Model of induced leakage of polymethylmethacrilate inside epidural space and prevertebral muscles during vertebroplasty in pigs: clinical, macroscopical, and histological study.

STUDY DESIGN: Experimental study in animals. PURPOSE: Study the clinical behavior of animals after an induced leakage of cement during vertebroplasty in pigs. Study the distribution of polymethylmetacrilate inside the epidural space and prevertebral muscle. Study the histological findings of the spinal cord and muscles, which contact with cement. OVERVIEW OF LITERATURE: Although vertebroplasty has a low rate of complication, leakage of cement is highly frequent. There is paucity, in how cement is distributed inside the spinal canal and what occurs when soft tissue comes into contact with pol

Evaluation of traumatic spinal cord edema using evoked potentials recorded from the spinal epidural space. An experimental study in the rat.

Spinal cord evoked potentials (SCEP) elicited by simultaneous distal tibial and sural nerve stimulation were continuously recorded from the epidural space at the T9 and T12 levels of urethane anaesthetized rats before and after a unilateral incision (about 3 mm deep and 5 mm long) in the right dorsal horn of the T10-11 segments. The changes in SCEP were correlated with the increase in spinal cord water content measured 5 h after injury. In addition, the influence of serotonin (5-HT) in mediating such changes was explored using a pharmacological approach. The changes in SCEP immediately after i

Comparison of Epidural Analgesic Efficacy of Direct Thoracic versus Caudally Inserted Catheter Positioned in Thoracic Space in Children Undergoing Cardiac Surgery.

INTRODUCTION: Effective pain management during paediatric cardiac surgery is essential to prevent complications such as metabolic, neuroendocrine, and immunological disturbances. Thoracic epidural anesthesia (TEA) offers significant benefits in terms of pain relief and postoperative recovery. However, its application in children is challenging due to the risk of dural puncture and nerve injury. This study compares the efficacy of two techniques: direct thoracic space insertion and caudal-to-thoracic space catheter placement for TEA in pediatric cardiac surgery. MATERIALS AND METHODS: This pro

Lipomatosis of spinal epidural space, peritoneum, and renal sinus: a rare complication of long-term steroid therapy in a child with nephrotic syndrome.

Excessive visceral adipose tissue proliferation, resulting in diffuse lipomatosis, is a rare complication of long-term steroid therapy. A 10-year-old boy presented with severe radicular back pain with limitation of lower limb movements. He was diagnosed with steroid-resistant nephrotic syndrome and was on unregulated steroid therapy. Magnetic resonance imaging of the spine showed increased adipose tissue in the epidural space of the lumbo-sacral spine causing clumping of cauda equina nerve roots along with marked proliferation of fat in the renal sinus as well as peritoneum. He was started on

Malignant melanoma of the spinal epidural space metastasized from the enucleated left eye after nine years without symptoms: report of a case.

A case of flaccid paraplegia due to a metastatic spinal epidural melanoma is reported. Symptoms occurred approximately 9 years after the enucleation of the left eye. A solid melanotic mass was removed almost totally. Three years after the operation, the patient had not regained the motility of the lower limbs. The need for long term follow-up and management of patients with such problems is discussed, and the pertinent literature is reviewed.

Comparative experimental anesthesia efficacy study of epidural injection at the sacrococcygeal space using ultrasound guidance versus blindness technique in Egyptian donkeys (Equus asinus).

The study investigates the effectiveness of blind and ultrasound-guided epidural injections in Egyptian donkeys in the sacrococcygeal region, a topic of limited research, by comparing and assessing their onset in 20 healthy adult donkeys of both sexes. In the first group, we used ten cadaver donkeys that were humanely euthanized. In the second group, we used 10 live, healthy adult donkeys to assess the efficacy and time of analgesia onset for blind and ultrasound-guided epidural injections at the sacrococcygeal region. Cadavers were randomly designated to blind (n = 5) or US-guided (n = 5) epi

Novel use of cranial epidural space in rabbits as an animal model to investigate bone volume augmentation potential of different bone graft substitutes.

BACKGROUND: The success of bone augmentation to a major degree depends on the biomechanics and biological conditions of the surrounding tissues. Therefore, an animal model is needed providing anatomical sites with similar mechanical pressures for comparing its influence on different biomaterials for bone regeneration. The present report describes the new bone formation associated to biomaterial in a bursa created in the epidural space, between dura mater and cranial calvaria, under the constant pressure of cerebrospinal fluid. METHODS: Five adult California rabbits were used for the trial. In

Effects of Medical Ozone Injection via Intervertebral Foramen Epidural Space on Postherpetic Neuralgia: Protocol for a Randomized Controlled and Double-Blind Clinical Trial.

