Evacuation of a 14-vertebral-level cervico-thoracic epidural abscess and review of surgical options for extensive spinal epidural abscesses.

Spinal epidural abscess (SEA) is a rare infection of the spinal peridural space, usually from haematogenous spread of bacteria from a distant source of infection or from direct extension of an osteomyelitis or paravertebral abscess into the spinal canal. An extensive SEA with involvement of five or more levels is even more exceptional and is a very threatening condition. Different surgical strategies have been used in the typically posteriorly located extensive SEA: multi-level laminectomies at all involved levels; catheter procedures, in which suction (irrigation) catheters are pushed cranial

Intracranial subdural empyemas and epidural abscesses in children.

OBJECTIVE: The authors conducted a retrospective analysis of a consecutive series of children with intracranial subdural empyemas (SEs) and epidural abscesses (EAs) to highlight the important clinical difference between these two entities. They describe the delays and pitfalls in achieving accurate diagnoses and make treatment recommendations based on clinical and imaging findings. METHODS: They reviewed their experience with children who had presented with intracranial SE and/or EA in the period from January 2013 to May 2018. They recorded presenting complaint, date of presentation, age, neu

Recent Developments in the Treatment of Spinal Epidural Abscesses.

Spinal epidural abscess (SEA) is a serious condition that can be challenging to diagnose due to nonspecific symptomology and delayed presentation. Despite this, it requires prompt recognition and management in order to prevent permanent neurologic sequelae. Several recent studies have improved our understanding of SEA. Herein, we summarize the recent literature from the past 10 years relevant to SEA diagnosis, management and outcome. While surgical care remains the mainstay of treatment, a select subset of SEA patients may be managed without operative intervention. Multidisciplinary management

Endoscopic transnasal drainage of frontal epidural abscesses.

BACKGROUND: Although rare, sinogenic intracranial epidural frontal abscesses remain a serious and potentially life-threatening complication of sinusitis. Traditional therapy involves a combination of intravenous antibiotics, sinus surgery, and neurosurgical drainage through an external approach. OBJECTIVE: In this report, we present a technique in which a purely endoscopic transnasal approach was used to treat both the sinusitis and the intracranial abscess. Four cases are discussed illustrating the technique and its outcomes. METHODS: In all cases, endoscopic sinus surgery was performed to

[Epidural spinal abscesses. Review of a clinical serie].

INTRODUCTION: The spinal abscess is an infrequent infectious complication whose diagnosis is often delayed, therefore bringing about significant morbidity and death rates. In the last years, its incidence has raised progressively. The objective of this work is to present the clinical features of a series of spinal abscesses. METHOD: The clinical histories of all patients diagnosed of spinal cord abscess were evaluated retrospectively. RESULTS: We obtained seven cases, with a 5/2 male/female proportion, all subjects being over 50 years old. Incidence was 0.58/10,000 admissions per year. All p

Acquired neuromyotonia in a patient with spinal epidural abscess.

We report a case of acquired neuromyotonia in a patient with Staphylococcus aureus septicemia and a spinal epidural abscess. Autoantibodies to voltage-gated potassium channels, which are associated with acquired neuromyotonia, were present during the patient's acute illness but became undetectable on clinical recovery. The spinal epidural abscess may have triggered the production of these specific autoantibodies, resulting in clinically and electromyographically detectable neuromyotonia.

Epidural abscess formation with an atypical pathogen following epidural steroid injection: A case report.

RATIONALE: Subcutaneous and epidural abscesses following epidural injection are a serious but rare complication. Epidural abscesses are typically caused by Staphylococcus aureus bacterial infection. In this case presented here, the causative bacterium was Enterococcus faecalis. PATIENT CONCERNS: A 67-year-old woman having chronic lower back and right leg pain with past history of 20 years of rheumatoid arthritis, diabetes mellitus, and osteoporosis (T-score: -2.7) visited the outpatient pain clinic. Magnetic resonance imaging (MRI) revealed L4-5 right central disc extrusion with inferior migr

A Pott's Puffy Tumor Associated with Epidural - Cutaneous Fistula and Epidural Abscess: Case Report.

