the Airway Scope (n = 5),

Clinical trials in restless legs syndrome--recommendations of the European RLS Study Group (EURLSSG).

The European Restless Legs Syndrome (RLS) Study Group (EURLSSG) is an association of European RLS experts who are actively involved in RLS research. A major aim of the Study Group is the development and continuous improvement of standards for diagnosis and treatment of RLS. Several members developed study designs and evaluation methods in investigator-initiated trials early in the 1990s, and all members have since contributed to many pivotal and nonpivotal drug trials for the treatment of RLS. The recommendations on clinical investigations of pharmacological treatment of RLS patients summarize
