Captopril in treatment-resistant essential and renal hypertension.

Captopril, an angiotensin converting enzyme inhibitor, was used to treat 25 patients with treatment-resistant hypertension of long duration. Seven patients had essential hypertension, 10 renovascular hypertension and 8 renoparenchymatous hypertension. All patients had GFR greater than 25 ml/min/1.73 m3 BSA. The acute blood-pressure-lowering response to the drug was shown to be dependent on the prevailing activity in the renin-angiotensin system. Its long-term effect was not correlated to the activation of the renin-angiotensin system as the mean blood-pressure decrease was not significantly di

[Total blood volume in essential and secondary hypertension].

To study the role of volume factors in the pathogenesis of hypertension, total blood volume (TBV) was determined in 43 patients with essential hypertension, 10 with primary aldosteronism, 5 with Cushing's syndrome, 5 with renovascular hypertension and 23 age-matched normotensives. The radioisotope (131I) labeled plasma tracer technique was employed under conditions of constant sodium intake (200mEq/day). The TBV values obtained were expressed as % normal against the predicted values according to the formulae of Fujita and his co-workers. The results were as follows: (1) TBV was increased in pa

Urinary kallikrein excretion in essential and mineralocorticoid hypertension.

Urinary kallikrein excretion has been reported to be decreased in patients with essential hypertension and elevated in patients with primary aldosteronism as a reflection of mineralocorticoid activity. Low renin essential hypertension (LREH) has been postulated to result from excess production of an unknown mineralocorticoid(s). Urinary kallikrein excretion was compared in outpatients with essential hypertension, mineralocorticoid hypertension (primary aldosteronism and 17alpha-hydroxylase deficiency), and in normal subjects of the same race. No significant difference in urinary kallikrein exc

Spironolactone and triamterene in volume-dependent essential hypertension.

The effects of 2 potassium-retaining diuretics on arterial pressure, intravascular volume, responses of the renin-angiotensin-aldosterone system, serum electrolytes, and renal function were compared by means of an 8-wk double-blind, crossover trial in 13 patients with "volume-dependent" essential hypertension. The fall in systolic, diastolic, and mean arterial pressures in the supine and erect positions (all p less than 0.005) induced by spironolactone was greater than that by triamterene. The pressure fall induced by spironolactone was also associated with a persistent contraction in plasma v

Essential Hypertension and Oxidative Stress: Novel Future Perspectives.

Among cardiovascular diseases, hypertension is one of the main risk factors predisposing to fatal complications. Oxidative stress and chronic inflammation have been identified as potentially responsible for the development of endothelial damage and vascular stiffness, two of the primum movens of hypertension and cardiovascular diseases. Based on these data, we conducted an open-label randomized study, first, to evaluate the endothelial damage and vascular stiffness in hypertense patients; second, to test the effect of supplementation with a physiological antioxidant (melatonin 1 mg/day for 1 y

[Essential hypertension: Definitions, hemodynamic, clinical and therapeutic review].

Arterial hypertension is à chronic disease that affects more than 25 % of the French adult population. Increased peripheral resistance combined with normal cardiac output is a special feature of arterial hypertension. The increase in the resistance of arterioles remains an important feature of arterial hypertension while the study of the rigidity of large arterials trunks remains poorly explored. Pulse wave velocity (PWV) measurement has been established as one of the major independent predictors of cardiovascular events in arterial hypertension.

Rifampicin reverses nicardipine effect inducing uncontrolled essential hypertension.

Dihydropyridine calcium-channel blockers are a known substrate for the cytochrome P450 isoform 3A4. Rifampicin, an antitubercular agent, is one of the most potent inducers of hepatic and intestinal CYP3A4 thus increasing dihydropyridine metabolism. We report a case of a 67-year-old hypertensive female treated with a four-drug antihypertensive regimen including a dihydropyridine (nicardipine 50 mg bid), who was admitted for septic arthritis of the knee requiring antibiotic treatment with teicoplanin 400 mg od and rifampicin 600 mg bid. Six days after rifampicin initiation, she presented with Po

microRNAs in Essential Hypertension and Blood Pressure Regulation.

Unravelling the complete genetic predisposition to high blood pressure (BP) has proven to be challenging. This puzzle and the fact that coding regions of the genome account for less than 2 % of the entire human DNA support the hypothesis that mechanisms besides coding genes are likely to contribute to BP regulation. Non-coding RNAs, especially microRNAs, are emerging as key players of transcription regulation in both health and disease states. They control basic functions in virtually all cell types relevant to the cardiovascular system and, thus, a direct involvement with BP regulation is hig

Heart rate affects endothelial function in essential hypertension.

