Augmented surgery for esotropia associated with high hypermetropia.

Historically, surgical formulas for the management of accommodative esotropia have been based on the residual deviation with full hypermetropic correction. This "standard surgery" has resulted in a high incidence of undercorrection. In response to the large number of undercorrections with standard surgery, the authors have devised a formula for augmenting the amount of rectus recession based on the average of the near deviation with and without correction. In this study, we compare augmented surgery to standard surgery in patients who underwent bilateral medial rectus recessions for residual e

Clinical characteristics and surgical treatment of intermittent esotropia.

This study describes the age of onset, symptomatology, and clinical characteristics of 25 patients with intermittent esotropia. This subgroup of esotropic patients typically has onset prior to 10 years of age and shows excellent bifoveal fixation ability, reflected by a high degree of stereopsis. In contrast to patients with other forms of esotropia, there is minimal to no hyperopia, an average esodeviation of approximately 20 prism diopters, and a low incidence of amblyopia, DVD, and oblique overaction. Symptomatic diplopia or cosmetic concern necessitated surgical intervention in 17 (68%) pa

Accommodative esotropia: review of current practices and controversies.

The treatment of accommodative esotropia requires a thorough evaluation at first presentation to elucidate all factors involved. The degree of hypermetropia, character of the esodeviation (i.e., intermittent or constant), presence or absence of amblyopia, and the AC/A ratio are all important in the initial management of these patients. In addition, the binocular status can be of prognostic value. Close follow-up is needed to ensure maintenance of binocularity and appropriate early intervention to modify the course before bifixation is lost.

Abnormalities of the vestibulo-ocular response in congenital esotropia.

In 32 infants with congenital esotropia, I measured the vestibuloocular response. The eight patients with nystagmus compensation syndrome exhibited brisk responses. Fourteen of the 24 who had estropia without nystagmus had severely diminished responses and the other ten had moderate responses. Vestibulo-ocular testing is useful in detecting subtle forms of nystagmus compensation syndrome, especially in young infants, in whom thorough examination of ductions and versions is difficult. Dysfunction of the vestibular system is not necessarily a primary cause of congenital estropia, but brain-stem

A new rationale for the management of large angle esotropia.

It is not only possible but safe to exceed the traditional 5 mm maximum recession of each medial rectus muscle to correct large angle esotropia. It is possible to grade the amount of recession to the preoperative deviation, similar to that which is done for esotropia less than 35 prism diopters. The results of this series indicate that alignment to within 10 prism diopters with one procedure is more likely by large recessions of the medial rectus than by other approaches. If a reoperation is necessary, there still remain two other untouched horizontal muscles on which to operate.

Lateral rectus dermoid cyst presenting as incomitant esotropia.

A 24-year-old male presented with decreased vision associated with inward deviation of his left eye since childhood and gradually progressive prominence of left eye for 6 months. Left eye examination revealed visual acuity of 2/60, convergent squint with restricted abduction and medial dystopia. Computed tomography (CT) scan showed a well-circumscribed mass with fat fluid levels temporal to the optic nerve indistinguishable from the left lateral rectus. Intraoperatively, a well-encapsulated mass was identified within the lateral rectus muscle which was confirmed as dermoid cyst on histopatholo

Risk factors for consecutive exotropia after esotropia surgery.

PURPOSE: To evaluate the risk factors in development of postoperative exotropia following bilateral medial rectus (BMR) recession for esotropia. METHODS: 108 patients who underwent BMR recession for esotropia between November 1996 and July 2010 were enrolled. 54 patients who required surgery for consecutive exotropia were compared to 54 controls whose angle of deviation showed less than five prism diopters of esotropia at all periods after BMR recession. Amblyopia, dissociated vertical deviation (DVD), refractive error, inferior oblique overaction, and multiple surgery history were evaluated

[Faden operations - indications beyond esotropia and surgical results].

BACKGROUND: Faden operations is a well-known surgical option for the treatment of esotropia. To the best of our knowledge there are only a few reports available dealing with this particular topic. PATIENTS AND METHOD: Retrospective series of patients in whom between 2000 and 2011 a faden operation was performed under all indications except esotropia. 23 patients (9 Graves' disease, 9 third nerve palsy, 3 sixth nerve palsy, 1 muscle dystrophy, 2 orbital trauma) were included. RESULTS: 5 out of 7 horizontally disturbed patients were operated unilaterally and the remaining 2 bilaterally. The fi

Head tilt-dependent esotropia associated with trisomy 21.

OBJECTIVE: To describe a series of patients who used a head tilt to control esotropia. DESIGN: Retrospective noncomparative case series. PARTICIPANTS: Seven children with esotropia that decreased with their compensatory abnormal head tilt. Six of the patients had trisomy 21. The patients had no other identifiable etiology for their head tilt, including no oblique muscle dysfunction, nystagmus that changed with head tilt, or uncorrected refractive error. INTERVENTION: Six patients underwent horizontal extraocular muscle surgery. Preoperative evaluation in 4 patients included assessment of th

Long-term surgical outcome of partially accommodative esotropia.

