Anaphylaxis Clinical Care Pathway: Incorporating Intranasal Epinephrine (Adrenaline).

Health care providers across settings must be trained to manage anaphylaxis by (1) removing the offending allergen if still present; (2) positioning the patient with legs elevated, and if respiratory distress is present, allow the patient to sit upright with legs extended and elevated; (3) immediately administering epinephrine (adrenaline) intramuscularly (IM) or intranasally (IN) every 5 to 15 minutes for persistent anaphylaxis; and (4) optimizing airway, breathing, and cardiovascular (ABC) resuscitation. This may include supplemental oxygen, noninvasive or invasive positive-pressure ventilat

Deprescribing Self-Administered Epinephrine: Why, Who, and When?

Epinephrine is established as the first-line treatment for anaphylaxis. Self-administered epinephrine (self-epi) is widely prescribed to patients considered at higher risk of future reactions. However, ongoing provision of self-epi is not without consequences: there is a financial cost, and evidence suggests that self-epi has a negative impact on quality of life by reinforcing a patient's perception of vulnerability. The decision to prescribe or continue self-epi therefore requires balancing clinical evidence with patient preferences through shared decision making. Importantly, self-epi should

Intramuscular Epinephrine-Induced Transient ST-Elevation Myocardial Infarction.

Introduction. Myocardial infarction in the setting of anaphylaxis may result from the anaphylaxis itself or from the epinephrine used to treat the anaphylaxis. While cases of myocardial infarction due to large doses of intravenous epinephrine have previously been reported, myocardial infarction after therapeutic doses of intramuscular epinephrine is rarely reported. Case Report. A 23-year-old male presented with sudden onset of difficulty in swallowing and speech after eating takeout food. He was treated with intramuscular epinephrine for presumed angioedema following which he immediately deve

Epinephrine in digital block: color Doppler flow imaging.

BACKGROUND: Epinephrine is currently added to local anesthetics for its advantageous vasoconstrictive effects. Traditionally, however, its use is not advised for digital block anesthesia due to the risk of digital gangrene. OBJECTIVE: To observe vasoconstrictive effect of epinephrine in digital block anesthesia. METHODS: Digital blocking with epinephrine containing lidocaine was performed in 24 subjects undergoing surgical procedure of their finger and toes. Digital artery blood flows were studied with color Doppler ultrasonography before digital blocking and at 10th, 60th, and if needed 90t

Clostridial myonecrosis resulting from subcutaneous epinephrine suspension injection.

A 32-year-old asthmatic man developed a life-threatening Clostridium perfringens infection after subcutaneous epinephrine suspension injection. Extensive surgical debridement, including forequarter amputation, hyperbaric oxygen therapy, and appropriate antibiotics were used as life-saving measures. The patient made a fair functional recovery. We could find no previously reported cases of gas gangrene following subcutaneous injection in otherwise healthy individuals. The pathogenesis and treatment of this infection are discussed.

Epinephrine-induced vasospasm reversed by phentolamine digital block.

A case of profound digital vasoconstriction caused by the accidental injection of epinephrine from a commercial bee sting kit is reported. One hour later the patient had a cold, painful, blanched finger. A digital block using a 2-mg dose of phentolamine mixed with 2% lidocaine was performed. Thirty minutes after treatment, the finger was pink and warm. The patient reported a marked decrease in pain. Reexamination 12 hours later showed only mild tenderness at the fingertip. No tissue necrosis occurred. One month after injection, there were no apparent sequelae. Although the use of alpha-adrener

A comparison of repeated high doses and repeated standard doses of epinephrine for cardiac arrest outside the hospital. European Epinephrine Study Group.

BACKGROUND: Clinical trials have not shown a benefit of high doses of epinephrine in the management of cardiac arrest. We conducted a prospective, multicenter, randomized study comparing repeated high doses of epinephrine with repeated standard doses in cases of out-of-hospital cardiac arrest. METHODS: Adult patients who had cardiac arrest outside the hospital were enrolled if the cardiac rhythm continued to be ventricular fibrillation despite the administration of external electrical shocks, or if they had asystole or pulseless electrical activity at the time epinephrine was administered. We

Epinephrine Improves Outcomes in Out-Of-Hospital Cardiac Arrests.