Increased heart rate (HR) is a risk factor for cardiovascular morbidity and mortality in the general population and in some clinical conditions. Endothelial dysfunction is an adverse prognostic factor for cardiovascular events. The aim of the study was to evaluate the effect of HR on central hemodynamic parameters and endothelial function in hypertension. We evaluated forearm blood flow (FBF) response to intra-arterial infusion of acetylcholine (ACh) and sodium nitroprusside (SNP) in 30 patients with HR ≤60 min(-1) and 30 with HR ≥80 min(-1). The FBF was measured by strain-gauge plethysmograph

Sympathetic nervous system, genes and human essential hypertension.

The sympathetic nervous system (SNS) is the first line of defense in the response to environmental stress through its regulation of second-to-second changes in blood pressure (BP). Both the activity of the SNS and the therapeutic responses to SNS agonists and antagonists are known to be highly variable in the population. "Small" changes caused by single nucleotide polymorphisms (SNPs) of SNS genes may have considerable impact on SNS function and individualized hypertension treatment. In this review, we first describe the physiology of the SNS and its influence on cardiovascular and renal mecha

Opposing glucose set points hypothesis of essential hypertension.

Understanding of the mechanisms involved in the development of essential hypertension is incomplete. Many studies have demonstrated that sympathetic overactivity is involved in the development of essential hypertension. In addition, hypertensive patients, as a group, are insulin resistant. The relationship of increased sympathetic activity to insulin resistance in essential hypertension has not been adequately explained. Hypertensive subjects have been reported to have rapid gastric emptying compared to control subjects. They also have increased postprandial hypotensive responses to high carbo

Renal participation in cardiovascular risk in essential hypertension.

The kidney plays a relevant role in the origin of essential hypertension in humans, and it suffers the consequences of sustained elevated blood pressure in the absence of therapy. Recently, a relevant prevalence of mild renal insufficiency both in general population than in hypertensive patients has been described. A direct relationship seems to exist between the level of cardiovascular risk and the prevalence of the renal disorder, whether this is detected as an elevation in serum creatinine or as a diminution of estimated creatinine clearance. This renal function impairment is a strong predi

Aldosterone antagonist improves diastolic function in essential hypertension.

Experimental studies demonstrated that mineralocorticoid antagonists prevent or reverse myocardial fibrosis. Therefore, we tested the hypothesis that the aldosterone antagonist canrenone can improve left ventricular diastolic function in essential hypertension. Using digitized M-mode echocardiography and 24-hour blood pressure monitoring (ABPM), we realized a prospective, randomized, controlled study on 34 never-treated essential hypertensives with left ventricular diastolic dysfunction. Echocardiogram and ABPM were repeated after 6 months of effective antihypertensive treatment with ACE inhib

[Cerebrovascular reactivity in elderly patients with essential hypertension].

Twenty-six patients (mean age 66.3 +/- 6.5 years) with long-term arterial hypertension were studied. In resting state the indices of cerebral hemodynamics did not differ substantially in the patients comparing to those in controls with normal arterial pressure. However, during functional tests with breath-holding and hyperventilation elderly patients with long-term mild and moderate essential arterial hypertension showed, disturbance of cerebrovascular reactivity manifesting in reduction of reactivity coefficients in response to hypo- and hypercapnic loading, earlier restoration of baseline bl

Microalbuminuria and subclinical cerebrovascular damage in essential hypertension.

BACKGROUND: Microalbuminuria is a marker of hypertensive and atherosclerotic organ damage in essential hypertension and has powerful prognostic value for cerebral and cardiovascular morbidity and mortality. An association between carotid artery intima-media thickness (IMT) and increased urinary albumin excretion (UAE) has been reported in patients with essential hypertension, suggesting a link between microalbuminuria and atherosclerotic stroke mechanism(s). METHODS: We investigated the relationships between UAE, carotid artery changes, and asymptomatic cerebrovascular damage in two groups of

Potassium depletion and salt sensitivity in essential hypertension.

To evaluate the actual role of potassium depletion on blood pressure, 11 hypertensive patients were placed on a 10-day isocaloric diet providing a daily potassium intake of either 18 or 80 mmol, with each subject serving as his or her own control; the intake of sodium (220 mmol/day) and other minerals was kept constant. On day 11 each patient was also subjected to central volume expansion by water immersion associated with either normal or low potassium intake. After a 10-day period of low potassium intake, systolic blood pressure increased (P < 0.02) by 5 mm Hg, whereas serum potassium decrea
