Generalized onset seizures with focal evolution (GOFE) - A unique seizure type in the setting of generalized epilepsy.

PURPOSE: We report clinical and electrographic features of generalized onset seizures with focal evolution (GOFE) and present arguments for the inclusion of this seizure type in the seizure classification. METHODS: The adult and pediatric Epilepsy Monitoring Unit databases at Vanderbilt Medical Center and Children's Hospital were screened to identify generalized onset seizures with focal evolution. We reviewed medical records for epilepsy characteristics, epilepsy risk factors, MRI abnormalities, neurologic examination, antiepileptic medications before and after diagnosis, and response to med

Retinal neuronal layers in children with idiopathic generalized epilepsy: effects of epilepsy and long-term valproate use.

OBJECTIVE: Idiopathic generalized epilepsies (IGEs) and long-term antiseizure treatment may differentially affect retinal neuronal structures. This study aimed to evaluate optical coherence tomography (OCT)-derived retinal and optic nerve head (ONH) parameters in adolescents with IGEs, in order to explore retinal alterations associated with epilepsy and valproic acid (VPA) exposure. METHODS: This cross-sectional study included three groups: patients with IGEs receiving VPA monotherapy for at least one year (PwE+VPA, n = 35), newly diagnosed patients with IGEs who had not yet initiated antisei

Persistent treatment resistance in genetic generalized epilepsy: A long-term outcome study in a tertiary epilepsy center.

OBJECTIVE: To assess prognostic patterns and investigate clinical and electroencephalography (EEG) variables associated with persistent treatment resistance in a population of genetic generalized epilepsy (GGE) patients with a long-term follow-up. METHODS: Data from GGE patients followed from 1975 to 2019 were reviewed retrospectively. Subjects with a follow-up >10 years, starting from epilepsy diagnosis, were included. Persistent treatment resistance was defined as the absence of any period of remission ≥1 year despite treatment with two appropriate and adequate antiepileptic drugs (AEDs).

Natural evolution from idiopathic photosensitive occipital lobe epilepsy to idiopathic generalized epilepsy in an untreated young patient.

Idiopathic photosensitive occipital lobe epilepsy (IPOE) is an idiopathic localization-related epilepsy characterized by age-related onset, specific mode of precipitation, occipital photic-induced seizures--frequently consisting of visual symptoms--and good prognosis. This uncommon epilepsy, which usually starts in childhood or adolescence, has rarely been observed in families in which idiopathic generalized epilepsy also affects other members. We describe a nuclear family in which the proband showed electro-clinical features of idiopathic photosensitive occipital lobe epilepsy in childhood, w

Personality Disorders in Patients with Idiopathic Generalized Epilepsy- An Underrecognized Comorbidity.

BACKGROUND AND OBJECTIVES: Personality disorders remain an illness in the shadows, with scarce literature on their prevalence in persons with epilepsy. Moreover, personality traits in idiopathic generalized epilepsy (IGE) have not been explored so far, except for a few authors investigating juvenile myoclonic variant alone. We aimed to explore the prevalence and types of personality traits in persons with all IGE subtypes in adults and to compare them with the general population. METHOD: We included 57 healthy controls and 57 IGE cases who were diagnosed according to the International League

Progressive Changes in Brain Morphology in People With Idiopathic Generalized Epilepsy.

BACKGROUND AND OBJECTIVES: Idiopathic generalized epilepsy (IGE) is typically responsive to treatment, yet some people remain poorly controlled despite multiple antiseizure medication trials. Whether this subgroup shows progressive structural brain changes remains uncertain. We aimed to investigate whether people with poorly controlled chronic IGE exhibit progressive morphological brain alterations and how these differ from those in early-stage IGE. METHODS: This longitudinal case-control neuroimaging study included 2 cohorts: an early-stage IGE group from West China Hospital (Chengdu, China)

Is remission in women with idiopathic generalized epilepsy possible without valproate?

AIM OF STUDY: This study aimed to evaluate changes in prescription practices for treating idiopathic generalized epilepsy (IGE) in women of childbearing age, and to assess how switching from valproate (VPA) affects seizure outcomes. IGE accounts for 15-20% of all epilepsy cases. While VPA is the most effective treatment, its teratogenic risk limits its use in women of reproductive age, leading to recommendations for safer alternatives such as lamotrigine (LTG) and levetiracetam (LEV). MATERIAL AND METHODS: We retrospectively analysed the data from 130 women aged 18-49 diagnosed with IGE from

Pathogenic Cav3.2 channel mutation in a child with primary generalized epilepsy.

Two paternally-inherited missense variants in CACNA1H were identified and characterized in a 6-year-old child with generalized epilepsy. Febrile and unprovoked seizures were present in this child. Both variants were expressed in cis or isolation using human recombinant Cav3.2 calcium channels in tsA-201 cells. Whole-cell patch-clamp recordings indicated that one variant (c.3844C > T; p.R1282W) caused a significant increase in current density consistent with a pathogenic gain-of-function phenotype; while the other cis-related variant (c.5294C > T; p.A1765V) had a benign profile.

Electroencephalography abnormalities in generalized epilepsy and their predictors: A multicenter experience.

BACKGROUND: In spite of the overwhelming significance of knowledge of basic elements of electroencephalography (EEG) in its application to the diagnostic workup and the management of patients with suspected or already established generalized epilepsy (GE), there is a dearth of data on the pattern and utility of clinical variables that can independently determine EEG abnormalities in GE. OBJECTIVE: The study was designed to evaluate the frequency and pattern of EEG abnormality as well as assess the utility of clinical variables in predicting the likelihood of an abnormal EEG in GE. METHODS: I

Epilepsy beyond seizures: Predicting enduring cognitive dysfunction in genetic generalized epilepsies.

Reduced cognitive functioning has been documented in the genetic generalized epilepsies (GGE). Among a number of hypothesized causal mechanisms, some evidence from other epilepsy syndromes suggests the impact of epileptiform discharges. This study investigates the relationship between cognitive function in GGE and burden of epileptiform discharges within a 24-hour EEG recording, controlling for variables relevant to cognitive function in epilepsy. As part of a larger prospective cohort study, 69 patients with EEG-confirmed GGE (11-58years) underwent 24-hour EEG and detailed neuropsychological

Vigabatrin-induced generalized epileptiform discharges in a patient with focal epilepsy.

Vigabatrin (VGB) may aggravate clinical seizures and epileptiform discharges especially in the patients with generalized epilepsy. This report is about the repetitive appearance of generalized spike-and-wave complexes in a patient with focal epilepsy. Though there were constant appearances of the generalized epileptiform discharges on the consecutive electroencephalograms (EEGs) taken over approximately four years under VGB monotherapy, clinical provocation of primary generalized seizures was not occurred. Because of the repetitive observations of the generalized epileptiform discharges, valpr

Myoclonus precipitated by oral suspension of oxcarbazepine in idiopathic generalized epilepsy.

Detection of circulating tumor cells remains a significant challenge due to their vast physical and biological heterogeneity. We developed a cell-surface-marker-independent technology based on telomerase-specific, replication-selective oncolytic herpes-simplex-virus-1 that targets telomerase-reverse-transcriptase-positive cancer cells and expresses green-fluorescent-protein that identifies viable CTCs from a broad spectrum of malignancies. Our method recovered 75.5-87.2% of tumor cells spiked into healthy donor blood, as validated by different methods, including single cell sequencing. CTCs we