Partially accommodative esotropia is an acquired strabismus characterized by high hyperopia, a normal AC/A ratio, and a deviation that responds only partially to spectacle correction. Surgery is done for the non-accommodative portion of the deviation. Over-corrections are managed by reducing the hyperopic power of the spectacles. This retrospective study was done to determine the risk factors for a poor surgical outcome, and to evaluate the efficacy of hyperopic spectacle reduction in the long-term management of consecutive exotropia. Of the 108 patients enrolled, 19% had a residual esotropia,

Characteristics and management of cyclic esotropia: a scoping review.

BACKGROUND: Cyclic esotropia is a rare form of strabismus characterized by alternating periods of esodeviation, typically occurring in a predictable cycle. Its etiology and treatment remain incompletely understood. This review aims to synthesize current evidence on the characteristics and treatment for cyclic esotropia. METHODS: Systematic searches were conducted in Ovid MEDLINE, Ovid Embase, Cochrane registered trials, Web of Science, and Scopus databases. The key terms used for searches were cyclic esotropia and treatment. Two independent reviewers screened the title and abstract of retriev

Management of childhood cyclic esotropia with botulinum toxin.

PURPOSE: We aimed to describe three cases of childhood cyclic esotropia and report the outcomes of botulinum toxin type A (BTA) treatment. MATERIALS AND METHODS: Three children with cyclic esotropia manifesting in a 48-hour cycle were included in the study. BTA (5 IU) was injected into the medial rectus muscles. RESULTS: The first patient was a 9-year-old boy with esodeviation (55-50 PD at near and distance) beginning after phacoemulsification surgery for juvenile cataracts. Injection of BTA resulted in breaking the cycle, and a second BTA injection was performed for residual deviation. Duri

Management of acute acquired comitant esotropia in children.

Aim: This report aims to discuss and review the diagnosis and management of acute acquired comitant esotropia (AACE) in children and to add several cases to the limited literature available on this unusual condition in the pediatric population. Materials and methods: We present two cases of AACE with large-angle deviations that were investigated and followed-up according to current recommendations. Both cases required strabismus surgery for AACE, but different procedures were chosen, with good postoperative results. Results: Unilateral recession of the medial rectus and resection of the latera

Evaluation and Prism Management of Divergence Insufficiency Esotropia.

An esodeviation that is greater at distance than near in an adult patient requires a full sensorimotor exam to rule out any cofounding neurological conditions. Many etiologies are described in the literature to cause an esodeviation that is greater at distance than near in adult patients and some exist in conjunction with a neurological condition. However, many adult patients present to the adult strabismus clinic with no other findings on exam and have a purely benign divergence insufficiency esotropia. A review of the literature on divergence insufficiency reveals a few attempts of classifyi

An expanded view of infantile esotropia: bottoms up!

A recent editorial in the Archives assigned the primary pathogenesis of infantile esotropia to the visual cortex. This analysis addresses the limitations of this timeworn view and advances the perspective that infantile esotropia is a cortico-mesencephalic-cerebellar disorder wherein binocular cortical maldevelopment permits atavistic subcortical visual pathways to remain operational. This integrated neuroanatomical model predicts that primary neurodevelopmental disorders involving the accessory optic system or its connections to the cerebellum can also give rise to infantile esotropia.

Identical twins with "mirror image" anisometropia and esotropia.

INTRODUCTION: Identical twins account for 0.2% of the world population and 8% of all twins. A "mirror image" variation can be found in 25% of identical twins. Studies of twins assume a special place in human genetics due to the possibility of comparing genetic and other factors. We present two pairs of identical male twins with mirror-image astigmatism and esotropia. CASE OUTLINE: The first was a pair of twelve-year old identical twins with "mirror image" myopic astigmatism. The Twin 1 had myopic astigmatism in the right eye, while the Twin 2 was affected by the left eye myopic astigmatism. T

Acute esotropia in heroin withdrawal: a case series.

BACKGROUND: Esotropia during opiate withdrawal is a new clinical syndrome that has only recently been reported in the literature. METHODS: Clinical case series. RESULTS: Five patients with acute esotropia during opiate withdrawal are presented. In four there was evidence of underlying hyperopia and/or other strabismogenic features. CONCLUSIONS: The precise cause of esotropia with heroin withdrawal is uncertain. Most (and possibly all) of these patients have one or more objective strabismogenic features, and these are probably a large factor in the causation.

Factors associated with horizontal reoperation in infantile esotropia.

PURPOSE: Risk factors for requiring multiple surgeries in infantile esotropia remain unclear. We identified clinical and demographic factors associated with horizontal reoperation in this disease. METHODS: A retrospective chart review of patients who underwent surgery from 1994-1997 was performed. Subjects were divided into 2 groups: those requiring only one operation and those requiring 2 or more operations to achieve orthotropia +/-10 PD. RESULTS: In 149 patients, the overall horizontal reoperation rate was 34%. There were no statistically significant differences between the 2 groups with
