BACKGROUND: Single-pulse ablation leads to irreversible electroporation (IRE) and has been introduced as a nonthermal ablation technology for pulmonary vein isolation (PVI). First-in-human studies demonstrated the acute feasibility and safety of IRE PVI.
OBJECTIVE: This study aimed to further investigate the safety of single-pulse ablation for PVI.
METHODS: Twenty patients with symptomatic atrial fibrillation underwent single-pulse PVI under conscious sedation. Nonarcing, nonbarotraumatic, 6 ms, 200 J IRE applications were delivered via a custom 14-polar circular IRE ablation catheter with a

A new operative method was designed for preventing gastroesophageal reflux after esophageal and cardiac cancer operation. From April, 1994 to December, 1995, sixty-one patients after resection of esophageal and cardiac cancer were randomly divided into two groups: experimental group with 34 patients and control group with the others. All patients were observed, monitored and followed up. It was found that no case in the experimental group showed symptoms of gastroesophageal reflux or marked reflux by esophagoscopy and barium meal examination. 10 patients (37%) in the control group had the symp

Flexible fiberoptic endoscopes are being increasingly used for direct visualization and biopsy of lesions of the gastrointestinal tract. The spectrum of gastrointestinal complications associated with fiber-optic esophagoscopy, gastroscopy, and colonoscopy is reviewed. Radiologic recognition and evaluation of these iatrogenic incidents is of critical importance.

Lower esophageal strictures in infants in more than 90% of cases are related to reflux esophagitis, while the remainder are congenital or induced by injuries such as penetrating trauma or corrosion by caustic agents. Among congenital disorders cartilaginous remnants in the esophagus are very rare, but it is a possibility that should always be considered, especially if signs of esophagitis are not seen on esophagoscopy. The first aim, when evaluating the vomiting or regurgitating infant, is to establish whether this is due to an organic disorder. When stricture of the esophagus becomes evident

The authors draw attention to cervical spine disorders that limit mobility of the vertebrae and thereby interfere with esophagoscopy. Four orthopedic disorders that either fuse the cervical spine or produce massive osteophytes are discussed and exemplified by specific cases, and appropriate techniques for endoscopy are recommended.

The aim of this study was to determine the necessity of endoscopy in cases in which a corrosive substance was ingested and to find a practical way to avoid unnecessary endoscopies for similar cases in the future. The clinical records of 458 hospitalized cases with clinical histories of corrosive substance ingestion between January 2007 and December 2013 were retrospectively reviewed. The demographics of the cases, the ingested substances, and the rigid endoscopy findings were evaluated. The three most commonly ingested corrosive agents were household bleach (22.9%), household degreaser (15.9%)

OBJECTIVE: Standard esophagogastroduodenoscopy (EGD) is costly and uses conscious sedation that cirrhotic patients may tolerate poorly. This study aimed to determine the feasibility and acceptance of unsedated esophagoscopy with an ultrathin battery-powered endoscope (BPE) in cirrhotic patients for diagnosing esophageal varices (EV).
METHODS: We first studied the prevalence of significant gastroduodenal pathology that could be missed if only esophagoscopy were performed in cirrhotic patients undergoing liver transplant evaluation. A prospective study was then done to evaluate a BPE in EV scre

OBJECTIVE(S): Coins are the most frequently ingested foreign bodies by children. While rigid esophagoscopy with concomitant intubation is frequently used for removal, it has the potential for serious complications. We aimed to assess and compare the safety, efficacy, and efficiency of two different techniques for pediatric cervical esophageal coin removal.
METHODS: Retrospective chart review identified patients who underwent esophageal coin removal from January 2017-August 2023 at one of two tertiary academic centers: Ann & Robert H. Lurie Children's Hospital and Yale-New Haven Hospital. Pati

The authors describe a rare complication following total gastrectomy or reconstruction using a Roux-en-Y loop: the presence of a metal mandrel used to insert the nasogastric tube in the end tract was discovered during esophagojejunal anastomosis. The object was extracted by esophagoscopy without damage to the patient.