Epinephrine use in cardiac arrest (CA) is associated with a greater likelihood of return of spontaneous circulation (ROSC) but this early potential benefit does not seem to translate to survival with good neurologic outcomes. Similarly, high-dose epinephrine may yield a small increase in ROSC but does not improve rates of hospital discharge, long-term survival, or improved neurologic outcomes compared to standard-dosed epinephrine. Higher cumulative doses of epinephrine (>3 mg) during CA has been consistently associated with poor neurologic outcomes following ROSC.

Molecularly engineered supramolecular fluorescent chemodosimeter for measuring epinephrine dynamics.

Accurately visualizing epinephrine (EP) activity is essential for understanding its physiological functions and pathological processes in brain. However, to the best of our knowledge, reliable, rapid, and specifical measurement of EP dynamics at cellular and in vivo level hasn't been previously reported. Herein, we report the probe for EP imaging and biosensing in neurons and living brain of freely behaving animals, based on creating a series of supramolecular fluorescent chemodosimeters through host-guest interaction. The optimized chemodosimeter enables real-time imaging and quantifying of E

Epinephrine-Induced Coronary Vasospasm Exacerbating Type I Myocardial Infarction.

Rapid administration of intramuscular epinephrine is a crucial lifesaving intervention for severe anaphylaxis. Owing to its alpha- and beta-agonistic effects, epinephrine may rarely induce coronary vasospasm leading to myocardial ischemia and infarction, which can be worse in patients with underlying coronary artery disease due to flow-limiting luminal irregularities who are concurrently predisposed to infarction via plaque rupture. We present an exceedingly rare case of a 48-year-old man with undiagnosed coronary artery disease who developed a type 1 ST-segment elevation myocardial infarction

Outcomes with intracoronary vs. intravenous epinephrine in cardiac arrest.

BACKGROUND: Advanced Cardiovascular Life Support (ACLS) guidelines recommend intravenous (IV) and intraosseous (IO) epinephrine as a basic cornerstone in the resuscitation process. Data about the efficacy and safety of intracoronary (IC) epinephrine during cardiac arrest in the catheterization laboratory are lacking. OBJECTIVE: To examine the efficacy and safety of IC vs. IV epinephrine for resuscitation during cardiac arrest in the catheterization laboratory. METHODS AND RESULTS: This is a prospective observational study that included all patients who experienced cardiac arrest in the cath

A Novel Method for Administering Epinephrine during Neonatal Resuscitation.

OBJECTIVE: This study aimed to determine if prefilled epinephrine syringes will reduce time to epinephrine administration compared with conventional epinephrine during standardized simulated neonatal resuscitation. STUDY DESIGN: Timely and accurate epinephrine administration during neonatal resuscitation is lifesaving in bradycardic infants. Current epinephrine preparation is inefficient and error-prone. For other emergency use drugs, prefilled medication syringes have decreased error and administration time. Twenty-one neonatal intensive care unit nurses were enrolled. Each subject engaged i

Intravenous epinephrine overdose in prehospital management of suspected anaphylaxis.

A 65-year-old woman called paramedics for stridor and neck swelling following an insect bite with a possible anaphylactic reaction. On arrival paramedics administered intramuscular epinephrine without any observed improvement in stridor. Paramedics then prepared 5 mg of 1:1000 epinephrine for nebulised administration, which was inadvertently given intravenously. The patient developed tachycardia, anxiety and a severe headache, with biochemical evidence of cardiac necrosis without any haemodynamic compromise. The patient recovered over the next 24 hours and no long-term sequelae were identified

Cardiac arrhythmia from epinephrine overdose in epidural test dose.

Medication shortages are a clinical reality that force changes in practice patterns leading to unintended consequences. Potential solutions to any drug shortage require a thoughtful, multidisciplinary and often creative approach. Here, we report a case of unintentional epinephrine overdose leading to an unstable cardiac arrhythmia and our subsequent development of a visual cue system to prevent future errors. A 56-year-old man with a history of rectal adenocarcinoma presented for low anterior resection and creation of diverting loop ileostomy. Epidural placement was requested by the surgical t
