Access to the epicardial space is fundamental to several cardiac procedures. While traditional indications include catheter ablation of ventricular arrhythmias and accessory pathways, novel indications include left atrial appendage occlusion, esophageal protection, mapping and ablation during atrial fibrillation procedures, implantation of epicardial pacing leads, and phrenic nerve displacement to facilitate safe ablation of atrial and ventricular arrhythmias. Accessing the epicardial space safely is a major challenge requiring intimate knowledge of cardiac anatomy, extensive training, and exp

Since its introduction over two decades ago, percutaneous epicardial procedures have become well-adopted by cardiac electrophysiologists, most commonly for catheter ablation of cardiac arrhythmias as well as left atrial appendage closure. The percutaneous epicardial approach has also been utilized for cardiac pacing and drug delivery. But still, its most common usage is for the treatment of intramural and subepicardial substrates that give rise to ventricular tachycardia, particularly in patients with nonischemic cardiomyopathy. In fact, subxiphoid, percutaneous epicardial mapping and ablation

INTRODUCTION: Recurrent ventricular tachycardia (VT) after percutaneous ablation is associated with a high morbidity and mortality. We assessed the feasibility of open chest extracorporeal circulation (ECC)-supported 3D multielectrode mapping and targeted VT substrate ablation in patients with previously failed percutaneous endocardial and epicardial VT ablations.
METHODS: In patients with previously failed percutaneous endocardial and epicardial VT ablations and a high risk of hemodynamic collapse during the procedure, open chest ECC-supported mapping and ablation were performed in a hybrid

BACKGROUND: Intracardiac echocardiography (ICE) has played a limited role in defining the substrate for ventricular tachycardia (VT). The purpose of this study was to assess whether ICE could identify abnormal epicardial substrate in patients with nonischemic cardiomyopathy (NICM) and VT.
METHODS AND RESULTS: We studied 18 patients with NICM and recurrent VT who had abnormal echogenicity identified on ICE imaging. Detailed left ventricular (LV) endocardial and epicardial electroanatomic mapping was performed in all patients. Low-voltage areas (<1.0 mV) in the epicardium were analyzed. ICE ima

In patients with nonischemic cardiomyopathy, epicardial ablation is critical in targeting epicardial paravalvular substrate. Epicardial access and ablation can be performed safely with attention to epicardial structures, such as the coronary arteries, phrenic nerve, and epicardial fat. This review explores the indications, techniques, complications, and outcomes of epicardial ablation in patients with nonischemic cardiomyopathy. Although epicardial ablation adds to the complexity and risk of the ablation procedure, it is a vital tool that, combined with endocardial mapping and ablation, improv

BACKGROUND: Pulsed-field ablation (PFA) may be effective for atrial tachycardia (AT) originating from Bachmann's bundle via epicardial mapping, but no such cases have been reported.
CASE SUMMARY: A 40-year-old man with recurrent AT postatrial fibrillation ablation underwent repeat ablation with a focal PFA catheter. Activation mapping identified the earliest activation site at the Bachmann's bundle insertion in the left atrium, but PFA there was ineffective. Following pericardial puncture, the earliest activation point was mapped on the epicardial side at the corresponding endocardial Bachman

BACKGROUND: Catheter ablation is an important therapeutic option in postmyocardial infarction patients with ventricular tachycardia (VT). We analyzed the endo-epicardial electroanatomical mapping (EAM) voltage and morphology characteristics, their association with clinical data and their prognostic value in a large cohort of postmyocardial infarction patients.
METHODS AND RESULTS: We performed total and segmental analysis of voltage (bipolar dense scar [DS] and low voltage areas, unipolar low voltage and penumbra areas) and morphology characteristics (presence of abnormal late potentials [LPs

Ventricular arrhythmias, either idiopathic or those associated with organic heart diseases, may originate from the epicardial side of the ventricular myocardium. Therefore, mapping and ablation via the coronary venous system offers a convenient, feasible, and effective option in such cases, considering the convenience of the epicardial location of the coronary venous system. Among epicardial idiopathic ventricular arrhythmias, the left ventricular summit is by far major region of interest closely associated with the coronary venous system. In this setting, the great cardiac vein/anterior inter

BACKGROUND: Studies of VT mechanisms are largely based on a 2D portrait of reentrant circuits on one surface of the heart. This oversimplifies the 3D circuit that involves the depth of the myocardium. Simultaneous epicardial and endocardial (epi-endo) mapping was shown to facilitate a 3D delineation of VT circuits, which is however difficult via invasive mapping.
OBJECTIVE: This study investigates the capability of noninvasive epicardial-endocardial electrocardiographic imaging (ECGI) to elucidate the 3D construct of VT circuits, emphasizing the differentiation of epicardial, endocardial, and