We discuss clinical symptoms and radiological findings of variable esophageal foreign bodies as well as therapeutic procedures in Caucasian pediatric patients. A retrospective study of 192 cases of suspected esophageal foreign bodies between 1998 and 2010 was conducted. Data were statistically analyzed by chi-square test. A foreign body was removed from a digestive tract of 163 children aged 6 months to 15 years (mean age 4.9). Most objects were located within cricopharyngeal sphincter. Dysphagia occurred in 43%, followed by vomiting (29%) and drooling (28%). The most common objects were coins

In 1967, care of esophageal burns from caustic substances was altered to include immediate esophagoscopy, high doses of steroid, and follow-up dilatations plus cine-esophagograms as needed. Previously, this area averaged six repairs of esophageal stricutre each year. Since 1967, there have been no esophageal strictures in 100 patients.

The accuracy of five tests for the diagnosis of gastroesophageal reflux in children was performed in 93 symptomatic children with gastroesophageal reflux and 16 nonreflux patients. These tests include the barium esophagram, the Tuttle test, extended esophageal pH monitoring, esophagoscopy, and esophageal biopsy. Esophagoscopy was less sensitive in detecting reflux in patients than any other test (P = less than 0.001), and biopsy was more likely to identify reflux patients than the barium swallow (P = less than 0.02), but there was no test superior to others. The severity of esophagitis noted a

A rare form of paroxysmal supraventricular tachycardia (SVT), associated with swallowing solid food, is described. Repeated triggering of the arrhythmia during esophagoscopy was produced by mechanical stimulation of the anterior esophageal wall, 30 cm from the incisors. Treatment with verapamil abolished this tachycardia. Esophagoscopic examination, the first of its kind in this setting, aided the investigation.

Dysphagia is a relatively uncommon presenting symptom of lung carcinoma that usually occurs in association with mediastinal adenopathy. However, a bronchogenic carcinoma will occasionally involve the esophagus by direct invasion. These central lesions can be difficult to visualize on chest radiographs and may not be detected by esophagoscopy or barium swallow. In such cases, a computed tomography scan of the thorax may suggest the correct diagnosis.

Caustic injury to the aerodigestive tract remains a significant medical and social concern despite various efforts to minimize hazards of caustic household products. Agents with a pH less than two or greater than 12 are extremely corrosive, causing damage that can range from mild to extensive, including esophageal perforation leading to mediastinitis and death at the extreme scale. Methods include retrospective case note review of all admissions to the otolaryngology unit with caustic injury that underwent esophagoscopy to the Children's Hospital Westmead between 1990 and 2007. A protocol-base

The management of gastrointestinal anastomotic leaks post surgery is a considerable challenge, characterized by elevated morbidity and mortality, particularly in cases of esophageal-jejunal anastomotic leaks. Diverse endoscopic intervention techniques have been utilized with enhanced success. We present a case where a 57-year-old patient with Siewert type II esophageal cardia cancer underwent endoscopic deployment of a fully covered stent into a fistula resulting from anastomotic leakage, following a laparoscopic proximal gastrectomy with Roux-en-Y and double tract reconstruction. On the third

Cervical stab wounds with a thoracic-inlet esophageal injury are extremely rare. A 30-year-old man presented with dysphagia and stridor. He had attempted suicide by stabbing his neck with a screwdriver followed by jumping from a building 10 days previously, when a cervical tracheal injury was found and surgically repaired. Physical examination was unremarkable. Lateral cervical radiography revealed an air-fluid level within an extensive retropharyngeal pocket. Follow-up radiography showed that the retropharyngeal lesion had shrunk spontaneously. Contrast esophagography demonstrated an extravas

A 17-year-old boy with immunodeficiency, elevated levels of IgM,and neutropenia developed distal esophageal ulcers and stricture. Although his lower esophageal sphincter pressure was decreased, he had normal acid reflux test results, normal findings from acid clearing studies, and absence of diffuse esophagitis at esophagoscopy. Neutropenia and hypogammaglobulinemia are postulated as pathogenic factors in his esophageal ulcers.

A 34-year-old man was admitted to our hospital because of a tumor shadow in the posterior mediastinum. Leiomyoma of the esophagus was suggested by the findings of CT, esophagography, and esophagoscopy. He underwent thoracotomy. The operative procedure was enucleation of the tumor. The histological examination confirmed it to be a leiomyoma. The postoperative course was uneventful, and the passage of the esophagus was good. He was discharged 36 days after the operation.