BACKGROUND: Pott's puffy tumour is characterized by a fluctuate swelling of the frontal region as a result of osteomyelitis of the frontal bone. This inflammatory lesion may propagate endocranially, resulting in acute meningitis, epidural abscess, subdural empyema, cavernous sinus trombophlebitis, cerebritis, and frontal lobe abscess of the brain. CASE REPORT: We present an unusual case of a 33-year-old man suffering from Pott's puffy tumour whose condition was further complicated by a draining epidural-cutaneous fistula and an epidural abscess. We confirmed the diagnosis by contrast-enhanced

Development of epidural abscess following surgical drainage of perianal abscess: report of a case.

PURPOSE: A case of epidural abscess originating from a perianal abscess is reported. METHODS: The history of the patient, erythrocyte sedimentation rate, magnetic resonance imaging, and bacteriological tests were used to reach a diagnosis and the possible mechanism. RESULTS: Epidural abscess was suspected because the patient had a fever and intense low back pain following drainage of a perianal abscess. Magnetic resonance imaging was used to correctly diagnose the epidural abscess and bacteriologic studies disclosed the pathophysiologic mechanism. CONCLUSIONS: Epidural abscess is an extreme

Management of spinal epidural abscess complicating psoas abscess: A retrospective study of 30 patients.

Psoas abscess is a quite rare clinical condition. Its extension to the spinal epidural space and formation of spinal epidural abscess (SEA) is another rare perilous complication which may result in significant neurological deficits. In this study, we report our experience with management of psoas abscess associated with SEA. This retrospective study included 30 patients with SEA complicating psoas abscess. Detailed imaging studies were done using plain X-ray, computed tomography and magnetic resonance imaging with contrast for spinal and paraspinal regions. Patients were managed medically or s

A Case Report of Frontal Sinus Abscess Complicated by Epidural Abscess with a Literature Review.

Inflammation of the frontal sinus is a relatively common clinical condition among paranasal sinusitis and is curable through anti-infection treatments, with a rare progression to frontal sinus abscess. An even rarer complication is the development of intracranial epidural empyema secondary to frontal sinus abscess. In this report, we describe a case of a 14-year-old male with a frontal sinus abscess that led to intracranial infection and was complicated by an epidural abscess misdiagnosed as an epidural hematoma. The primary symptoms were headache, dizziness, and fever. Following combined anti

Lumbar spinal brucellosis presenting as an atypical epidural abscess.

Brucellosis is a zoonotic infection that affects the spine, typically presenting as spondylodiscitis. Predominantly, epidural involvement without significant concurrent disc or vertebral involvement is rare and may closely mimic non-infectious pathologies. We report a man in his 50s presenting with low back pain and lower limb radiculopathy. Initial MRI demonstrated an epidural-predominant ventral epidural collection at L4-L5 associated with a posterior annular fissure, without significant vertebral or disc involvement, mimicking a sequestered disc fragment. Diffusion-weighted imaging showed p

Staphylococcus aureus Dissemination Presenting With Encephalopathy and Epidural Abscess.

Background: Staphylococcal infection is a common bacterial disease with common clinical features. Untreated infection, especially in immunosenescence cases, can affect other organs. This can lead to multiorgan dysfunction and cause increased morbidity and mortality. Unlike commonly presented features of pneumonia, dissemination of infection can pose diagnostic and therapeutic enigma. Therefore, any such presentation in common clinical practice can yield a conundrum of diagnoses. Case Report: A 69-year-old elderly male presented to the Emergency Department with acute onset encephalopathy. Histo

Streptococcus intermedius: an unusual cause of spinal epidural abscess.

Spinal epidural abscess (SEA) following dental procedures is a rarely reported entity. Here, we present a unique case of a 74-year-old immunocompetent man who developed severe lower back pain and bilateral lower extremity weakness 4 days following two root canal procedures. Lumbar spine magnetic resonance imaging (MRI) showed several pockets of epidural abscesses extending from L1 through L5. Blood cultures grew Streptococcus intermedius, an anaerobic commensal bacterium of the normal flora of the mouth and upper airways. The patient was treated with IV penicillin for 7 days but his symptoms c