BACKGROUND: Ripple mapping displays every deflection of a bipolar electrogram and enables the visualization of conduction channels (RMCC) within postinfarction ventricular scar to guide ventricular tachycardia (VT) ablation. The utility of RMCC identification for facilitation of VT ablation in the setting of arrhythmogenic right ventricular cardiomyopathy (ARVC) has not been described.
OBJECTIVE: We sought to (a) identify the slow conduction channels in the endocardial/epicardial scar by ripple mapping and (b) retrospectively analyze whether the elimination of RMCC is associated with improved

BACKGROUND: Atypical atrial flutter (AFL) after atrial fibrillation (AF) ablation, particularly when mediated by epicardial structures such as the vein of Marshall (VOM), poses a therapeutic challenge.
CASE SUMMARY: A 64-year-old woman presented with recurrent AFL after AF ablation. High-density and entrainment mapping confirmed a macro-re-entrant circuit involving the VOM via epicardial conduction. After failed endocardial ablation, ethanol infusion into the VOM immediately terminated the tachycardia. At 1-year follow-up, the patient maintained sinus rhythm with a marked improvement in left

INTRODUCTION: Epicardial access for mapping and ablation of the epicardial substrate may be required in catheter ablation of ventricular tachycardias (VT). However, high complication rates are associated with the standard epicardial access approach. Recently, a novel method of intentional coronary vein (CV) exit with pericardial CO2 insufflation to facilitate epicardial access has been described. This study describes our initial experience with this technique.
METHODS: Patients undergoing epicardial VT ablation between 1 February 2021 to 31 May 2022 at the Royal Prince Alfred Hospital, Sydney

Left ventricular outflow tract arrhythmias that fail endocardial mapping and ablation have traditionally been labeled as originating from the epicardial left ventricular summit. Although these sometimes can be targeted from the epicardial surface of the left ventricular ostium, such approach poses significant technical challenges. A significant proportion of such arrhythmias, however, exhibit intramyocardial origin, demonstrated by mapping intraseptal branches of the anterior interventricular vein, and henceforth defined as the basal superior intraseptal space.

BACKGROUND: Atrial tachycardia after atrial fibrillation ablation may arise from uncommon epicardial conduits.
CASE SUMMARY: A 73-year-old man with persistent atrial fibrillation underwent redo ablation for recurrent atrial tachycardia. Electroanatomic mapping showed durable pulmonary vein and posterior wall isolation with a blocked anterior mitral line. Tachycardia (cycle length: 252 milliseconds) appeared mitral isthmus-dependent, but endocardial ablation only prolonged the cycle without termination. Mapping revealed a macroreentrant biatrial tachycardia incorporating the persistent left su

OBJECTIVES: We aimed to describe the feasibility of a surgical left thoracotomy for catheter ablation of scar-related ventricular tachycardia (VT) in patients with inaccessible pericardial access.
BACKGROUND: Pericardial adhesion due to prior cardiac surgery or previous epicardial ablation procedures limits epicardial access in patients with drug-refractory VT originated from the epicardium.
METHODS: Six patients who underwent a surgical left lateral thoracotomy epicardial access for catheter ablation of VT after failed subxiphoid percutaneous epicardial access were reviewed. Patients' basel

BACKGROUND: Noninvasive electrocardiographic mapping of ventricular tachycardia (VT) and ablation using stereotactic radiotherapy was recently reported. This strategy does not directly evaluate the critical diastolic components and assumes that the epicardial exit site of VT subtends closely over the endocardial mid-diastolic isthmus.
OBJECTIVE: To determine if the epicardial exit site of VT spatially corresponds to the critical diastolic components of ischemic scar-related VT.
MATERIALS AND METHODS: Intraoperative simultaneous endocardial and epicardial mapping were performed during VT usin

BACKGROUND: The presence of focal fibrillation waves during atrial fibrillation (AF) can, besides ectopic activity, also be explained by asynchronous activation of the atrial endo- and epicardial layer and transmurally propagating fibrillation waves. To provide direct proof of endo-epicardial asynchrony, we performed simultaneous high-resolution mapping of the right atrial endo- and epicardial wall during AF in humans.
METHOD AND RESULTS: Intraoperative mapping of the endo- and epicardial right atrial wall was performed during (induced) AF in 10 patients with AF (paroxysmal: n=3; persistent:

PURPOSE: Recent investigations into the treatment of atrial fibrillation have suggested improved outcomes after concomitant pulmonary vein isolation (PVI) and ganglionic plexi (GP) ablation. We investigated the impact of left atrial ablation with substrate modification (left atrial maze) or epicardial PVI, combined with GP mapping and ablation, in patients with paroxysmal or longstanding persistent atrial fibrillation undergoing additional off-pump or on-pump cardiac surgery.
DESCRIPTION: Twelve patients aged 74.9 +/- 3.8 years, with atrial fibrillation for 4.5 +/- 1.5 years, underwent left a