BACKGROUND: Previous studies have demonstrated that ozone injection into the dorsal root ganglion significantly reduces pain scores associated with herpes zoster, suggesting its therapeutic potential for managing postherpetic neuralgia (PHN) via the intervertebral foramen epidural space. However, there are no specific reports addressing the treatment of herpes zoster and PHN by involving the thoracic and lumbar nerves under computed tomography (CT) guidance. Our research focuses on the effect of medical ozone administered through the intervertebral foramen epidural space on PHN. OBJECTIVE: Th

An inflammatory pseudotumor in the thoracic epidural space presenting with progressive paraplegia: a histopathological diagnosis with clinical and radiological uncertainty. Case report with literature review.

INTRODUCTION: Inflammatory pseudotumors (IPTs) are benign lesions with unknown etiology, probably an immunological reaction to a traumatic or an infective insult or sometimes considered as an IgG4-related autoimmune disorder. It can occur as an isolated or multi-centric lesion and are reported to involve almost all parts of the human body. Although lung and orbital IPTs are reported commonly, central nervous system involvement is a rare occurrence. Only seven cases of spinal epidural IPTs have been reported to date. These are clinically and radiologically a diagnosis of exclusion. It is an exc

Unintentional injection of contrast agent into the subdural space during an epidural blood patch procedure identified the site of cerebrospinal fluid leak: a case report.

BACKGROUND: CT myelography is a definitive diagnostic tool for cerebrospinal fluid leakage. We report a case in which the site of a cerebrospinal fluid leak that had been difficult to identify on CT myelography was fortuitously localized owing to incidental injection of a contrast agent into the subdural space. CASE PRESENTATION: A 37-year-old man developed headache and neck pain following a massage. He was diagnosed with intracranial hypotension, but the exact site of cerebrospinal fluid leakage could not be identified by CT myelography. An epidural blood patch was performed at the Th11/12 l

Confirming identification of the epidural space: a systematic review of electric stimulation, pressure waveform analysis, and ultrasound and a meta-analysis of diagnostic accuracy in acute pain.

To review the use of epidural electric stimulation test, pressure waveform analysis, and ultrasound assessment of injection as bedside methods for confirming identification of the epidural space in adults with acute pain, the PubMed database was searched for relevant reports between May and August 2022. Studies reporting diagnostic accuracy with conventional Touhy needles and epidural catheters were further selected for meta-analysis. Sensitivity and specificity were estimated using univariate logistic regression for electric stimulation and pressure analysis, and pooling of similar studies fo

The "Eagle Ray Sign" of the lumbar epidural space: A case of vascular-dominant spinal angiolipoma mimicking Aetobatus spp. morphology with favorable outcome and literature review.

INTRODUCTION/RESEARCH QUESTION: Spinal epidural angiolipomas (SEALs) are rare, benign mesenchymal tumors composed of mature adipocytes and anomalous vascular channels, representing only 0.04-1.2% of all spinal tumors. This study presents a rare case of a lumbar SEAL and proposes the "Eagle Ray Sign" as a morphological descriptor to characterize its unique macroscopic appearance during surgical resection. MATERIAL AND METHODS: A 32-year-old woman presented with a history of insidious, progressive, and mechanically induced low back pain. Diagnostic evaluation included lumbar CT, MRI, and spinal

Detection of Severe Neurological Complications Caused by the Removal of Central Venous Catheter Accidentally Inserted in the Epidural Space via Motor-Evoked Potential: A Case Report.

The central venous catheter (CVC) has been in clinical use for more than half a century. It was initially used for total parenteral nutrition. However, its indication gradually expanded to chemotherapy, intensive care, anesthesia, and other areas. As the application of CVCs increased, complications also increased. Nevertheless, some guidelines for CVC insertion have been implemented, and clinicians worldwide are working hard to prevent complications during CVC insertion. However, the safety of CVC removal has not been given adequate attention. Because of a few reports on complications, such as

Invasive Pulmonary Aspergillosis in the Epidural Space in a Patient with Acute Myelogenous Leukemia with Myelodysplasia-related Changes: A Case Study and Literature Review of Vertebral Aspergillosis in Japan.

Vertebral aspergillosis is a rare infectious disease with a high mortality rate. We herein report a 70-year-old woman with acute myelogenous leukemia with myelodysplasia-related changes, nontuberculous mycobacteriosis, and bronchiectasis who presented with a fever and cough. Her clinical symptoms and laboratory test results suggested febrile neutropenia and pneumonia. However, her clinical course was further complicated by lower extremity weakness. Magnetic resonance imaging of the spine showed consolidation contiguously spreading toward the epidural space between the T4 and T5. Cytological te

[Post-operative complications peculiar to lumboperitoneal shunt: possible consequences due to side leakage of CSF from around the inserted spinal tube into the lumbar epidural space].

This is a report on a few possible consequences of occasional side leakage of CSF into the lumbar epidural space. In this report, we refer to such CSF side leakage from around the inserted spinal tube which occurred during the early stages following our lumboperitoneal shunt operations in two of our patients. This leakage can lead to over-drainage of CSF, and such complications as post-operative orthostatic headache, subdural effusion, and/or subdural hematoma can also result. The diagnosis of these complications can be verified if CSF components in the lumbar epidural space are visible in T2
